Dysregulation of alternative splicing - mediated by factors such as RBM20 or SLM2 - can affect proper gene isoform control, disrupting gene isoform homeostasis and underpins severe cardiomyopathy in both animal models and patients. Although innovative therapies target various sarcomeric components, the impact of isoform switching in cardiac disease remains poorly understood. Here, we applied nanopore long-read sequencing to map the full-length transcriptome of left ventricular tissue from thirteen nonfailing controls, ten patients with dilated cardiomyopathy (DCM), and ten with ischemic cardio

Doxorubicin (Dox)-induced cardiomyopathy (DIC) is characterized by significant myocardial damage that can progress to dilated cardiomyopathy and potentially lead to heart failure. The rate of mortality due to heart disease in patients undergoing cancer chemotherapy has even surpassed that caused by tumor recurrence. However, there is a lack of effective treatments for DIC in clinical practice. Lithospermic acid (LA), a polycyclic phenolic carboxylic acid isolated from the traditional Chinese herb Salvia miltiorrhiza , exhibits superior efficacy in inhibiting oxidative stress damage across vari

Coagulation disturbances in patients with end-stage heart failure receiving intra-aortic balloon pump (IABP) support present significant management challenges. We describe a 49-year-old male with dilated cardiomyopathy awaiting transplantation who developed secondary hyperfibrinolysis following IABP-associated infection and hemodynamic instability. The patient exhibited pronounced D-dimer elevation (peak: 55.84 μg/mL) and persistent oozing at the puncture site. At the onset of hyperfibrinolysis, laboratory tests demonstrated a markedly increased plasmin-α2-plasmin inhibitor complex (PIC: 26.56

INTRODUCTION: Multifocal ectopic Purkinje-related premature contractions (MEPPC) is a syndrome caused by gain-of-function SCN5A variants, characterized by multifocal ventricular ectopy and dilated cardiomyopathy (DCM).
METHODS AND RESULTS: We report a case of a 63-year-old woman with longstanding DCM and a high burden of multifocal premature ventricular contractions arising from the His-Purkinje system, refractory to multiple antiarrhythmic drugs and associated with progressive heart failure. Genetic testing identified the pathogenic SCN5A R814W variant. Quinidine resulted in complete suppres

INTRODUCTION: Inherited cardiac conditions, including dilated cardiomyopathy (DCM) and hypertrophic cardiomyopathy (HCM), may have a monogenic cause identified through genetic testing (GT). Confirmation of pathogenic gene variants can have important implications for the patient and their relatives. The UK National Genomic Test Directory (NGTD) provides strict criteria on the indications for GT; however, the European Society of Cardiology (ESC) recommends wider GT. We reviewed the prevalence of pathogenic genotypes in patients undergoing GT who did not meet the NGTD criteria.

Dilated cardiomyopathy (DCM)-induced heart failure (HF) remains a major global health burden, highlighting the need to identify disease-associated molecular signatures and potential therapeutic targets. In this study, we performed differential expression analysis and weighted gene co-expression network analysis (WGCNA) on the GSE141910 dataset, which identified 138 genes associated with DCM-induced HF. These were further refined to 48 candidate genes using support vector machine-recursive feature elimination (SVM-RFE). Protein-protein interaction (PPI) network analysis revealed 10 hub genes, a

AIMS: Assessing cardiac function is critical for managing cardiovascular disease, guiding treatment, monitoring progression, and risk stratification. While left ventricular (LV) ejection fraction (LVEF) is firmly established, it has limitations. Myocardial contraction fraction (MCF)-the ratio of stroke volume to myocardial volume, is simple to compute without additional analysis and offers a promising alternative to LVEF.
METHODS AND RESULTS: MCF was assessed across four datasets spanning healthy controls and chronic structural cardiac disease, with direct comparison to LVEF. Association betw

Dilated cardiomyopathy (DCM) is a myocardial disease, characterized by ventricular enlargement and reduced contractile ability, which frequently leads to heart failure and arrhythmias. This condition is related to loss-of-function mutations in the RNA-binding motif protein 20 (RBM20), which disrupts target gene splicing. Two induced pluripotent stem cell (iPSC) lines, each harboring a single heterozygous RBM20 mutation, were generated from DCM patients. Both cell lines retain normal karyotypes and exhibit robust undifferentiated iPSC state markers. They have the capability to differentiate int

BACKGROUND: Myocarditis is a leading cause of heart failure in young individuals and may progress to dilated cardiomyopathy due to inflammation-driven cardiac fibrosis. Cardiac fibroblasts (CFs) are key contributors to this process, mediating both IL-17A-driven inflammatory responses and TGF-β-induced fibrotic remodeling. However, effective therapies that concurrently target these dual pathological pathways in CFs are lacking.
METHODS: We evaluated the neddylation inhibitor MLN4924 in a mouse model of experimental autoimmune myocarditis (EAM). A fibroblast-targeted biomimetic nanoparticle (FM

BACKGROUND: Coronary microvascular dysfunction (CMD) is associated with poor prognosis in patients with dilated cardiomyopathy with reduced ejection fraction (DCMrEF). However, its assessment remains challenging in routine clinical practice. This study aimed to explore the potential value of angiography-derived microcirculatory resistance (AMR) in predicting clinical outcomes in DCMrEF patients.
METHODS: This multicentre retrospective study included DCMrEF patients (2019-2024) with a 5-year major adverse cardiovascular events (MACE) end point. AMR was calculated for all major arteries. Its pr

Dilated cardiomyopathy caused by variants in the LMNA gene leads to malignant arrhythmogenic events, faster phenotype progression and high risk of sudden cardiac death. The pathophysiological mechanisms triggering disease progression remains poorly understood. We investigated the mRNA and miRNA transcriptome in the myocardial tissue of 50-week-old LMNAR249W mice developing dilated cardiomyopathy. We found 2148 genes and 53 miRNAs that were differentially expressed in LMNAR249W hearts. Gene ontology and pathway enrichments showed that differentially expressed genes were enriched mainly for fatt

Non-coding RNA (ncRNA), including microRNA, long non-coding RNA, and circular RNA, are epigenetic signals acting as upstream regulators in several cardiovascular disease processes. This review explores how ncRNA profiling complements non-invasive cardiac imaging modalities by providing biological insights into structural and functional phenotypes. In conditions such as hypertensive heart disease and aortic stenosis, ncRNAs like microRNA (miR)-29 and miR-155 correlate with left ventricular hypertrophy and fibrosis. In dilated cardiomyopathy and heart failure, circulating miR-150-5p, miR-21, and

Cardiac fibrosis is a prevalent pathological feature in the progression of various cardiovascular diseases, including heart failure, myocardial infarction, and dilated cardiomyopathy, particularly in their advanced stages. Its primary mechanism involves the abnormal activation of cardiac fibroblasts and excessive deposition of extracellular matrix, which ultimately results in decreased myocardial compliance and cardiac dysfunction. The Hippo signaling pathway, an evolutionary conserved kinase cascade, not only regulates organ development and tissue homeostasis but has also been shown to play a

INTRODUCTION: Heart muscle diseases are challenging for the aeromedical examiner due to their heterogeneous nature and widely varying natural history. The diagnosis of heart muscle disease is likely to have a significant impact on flying privileges, with the risk assessment requiring careful testing and careful follow up. Consideration must also be given to the treatments used in each individual, as these may also pose an aeromedical risk. In athletic aircrew, differentiating athletic adaptation from a cardiomyopathy is essential to decide if flying restrictions are necessary.

Dilated cardiomyopathy is characterized by progressive left ventricular dilation and impaired systolic function, with inflammation recognized as a key contributor to disease onset and adverse outcomes. C-reactive protein reflects systemic biochemical inflammation, whereas Delta Neutrophil Index represents the circulating immature neutrophil fraction and provides a cellular dimension of inflammatory burden. The combined diagnostic and prognostic value of these two biomarkers in dilated cardiomyopathy has not been adequately explored. This retrospective study included one hundred and fifty patie
