PURPOSE OF REVIEW: This review summarises recent advances in cardiac involvement in eosinophilia, with particular emphasis on pathophysiology, diagnostic challenges and therapeutic options.
RECENT FINDINGS: Up to 43% of patients with histologically proven eosinophilic myocarditis (EM) have no evidence of peripheral blood eosinophilia. Aberrations in the interleukin-5 pathway are central to the pathophysiology of EM. In addition, other contributing factors for specific aetiologies are being recognised, such as susceptibility loci GPA33 and IRF1/IL5 in patients with ANCA-negative eosinophilic g

Myocarditis is an inflammatory disease of the heart muscle that can be triggered by various causes, including viruses, autoimmune response, molecular mimicry and exposure to immune-stimulating drugs or vaccines. Most cases of myocarditis heal, and cardiac dysfunction, if present, recovers; however, selected forms may require targeted therapy to improve outcomes. We herein review five conditions presenting with or mimicking myocarditis that require targeted diagnostic approaches, including endomyocardial biopsy, and/or targeted treatments. Giant cell myocarditis is an intense and unresolving in

Eosinophilic heart disease (EHD) is an inflammatory cardiac disorder characterized by the infiltration and subsequent heart tissue damage mediated by eosinophils. The clinical manifestations of EHD vary widely, ranging from asymptomatic to severe, life-threatening conditions. Diagnosis relies on a multimodal approach, incorporating clinical presentations, laboratory findings (e.g. hypereosinophilia), and advanced imaging studies such as cardiac magnetic resonance imaging (MRI) and echocardiography. However, the gold standard for definitive diagnosis remains endomyocardial biopsy, which demonst

BACKGROUND: Anaphylaxis is a severe hypersensitivity reaction that may involve the heart through overlapping inflammatory, vascular, and catecholaminergic mechanisms. The ATAK (adrenaline, Takotsubo cardiomyopathy, anaphylaxis, Kounis syndrome) complex describes a clinical continuum in which hypersensitivity-mediated inflammation and endogenous or exogenous catecholamines promote endothelial injury, coronary vasomotor and microvascular dysfunction, and myocardial damage.
CASE SUMMARY: A 79-year-old man with urinary sepsis developed an antibiotic-related hypersensitivity reaction followed by a

Heart involvement in eosinophilia represents a rare entity with substantial cardiac morbidity and mortality. This disorder is often overlooked and diagnosed in its advanced stages. In a subgroup of patients with eosinophilic heart disease, peripheral blood eosinophilia may not be present, and the biopsy of the affected organ or tissue is essential to establish the correct diagnosis. In this clinical consensus statement, an expert consensus group reviews the epidemiology, aetiology, diagnosis, and management of eosinophilic heart disease. The aim of the document is to increase awareness about t

BACKGROUND: Eosinophilic myocarditis (EM) is a rare and potentially life-threatening inflammatory heart disease. Diagnosis is frequently delayed because clinical presentations are heterogeneous, early cardiovascular magnetic resonance (CMR) lacks a pathognomonic pattern, and endomyocardial biopsy (EMB) is often performed late or after glucocorticoid exposure. A further pitfall is assuming that EM can be excluded in the absence of peripheral eosinophilia or overt extracardiac disease. Delayed recognition may result in irreversible myocardial injury, endomyocardial fibrosis and adverse clinical

BACKGROUND: Dupilumab, an IL-4Rα antagonist used in type 2 inflammatory diseases, is associated with transient peripheral eosinophilia, yet progression to eosinophilic granulomatosis with polyangiitis (EGPA) remains rare.
CASE SUMMARY: A 64-year-old man presented with acute chest pain, dyspnea, marked hypereosinophilia, elevated cardiac biomarkers, and systemic symptoms 3 months after dupilumab initiation. Echocardiography and cardiac magnetic resonance imaging demonstrated left ventricular dysfunction, and endomyocardial biopsy confirmed eosinophilic myocarditis. High-dose corticosteroids an

BACKGROUND: Loeffler endocarditis is a late manifestation of hypereosinophilic syndromes characterized by endomyocardial fibrosis and intracardiac thrombosis. Early recognition is essential yet challenging because presentation is often nonspecific.
CASE SUMMARY: Case 1 involved a 43-year-old man with long-standing atopy and persistent eosinophilia who presented with heart failure symptoms. Echocardiography demonstrated biventricular apical thrombi, and cardiac magnetic resonance confirmed ventricular thrombi and revealed diffuse subendocardial late gadolinium enhancement confirming fibrotic-s

The pathophysiology of organ involvement in eosinophilic granulomatosis with polyangiitis is characterised by vasculitis and eosinophilic inflammation, which can be accompanied by eosinophil extracellular trap cell death (EETosis). Clinically, eosinophilic inflammation is often observed in the lungs, gastrointestinal tract, and heart, whereas vasculitis more frequently affects the peripheral nerves, skin, and kidneys. However, histopathological confirmation of vasculitis and EETosis in different organs within the same patient has rarely been reported. We report a patient with EGPA presenting w

OBJECTIVE: In eosinophilic diseases, myocardial involvement critically impacts prognosis. Given the rarity and heterogeneity of eosinophilic myocarditis (EM), diagnosis and risk-adapted treatment remain challenging, and longitudinal data on cardiac function are scarce. Therefore, this study investigated the course of cardiac function and aimed to identify prognostic biomarkers in a real-world EM cohort across inflammatory disorders.
METHODS: Clinical, laboratory, and imaging data of all patients with eosinophilic myocarditis treated at Ludwig-Maximilians-University Hospital in Munich between

We present the case of a 58-year-old man with negative T waves in the inferior and lateral leads observed on a routine ECG in the context of persistent peripheral eosinophilia, 6 months after an intestinal parasitic infection. The diagnostic workup excluded coronary artery disease and led to a diagnosis of endomyocardial fibrosis with apical thrombosis. Endomyocardial fibrosis, also known as Loeffler endocarditis, represents the cardiac involvement of the hypereosinophilic syndrome, resulting in a cardiomyopathy with a restrictive phenotype. After initiating specific therapy, further investiga

Parasitic infections are an underrecognized but important cause of cardiovascular disease, primarily in endemic regions and increasingly in non-endemic settings as a consequence of global migration and travel. Cardiac involvement may result from direct parasitic invasion or, more commonly, from immune-mediated and inflammatory mechanisms, with predominant effects on the myocardium and pericardium. Clinical manifestations include acute myocarditis, dilated or restrictive cardiomyopathy, pericardial effusion, tamponade, constrictive pericarditis, and occasionally cystic lesions identified on car

BACKGROUND: Loeffler's endomyocarditis, the cardiac manifestation of hypereosinophilic syndrome, is characterized by eosinophilic myocardial infiltration and thrombus formation leading to restrictive cardiomyopathy.
METHODS: In a retrospective single-center registry, patients diagnosed with Loeffler's endomyocarditis between 2017 and 2025 were included. Clinical data, biomarkers, echocardiography, cardiac magnetic resonance imaging (CMR), and endomyocardial biopsies (n = 8) were analyzed.
RESULTS: Eighteen patients were included. Hierarchical clustering identified two patterns: an acute infl

Secondary plasma cell leukemia (sPCL) represents one of the most aggressive forms of Multiple Myeloma (MM) progression and remains associated with poor outcomes despite the availability of novel immunotherapeutic approaches. Severe hypereosinophilia is exceptionally rare in plasma cell neoplasms and may occasionally occur as a paraneoplastic manifestation of aggressive disease biology. In selected cases, eosinophilic infiltration can result in life-threatening organ damage, particularly involving the cardiovascular system. We report the case of a 40-year-old woman with IgG lambda MM who achiev

Hypereosinophilic syndrome (HES) is defined by marked eosinophilia accompanied by eosinophil-mediated organ injury. Cardiac involvement can progress from endomyocardial inflammation to mural thrombosis and fibrosis, with systemic embolization. A 58-year-old man presented with recurrent chest distress and back pain. Laboratory testing showed an absolute eosinophil count of 2.01 × 10⁹/L, cardiac troponin I of 0.25 ng/mL, N-terminal pro-B-type natriuretic peptide of 3,272 pg/mL, and D-dimer of 1.12 mg/L. Coronary angiography excluded fixed obstructive coronary disease and demonstrated dynamic sys

BACKGROUND: No large registries of patients with acute eosinophilic myocarditis (EM) are available. However, EM is perceived as a cardiac disease with high mortality, affecting mainly young and middle-aged adults according to small series and case reports. Awareness of the clinical presentation, associated systemic conditions, treatments, and outcomes of this uncommon condition is an unmet need.
METHODS: In this international, multicenter, retrospective cohort study, 53 centers screened 193 patients with histologically proven acute EM between 1992 and 2023. After the exclusion of patients wit

Myocarditis and inflammatory cardiomyopathy are inflammatory diseases of the heart muscle that can have both infectious and non-infectious causes. They can be caused by an unresolved viral infection or other infection, or they can be autoimmune, toxic, or allergic in nature. The specific identification of the pathogen and/or confirmation of inflammation can only be achieved through direct tissue analysis using endomyocardial biopsy (EMB), as neither detection of the virus nor assessment of the quality and intensity of the inflammation is possible using non-invasive methods. Accordingly, the re

BACKGROUND: Eosinophilic myocarditis has heterogeneous presentations and may cause fulminant cardiogenic shock.
CASE SUMMARY: A 35-year-old man with ulcerative colitis and eosinophilia developed cardiogenic shock after sequential biologic therapy (most recently vedolizumab) and corticosteroid withdrawal. He rapidly deteriorated with severe biventricular dysfunction (left ventricular ejection fraction 10%) and hypereosinophilia (eosinophil count 2,154/μL). Coronary angiography showed no obstructive disease, and cardiac magnetic resonance suggested diffuse myocardial involvement. Endomyocardial
