Transient Ventricular Capture Loss Following Alcohol Septal Ablation in Left Bundle Branch Area Pacing: A Case Report.

BACKGROUND: Alcohol septal ablation (ASA) is an established therapy for symptomatic hypertrophic obstructive cardiomyopathy (HOCM) but may cause conduction disturbances. Data on interactions between ASA and left bundle branch area pacing (LBBAP) leads are limited. CASE SUMMARY: A 70-year-old woman with complete AV block and dual-chamber pacemaker with LBBAP developed severe LVOT obstruction (Valsalva 93.3 mmHg) with syncope and dyspnea. ASA of the first septal branch (4 mL 95% ethanol) caused immediate hypotension and transient loss of ventricular capture due to abrupt LBBAP threshold rise, r

Immune Checkpoint Inhibitor-Associated Myocarditis With Multisystem Immune Toxicity in Renal Cell Carcinoma.

BACKGROUND: Immune checkpoint inhibitors are widely used in the adjuvant treatment of renal cell carcinoma. Although immune-related adverse effects are well described, myocarditis remains an uncommon complication. Ealy multisystem immune-related toxicity after limited exposure to therapy is uncommon and may be difficult to recognize in older patients. CASE SUMMARY: An 84-year-old woman receiving adjuvant pembrolizumab for renal cell carcinoma presented with dyspnea shortly after her second dose. She was found to have a new left bundle branch block, with marked troponin elevation and mild left

Performance of automatic capture confirmation algorithm in a large cohort of pacemaker patients with left bundle branch area pacing.

BACKGROUND: Automatic capture confirmation algorithms regularly evaluate pacing capture threshold (PCT) and adjust energy output to deliver a tailored safety margin over the PCT. Abbott AutoCapture™ Algorithm provides beat-by-beat capture confirmation and delivers a high-output backup safety pulse in the event of non-capture. Our objective was to evaluate the longitudinal performance and stability of the Abbott AutoCapture™ algorithm in patients with LBBAP. METHODS: De-identified remote device data were retrospectively analyzed from consecutive patients in our hospital who received AutoCapture

Research on postoperative complications following transcatheter aortic valve replacement: a bibliometric and visual analysis from 2006 to 2025.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) has transformed the treatment of severe aortic stenosis in patients at high or prohibitive surgical risk. Despite high procedural success, postoperative complications remain common and clinically consequential. Understanding how research on these complications has evolved is essential for guiding future investigations and improving outcomes. This study maps the global research landscape of TAVR-associated postoperative complications, with emphasis on thematic evolution, collaboration patterns, and emerging mechanistic and analytical app

Comparative Left Bundle Branch Block Correction With Physiological Pacing-An Intrapatient Electrocardiographic Study.

INTRODUCTION: Several studies have compared His bundle pacing (HBP) and left bundle branch area pacing (LBBAP) regarding paced QRS duration and clinical outcomes. This study compared acute intraprocedural depolarization and repolarization electrocardiographic changes between corrective HBP and LBBAP in the same patient with baseline left bundle branch block and reduced left ventricular ejection fraction. MATERIAL AND METHODS: Thirty-six patients with successful corrective LBBAP for cardiac resynchronization therapy (CRT), in whom corrective HBP was also demonstrated, were retrospectively revi

Impact of short membranous septal length on overall survival after transcatheter aortic valve replacement.

Transcatheter aortic valve replacement (TAVR) is an established therapy for severe aortic stenosis, although conduction disturbances, especially atrioventricular block and new-onset left bundle branch block (LBBB) remain associated with adverse outcomes. Short membranous septal length (MSL) has been linked to post-TAVR conduction abnormalities, but its prognostic significance is unclear. We retrospectively analyzed 258 patients who underwent transfemoral TAVR. MSL was measured on contrast-enhanced CT, and left ventricular ejection fraction (LVEF) was assessed at baseline, 1 week, and 1 year. T

AI-ECG classification for Brugada syndrome: A study of machine learning techniques to optimise for limited datasets.

Deep neural networks can classify ECGs with high accuracy when training data is abundant. Rare conditions like Brugada syndrome, an inherited arrhythmia syndrome predisposing to sudden death, pose challenges due to data scarcity hindering model training. We evaluated multiple machine learning (ML) approaches to optimise a Brugada ECG classification model using limited training data. The baseline model was trained on a dataset comprising 176 Brugada, 176 right bundle branch block (RBBB) and 352 normal ECGs from Zhongshan Hospital (Zhongshan-baseline dataset), framed as a binary classification t

Novel electrocardiographic diagnosis of left ventricular hypertrophy with right bundle branch block in a Taiwanese clinical cohort.

BACKGROUND: The diagnostic accuracy of electrocardiographic (ECG) criteria for left ventricular hypertrophy (LVH) in Taiwanese adults remains uncertain, particularly in the presence of complete right bundle branch block (CRBBB). METHODS: We retrospectively enrolled 431 Taiwanese adults, including 205 with normal conduction and 226 with CRBBB. Echocardiographic left ventricular mass index (LVMI) served as the reference standard. We evaluated the sensitivity, specificity and area under the receiver operating characteristic curve (AUC) of six traditional ECG criteria: Sokolow-Lyon index, RaVL, m

Sex-Based Differences in Left Bundle Branch Capture During Left Bundle Branch Area Pacing.

OBJECTIVE: To identify sex-based differences in meeting left bundle branch (LBB) capture criteria on postimplant ECGs. BACKGROUND: Despite differences in QRS duration (QRSd) between males and females, no sex-based criteria have been established for defining LBB capture during LBB area pacing (LBBAP). METHODS: Patients aged >18 y.o. with a pacing indication and dual-chamber LBBAP pacemaker implantation from 2020 to 2023 were studied. Baseline and postoperative day 1 (POD1) ECGs were assessed for electrical dyssynchrony (QRS-area, sum-absolute integral of QRS-T (SAI QRS-T)), with LBB capture d

Delayed Partial Protrusion of the Lead After Left Bundle Branch Pacing: The Importance of Programming-A Case Report.

Delayed lead perforation into the left ventricular (LV) cavity after left bundle branch area pacing (LBBAP) is less common than in the acute setting and could be associated with severe adverse events. We present the case of a young patient with symptomatic second-degree atrioventricular block who developed partial protrusion of the LBBAP lead into the LV cavity after discharge. While adequate conduction system capture was still seen with bipolar pacing, this potentially serious complication was self-diagnosed by the patient due to the initial ventricular pacing programming in a unipolar settin

Atrioventricular Conduction Disturbance After Perimembranous Ventricular Septal Defect Closure.

Transcatheter closure of perimembranous ventricular septal defect (VSD) offers a minimally invasive alternative to surgery but carries a rare risk of atrioventricular block (AVB). This report describes 2 cases of conduction disturbances following transcatheter VSD closure and discusses their management. A 29-year-old man developed symptomatic high-grade AVB 5 days after successful device closure, which resolved with corticosteroid therapy and did not recur. A 25-year-old man developed new-onset left bundle branch block during device deployment using the retrograde approach; the procedure was a

Outcomes of Patients With New Left Bundle Branch Block After TAVR: TVT Registry Insights.

BACKGROUND: Cardiac conduction disturbances remain the most frequent complication of transcatheter aortic valve replacement (TAVR), but the clinical implications of new left bundle branch block (LBBB) after TAVR remain controversial. Here, we aim to assess the impact of new LBBB after TAVR on patient outcomes in a large, real-world registry. METHODS: The study population consisted of patients in the TVT registry (Society of Thoracic Surgery and American College of Cardiology Transcatheter Valve Therapy Registry) who underwent TAVR for aortic stenosis between 2016 and 2022 and were discharged

Diagnostic Accuracy Improvement of an Updated HEART Score to Predict Coronary Artery Disease as Detected by Coronary Computed Tomography Angiography.

Background: The HEART score is a widely used risk-stratification tool in suspected acute coronary syndrome (ACS), but it still suffers from several limitations. We aim to assess its diagnostic accuracy for predicting coronary computed tomography angiography (CCTA) findings and explore possible enhancement by integrating additional clinical variables. Methods: In this retrospective, observational study, consecutive patients presenting to the Emergency Department with suspected ACS and undergoing CCTA were analyzed. The study assessed the HEART score's diagnostic accuracy for predicting signific

Left Ventricular and Biventricular Multipoint Pacing With Dynamic Atrioventricular Delays: 6-Month Cardiac Resynchronization Therapy Response.

BACKGROUND: The response to cardiac resynchronization therapy (CRT) can depend on when and where the right and left ventricles (RV, LV) are paced. Dynamic atrioventricular (AV) delays (SyncAV CRT, Abbott) and multi-site LV pacing (e.g., MultiPoint Pacing [MPP], Abbott) are features that may enhance CRT response. Their combined impact has not been evaluated in a chronic clinical setting. OBJECTIVE: Evaluate the 6-month echocardiographic and clinical response to SyncAV when combined with either biventricular MPP (MPP+SyncAV: RV + LV1 + LV2 pacing) or LV-only MPP (LVMPP+SyncAV: LV1 + LV2 pacing)

Feasibility and Safety of Left Bundle Branch Pacing in Nonagenarians: A Single Center Observational Study.

BACKGROUND: Left bundle branch pacing (LBBP) is emerging as a physiological pacing strategy. However, data on its feasibility and safety in nonagenarians (≥90 years) with symptomatic bradycardia are limited. This study aims to evaluate the outcomes of LBBP in this frail, high-risk population. METHODS: A retrospective analysis was conducted on 8 patients aged 90 and older who underwent LBBP between December 2023 and December 2024. Procedural outcomes, complications, and pacing parameters were assessed. RESULTS: LBBP was successfully performed in 8 patients (mean age 93 ± 2 years, mean Clinica

Conduction abnormalities after TAVI and their role in patient management: a systematic review.

OBJECTIVE: Transcatheter aortic valve implantation (TAVI) is associated with a relatively high incidence of permanent pacemaker implantation (PPM-I). We aimed to evaluate the existing literature on electrocardiographic (ECG) changes before and after TAVI, identify predictors for PPM-I, and suggest a standardized post-TAVI ECG monitoring protocol. METHODS: A systematic literature review was conducted across multiple databases, including PubMed, Web of Science, and JSTOR, to identify studies published between 2001 and 2024. RESULTS: From an initial pool of 24,170 records, 17 studies met the in

Impacts of Cardiac Dyssynchrony and Resynchronisation on Cellular Metabolism and Mitochondria: Current Understanding for Future Applications.

Ventricular dyssynchrony serves as a cause of heart failure, resulting from impaired electrical conduction within the heart, commonly from bundle branch block or right ventricular pacing. It exceeds structural remodelling to cause significant cellular dysfunction, particularly in aberrant myocardial metabolism, alterations to myocardial oxygen consumption and cardiac efficiency, along with compromised mitochondrial activity. CRT is a therapeutic strategy that is used for the dyssynchronous ventricle by correcting electrical aberrance, while also improving cellular metabolic homeostasis, cardia
