Current treatment decisions in cardiac transthyretin amyloidosis: a multicentre analysis.

BACKGROUND: The efficacy of transthyretin stabilisation in cardiac transthyretin amyloidosis (ATTR-CM) has been demonstrated in a clinical trial setting, but little is known about treatment decision-making in the real world. Particularly, initiating or discontinuing specific therapy is challenging in early and advanced disease. We evaluated current decision pathways for tafamidis in ATTR-CM. METHODS: This multicentre retrospective study included consecutive patients from 15 tertiary centres in Germany in whom ATTR-CM was newly diagnosed between January and June 2024, as well as patients, in w

Transthyretin Amyloid Cardiomyopathy: A Rapidly Evolving Landscape.

Transthyretin amyloid cardiomyopathy (ATTR-CM) has emerged as a significant yet underdiagnosed cause of heart failure. Advancements in the noninvasive imaging that facilitates diagnosis at an earlier stage of disease and treatment have marked a paradigm shift in the management of this previously fatal disease. This review outlines the epidemiology, pathophysiology, diagnosis, and treatment of ATTR-CM, with an emphasis on the recent developments transforming the field, including an ability to diagnose most patients without a biopsy, the shift toward screening or active ascertainment, the develo

Multimodal Diagnosis of Cardiac Amyloidosis: Integrating Imaging, Histochemistry, and Proteomics of Precise Typing.

Amyloidosis is a group of disorders caused by extracellular deposition of insoluble fibrillar proteins, leading to progressive organ dysfunction. Cardiac amyloidosis is clinically significant, as myocardial infiltration results in restrictive cardiomyopathy, arrhythmias, and heart failure. The main subtypes are light-chain (AL) and transthyretin (ATTR) amyloidosis, while AA and isolated atrial amyloidosis (IAA) are less common. Accurate subtype identification is crucial for management and prognosis. Diagnosis requires a multimodal approach combining imaging and tissue-based techniques. Echocar

Multiparametric CMR for detection of cardiac amyloidosis in patients with undifferentiated concentric left ventricular hypertrophy.

Multiparametric CMR can detect cardiac amyloidosis (CA) in patients with undifferentiated concentric left ventricular hypertrophy (LVH). A retrospective patient search was performed for patients ≥ 18 years of age who received 1.5T CMR from January 2018 to August 2022 to evaluate for myocardial infiltration in the setting of known concentric LVH at a tertiary medical center. Clinical records were reviewed for positive diagnosis or exclusion of CA. CMR were post-processed to evaluate ventricular volumes and function, native T1 mapping, and extracellular volume (ECV). Late gadolinium enhancement

Acoramidis in transthyretin amyloid cardiomyopathy: expanding evidence from ATTRibute-CM.

Transthyretin (TTR) amyloid cardiomyopathy (ATTR-CM) is a progressive, often fatal disease. TTR stabilizers bind directly to TTR, inhibiting tetramer dissociation and the resulting amyloidogenic process. This comprehensive review synthesizes clinical outcomes data from the ATTRibute-CM study program, including primary analyses, prespecified sensitivity studies, and open-label extension (OLE) follow-up, to characterize the clinical profile of acoramidis, an oral TTR stabilizer approved for ATTR-CM treatment. In clinical trials, acoramidis demonstrated consistent clinical benefits, with statisti

Diagnosis of Cardiac Amyloidosis on Echocardiography Using Artificial Intelligence.

BACKGROUND: Diagnosing cardiac amyloidosis (CA) on echocardiography can be challenging due to the imaging overlap between CA and more prevalent causes of a hypertrophic phenotype. This study sought to (1) evaluate the performance of artificial-intelligence (AI) derived measurements incorporated into the established multiparametric echocardiographic scoring system to detect CA; (2) develop and validate an AI-based deep-learning model for video-based detection of CA on echocardiography. METHODS: The study population comprised 5776 patients (CA, 2756; controls, 3020). The training data set inclu

Pyrophosphate Scintigraphy Changes After Tafamidis Therapy - Proposed Novel Index From Quantitative Single-Photon Emission Computed Tomography.

BACKGROUND: Because a disease-modifying therapy is now available, bone scintigraphy plays a crucial role in diagnosing transthyretin cardiac amyloidosis (ATTR-CM). METHODS AND RESULTS: We retrospectively analyzed 24 patients with ATTR-CM to determine the amyloid accumulation volume (AAV: mean standardized uptake value×volume) of the left ventricular wall. AAV showed a significant decrease (591.1±426.0 vs. 213.8±201.9, P<0.0001) after tafamidis treatment, and correlated with troponin T (R=0.49, P=0.022). CONCLUSIONS: AAV provided a precise quantitative evaluation of the amyloid burden in ATTR

Aortic valve calcium score from computed tomography for signal localization by cardiac amyloid radionuclide imaging to predict aortic stenosis and assess prognosis in patients with transthyretin cardiomyopathy.

PURPOSE: Transthyretin cardiomyopathy (ATTR-CM) and aortic stenosis (AS) are frequently associated. The diagnosis of ATTR-CM can be made by performing a cardiac amyloid radionuclide imaging (CARI) with a computed tomography (CT) scan to localize the signal, which allows visualization aortic valve calcifications (AVC). The aim of this study was to determine whether AVC on the CT performed at the time of the CARI for the diagnosis of ATTR-CM were predictive of AS and had an impact on prognosis. METHODS: CT images of patients diagnosed with ATTR-CM by CARI were reviewed for the presence of AVC an

Identifying Subclinical Transthyretin Cardiac Amyloidosis in V142I TTR Carriers: Design, Rationale, and Methods of the VISTA (Variant Imaging of Subclinical Transthyretin Amyloidosis) Study.

BACKGROUND: Approximately 1.5 million of the 44 million individuals of African descent in the United States are carriers of the p.V142I variant in transthyretin (TTR). This is the most common cause of variant amyloid cardiomyopathy (ATTR-CM) worldwide which leads to heart failure (HF) and premature death. Despite promising new treatments, ATTR-CM is often diagnosed at an advanced stage and conventional diagnostic tools lack specificity to detect early disease. Thus, the overall objectives of the Variant Imaging of Subclinical Transthyretin Amyloidosis (VISTA) study are to determine imaging and

How to monitor disease progression in ATTR amyloid cardiomyopathy: Implications for clinical practice and trial design.

Transthyretin amyloid cardiomyopathy (ATTR-CM) has been traditionally considered a rare and inexorably fatal condition. However, the development of therapies able to slow or halt ATTR-CM progression and increase survival have transformed the management of this condition. As these treatments become more accessible, the need for clinical indicators of disease progression has become increasingly important to guide clinical decision-making and personalise treatment strategies. Changes in widely available parameters have been shown to track disease evolution, which include worsening heart failure s

Hyperglycemia-induced SIRT3 inhibition contributes to diabetic cardiomyopathy by impairing Cav-3-mediated mitophagy.

Diabetic cardiomyopathy (DCM) is a major cause of mortality in diabetic patients, with impaired mitophagy contributing its pathogenesis. Sirtuin 3 (SIRT3) and caveolin-3 (Cav-3) are protective proteins involved in mitophagy, although their precise mechanisms remain unclear. This study investigated the interplay between SIRT3, Cav-3, and mitophagy in DCM. We found that the diabetic C57BL/6 mice exhibited impaired cardiac structure and function, accompanied by reduced mitophagy and decreased expression of SIRT3 and Cav-3. Cav-3 KO mice with diabetes showed further worsened cardiac dysfunction an
