Conduction system pacing in the Italian clinical practice: results of a nationwide survey.

BACKGROUND: Conduction system pacing (CSP) is an increasingly used approach for physiological ventricular pacing. We conducted a nationwide survey aimed to evaluate the adoption of CSP in the Italian clinical practice. METHODS: An online survey consisting of 20 questions, distributed to Italian centres performing cardiac pacing, was conducted from October to November 2024. RESULTS: A total of 136 centres took part in the survey (37% of all arrhythmia centres operating in Italy), 119 of which (86%) declared to perform CSP implants. The lack of trained operators was the main perceived barrier

Physiologic Pacing in Heart Failure.

Cardiac physiologic pacing, also known as cardiac resynchronization therapy, is indicated in patients with heart failure, reduced left ventricular ejection fraction (LVEF) of 50% or less, and either a high (or anticipated high) ventricular pacing burden or a wide QRS complex. Traditionally, physiologic pacing has been achieved with biventricular pacing with a right ventricular lead and a coronary sinus branch lead. Randomized trials involving more than 10,000 patients with heart failure have shown clinical, exercise, and quality-of-life benefits associated with biventricular pacing, as well as

Association between remnant cholesterol and cardiac conduction block: A prospective cohort study.

BACKGROUND: Elevated remnant cholesterol (RC) is recognized as a risk factor for atherosclerotic cardiovascular disease. However, its association with cardiac conduction block (CCB) remains unclear. OBJECTIVE: This study aimed to investigate the relationship between serially measured RC levels and incident CCB. METHODS: This study used data from the Kailuan study. RC level was measured at baseline in 2006 and at biennial follow-up visits. Cumulative average RC levels were calculated using all available RC measurements before incident cases of CCB or the end of follow-up (December 31, 2019).

Transaortic and Right Atrial Approach for Pediatric Tunnel-Type Left Ventricular Outflow Tract Obstruction: An Alternative to Modified Konno.

BACKGROUND: Tunnel-type left ventricular outflow tract obstruction (LVOTO) in children requires extensive resection of long-segment fibromuscular narrowing and is traditionally treated with Konno or modified Konno procedures, which involve right ventriculotomy and may risk right ventricular dysfunction and conduction injury. METHODS: Between 2012 and 2023, 26 children (mean ± SD age, 5.4 ± 1.3 years; 34.6% with prior septal myectomy) underwent repair of tunnel-type LVOTO using a combined transaortic and right atrial approach at 3 tertiary centers. The operation consisted of transaortic exposu

SGLT2 inhibitors and risk of new-onset left bundle branch block in heart failure with preserved ejection fraction.

BACKGROUND: Whether sodium-glucose cotransporter 2 inhibitors (SGLT2i) may reduce the risk of developing new-onset left bundle branch block (LBBB) in patients with heart failure with preserved ejection fraction (HFpEF) remains unknown. We investigate the association between SGLT2i use and incident LBBB in patients with HFpEF. METHODS: We conducted a cohort study using a prospectively database of patients with HFpEF. Patients receiving SGLT2i were propensity score-matched 1:1 with historical controls not receiving SGLT2i. The primary endpoint was new-onset LBBB, over a median 24-month follow-u

Conduction system pacing versus biventricular pacing in patients with heart failure and left bundle branch block: a systematic review and meta-analysis.

BACKGROUND: Heart failure (HF) with left bundle branch block (LBBB) is associated with adverse clinical outcomes. Although biventricular pacing (BVP) remains the standard therapy, up to 30% of patients show suboptimal response. Conduction system pacing (CSP), including His bundle pacing (HBP) and left bundle branch pacing (LBBP), provides more physiological ventricular activation, but comparative evidence remains limited. This study aimed to analyze available data comparing HBP or LBBP with BVP in patients with HF and LBBB, focusing on electrophysiological, echocardiographic, and clinical outc

Case Report: A case of heart failure recovery after non-ischemic cardiomyopathy following chimeric antigen receptor T-cell therapy.

INTRODUCTION: Chimeric Antigen Receptor T-cell (CAR-T) therapy has emerged as a highly promising immunotherapy for cancer treatment. Intensive research is being conducted focusing on adverse effects that CAR-T infusions may have due to the development of cytokine release syndrome (CRS). We aim to highlight a noteworthy case of ventricular tachycardia presented in the setting of acute non-ischemic cardiomyopathy following CRS after CAR-T therapy, along with providing a detailed discussion of subsequent inpatient and outpatient management. CASE PRESENTATION: A 78-year-old man with a past medica

Case Report: Exercise-induced normalization of chronic left bundle branch block: a rare recovery-phase phenomenon.

BACKGROUND: Transient left bundle branch block (LBBB) is associated with various cardiac and systemic conditions, including exercise-induced ischemia. However, normalization of baseline LBBB following exertion is a rare and poorly understood phenomenon. CASE SUMMARY: We report the case of an 83-year-old male with longstanding coronary artery disease and baseline LBBB who underwent a routine stress echocardiogram. During the recovery phase, ECG monitoring revealed normalization of LBBB with first-degree AV block. Concurrent stress echocardiography showed improved segmental and global ventricul

Transient ischemic attack and acute ischemic stroke: evidence of altered corrected index of cardiac electrophysiological balance.

BACKGROUND: Several electrocardiogram (ECG) abnormalities have been reported in patients with acute cerebral ischemic events. The index of cardiac electrophysiological balance (iCEB) and corrected index of cardiac electrophysiological balance (iCEBc) are potential non-invasive markers for arrhythmogenesis. Therefore, Chinese patients diagnosed with transient ischemic attack (TIA) or acute ischemic stroke (AIS) were included in this case-control study to explore the potential alterations in these markers and to determine the relationships between them and the National Institutes of Health Strok

Mechanical Dyssynchrony and Perfusion Heterogeneity Predict Adverse LV Remodeling in Patients with and without LBBB.

AIMS: Left bundle branch block (LBBB) is associated with mechanical dyssynchrony, heterogeneous perfusion, and adverse left ventricular (LV) remodeling. However, not all patients with LBBB develop cardiomyopathy, and dyssynchrony can occur without conduction defects. The role of microvascular dysfunction remains uncertain. We aimed to assess how mechanical dyssynchrony and perfusion heterogeneity relate to LV remodeling and function in patients with and without LBBB. METHODS AND RESULTS: We retrospectively analyzed 233 patients with isolated LBBB and 932 matched controls who underwent PET myo

Incidence and Predictors of Pacemaker Implantation Post-Transcatheter Aortic Valve Replacement in Patients with Pre-existing Right Bundle-Branch Block.

BACKGROUND: Pre-existing right bundle branch block (RBBB) is the leading predictor of permanent pacemaker implantation (PPI) following transcatheter aortic valve implantation (TAVI). The applicability of conventional PPI predictors in this high-risk subgroup remains unclear. This review evaluates PPI incidence and associated predictors in TAVI patients with pre-existing RBBB. METHODS: A systematic review of PubMed, Scopus, Embase, and Web of Science identified studies reporting PPI incidence and predictors in RBBB patients undergoing TAVI. Studies not addressing RBBB-specific factors were exc

Electrocardiographic Characteristics of Escape Rhythm During Complete Atrioventricular Block After Transcatheter Aortic Valve Replacement.

BACKGROUND: Complete atrioventricular block (CAVB) following transcatheter aortic valve replacement (TAVR) is primarily attributed to mechanical compression of the penetrating or branching portions of the His bundle, and less commonly, the atrioventricular (AV) node. This study aimed to characterize the electrocardiographic features of stable escape rhythms (ERs) occurring during CAVB after TAVR. METHODS: This retrospective study analyzed 12-lead electrocardiograms (ECGs) obtained at three time points: before TAVR (ECG 1), after TAVR but before CAVB (ECG 2), and during CAVB (ECG 3). The ERs o

Artificial Intelligence-Enabled 8-Channel ECG Diagnosing of Abnormalities with Wide QRS Complexes.

Background: There is a substantial number of research exploring the application of artificial intelligence (AI) in identifying electrocardiogram (ECG) abnormalities related to heart rhythm or conduction with the 12-channel format. However, there is a scarcity of studies focusing on refined differentiation of serials of ECG abnormalities with wide QRS complexes in a simplified channel format. Methods: We constructed an ECG dataset (standard 10-s, 12-channel format) from adult patients from Tongji Hospital of Huazhong University of Science and Technology, Wuhan, China. This dataset was consisted

Incidence and Predictors of Permanent Pacemaker Implantation After Balloon-Expandable TAVI: Applicability of the RITMO Score.

BACKGROUND: Permanent pacemaker implantation (PPMI) remains a common complication after transcatheter aortic valve implantation (TAVI). While the RITMO score-incorporating right bundle branch block (RBBB), membranous septum length (MSL), and aortic valve calcium load-was developed for self-expanding valves, its performance in balloon-expandable valves (BEVs) is uncertain. AIMS: To evaluate the incidence and predictors of PPMI following balloon-expandable TAVI and to assess the applicability of the RITMO score in this valve platform. MATERIALS AND METHODS: We retrospectively analyzed 310 pati

Case Report: A case of myocardial calcification combined with abnormal Q waves on electrocardiogram.

BACKGROUND: Myocardial infarction is the most common cause of pathological Q waves on electrocardiogram (ECG), but other conditions that cause myocardial injury or abnormal conduction (such as cardiomyopathy, myocarditis, cardiac tumors or amyloidosis, WPW, etc.) can also produce pathological Q waves. Whether myocardial calcification can lead to pathological Q waves remains uncertain. CASE SUMMARY: A 24-year-old man with a solitary kidney and multiple childhood abdominal surgeries had been on maintenance hemodialysis since 2017 (three times weekly), with a dialysis vintage of approximately 8
