In Vitro Models to Study Fibroelastic Remodeling of the Endocardium in Congenital Heart Disease.

Congenital heart defects (CHD) are characterized by pathological intracardiac blood flow, exposing the endocardium to disturbed shear stress. This contributes to the development of endocardial fibroelastosis (EFE), a restrictive fibrotic remodeling process associated with diastolic dysfunction. Despite the pathomechanistic role of the endocardium in CHD, EFE-derived endocardial endothelial cells (EdEECs) remain largely understudied. No standardized protocol exists for isolating and purifying them from human pediatric tissue. Tissue from N = 15 pediatric patients with (1) hypoplastic left heart

Atrial Cardiomyopathy Through the Pathologist's Lens: From Tissue Remodeling to Clinical Expression.

Atrial cardiomyopathy (ACM) provides a useful framework for understanding structural, architectural, contractile, and electrophysiological disease of the atria. ACM and atrial fibrillation (AF) are closely linked but are not interchangeable: ACM favors arrhythmogenesis, whereas sustained AF can accelerate atrial injury. This review considers ACM from the pathologist's perspective, encompassing primary atrial disease and secondary remodeling associated with valvular disease and classical cardiomyopathies. The histologic spectrum includes cardiomyocyte hypertrophy and myocytolysis, interstitial

Endocardial thickening in dilated cardiomyopathy associated with FHL2 variant: a case report.

BACKGROUND: FHL2 is critical for cardiac development, and its variants are associated with early-onset severe dilated cardiomyopathy (DCM), though such reports remain rare. CASE PRESENTATION: The patient was a 30-day-old male infant admitted with a 3-day history of cough, a 2-day history of poor feeding, and lethargy lasting half a day. Echocardiography revealed left ventricular enlargement, with a minimum left ventricular ejection fraction (LVEF) of only 13%, and endocardial thickening. Genetic testing identified a heterozygous missense variant c.391C>T (p.Arg131Cys) in FHL2. This variant wa

Primary biventricular repair in neonates with borderline left ventricle.

BACKGROUND: Borderline left ventricle (LV) represents a spectrum of defects, from mild to severe left heart hypoplasia, often with other cardiac anomalies. Decision-making regarding primary biventricular (BiV) repair is challenging. We sought to identify risk factors associated with adverse outcome after neonatal primary BiV repair for borderline LV. METHODS: A retrospective review was undertaken of neonatal primary BiV repair for borderline LV between 2001 and 2022. The primary outcome was composite of death, transplant, conversion to single-ventricle circulation, pulmonary vascular resistan

[Analysis of optimizing indications for fetal pulmonary valvuloplasty based on a multi-parameter scoring system].

Objective: To establish a multi-parametric scoring system for predicting postnatal circulatory outcomes in fetuses with critical pulmonary stenosis (CPS) and pulmonary atresia (PA) with intact ventricular septum (IVS), in order to optimize the indications for fetal pulmonary valvuloplasty (FPV). Methods: This was a retrospective cohort study. A total of 96 fetuses diagnosed with CPS-IVS or PA-IVS at Women and Children's Hospital, Qingdao University between July 2018 and June 2021 were included. Fetuses who did not undergo FPV constituted the modeling cohort, while those who underwent FPV const

Severe Aortic Regurgitation with New Congestive Heart Failure Following Fetal Aortic Valvuloplasty - A Case Report.

Fetal aortic valvuloplasty (FAV) is performed in fetuses with critical aortic stenosis (CAS) to prevent progression to hypoplastic left heart syndrome (HLHS). While successful FAV often results in new-onset aortic regurgitation (AR), this typically improves during gestation. We present a rare case of persistent severe AR following FAV, complicated by fetal congestive heart failure. A 28-year-old woman was referred at 23 + 0 weeks of gestation due to CAS of the fetus. Fetal echocardiography showed CAS with a peak velocity of 2.79 m/s, significant LV dysfunction and grade 2–3 endocardial fibroel

Fibroelastic Remodelling of the Endocardium on the Right Side of the Heart: Endothelial-to-Mesenchymal Transition in Pulmonary Atresia With Intact Ventricular Septum.

OBJECTIVES: Patients with pulmonary atresia/critical pulmonary stenosis with intact ventricular septum (PA/cPS-IVS) show fibrous subendocardial tissue of unclear origin accompanied by right ventricular (RV) hypoplasia. While in hypoplastic left heart syndrome, it is known that flow-induced endothelial-to-mesenchymal transition (EndMT) of endocardial endothelial cells (EECs) is the source of this fibrous tissue, it remains unclear whether similar mechanisms exist in PA/cPS-IVS. METHODS: We analysed 13 PA/cPS-IVS patients who underwent staged ventricular rehabilitation surgery aimed at preservi

Left ventricular rehabilitation after Ross operation for critical aortic stenosis with endocardial fibroelastosis.

OBJECTIVES: Congenital critical aortic stenosis is frequently accompanied by endocardial fibroelastosis (EFE), impairing diastolic compliance and long-term ventricular performance. An early Ross procedure, with or without concomitant EFE resection may restore left ventricular (LV) function; however, long-term functional outcomes remain poorly characterized. We evaluated long-term clinical and functional outcomes after Ross procedure in neonates and infants with and without EFE. METHODS: Patients who underwent a Ross operation at <1 year of age between January 2005 and January 2025 were includ

Endocardial Fibroelastosis as an Early and Isolated Ultrasound Marker of Anti-Ro/SSA(B) Antibodies in the Fetus.

A rare case of isolated fetal endocardial fibroelastosis associated with anti-Ro/SSA and SSB antibodies is presented. The only abnormal ultrasound finding was excessive brightness of the endocardium, particularly at the level of the crux cordi, the atrioventricular plane, and the outflow tracts, in the presence of normal myocardial contractility and normal function of the semilunar and atrioventricular valves.

Echogenicities in the Fetal Heart: Echocardiographic Evaluation, Differential Diagnosis and Clinical Outcomes.

Intracardiac echogenicity during routine obstetric ultrasound may represent a benign finding or may indicate underlying pathology, from genetic syndromes to critical structural heart disease. Distinguishing benign intracardiac echogenic foci from pathology has important implications for parental counselling and decision-making. This review synthesizes current knowledge of the various etiologies of intracardiac echogenicity and outlines prognostic considerations to help guide clinical management.

Left Ventricular Pacing-Associated Cardiomyopathy Revealing Myocardial Vulnerability in Congenital Complete Heart Block.

BACKGROUND: Mechanisms underlying cardiac manifestations of immune-mediated congenital complete heart block (CCHB) remain unclear. Dilated cardiomyopathy (DCM) in immune-mediated CCHB has been associated with RV pacing, whereas reports of LV pacing-associated DCM are lacking. CASE SUMMARY: A girl with immune-mediated CCHB showed endocardial fibroelastosis (EFE) at 20 weeks of gestation before heart block developed at 22 weeks. After LV basal-lateral pacing, she developed DCM with LV dyssynchrony. Cardiac resynchronization therapy (CRT) improved LV function and induced reverse remodeling. CON

An ALMS1 variant disrupts proximal centriole organization and promotes a myofibroblast-like phenotype that is regulated by THY1.

Primary endocardial fibroelastosis (pEFE) is characterized by accumulation of elastin-rich extracellular matrix (ECM) in the left ventricular endocardium in the absence of structural defects, but its pathophysiology remains unclear. We investigated dermal fibroblasts from a pEFE patient (proband) harboring a loss-of-function ALMS1 variant. Compared with control fibroblasts, proband cells displayed a myofibroblast-like phenotype, with increased ECM proteins and markers of fibroblast activation and endothelial-to-mesenchymal transition. Conversely, the thymus cell surface antigen (THY1) was down

Is There a Unified Etiology of Hypoplastic Left Heart Syndrome? Evaluating Genetic, Structural, and Hemodynamic Models of Disease Initiation.

Background: Hypoplastic left heart syndrome (HLHS) is defined as "a spectrum of congenital cardiovascular malformations with normally aligned great arteries without a common atrioventricular junction, characterized by underdevelopment of the left heart with significant hypoplasia of the left ventricle including atresia, stenosis, or hypoplasia of the aortic or mitral valve, or both valves, and hypoplasia of the ascending aorta and aortic arch". Without treatment, HLHS is usually lethal in the neonate. Many hypotheses have been advanced to explain the etiology of HLHS; however, no single theory

An infant case of double-chambered left ventricle with extensive non-compaction revealed by intraoperative endoscopy.

UNLABELLED: Coexistence of double-chambered left ventricle (DCLV) and left ventricular non-compaction (LVNC) is exceptionally rare and may present diagnostic challenges. We report a case of an 8-month-old girl who developed severe heart failure due to the combination of these two conditions. Transthoracic echocardiography revealed left ventricular dilatation and hypokinesia, consistent with a dilated cardiomyopathy-like phenotype, together with a septum-like structure dividing the cavity and swirling flow, raising suspicion of DCLV, although hyper-trabeculation could not be fully assessed. Assessment of right ventricular endocardial fibroelastosis in fetuses with critical pulmonary stenosis and pulmonary atresia with intact ventricular septum.

BACKGROUND: This study aimed to assess right ventricular (RV) endocardial fibroelastosis (EFE) in fetuses with critical pulmonary stenosis (CPS) and pulmonary atresia with intact ventricular septum (PA-IVS) and to investigate the implications of RV EFE for circulatory outcomes. METHODS: Fetal echocardiographic data from July 2018 to January 2021 were collected. Three reviewers independently graded EFE based on the presence and extent of endocardial echogenicity. Since this is a novel study on grading RV EFE, intra- and interobserver comparisons were performed. The associations among EFE sever

Usefulness of serial N-terminal pro-B-type natriuretic peptide values after biventricular repair in patients with borderline hypoplastic left ventricle.

OBJECTIVES: Biventricular repair (BVR) for patients with borderline hypoplastic left ventricle is challenging, and a predictor of failing BVR has not been clarified. This study aimed to analyse the z-log N-terminal pro-B-type natriuretic peptide and evaluate its usefulness in predicting outcomes after BVR. METHODS: Patients who were diagnosed with borderline left heart hypoplasia and underwent BVR from 2012 to 2022 were included. Serial N-terminal pro-B-type natriuretic peptide values were evaluated using its age-adjusted z-score. The data were collected from the first admission to the last f

Eliminating left ventricular outlet stenosis lowers the risk for endocardial fibroelastosis recurrence†.

OBJECTIVES: Patients with endocardial fibroelastosis (EFE) in the setting of congenital critical aortic valve (AoV) stenosis and left ventricular outflow tract obstruction (LVOTO) are at risk for diastolic dysfunction, limiting biventricular circulation. EFE resection is the only available treatment option, but frequently recurs requiring re-resections. We aimed to investigate whether augmentation of a left ventricular outlet stenosis (AoV stenosis ± LVOTO) with a Ross/Ross-Konno procedure prevents EFE recurrence. METHODS: Patients born with AoV stenosis ± LVOTO and treated with primary left
