The Therapeutic Potential of Berberine in Treating Diabetes and Its Complications: Molecular Mechanisms and Future Perspectives.

Diabetes mellitus (DM), a chronic metabolic disorder characterized by persistent hyperglycemia, poses a significant global health burden. Berberine (BBR), a natural isoquinoline alkaloid derived from medicinal plants such as Coptidis Rhizoma, has shown therapeutic promise for DM and its complications. Preclinical and clinical studies have revealed that BBR exerts multi-target effects, including remodeling the gut microbiome, promoting insulin secretion, ameliorating insulin resistance, and inhibiting hepatic gluconeogenesis. Its protective mechanisms against diabetic complications, such as nep

Impact of amyloid cardiomyopathy diagnosis on heart failure readmission and length of stay.

BACKGROUND: Amyloid cardiomyopathy (CM) portends poor outcomes in heart failure (HF). Whether establishing a diagnosis of amyloid CM influences healthcare utilization remains unclear. METHODS: We identified patients admitted with a primary diagnosis of HF between January 2018 and December 2020, using medical record-linked administrative data. Amyloid CM was diagnosed using technetium-99m pyrophosphate scintigraphy and assessment for paraproteinemia, with endomyocardial biopsy as needed. Annualized HF readmission rates and length of stay (LOS) were compared between patients with confirmed amyl

Cyclic azapeptide CD36 ligand attenuates cardiac injury and reduces long-chain fatty acid accumulation after myocardial ischemia-reperfusion in mice.

Ischemic heart disease remains a leading global cause of death. We investigated the cardioprotective effects of the selective cluster of differentiation-36 receptor (CD36) modulator azapeptide MPE-298 in a mouse model of myocardial ischemia-reperfusion. Given before reperfusion, a single intravenous dose of azapeptide MPE-298 reduced infarct size by 44% of the area-at-risk and transiently decreased left ventricular long-chain fatty acids (LCFA) accumulation, independently of saturation status. Metabolomic profiling identified changes in amino acids that may fuel the tricarboxylic acid cycle an

Clinical consensus guidelines for the management of Noonan syndrome in Japan.

Noonan syndrome (NS) is a multisystem RASopathy marked by characteristic facies, congenital heart disease, short stature, and variable neurodevelopmental and systemic complications. Although molecular and clinical knowledge has advanced, evidence-based clinical guidance in Japan has been limited. A multidisciplinary committee in pediatric endocrinology, clinical genetics, and pediatric cardiology developed consensus guidelines on diagnosis, comorbidity assessment, treatment, and transitional care. Clinical questions were defined, and the literature published through March 2024 was systematical

Altered Acylcarnitine Metabolism in cardiac diseases: From molecular mechanisms to translational potential.

Cardiovascular diseases (CVDs) are a leading global cause of mortality and disease burden, closely associated with the remodeling of myocardial energy metabolism. Acylcarnitines (Acs) are key intermediates in the carnitine shuttle and fatty acid β-oxidation (FAO) processes. They play a critical role in transporting long-chain fatty acids across the mitochondrial membrane, maintaining the intracellular acyl-CoA/CoA balance, and buffering metabolic flux. Recent metabolomics studies have demonstrated significant abnormalities in acylcarnitine profiles across various cardiac diseases, including co

Current and emerging treatment options for transthyretin amyloid cardiomyopathy.

Transthyretin amyloidosis (ATTR) is a condition caused by TTR protein misfolding and amyloid deposition, particularly in the heart and nervous system, leading to organ dysfunction. Advances in therapeutic strategies have revolutionised the management of ATTR amyloidosis. Treatments available in clinical practice include TTR stabilisers (tafamidis and acoramidis), which prevent the dissociation of TTR tetramer into monomers and oligomers that subsequently form amyloid fibrils, and gene-silencing therapies (patisiran, inotersen and vutrisiran), which suppress the hepatic synthesis of TTR, which

Additive cardioprotection by dapagliflozin and pioglitazone co-therapy in streptozotocin-induced diabetic rats.

OBJECTIVES: In diabetes mellitus, inflammation and apoptosis contribute to cardiovascular injury and oxidative damage. Dapagliflozin and Pio, beyond their hypoglycemic effects, mitigate organ damage. This study explored whether their combination provides enhanced protection against cardiac damage in streptozotocin-induced diabetic rats. METHODS: Wistar rats were divided into control, diabetic (STZ, 55 mg/kg, i.p.), dapagliflozin-treated (10 mg/kg, chow), pioglitazone-treated (12 mg/kg, chow), and combination groups. After six weeks, systolic blood pressure, glucose, and lipids were assessed;

Transthyretin amyloid cardiomyopathy: from cause to novel treatments.

Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive disorder marked by amyloid deposition in the heart, ultimately impairing cardiac function. Recent treatment advances have paralleled an evolving understanding of ATTR-CM pathophysiology. One emerging hypothesis suggests that an imbalance between in vivo amyloid production and natural clearance may drive disease progression, a conceptual framework that still requires validation. Transthyretin (TTR) stabilizers, such as tafamidis and acoramidis, mitigate amyloid formation by promoting the native tetrameric conformation of circulatin

Revisiting Congenital Atransferrinemia: A Rare but Treatable Cause of Pediatric Heart Failure.

Congenital atransferrinemia is an extremely rare autosomal recessive disorder characterized by functional or quantitative deficiency of transferrin. This leads to a characteristic clinical picture that includes heart failure due to iron overload cardiomyopathy, severe anemia, as well as hepatic and endocrine dysfunction in early infancy. Although rare or may be underdiagnosed, congenital atransferrinemia is a treatable cause of infantile heart failure and iron overload cardiomyopathy. Therefore, it is important to keep this diagnosis as a possibility in childhood presentation of heart failure

P-Selectin and Platelet-Monocyte Interaction in Inflammation: Mechanisms, Hypothesis, and Open Questions.

This review provides an integrated overview of how P-selectin-mediated platelet-monocyte interactions orchestrate key inflammatory processes in sterile and non-sterile pathological contexts. Inflammation presents a paradox: although it is a fundamental physiological process, uncontrolled activation can progress to a chronic state, contributing to the development of various inflammatory diseases. Platelets, previously considered to play a role exclusively in hemostasis, are now recognized as important immunomodulatory effectors capable of releasing inflammatory mediators and interact with leuko

Practical approach to the management of diabetic cardiomyopathy: From molecular pathways to treatment strategies.

Diabetic cardiomyopathy (DCM) has emerged as a major health challenge, further intensified by the rising global prevalence of type 2 diabetes mellitus. It plays a key role in the morbidity and mortality linked to diabetes, mainly through myocardial dysfunction that develops independently of coronary artery disease or hypertensive heart disease, and it is a major contributor to heart failure. Despite the significance of DCM, there is no gold standard diagnostic test; rather, a multimodal approach is required that includes clinical evaluation (risk factors, and symptoms and signs), cardiac imagi

Reparative effects of VCAM-1 high-performance MSC-derived exosomes on aged diabetic cardiomyocyte injury: a focus on ferroptosis suppression.

Cardiac dysfunction in elderly diabetes, due to superimposition of age-related myocardial senescence and diabetes-induced injury, lacks effective therapeutic strategies. Ferroptosis may be a key mechanism underlying cardiomyocyte injury in diabetic cardiomyopathy. Mesenchymal stem cells (MSCs) and their exosomes show potential for repairing cardiomyocytes, restoring cardiac function, improving insulin sensitivity, and mitigating diabetes-related complications, but their mechanisms and relationship with ferroptosis remain unclear. The present study aimed to investigate reparative effects and fe

Pathological mechanisms and treatment strategies for immune checkpoint inhibitor-associated myocarditis: insights from single-cell sequencing.

Immune checkpoint inhibitors have significantly advanced cancer treatment, but they can lead to immune-related adverse events. Among these, immune checkpoint inhibitor-associated myocarditis (ICI-M) represents a rare yet severe cardiotoxicity. The underlying pathogenesis of ICI-M is not fully elucidated, necessitating deeper mechanistic investigation. Traditional bulk sequencing technology measures average gene expression across entire cell populations, which affects the interpretation of gene function in complex tissues. In contrast, single-cell sequencing technology represents a transformati
