Mitochondrial iron overload is associated with lysosomal dysfunction-mediated mitophagy impairment in the heart of Friedreich's ataxia.

Friedreich's ataxia (FRDA) is a rare disease caused by deficiency of frataxin, a mitochondrial protein essential for iron-sulfur cluster assembly and iron homeostasis. In addition to neurological symptoms, cardiac dysfunction is common and represents a major cause of premature death in FRDA. Although iron overload has been suggested as a major player for FRDA-related cardiomyopathy, its underlying mechanisms remain unclear. Using heart-specific frataxin deficient mice, we observed that FRDA-related cardiac hypertrophy is accompanied by mitochondrial iron overload. Transmission electron microsc

Dietary regulation of peroxisome proliferator-activated receptors (α and γ) and IL-1R-associated kinase proteins in experimental Trypanosoma cruzi infection: Insights into one health.

Chagasic cardiomyopathy, caused by Trypanosoma cruzi, is influenced by host-parasite genetics and environmental factors such as diet. High-fat diets can intensify inflammation via pathways involving IRAK-1 and PPARs (α/γ). This study examined the impact of monounsaturated (olive oil) and saturated (lard) fat-rich diets on the expression of IRAK-1, PPAR-α, and PPAR-γ in cardiac and adipose tissues of mice infected with the Colombian strain of T. cruzi. Mice received control, olive oil, or lard diets during the experiment, and the infection was evaluated for 30 or 100 days. Infection increased I

The Ketogenic Diet in the Neonatal Intensive Care Setting: The Case of a Preterm Newborn With Mitochondrial DNA Depletion Syndrome Type 13 (MTDPS13).

BACKGROUND: Mitochondrial DNA depletion syndrome 13 (MTDPS13) is an autosomal recessive disorder presenting in early infancy with encephalopathy, hypotonia, lactic acidosis, and severe global developmental delay. Patient-derived cells typically exhibit impaired mitochondrial oxidative phosphorylation and a marked reduction in mitochondrial DNA (mtDNA) copy number. CASE REPORT: We report the case of a male preterm neonate born at 31 + 3 weeks of gestation following a pregnancy marked by severe polyhydramnios. At birth, his weight was 1400 g. Physical examination revealed dysmorphic features, r

Leptin Activates Brain-BAT-Heart Crosstalk to Promote Cardiac Protection.

BACKGROUND: Cardiovascular disease remains the leading cause of death worldwide, with coronary artery disease being the primary contributor. Our recent studies suggest that activation of LepRs (leptin receptors) in the brain can improve cardiac function after myocardial infarction. However, the mechanism by which this cardioprotective effect is transmitted from the brain to the heart remains unclear. We hypothesize that brain LepR activation stimulates brown adipose tissue (BAT) to secrete extracellular vesicles (EVs) enriched with cardioprotective factors. These EVs may safeguard the heart by

Dapagliflozin ameliorates myocardial ischemia/reperfusion injury by modulating EGFR signaling and targeting NCOA4-mediated ferritinophagy.

BACKGROUND: SGLT2 inhibitor dapagliflozin (Dapa) has gained increasing attention in the treatment of myocardial ischemia-reperfusion injury (IRI). However, the mechanism of action of the cardiovascular benefits of Dapa is unclear. The present study aimed to investigate the effects of Dapa on myocardial IRI and the underlying molecular mechanisms. METHODS: The effects of Dapa on myocardial IRI were investigated using the in vitro perfusion Langendorf model and the in vitro hypoxia/reoxygenation (H/R) cell model. Histological changes, myocardial enzymes, oxidative stress and mitochondrial struc

Leveraging the shared and opposing genetic mechanisms in the heritable cardiomyopathies.

Dilated cardiomyopathy (DCM) and hypertrophic cardiomyopathy (HCM) are heart muscle diseases with largely opposing structural and functional phenotypes. Yet, both may lead to the same devastating outcomes of advanced heart failure and life-threatening arrhythmias. Using genome-wide association data from 9,365 DCM cases, 5,900 HCM cases, and over 1.2 million controls, we show that DCM and HCM are largely inversely associated across multiple genomic levels. Modeling both disorders as opposing genetic entities, in case-case GWAS approaches, we identify 100 loci (17 novel) underlying the cardiomyo

Redefining optimal medical therapy for heart failure in amyloid transthyretin cardiomyopathy.

Amyloid transthyretin cardiomyopathy (ATTR-CM) is an infiltrative cardiomyopathy and an underdiagnosed cause of heart failure (HF) across the left ventricular ejection fraction spectrum. As disease-modifying therapies prolong survival, outcomes are increasingly shaped by non-amyloid-specific HF care. Restrictive physiology and frequent autonomic dysfunction create a narrow therapeutic window in which small changes in preload or afterload may precipitate hypotension, renal injury, and low-output symptoms. Loop diuretics remain foundational, requiring individualized dosing, preference for agents

Tissue characterization in cardiac amyloidosis: a joint consensus document by the gruppo di studio di cardiopatologia (SIAPEC) and the SIC/ANMCO Italian cardiac amyloidosis network (RIAC).

Cardiac amyloidosis (CA) is an infiltrative cardiomyopathy caused by the accumulation of protein fibrils with an abnormal 3D configuration in the myocardium. The introduction of targeted therapies for the two most common forms, light-chain amyloidosis (AL) and transthyretin amyloidosis (ATTR), has made prompt recognition and accurate typing of the amyloid protein indispensable. Although in some ATTR cases a diagnosis can be reached with noninvasive imaging methods, diagnostic confirmation by peripheral or endomyocardial biopsy remains fundamental for many patients with ATTR and for all those w

Abnormal Lipid Signaling Characterizes Diastolic Dysfunction in Pediatric Cardiomyopathy.

Pediatric cardiomyopathy is a major cause of diastolic heart failure; yet, its mechanisms remain unclear. Global myocardial transcriptomic and blood lipidomic profiling revealed a distinct metabolic signature of diastolic dysfunction marked by dysregulated lipid signaling. A machine learning model using these gene markers accurately classified diastolic dysfunction across cardiomyopathy subtypes. Lipidomic changes included excess saturated lipids and impaired oxidation that correlated with myocardial gene expression. Induced pluripotent stem cell cardiomyocytes from patients with diastolic dys

Pregnancies in Women With Long-Chain Fatty Acid Oxidation Disorders: Results of a European and North American Survey.

Long-chain fatty acid oxidation disorders (lcFAODs) are genetic disorders of energy metabolism that are associated with a risk of metabolic decompensation, especially during catabolic episodes. With improvement in diagnostics and treatment, more women with lcFAODs now reach child-bearing age. So far, little is known about the risk and outcome of pregnancies, particularly in women with more severe forms of lcFAODs. We performed an international web-based survey among health care professionals involved in the care of individuals with lcFAODs and collected data on 89 pregnancies in 39 women (mild

Point-of-care Ultrasound (POCUS)-Guided Pragmatic Fluid and Albumin Resuscitation and Hemodynamic Monitoring in Cirrhosis and Septic Shock.

BACKGROUND & AIMS: Fluid resuscitation strategy in cirrhosis and septic shock remains unclear, especially in the setting of cirrhotic cardiomyopathy (CCM). In a single-center propensity-matched analysis (discovery cohort; group 1) and in a subsequent multicenter validation cohort (group 2), we compared two pragmatic point-of-care ultrasound (POCUS)-guided fluid resuscitation strategies in cirrhosis and septic shock. Patients received a crystalloid challenge to assess fluid responsiveness, followed by either balanced salt solution (BSS) with 20% albumin vs. BSS alone. Outcomes of interest were

Hypertrophic cardiomyopathy as a novel phenotypic feature of NSUN3-related mitochondrial disease: a case report with review of the literature.

OBJECTIVES: NSUN3 encodes a mitochondrial RNA methyltransferase responsible for cytosine methylation at the tRNA wobble base, which plays a role in the regulation of mitochondrial translation. Variants in NSUN3 lead to impaired oxidative phosphorylation resulting in multisystem involvement. Reported cases to date have described developmental delay, hypotonia, optic atrophy and neurological involvement. Cardiac manifestations, however, have not yet been clearly associated with NSUN3 deficiency. Here, we report hypertrophic cardiomyopathy (HCM) as a new clinical feature and the long-term follow-

Laminopathies: natural history and risk prediction of heart failure.

BACKGROUND AND AIMS: Patients with LMNA gene variants are at high risk for dilated cardiomyopathy and heart failure (HF), but no prediction model for severe HF events exists. This study aimed to describe the incidence of severe HF events and develop a prediction model in a large cohort of patients with adult-onset laminopathies. METHODS: From a population of 660 patients enrolled in the French LMNA nationwide registry, 470 adults were included in the derivation cohort. An independent international validation cohort included 245 additional patients. Baseline characteristics at genetic testing

Atrial fibrillation: genetic architecture and polygenic risk prediction.

Atrial fibrillation (AF) is a common arrhythmia associated with increased risk of stroke, heart failure and mortality. Advances in genomic research have revealed a complex genetic architecture underlying AF. Genome-wide association studies have identified hundreds of loci of common variants, while sequencing efforts have linked rare variants to cardiomyopathy-related pathways. Polygenic risk scores (PGS) offer a promising tool for AF risk stratification, demonstrating predictive value for disease onset, complications and perioperative outcomes. Multiancestry studies have improved the performan

Long-Term Follow-Up of Patients With Transaldolase Deficiency.

Transaldolase (TALDO) deficiency has a well-characterized phenotype. However, there are few large cohort studies, and little is known about the long term, including the need for organ transplantation. Our aim was to share a long multicenter experience in managing these patients. We retrospectively collected data on 16 patients followed for up to 20 years. Liver involvement was present in all patients, with six presenting liver failure at birth. During long-term follow-up, 10 patients developed cirrhosis and four underwent liver transplantation (LTx). Two patients died from severe liver failure

Multimodality Imaging Approach in the Diagnosis of Constrictive Pericarditis.

Constrictive pericarditis (CP) results from persistent or insufficiently controlled pericardial inflammation arising from diverse etiologies. It remains a challenging clinical condition, typically presenting with non-specific symptoms that demand a high degree of clinical suspicion and meticulous imaging assessment. As CP progressively impairs both left- and right-sided cardiac function, it can lead to overt heart failure and a marked decline in quality of life, making early recognition crucial. Multimodality imaging plays an essential role in the diagnostic and prognostic evaluation of CP, en
