Mitral valve prolapse and conduction defects in Ehlers-Danlos syndrome.

A case of type 3 Ehlers-Danlos syndrome with associated mitral valve prolapse, right bundle-branch block, and left anterior fascicular block was studied, including His bundle electrocardiography and echocardiography. Mitral valve prolapse and HV prolongation were reported. The present study suggests that the basic underlying collagen defect in this syndrome is responsible for these findings, and that the association may be more widespread in the syndrome than currently acknowledged. The possibility also is considered that the connective tissue abnormalities of Ehlers-Danlos syndrome involving

Rate-dependent variation in the duration of the QRS complex with left anterior fascicular block.

The case of a 67-year-old man with combined ischemic and valvular heart disease is presented. Electrocardiographic abnormalities included left anterior fascicular block with a variable duration of the QRS complex. The width of the QRS complex was dependent on the length of the cycle, being broader with short than with long preceding R-R intervals. This is interpreted as a tachycardia-dependent focal block coexisting with a fixed delay in fascicular conduction.

Bilateral bundle branch block. Right bundle branch block associated with left anterior fascicular block.

This report describes the clinical and electrocardiographic features in 30 cases of complete right bundle branch block associated with a block of the anterior fascicle of the left bundle branch, 7 of which also featured a block of the posterior fascicle of the left bundle branch (trifascicular block). The majority of the patients suffered from coronary atherosclerotic heart disease and/or hypertensive heart disease. Complete right bundle branch block associated with fascicular block of the left bundle branch constitutes a special electrocardiographic pattern. The prognosis of the patients pres

Left anterior fascicular block concealing anteroseptal infarction.

Fascicular blocks may modify electro-cardiographic morphologies and signs of necrosis may be masked. This report concerns the case of a patient with an anteroseptal myocardial infarction who developed a transient left anterior fascicular block. An explanation for the concealment of the signs of necrosis is proposed, based on an analysis of the modified activation process by means of a vectorial representation.

Mobitz II atrioventricular block due to infra-Hisian block and bundle branch extrasystoles.

An asymptomatic elderly male presented with complete right bundle branch block, left anterior fascicular block and Mobitz Type II second degree atrioventricular block. In addition, he was noted to have ventricular premature systoles. Electrophysiologic studies demonstrated two apparently different mechanisms for the second degree A-V block--infra-Hisian block and A-V block due to bundle branch extrasystoles arising in the affected right bundle branch. Concealed conduction of the bundle branch extrasystoles as the proximate cause of infra-Hisian block could not be excluded. However, both forms

Reassessment of Q waves in left bundle branch block.

This study disputes a number of recent reports claiming that abnormal Q waves or a QS configuration in inferior leads (II, III and AVF) coexisting with left bundle branch block is highly suggestive of, and indeed specific for, myocardial infarction. Five patients reported herein demonstrate disappearance of Q waves in inferior leads on spontaneous reversal of LBBB to normal conduction. This necessitates the conclusion that these Q waves represent a postdivisional conduction variant most closely equivalent to left anterior fascicular block coexisting with predivisional LBBB. Absence of inferior

Congenital aneurysm of the sinus of Valsalva protruding into the left ventricle. Review of diagnosis and treatment of the unruptured aneurysm.

During a routine physical examination, a 34-year-old man, in apparent good health, had an electrocardiogram which revealed left axis deviation. Cardiac evaluation disclosed mild aortic regurgitation and left anterior fascicular block. Nine months later, the patient was admitted to the critical care unit after he had experienced cardiac arrest while jogging. Angiograms demonstrated a large unruptured aneurysm in the right coronary sinus of Valsalva protuding into the left ventricle. Echocardiographic manifestations also suggested also suggested a sinus of Valsalva aneurysm. The defect was repai

[Systolic intervals in disorders of intraventricular conduction].

The duration of the systolic intervals in 50 patients was studied. They were divided into the following groups of 10 each: 1) with normal intraventricular conduction; 2) with advanced right bundle branch block; 3) with advanced left bundle branch block; 4) with left anterior fascicular block, and 5) endocardial pacemaker in the right ventricle. The systolic intervals do not suffer modification in the presence of a right bundle branch block or a left anterior fasicular block. The left bundle branch block produces changes fundamentally in the preejection period at the expense of its two componen

[Differential diagnosis of the deviation of the QRS left axis in ECG].

100 ECGs with left axis deviation have been analyzed with respect to left anterior fascicular block (LAFB). On the basis of conventional criteria LAFB was diagnosed in 59 cases, but could be rules out in 26. Of the remaining 15 ECGs which at first seemed doubtful, 4 were identified as LAFB on the basis of a new criterion: duration of qR in a VL at least 40 msec longer than in V6.

Electrocardiographic criteria for the diagnosis of left anterior fascicular block. Left axis deviation and delayed intraventricular conduction.

The two current criteria for diagnosis of left anterior fascicular block (LAFB) were evaluated; they are marked left axis deviation (LAD) and a delay in the time of inscription of the intrinsicoid deflection (ID) in lead aVL asynchronous to V6. From 400 electrocardiograms with a LAD of --30 degrees or greater, 62 percent showed asynchronous activation of the left ventricle. There was only a general relationship between the degree of LAD and delayed ID in aVL. The incidence of delayed ID in aVL was as follows: 2 percent with mean frontal QRS axis at 0 degrees; 9 percent at --15 degrees; 41 perc

[The left-fascicular blocks ("hemiblocks")].

The left ventricular impulse conduction system is composed of two, three or more fasciculi. The blocking of the impulse in one fasciculus is termed a left fascicular block or "hemiblock" for short. The anatomical variations of the left ventricular impulse conduction system, and the possibility of partial blocking, makes the electrocardiographic diagnosis more difficult. The ECG criteria of left fascicular blocking - with special regard to the left anterior fascicular block - and their combinations with the right block (bilateral bifascicular block) are discussed. A bilateral trifascicular bloc
