Pregabalin is widely prescribed for neuropathic pain and anxiety disorders and is generally considered to have a favorable cardiovascular safety profile. However, rare cardiac conduction disturbances have been reported, mainly in overdose settings. We report the case of a 29-year-old woman admitted to the intensive care unit for toxic coma secondary to acute pregabalin intoxication, complicated by a second-degree atrioventricular block with 2:1 conduction. The patient required mechanical ventilation and transient chronotropic support with isoproterenol. Cardiac investigations were unremarkable

Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common cause of paroxysmal supraventricular tachycardia in adults and frequently presents with sudden-onset palpitations and regular narrow-complex tachycardia. Although generally considered benign, recurrent symptomatic episodes often lead to repeated emergency department visits and reduced quality of life, warranting definitive therapy. We report the case of a 42-year-old woman who presented with several hours of palpitations and was found to have a regular narrow-complex tachycardia at approximately 200 beats per minute withou

BACKGROUND: Chagas disease (CD) remains a significant public health problem and may progress to Chagas cardiomyopathy (ChCM), a distinct inflammatory–fibrotic myocardial phenotype characterized by a high burden of atrial fibrillation (AF) and conduction system abnormalities. Updated estimates of the prevalence of these electrical disturbances are essential to inform clinical risk stratification and guideline development. OBJECTIVE: This updated meta-analysis aimed to evaluate the prevalence of AF and Conduction Abnormalities in ChCM. METHODS: We conducted a comprehensive systematic review and

BACKGROUND: Catheter ablation is a validated treatment for ventricular arrhythmias (VA), but conventional radiofrequency (RF) energy may cause collateral injury due to non-selective thermal damage. Pulsed Field Ablation (PFA), a non-thermal modality based on irreversible electroporation, offers myocardial tissue selectivity and enhanced safety. While PFA is widely adopted for atrial arrhythmias' ablation, its application in the ventricles remains an evolving frontier.
METHODS: We report a single-center experience using the Centauri PFA system integrated with a focal, contact-force sensing irr

This case report explores the spectrum and mechanisms of myocardial involvement in Lyme carditis, integrating clinical and imaging findings to better understand the disease course and its management implications. While conduction abnormalities are the most well-recognized cardiac manifestation of Lyme carditis, our findings highlight the importance of myocardial work assessment by echocardiography in the early stage of the disease and by cardiac magnetic resonance in the late stage. These insights may guide clinicians in early recognition, targeted therapy, and prevention of adverse cardiac ou

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have significantly advanced the treatment of malignant tumors. Tislelizumab, a recombinant humanized anti-programmed death 1 (PD-1) monoclonal antibody, is widely used in the management of advanced cancers. However, like other ICIs, tislelizumab can trigger immune-related adverse events (irAEs). Among irAEs, immune checkpoint inhibitor-associated myocarditis (ICI-associated myocarditis) is rare but life-threatening, underscoring the need for early recognition and intervention. This case report describes a case of highly suspected fulminant myoc

Complete heart block (CHB) is an uncommon but life-threatening complication of anterior ST-elevation myocardial infarction (STEMI). We report a case of a 62-year-old male who presented to the emergency department with severe chest pain, profound bradycardia, and an undetectable blood pressure. Initial electrocardiogram demonstrated complete atrioventricular dissociation, and emergent transcutaneous pacing and vasopressor support were initiated. The patient subsequently developed ventricular tachycardia degenerating into recurrent conduction abnormalities with evolving ST-segment elevation. Eme

Cardiac involvement in human echinococcosis is rare, with isolated interventricular septal (IVS) cysts representing an uncommon presentation. This report details the case of a 41-year-old male who presented to urgent care with fever, cough, and exertional dyspnea. Initial evaluation revealed high-grade atrioventricular (AV) block and markedly elevated inflammatory markers. Advanced cardiac imaging (echocardiography, CT, and MRI) identified a large, complex cystic mass within the basal IVS. Serology was positive for Echinococcus antibody, and a history of residence in an endemic region with con

OBJECTIVE: Postoperative high-grade atrioventricular block is prevalent following congenital aortic valve, root and left ventricular outflow tract (LVOT) surgery. We aimed to evaluate the application of presurgical cardiac computed tomography (CT) estimation of the conduction axis in mitigating this issue.
METHODS: Patients with congenital aortic valve, root and LVOT disease evaluated and operated in our center from February 2022 to August 2025 who underwent presurgical CT with intact central fibrous body were included. Anatomical landmarks were used to estimate the atrioventricular node (poi

BACKGROUND: There is no universal agreement on the diagnostic criteria for cardiac sarcoidosis (CS). In 2024, the American Heart Association (AHA) introduced a new diagnostic scheme featuring a quaternary likelihood stratification for CS instead of dichotomy. Its clinical implications are unknown.
METHODS: We analyzed 252 patients with CS and proven histology (mean age, 49 years; 61% women). Following the AHA diagnostic stratification, CS was retrospectively considered definite given sarcoid granulomas on myocardial biopsy (n=109), highly probable with both clinical and cardiac imaging findin

Erdheim-Chester disease (ECD) is a rare, non-Langerhans cell histiocytosis characterized by excessive production and accumulation of histiocytes within various tissues. The discovery of activating mutations within the Mitogen-Activated Protein Kinase (MAPK) pathway has established its neoplastic nature and enabled the use of targeted therapeutic approaches. Despite these advances, ECD remains a diagnostic challenge, often due to its nonspecific symptoms, multisystemic involvement, and consequent delay in diagnosis. We report three cases of ECD illustrating its diverse clinical presentations an

BACKGROUND: Safety and efficacy have been well demonstrated for Micra™ leadless cardiac pacemakers (LCPs). However, the presence of sex-specific disparities remains unclear.
METHODS: The aim of this single-centre observational study was to assess the sex-specific short- and long-term outcomes in patients undergoing LCP implantation.
RESULTS: In total, 378 LCPs were implanted in 127 women (33.6%) and 251 men (66.4%). The most frequent indications included atrial fibrillation with slow conduction (women: 31.5%, men: 44.6%), third-degree atrioventricular block (women: 31.5%, men: 33.5%) and sic

BACKGROUND: T-wave inversion (TWI) on electrocardiography often raises concern for myocardial ischemia or other structural heart disease. However, abnormal ventricular activation caused by cardiac pacing can induce transient repolarization changes known as cardiac memory. Awareness of this phenomenon is essential to avoid unnecessary diagnostic procedures. CASE PRESENTATION: An 80-year-old woman underwent implantation of a dual-chamber pacemaker for symptomatic sick sinus syndrome. Her pre-operative electrocardiogram showed complete right bundle branch block with T-wave inversion limited to le

BACKGROUND: Postoperative junctional ectopic tachycardia (JET) is a frequent, hemodynamically consequential arrhythmia after congenital heart surgery. Ivabradine is increasingly used off-label, but its effectiveness and safety in pediatric postoperative JET remain uncertain. METHODS: We conducted a systematic review and single‑arm meta‑analysis of studies enrolling children (< 18 years) with postoperative JET treated with oral or nasogastric ivabradine. Outcomes included conversion to sinus rhythm, time to conversion, recurrence, use of concomitant antiarrhythmic drugs (AADs), adverse events (

BACKGROUND: Deep brain stimulation (DBS), used in the management of neurologic movement disorders, can generate electrocardiographic artifacts that mimic atrial arrhythmias.
CASE SUMMARY: A 63-year-old man with a history of atrial fibrillation ablation, left bundle branch block, and atrioventricular block status post-cardiac resynchronization therapy pacemaker developed Parkinson disease and underwent right-sided deep brain stimulator implantation. A routine electrocardiogram demonstrated apparent atrial flutter. However, interrogation of his pacemaker confirmed normal sinus rhythm, indicatin

BACKGROUND: His bundle pacing (HBP) provides physiological ventricular activation by engaging the native His-Purkinje system. Although midterm safety and efficacy are established, long-term outcomes beyond 5 years remain insufficiently defined.
OBJECTIVE: This study aimed to evaluate long-term performance, device behavior, and clinical outcomes of permanent HBP over a follow-up period of up to 10 years.
METHODS: We retrospectively analyzed 500 consecutive patients who underwent successful HBP between 2003 and 2013 at 2 high-volume centers. Clinical, device, and echocardiographic data were co

BACKGROUND: Rupture of the left coronary aortic sinus complicated by conduction disturbances has not been reported.
CASE SUMMARY: An 85-year-old woman presented with chest and back pain and a complete atrioventricular block, which led to low-output syndrome. Imaging revealed a rupture of the left coronary aortic sinus with a pseudoaneurysm and a hematoma extending toward the central fibrous body. Conservative management was selected, and atrioventricular conduction gradually improved as the hematoma regressed.
DISCUSSION: The hematoma compressed the atrioventricular node region, causing a re
