Mavacamten in Chinese Patients with Obstructive Hypertrophic Cardiomyopathy: Patient-Reported Health Status Analysis up to 78 Weeks in the EXPLORER-CN Study.

INTRODUCTION: Improvement of symptoms, physical function, and quality of life is one treatment goal for patients with obstructive hypertrophic cardiomyopathy (HCM). Mavacamten improved left-ventricular outflow tract gradients in Chinese patients with obstructive HCM in the EXPLORER-CN trial. We report here a detailed analysis of patient-reported health status per the 23-item Kansas City Cardiomyopathy Questionnaire (KCCQ-23) in Chinese patients after 78 weeks of mavacamten. METHODS: Patients who completed the double-blind, placebo-controlled (DBPC) period of EXPLORER-CN with no active safety

Anesthetic management of transapical beating-heart septal myectomy in a child with obstructive hypertrophic cardiomyopathy: a case report.

BACKGROUND: Transapical beating‑heart septal myectomy (TA‑BSM) is considered an effective surgical approach for obstructive hypertrophic cardiomyopathy (oHCM). However, data regarding its application in pediatric patients remain limited. To date, no cases have been reported detailing the anesthetic management for pediatric oHCM patients undergoing TA‑BSM. CASE PRESENTATION: We report the case of a 9‑year‑old girl with oHCM who underwent TA‑BSM under general anesthesia. Despite thorough preoperative preparation, she experienced significant perioperative complications, including hypotension and

Cardiogenic shock and atrial fibrillation after mavacamten dose escalation in obstructive hypertrophic cardiomyopathy: a case report.

BACKGROUND: Mavacamten is a first-in-class cardiac myosin inhibitor (CMI) that reduces left ventricular outflow tract (LVOT) obstruction in patients with symptomatic obstructive hypertrophic cardiomyopathy (HCM). However, excessive myosin inhibition may cause significant systolic dysfunction and rarely requires intensive care. CASE SUMMARY: A 40-year-old woman with obstructive HCM presented with dyspnoea and palpitations. Mavacamten 5 mg daily improved symptoms after an inadequate response to β-blocker therapy, although significant LVOT obstruction persisted. Following dose escalation to 10 m

Cardiac Myosin Inhibitors (CMIs) and Surgical Referral in Patients with Hypertrophic Cardiomyopathy.

The management of obstructive hypertrophic cardiomyopathy (HCM) has been transformed by the advent of cardiac myosin inhibitors (CMIs), such as mavacamten and aficamten. Unlike traditional pharmacotherapy, which primarily addresses symptoms, CMIs target the underlying mechanism of sarcomeric hypercontractility, offering significant reductions in left ventricular outflow tract (LVOT) gradients and improved functional capacity. This review evaluates the evolving role of CMIs in refining surgical candidate selection and postoperative care. Clinically, CMIs function as an in vivo "biological test"

Big Endothelin-1 Predicts the long-term survival in patients undergoing septal myectomy.

BACKGROUND: The prognostic role of endothelin-1 (ET-1) in patients with obstructive hypertrophic cardiomyopathy (oHCM) who underwent septal myectomy remains unclear. We aimed to assess its prognostic importance in patients undergoing septal myectomy. METHODS: We recruited 1207 oHCM patients for this analysis. The participants were divided into three groups according to the tertile of big ET-1. Cox regression analyses were conducted to assess the prognostic value of big ET-1 on all-cause death and cardiovascular death. RESULTS: The mean age of the participants was 49.9 ± 12.0 years, 713 (59%)

Venous Septal Alcohol Ablation as a Salvage Strategy in Obstructive Hypertrophic Cardiomyopathy.

Septal alcohol ablation (SAA) is an established septal reduction therapy for symptomatic obstructive hypertrophic cardiomyopathy (HCM); however, its efficacy depends on favorable septal arterial anatomy. In a subset of patients, arterial access may be limited by unfavorable anatomy or prior septal artery occlusion, rendering conventional SAA ineffective. We report the first case series describing venous SAA as an alternative and salvage strategy. Two symptomatic men with obstructive HCM underwent venous septal ethanol delivery after inadequate arterial septal ablation. In the first case, limit

Myectomy with secondary chordal resection in hypertrophic obstructive cardiomyopathy: the mid-term outcomes.

BACKGROUND: It is not clear whether septal myectomy with secondary chordal resection (SM-SCR) can achieve similar postoperative outcomes among hypertrophic obstructive cardiomyopathy (HOCM) patients exhibiting different severity of septal hypertrophy. METHODS: This was a single-center retrospective cohort study with a nested case-control study. HOCM patients received SM-SCR between 2014 and 2023 were retrospectively included and were grouped based on their septal thickness. The main outcome was the relief of the left ventricular outflow tract (LVOT) obstruction. Besides, predictors for recurr

Long-term outcomes after septal reduction therapy for obstructive hypertrophic cardiomyopathy: a nationwide cohort study.

AIMS: With cardiac myosin inhibitors emerging as a novel pharmacological option instead of septal reduction therapies (SRT) in obstructive hypertrophic cardiomyopathy (HCM), contemporary data on national long-term outcomes after SRT are needed. METHODS AND RESULTS: In this nationwide cohort study from 2015 to 2021, patients with obstructive HCM undergoing SRT (surgical myectomy or transcoronary ablation of septal hypertrophy) were 1:8 propensity score-matched with non-obstructive HCM patients or surgical controls undergoing appendectomy without heart disease in recent records. As assessed in

Obstructive Hypertrophic Cardiomyopathy With Degenerative Mitral Regurgitation: A New Treatment Strategy Using Mavacamten Combined With M-TEER.

BACKGROUND: Transcatheter edge-to-edge mitral valve repair (M-TEER) has been demonstrated as an effective therapeutic option for patients with obstructive hypertrophic cardiomyopathy (oHCM) and concomitant degenerative mitral regurgitation. Mavacamten is a mechanism-based therapy for oHCM that can improve hemodynamics. However, clinical evidence on the concomitant use of mavacamten and M-TEER remains limited, and related case reports are scarce. CASE SUMMARY: We present the case of a 79-year-old woman with symptomatic severe oHCM complicated by mitral valve prolapse, aortic stenosis, and seco

A global model for symptomatic obstructive hypertrophic cardiomyopathy to assess the cost-effectiveness of mavacamten: results from a Dutch societal perspective.

OBJECTIVES: To explore the cost-effectiveness of mavacamten + beta-blocker/calcium channel blocker therapy (BB/CCB) versus BB/CCB monotherapy for the treatment of symptomatic obstructive hypertrophic cardiomyopathy (HCM). MATERIALS AND METHODS: A 5-state Markov model (New York Heart Association classes I-IV, death) that included treatment sequencing was developed. It used a Dutch societal perspective and lifetime horizon stratified into short-term (mavacamten + BB/CCB: 30 weeks; BB/CCB: 46 weeks) and long-term (i.e. post short-term) periods. The model population reflected the EXPLORER-HCM tri

Use of Disopyramide in Obstructive Hypertrophic Cardiomyopathy: A European Insight.

Background/Objectives: Guidelines for obstructive hypertrophic cardiomyopathy (HCM) recommend treatment with disopyramide as an add-on to beta-blockers or calcium-channel blockers when symptoms persist. Data pertaining to effective disopyramide use in practice beyond single-center experience are very limited. This study aimed to quantify disopyramide use in patients with obstructive HCM in England, France and Germany, before the availability of cardiac myosin inhibitors. Methods: This retrospective study used nationally representative databases from England (Clinical Practice Research Datalink

Systematic Review Certainty of Evidence and Procedural Durability of Septal Reduction Therapies in Obstructive Hypertrophic Cardiomyopathy: An Umbrella Review of Meta-Analyses.

INTRODUCTION: This umbrella review aimed to evaluate the certainty and consistency of the available evidence rather than to provide a new procedural comparison. We synthesized systematic reviews comparing surgical septal myectomy (SM) versus alcohol septal ablation (ASA) for symptomatic obstructive hypertrophic cardiomyopathy (oHCM) to critically assess the certainty of findings. METHODS: An umbrella review of meta-analyses was conducted following Cochrane and PRISMA guidelines. A systematic search of four databases was performed. Methodological quality, risk of bias, and study overlap were a

Long-Term Outcome of Myectomy Associated With Secondary Mitral Valve Chordal Cutting in Obstructive Hypertrophic Cardiomyopathy.

BACKGROUND: The recent introduction of cardiac myosin inhibitors has modified the therapy of left ventricular outflow obstruction in patients with hypertrophic cardiomyopathy (HCM) and is raising uncertainties regarding the future role of surgery in the management of obstructive HCM. However, it is likely that a proportion of patients will continue to present with advanced, highly symptomatic HCM, despite medical therapy, and require surgical intervention. We previously reported the short-term results of cutting anterior mitral leaflet secondary chordae in combination with septal myectomy in p
