FLNC-Related Cardiomyopathies in an Admixed Population: Expanding the Genotype-Phenotype Spectrum and Clinical Correlates.

BACKGROUND: Pathogenic and likely pathogenic variants in filamin C (FLNC) cause a broad spectrum of cardiomyopathies. However, available genotype-phenotype data are derived predominantly from European and North American populations, with limited representation of admixed populations. METHODS: We characterized carriers of pathogenic/likely pathogenic FLNC variants identified among 2654 unrelated probands undergoing exome sequencing for suspected inherited cardiomyopathy and their relatives identified through cascade screening. Clinical, electrocardiographic, imaging, genetic, and longitudinal

Is That a Cardiac Mass or Just a Pseudoaneurysm of the Mitral-Aortic Intervalvular Fibrosa?

A 64-year-old male veteran with a history of aortic valve (AV) replacement for severe bicuspid AV regurgitation from infective endocarditis was diagnosed with acute decompensated heart failure. Initial transthoracic echocardiogram evaluation demonstrated a dilated cardiomyopathy with a suspicious mass. Multimodality imaging evaluation, including cardiac computed tomography and cardiac magnetic resonance imaging, confirmed the diagnosis of a Pseudoaneurysm of the Mitral-Aortic Intervalvular Fibrosa. Pseudoaneurysm of the Mitral-Aortic Intervalvular Fibrosa may be a potential congenital abnormal

Physical Activity and Cardiovascular Outcomes in Phenotype-Negative Cardiomyopathy Variant Carriers.

IMPORTANCE: Exercise may lead to disease progression and higher risk of sudden death in individuals with genetic cardiomyopathies, but the effects of exercise among individuals carrying a cardiomyopathy-associated variant without clinical manifestations (G+P-) are unclear. OBJECTIVE: To examine whether the effects of moderate to vigorous physical activity (MVPA) on cardiovascular (CV) outcomes, cardiac structure and function, and risk of developing overt cardiomyopathy and malignant ventricular arrhythmias (VAs) vary by G+P- status. DESIGN, SETTING, AND PARTICIPANTS: UK Biobank participants

Dystrophin Loss in Engineered Heart Tissues Recapitulates Clinically Relevant Aspects of Dystrophic Cardiomyopathy.

Heart failure is the leading cause of death in patients with Duchenne muscular dystrophy (DMD), but the mechanisms underlying the associated dilated cardiomyopathy (DCM) are not fully understood. To address this gap, we generated engineered heart tissues (EHTs) using CRISPR-edited human induced pluripotent stem cell-derived cardiomyocytes that lack dystrophin. These dystrophic EHTs reproduced aspects of systolic and diastolic dysfunction seen in DMD-related DCM as they showed impaired contractile function and slower kinetics. Increased beat rate variability was also observed in dystrophic EHTs

Cardiomyopathy in an Adolescent with McLeod Syndrome: Searching Beyond the Routine Assessment.

There are only few reports in the literature regarding McLeod Syndrome and cardiomyopathy and all of them are referred to adults with neurological symptoms and hyperCKemia. To our knowledge, this is the first reported case of a neurologically asymptomatic 14-year-old male adolescent with hyperCKemia and dilated cardiomyopathy, due to McLeod syndrome. In our patient, the documentation of clinical suspicion of McLeod syndrome was achieved by genetic assessment. Our clinical awareness guided the decision to perform a cardiovascular magnetic resonance (CMR) evaluation although the echocardiogram w

BET inhibitors in cardiovascular diseases: from atherosclerosis to heart failure.

Bromodomain and extra-terminal domain (BET) proteins act as acetyl lysine ‘readers’ that summon transcriptional factors and regulate gene expression. Deregulation of bromodomain-containing protein 4 (BRD4), a transcriptional and epigenetic regulator, is associated with increased epithelial-mesenchymal transition and profibrotic changes in the cardiovascular system. Consistently, BET inhibitors were shown to attenuate cardiac and vascular remodeling induced through several signaling pathways. BRD4 upregulation has been reported in essential hypertension, dyslipidemia, myocardial infarction, hea

Subendocardium-involved late gadolinium enhancement in non-ischemic dilated cardiomyopathy improves risk stratification of sudden cardiac death.

AIMS: The prognostic value of late gadolinium enhancement (LGE) phenotypes-particularly subendocardial involvement-for sudden cardiac death (SCD) remains unclear in dilated cardiomyopathy (DCM). Whether LGE phenotype integrating pattern and location can improve SCD risk stratification is an unmet need. METHODS AND RESULTS: DCM patients who underwent cardiac MRI were retrospectively enrolled. The endpoint was a composite of SCD and surrogate SCD events. Among 902 patients (mean age 46 ± 14 years, 78.7% men), subendocardium-involved and mid-wall LGE were observed in 129 (14.3%) and 263 (29.1%)

AAV-TNNI3 rescues an experimental murine Tnni3 mutation resulting in thin filament mediated DCM.

Cardiac thin filament mutations in TNNI3 are associated with up to 3 % of hypertrophic (HCM) cardiomyopathy cases and contribute to severe restrictive (RCM) and dilated (DCM) cardiomyopathy caseloads. As such, thin filament cardiomyopathy mediated by TNNI3 mutations is an orphan disease with unmet therapeutic need. Gene therapy is one approach to addressing orphan disease but has been restricted to the repletion of protein deficiency. Based on the best available knowledge, TNNI3 gene therapy has never been applied in the context of a functional mutant protein. Described here is the viral gene

Cardiomyocyte β-arrestins mediate inflammation and cGAS-STING activation in CVB3 viral myocarditis.

Viral myocarditis is a major cause of sudden cardiac death and can lead to dilated cardiomyopathy in adults. However, effective treatments remain elusive due to an incomplete understanding of its molecular drivers. Here, we investigate the role of β-arrestins (βarrs), scaffolding proteins that regulate GPCR signaling, in acute viral myocarditis. Using global βarr1 and βarr2 knockout (KO) mice, we assessed immune cell infiltration and apoptosis as markers of cardiac inflammation under Coxsackievirus (CVB3) infection. CVB3-infected βarr1 and 2 KO mice exhibited suppressed recruitment of NK cells

Pathogenetic mechanisms of muscle-specific ribosomes in dilated cardiomyopathy.

The heart uses a muscle-specific ribosome in cardiomyocytes, where the ribosomal protein RPL3 is replaced by its paralog RPL3L. Rare biallelic RPL3L mutations cause fatal neonatal dilated cardiomyopathy, yet the mechanisms that link genotype to heart failure are unclear. Despite the recessive inheritance pattern in humans, Rpl3l knockout mice show no overt cardiac phenotype, probably because of compensatory RPL3 upregulation through unknown mechanisms. Here we report four additional cases and propose a unifying pathogenetic model by integrating human genetics, patient tissues and isogenic cell

Computational identification of necroptosis-related hub genes and therapeutic targets in dilated cardiomyopathy via integrated analysis of bulk and single-cell RNA sequencing public cohorts.

Dilated cardiomyopathy (DCM) is a prevalent myocardial disease with a grim poor prognosis, and its pathogenesis remains poorly understood. Necroptosis, a programmed cell death mechanism, is believed to be significantly involved in the pathogenesis of DCM. This study aimed to identify necroptosis-related hub genes associated with DCM and predict potential agents for DCM. All analyses exclusively utilized publicly available datasets from the Gene Expression Omnibus database. Differentially expressed genes were identified in the DCM dataset GSE128095. The least absolute shrinkage and selection op

MDH2 S246 phosphorylation protects mitochondria against hypoxia/reoxygenation injury due to acetate/lactate metabolism.

Ischemic cardiomyopathy (ICM) is characterized by impaired myocardial function resulting from reduced coronary blood flow, resulting in heart failure. Emerging evidence from proteomic studies indicates that ischemic preconditioning confers cardioprotection against ICM through the regulation of mitochondrial proteins. Proteomic analyses have identified phosphorylation in mitochondrial proteins, including malate dehydrogenase 2 (MDH2), as mediators of cardioprotective mechanisms. However, the mechanism by which MDH2 phosphorylation contributes to cardioprotection in cardiovascular diseases remai

Unravelling the Complications of Dilated Cardiomyopathy in Duchenne Muscular Dystrophy: From Molecular Pathways to Disease Management.

INTRODUCTION: Duchenne Muscular Dystrophy (DMD) is a rare X-linked recessive disorder caused by mutations in the dystrophin gene, leading to progressive muscle weakness. Cardiomyopathy and respiratory failure remain leading causes of mortality despite improvements in respiratory, cardiac, and pharmacological management. This review aims to summarize current knowledge on the pathophysiology, management strategies, and emerging therapies for DMD-associated dilated cardiomyopathy. METHODS: A comprehensive literature search was performed for studies published up to May 2025 using PubMed, Scopus,

Clinical and prognostic impact of left atrial strain in non-ischemic cardiomyopathies.

Over the last decade, left atrial (LA) strain has gained attention as a clinical diagnostic and prognostic non-invasive imaging parameter. LA strain measures the myocardial deformation of the LA in three phases; the reservoir (LA filling during ventricular systole), the conduit (passive filling of the left ventricle [LV]) and the booster phase (late-diastolic active atrial contraction). LA strain reflects both diastolic and systolic myocardial function and is presumed to be an early marker of atrial and ventricular dysfunction. The aim of this review article is to discuss the (patho)physiology

RNA-Based Therapies for Treating Monogenic Cardiomyopathies.

As the field of cardiovascular medicine advances toward precision and personalized care, ribonucleic acid (RNA)-based therapies have emerged as powerful tools with the potential application for monogenic cardiomyopathies. Multiple classes of RNA-based therapeutics are currently being investigated, including antisense oligonucleotides, microRNAs, small interfering RNAs, small hairpin RNAs, and long noncoding RNAs. Recent progress in viral and nonviral delivery systems has enhanced the feasibility of these approaches. These advances are particularly relevant to a growing list of genetic disorder

Implication of intracellular chloride channel in extracellular matrix remodeling in pressure-overloaded mice and patients with dilated cardiomyopathy.

Chloride intracellular channels (CLICs) are important in cardiac cellular physiology. We aimed to determine the pathophysiological roles of CLICs in the heart. For this, we analyzed CLIC expression in cardiomyocytes in a mouse transverse aortic constriction (TAC) model to induce cardiac hypertrophy and failure, as well as in ventricular myocytes from patients with dilated cardiomyopathy (DCM) using single-cell RNA-sequencing. Single-ventricular myocytes were isolated from the left ventricular free wall of C57BL/6J mice after TAC (pre-TAC; Day 3 post-TAC; and Weeks 1, 2, 4, and 8 post-TAC). Gen

Dilated cardiomyopathy-associated RNA-binding motif protein 20 regulates long pre-mRNAs in neurons.

Precise coordination of molecular programs and neuronal growth governs the formation, maintenance, and adaptation of neuronal circuits. RNA metabolism has emerged as a key regulatory node of neural development and nervous system pathologies. To uncover cell-type-specific RNA regulators, we systematically investigated expression of RNA recognition motif-containing proteins in the mouse neocortex. Surprisingly, we found RNA-binding motif protein 20 (RBM20), an alternative splicing regulator associated with dilated cardiomyopathy, to be expressed in cortical parvalbumin interneurons and mitral ce

Chamber-specific chromatin architecture guides functional interpretation of disease-associated Cis-regulatory elements in human cardiomyocytes.

Cis-regulatory elements (CREs) are noncoding DNA regions regulating cell-type-specific gene expression programs by interacting with distal gene promoters. Here, we aim to decode the function and spatial organization of CRE-promoter interactions in human cardiomyocytes. We analyzed the epigenome and chromatin interactions of human male atrial, ventricular, and failing cardiomyocytes. Atrial and ventricular cardiomyocytes harbored chamber-specific CRE-promoter interactions modulating gene expression as confirmed by functional epigenetic silencing. These CRE-promoter interactions explain the dist
