Mavacamten Versus Alcohol Septal Ablation in Obstructive Hypertrophic Cardiomyopathy: An Echocardiography-Derived Pressure-Volume Analysis.

BACKGROUND: Obstructive hypertrophic cardiomyopathy (oHCM) is characterized by left ventricular (LV) outflow tract obstruction, which increases afterload and chronically activates the Anrep response, a compensatory (afterload-driven) state of hyperdynamic systole, prolonged systolic ejection time, and increased myocardial workload. We investigated whether the myosin inhibitor mavacamten reverses this state, comparing its effects to the anatomic relief achieved by alcohol septal ablation. METHODS: Thirty-six patients with symptomatic oHCM were treated with mavacamten. Of these, 29 who achieved

Cardiology: What You May Have Missed in 2025.

During 2025, new data emerged that are important to internal medicine specialists and subspecialists who frequently encounter and manage patients with common cardiovascular conditions. Among these conditions, the landscape of therapy is changing with regard to hypertension, heart failure, and anticoagulation in atrial fibrillation. Management of acute cardiovascular diseases often involves collaboration between specialists and generalists. The choice of initial imaging modality in patients with suspected coronary artery disease, whether β-blockers are routinely indicated after myocardial infar

Efficacy of Aficamten in Hypertrophic Cardiomyopathy Patients With Very-High Left Ventricular Outflow Tract Gradients.

This sub-analysis of SEQUOIA-HCM (NCT05186818) evaluated the efficacy and safety of aficamten in obstructive hypertrophic cardiomyopathy (oHCM) and very-high Valsalva left ventricular outflow tract gradients (LVOT-G). Patients with oHCM and Valsalva LVOT-G ≥100 mm Hg (n = 73) were randomized to aficamten (n = 33) or placebo (n = 40) for 24 weeks. The primary endpoint was proportional change in Valsalva LVOT-G from baseline to Week 24. Secondary endpoints included the proportion of patients achieving Valsalva LVOT-G < 30 mm Hg, absolute change in Valsalva LVOT-G, Kansas City Cardiomyopathy Ques

Multimodality imaging in a patient with non-obstructive hypertrophic cardiomyopathy and multivessel coronary artery disease presenting with chest pain: a case report.

INTRODUCTION: Chest pain in hypertrophic cardiomyopathy (HCM) is commonly attributed to microvascular dysfunction, diastolic dysfunction, or left ventricular outflow tract obstruction, which may obscure concomitant epicardial coronary artery disease (CAD). Identifying the dominant ischemic mechanism is particularly challenging in patients with non-obstructive HCM and multivessel CAD. CASE PRESENTATION: A 43-year-old man with hypertension, dyslipidemia, and a smoking history presented with progressive chest pain. Transthoracic echocardiography and cardiac magnetic resonance confirmed non-obstru

Efficacy and safety of mavacamten in non-obstructive hypertrophic cardiomyopathy: A systematic review and meta-analysis of randomized trials.

BACKGROUND: Non-obstructive hypertrophic cardiomyopathy (nHCM) remains a therapeutic challenge, with limited pharmacological options beyond symptomatic management. Mavacamten, a selective allosteric cardiac myosin inhibitor, has shown benefit in obstructive HCM, but its role in nHCM is uncertain. METHODS: A systematic search of PubMed, Embase, CENTRAL, and ClinicalTrials.gov was conducted up to November 2025 for placebo-controlled randomized controlled trials (RCTs) enrolling patients with nHCM. Outcomes included Kansas City Cardiomyopathy Questionnaire (KCCQ) score, peak oxygen consumption (

New and recurrent atrial fibrillation in patients with obstructive hypertrophic cardiomyopathy receiving mavacamten.

BACKGROUND: Patients with obstructive hypertrophic cardiomyopathy (oHCM) are at an increased risk of the development of atrial fibrillation (AF) and subsequent AF-related morbidity. The impact of treatment with the novel cardiac myosin inhibitor mavacamten on AF is unknown. OBJECTIVE: This study aimed to evaluate incident and recurrent AF rates in patients with oHCM receiving mavacamten. METHODS: This was a retrospective observational study of patients with oHCM undergoing mavacamten treatment at the Johns Hopkins Hypertrophic Cardiomyopathy (HCM) Center. Predicted AF risk was calculated usi

Mavacamten in Obstructive Hypertrophic Cardiomyopathy: National Insights on Access and Safety Profile From Real-World Data.

BACKGROUND: Mavacamten improves symptoms in obstructive hypertrophic cardiomyopathy, but real-world prescription patterns, safety profile, and the effect of atrial fibrillation (AF) on outcomes remain unclear. METHODS: An observational multicenter analysis using patient-level data from the TriNetX database (2011-2023) compared patients with obstructive hypertrophic cardiomyopathy treated with mavacamten versus those not treated with mavacamten (controls). Multivariable logistic regression identified predictors of mavacamten use. Propensity-score matching was used to reduce confounding bias. O

Obstructive Hypertrophic Cardiomyopathy and Interarterial Anomalous Aortic Origin of the Right Coronary Artery (AAORCA) Presenting With Syncope: Multimodal Diagnostic Assessment and Individualized Management.

The coexistence of obstructive hypertrophic cardiomyopathy (oHCM) and anomalous aortic origin of the right coronary artery (AAORCA) with an interarterial course is exceptionally rare and poses a significant diagnostic and therapeutic challenge, particularly in patients presenting with syncope. We report the case of a 63-year-old man referred for evaluation of a sudden syncopal episode that occurred at rest while he was seated on an airplane, followed by a fall on standing. Transthoracic echocardiography revealed asymmetric septal hypertrophy, systolic anterior motion of the mitral valve. and a

Towards a Quantitative Understanding of Aficamten Clinical Pharmacology: Pharmacokinetic-Cardiodynamic Modeling to Support Safety and Efficacy.

In the phase 3 study SEQUOIA-HCM (NCT05186818), aficamten, a next-in-class cardiac myosin inhibitor, was safe and efficacious in participants with obstructive hypertrophic cardiomyopathy (oHCM). Using pharmacokinetics/pharmacodynamics (PKPD) modeling, we quantified the relationship between aficamten exposure and cardiodynamic measures of safety (left ventricular ejection fraction [LVEF]) and efficacy (post-Valsalva left ventricular outflow tract gradient [LVOT-G]), and used Clinical Trial Simulations (CTS) to predict cardiodynamics for a flexible dose regimen in a post-approval setting. Are clinical trajectories in obstructive hypertrophic cardiomyopathy mutation-specific? A systematic review and meta-analysis of MYH7 vs. MYBPC3.

BACKGROUND: The clinical course of obstructive hypertrophic cardiomyopathy (HCM) is highly variable. Whether this heterogeneity reflects mutation-specific clinical trajectories remains a pivotal question for prognostication and personalized management. OBJECTIVES: This study aimed to determine if pathogenic sarcomeric variants in MYH7 and MYBPC3 are associated with distinct clinical pathways in obstructive HCM, by comparing their associations with age at presentation, severity of hypertrophy, left ventricular outflow tract (LVOT) obstruction, and arrhythmic outcomes. METHODS: We conducted a

Long-term outcomes of obstructive hypertrophic cardiomyopathy undergoing surgical myectomy: impact of symptom severity at presentation.

Surgical myectomy (SM), performed to relieve dynamic left ventricular outflow tract obstruction in symptomatic obstructive hypertrophic cardiomyopathy (oHCM) patients, provides improved quality of life and symptoms and excellent long-term survival. Guidelines recommend SM in symptomatic oHCM patients refractory to optimal medical therapy. We sought to assess whether SM in patients with fewer symptoms at initial referral is associated with improved long-term outcomes in oHCM. Baseline symptom burden at the time of referral emerged as a powerful prognostic discriminator among 3546 HCM patients u

Obstructive Hypertrophic Cardiomyopathy in a Symptomatic Professional Athlete Treated With Mavacamten.

BACKGROUND: Management of left ventricular outflow tract obstruction (LVOTO) in competitive athletes with hypertrophic cardiomyopathy (HCM) is challenging, particularly when obstruction is exercise-induced and medical therapy is poorly tolerated. CASE SUMMARY: A 28-year-old professional athlete presented with recurrent syncope. Resting imaging showed asymmetric septal hypertrophy without obstruction, but stress echocardiography revealed severe dynamic LVOTO (peak gradient: 150 mm Hg). Beta-blockers caused fatigue with persistent gradients. After shared decision-making, mavacamten was initiate
