[Correlation between right ventricular dyssynchrony and early ventricular dysfunction].

OBJECTIVE: To assess the association between right ventricular dyssynchrony and systolic dysfunction in patients with pulmonary hypertension, considering the distinct pathophysiological mechanisms from those affecting the left ventricle. METHOD: Prospective observational study including patients with pulmonary hypertension (groups 1, 3, 4, and 5). Right ventricular dyssynchrony was evaluated using speckle tracking, tissue Doppler imaging, and 3D echocardiography, measuring time-to-peak strain and systolic dyssynchrony index. RESULTS: A strong correlation was found between increased dyssynchr

Baseline right bundle branch block and clinical outcomes in patients undergoing transcatheter aortic valve implantation: a Danish nationwide cohort study.

AIMS: In the largest nationwide study of Danish patients undergoing transcatheter aortic valve implantation, we examined the clinical impact of pre-existing right bundle branch block - a known risk factor for permanent pacemaker implantation - on outcomes including pacemaker implantation, heart failure, and all-cause mortality. METHODS AND RESULTS: We included first-time transcatheter aortic valve implantation patients in Denmark from 2008 to 2021. Patients were stratified by baseline right bundle branch block status using the digital Danish Nationwide Electrocardiogram Cohort. The study outc

Longevity and Innovation: Cardiac Resynchronization Therapy in a 100-Year-Old Patient With Severe Heart Failure-A 4-Year Follow-Up.

Cardiac resynchronization therapy (CRT) has been shown to improve outcomes in patients with severe left ventricular systolic dysfunction (LVSD) and electrical dyssynchrony. This case report presents a 100-year-old male with a history of atrial fibrillation (AF), left bundle branch block (LBBB), and ischemic heart disease, who was admitted with chest pain and episodes of bradycardia. After careful consideration, CRT-P was successfully implanted. The patient's condition was closely monitored postimplantation, and a 4-year follow-up revealed significant symptomatic improvement and functional inde

A rare ORAI1 missense variant associates with risk of vascular diseases in White British adults.

BACKGROUND: Pathological remodelling of native vascular smooth muscle cells (VSMC) within the arterial wall is a key contributor to vascular disease. A driver of this remodelling is platelet-derived growth factor BB (PDGF-BB) and its signalling via activation of the store-operated calcium ion channel, ORAI1. Here, we investigated if there are associations of ORAI1 polymorphisms with human cardiovascular disease. METHODS AND RESULTS: We conducted candidate gene association analysis and revealed that a missense ORAI1 variant (rs3741596, S218G) associates with an increased risk of hospital-diagn

Conduction disorders after combined surgical or transcatheter treatment for severe aortic stenosis and coronary artery disease.

BACKGROUND: Conduction disturbances, including bundle branch and atrioventricular blocks, are frequent after aortic valve replacement and may require permanent pacemaker implantation (PPI). In patients with concomitant severe aortic stenosis and coronary artery disease, the fully percutaneous approach (TAVR + PCI) has not been sufficiently compared to SAVR + CABG regarding conduction disturbances and long-term outcomes. METHOD: A total of 1,232 patients undergoing either TAVR + PCI or SAVR + CABG between January 2010 and December 2020 were retrospectively evaluated. Propensity score matching (

Detection and prognostic stratification of left ventricular systolic dysfunction in left bundle branch block using an artificial intelligence-enabled electrocardiography.

BACKGROUND: Left bundle branch block (LBBB) significantly increases the risk of left ventricular systolic dysfunction (LVSD) due to cardiac dyssynchrony. Although artificial intelligence-enabled electrocardiography (AI-ECG) models show promise in detecting LVSD, their performance in LBBB patients remains underexplored. We hypothesized that an AI-ECG model clinically validated for detecting LVSD would accurately detect LVSD and predict future clinical outcomes in LBBB patients. METHODS: In this retrospective multicenter study, 5,689 expert-validated LBBB ECGs collected from 2,813 patients betw

Late left ventricular summit premature ventricular contractions elimination with new TactiFlex irrigation technology.

BACKGROUND: Premature ventricular contractions (PVCs) originating from the left ventricular summit (LVS) could be challenging to ablate due to anatomical reasons. AIM: To study the role of new irrigation technologies, like the TactiFlex catheter, could facilitate deeper radiofrequency (RF) penetration and thus increase success in the ablative treatment of this PVC subset. METHODS: Three PVCs focus (left bundle branch block morphology, inferior axis on the frontal plane, early R/S transition in V1-V2) were accurately mapped with Ensite X omnipolar technology. RESULTS: RF was delivered at mit

Fascicular Ventricular Tachycardia: An Electrophysiological Paradox.

We present a case of fascicular ventricular tachycardia (VT) in a 27-year-old man. Fascicular VT arises within the fascicles of the left bundle of His and is associated with right bundle branch block and axis deviation on ECG. Fascicular VT can often be mislabelled as a narrow-complex tachycardia due to the proximity of the arrhythmic origin to the normal conduction system, creating a widened, but not classically "broad," QRS complex, as occurred in our case. Our patient was resistant to adenosine but sensitive to verapamil, as is characteristic of fascicular VT owing to its origin in a macros

Distinct intracardiac electrogram waveforms with perforation during left bundle branch area pacing implantation.

AIMS: Perforation during left bundle branch area pacing (LBBAP) results in a fall in the current of injury (COI) amplitude in the unipolar unfiltered electrogram (iEGM), but systematic waveform analyses have not been performed. Our aim was to investigate unipolar iEGM waveforms during perforation and to compare them to those at the final lead position. METHODS AND RESULTS: The iEGMS of consecutive patients who had perforation during LBBAP implantation were systematically analysed. A total of 92 patients with perforation were included. In the unfiltered channel, sensed COI amplitude was lower

Conduction system pacing using a rotatable connector enabling continuous pacing during lumenless lead deployment: a case report.

BACKGROUND: Conduction system pacing (CSP) is an alternative to right ventricular pacing, providing more physiological ventricular activation. Current guidelines recommend CSP for patients with an expected ventricular pacing burden >20% and mildly reduced left ventricular ejection fraction. However, consistent conduction capture is not always achieved, with success rates of around 80% in the MELOS trial. Lumenless leads (LL) are widely used for CSP, but sustained, real-time pacing and impedance monitoring could enhance procedural control and decrease procedure time. The integration of a tool t

Bidirectional ventricular tachycardia caused by occlusion myocardial infarction-a case report.

BACKGROUND: Bidirectional ventricular tachycardia (BdVT) is an uncommon form of ventricular arrhythmia typically associated with digoxin toxicity or catecholaminergic polymorphic ventricular tachycardia. BdVT in the setting of acute myocardial ischaemia is limited to case reports. We present an example of BdVT caused by occlusion myocardial infarction with original and modified Sgarbossa criteria being positive. CASE SUMMARY: An elderly male with known coronary artery disease presented with chest pain. The initial ECG demonstrated a regular wide QRS complex tachycardia with alternating fronta

Protein biomarkers associated with left bundle branch block in patients with heart failure and reduced ejection fraction.

INTRODUCTION: Left bundle branch block (LBBB) is common in heart failure with reduced ejection fraction (HFrEF) and causes dyssynchrony, which accelerates cardiac remodelling. The biological mechanisms behind LBBB-associated remodelling remain largely unknown. Therefore, we used an omics approach to test the hypothesis that LBBB is associated with plasma protein dysregulation aiming at defining a proteome that contributes to the specific disease driving phenotype in dyssynchronopathy. METHODS: Patients were selected from the BIOlogy Study to TAilored Treatment in Chronic Heart Failure databas

Conduction disturbances after transcatheter aortic valve implantation.

Conduction disturbances and permanent pacemaker implantation remain the most common complications after transcatheter aortic valve implantation. The strongest predictors of conduction abnormalities and subsequent permanent pacemaker implantation after transcatheter aortic valve implantation include pre-existing right bundle branch block, a short membranous interventricular septum, deep transcatheter heart valve implantation, and valve type. Importantly, both new permanent pacemaker implantation and new left bundle branch block after transcatheter aortic valve implantation are associated with i

Electrocardiographic predictors of major adverse cardiovascular events in immune checkpoint inhibitor-associated myocarditis.

BACKGROUND: Immune checkpoint inhibitor (ICI)–associated myocarditis carries a high risk of mortality, yet electrocardiographic (ECG) predictors of adverse outcomes remain unclear. The present study aimed to evaluate the predictive value of clinical and admission ECG parameters for major adverse cardiovascular events (MACE). METHODS: We retrospectively analyzed 60 consecutive patients with ICI myocarditis. Patients were stratified according to the occurrence of major adverse cardiovascular events (MACE), defined as myocarditis-related cardiogenic shock, high-degree atrioventricular block requi

Zero-Fluoroscopy Left Bundle Branch Pacing in Pregnancy Using Electroanatomic Mapping.

BACKGROUND: Left bundle branch pacing (LBBP) offers physiological ventricular activation and has been proposed as an effective pacing strategy in young patients, particularly to prevent pacing-induced cardiomyopathy. In vulnerable populations, such as pregnant women, eliminating fluoroscopy is paramount to avoid fetal radiation exposure. CASE SUMMARY: We describe a 23-year-old pregnant woman (20 weeks gestation) presenting with symptomatic complete atrioventricular (AV) block. A dual-chamber pacemaker with LBBP was implanted using a three-dimensional (3D) electroanatomic mapping (EAM) system,

A chicken and egg situation: a case of left bundle branch block-induced dilated cardiomyopathy.

BACKGROUND: Left bundle branch block (LBBB) mainly occurs in those with heart disease, but also appears in asymptomatic patients with normal heart structure. Determining the causal relationship between LBBB and cardiomyopathy is a current clinical challenge. CASE PRESENTATION: This article describes a case of a middle-aged female patient with complete LBBB and cardiac dysfunction whose symptoms significantly improved after cardiac resynchronization therapy (CRT). During follow-up assessments at 3 and 6 months postsurgery, echocardiography revealed good recovery of cardiac structure and functio

Transhepatic Left Bundle Branch Area Pacing in a Patient With Ebstein Anomaly and Prior Glenn Surgery: A First Report.

BACKGROUND: Venous access exhaustion poses a significant challenge for the implantation of intracardiac devices. Epicardial alternatives are often used in these cases, despite their long-term deleterious effects. This case presents a transhepatic approach for left bundle branch pacing in a patient with Ebstein anomaly and a prior Glenn procedure. METHODS: Following pre-procedural evaluation with triphasic CT, ultrasound-guided hepatic vein puncture was performed. Guidewires were advanced into the right heart chambers, allowing passage of a delivery sheath for physiologic pacing. Atrial lead p

The Role of Paced QRS Morphology in Determining Initial Site for Lead Placement in Left Bundle Branch Area Pacing.

BACKGROUND: Determining proper initial site on the right side of interventricular septum is the first step to implant LBBAP lead. Electrocardiographic features such as discordance of paced QRS polarity in leads II and III (alternatively in aVL and aVR) and the presence of notch in lead V1 are in use but have not yet been evaluated. This study aimed to establish the role of paced QRS morphology in determining the initial site for LBBAP lead deployment. METHODS: A group of 150 LBBAP device implantations containing 327 lead placement attempts were analyzed. Final electrocardiographic results of
