Prolongation of the cardiac QTc interval in Turner syndrome.

Anatomic anomalies of the cardiovascular system occur in approximately 50% of individuals with Turner syndrome (TS), with the specific genetic cause(s) for the heart defects still unknown. Because congenital heart disease may be associated with conduction system abnormalities, we compared electrocardiograms (ECGs) in 100 women with TS and 100 age-matched female controls. Women with TS were significantly more likely to demonstrate left posterior fascicular block (p < 0.005), accelerated AV conduction (p < 0.006), and T wave abnormalities (p < 0.006). The PR interval was significantly shorter (1

Clinical predictors of cardiac events in patients with isolated syncope and negative electrophysiologic study.

BACKGROUND: Patients with syncope or near syncope of unknown etiology represent a great challenge to cardiologists. An initial symptomatic episode triggers a series of diagnostic analysis which remain unsatisfactory when negative. More invasive tools such as electrophysiologic testing yield only partial answers to risk stratification while the complementary implantable holter diagnostics are not usually considered until a recurrent episode is documented. OBJECTIVE: This study targets predictors of significant cardiac rhythmic events in patients with a reported episode of syncope or near synco

Idiopathic fascicular left ventricular tachycardia: linear ablation lesion strategy for noninducible or nonsustained tachycardia.

BACKGROUND: Idiopathic "fascicular" left ventricular tachycardia (IFLVT) is frequently not inducible or nonsustained at the time of planned catheter ablation. The mechanism of the arrhythmia has been suggested to be reentry involving a sizable area of the LV inferior septum extending from base toward the apex. OBJECTIVE: We tested the ability of a series of radiofrequency lesions delivered in a linear fashion to the inferior-mid septum to control ventricular tachycardia not amenable to standard mapping ablation strategies. METHODS: Programmed stimulation both at baseline state and with isopr

Cardiac resynchronization therapy in patients with heart failure and conduction abnormalities other than left bundle-branch block: analysis of the Multicenter InSync Randomized Clinical Evaluation (MIRACLE).

BACKGROUND: Cardiac resynchronization therapy (CRT) has been proposed as a treatment for patients with congestive heart failure (CHF) and prolonged QRS durations. Previous studies have predominantly included patients with left bundle-branch block (LBBB). The Multicenter InSync Randomized Clinical Evaluation (MIRACLE) investigators assessed the efficacy of CRT in patients with CHF with QRS durations > or = 130 ms and found that CRT lead to improvement in several measures of functional capacity and exercise tolerance. HYPOTHESIS: We designed this retrospective study to determine whether patient

Quantitative effects of functional bundle branch block in patients with atrioventricular reentrant tachycardia.

Changes in the retrograde conduction time (ventriculoatrial [VA]) interval during functional bundle branch block (BBB) have been used to separate septal from free wall accessory pathways (APs), but different values of the VA interval prolongation (deltaVA) have been described in different reports. A total of 95 patients with single nondecremental APs who developed BBB during atrioventricular reentrant tachycardia were studied. Free wall APs were found in 60 patients, and 35 had septal APs. For patients with free wall APs, complete and incomplete BBB ipsilateral to the atrial insertion site of

Specificity of electrocardiographic myocardial infarction screening criteria in patients with nonischemic cardiomyopathies.

BACKGROUND: The 32-point, 54-criteria Selvester QRS scoring system has been successfully used to estimate the size of nonacute myocardial infarction (MI). Three criteria of the system have been shown to be sensitive for the identification of nonacute MI and specific in normal control subjects. The validity of the system has not been tested in patients with cardiomyopathy of nonischemic origin. The purpose of this study was to examine the electrocardiographs (ECGs) of patients with abnormal left ventricular function but no presence of coronary disease to determine the diagnostic specificity of

Cure of interfascicular reentrant ventricular tachycardia by ablation of the anterior fascicle of the left bundle branch.

INTRODUCTION: Fascicular reentrant ventricular tachycardia (VT) using the anterior fascicle of the left bundle anterogradely is rare and may produce identical QRS morphology during sinus rhythm and VT. Catheter ablation of this type of VT has not been described in detail. METHODS AND RESULTS: In a postinfarct patient with dilated left ventricle and recurrent VT (showing a QRS configuration of right bundle branch, left posterior fascicular block), endocardial recordings from the His-Purkinje system showed that VT was due to interfascicular reentry. Induction of VT occurred after progressive re

Isolated left posterior fascicular block: a reliable marker for inferior myocardial infarction and associated severe coronary artery disease.

The electrocardiographic (ECG) pattern of isolated left posterior fascicular block (LPFB) is a rare condition. It seems that true LPFB is often not recognized, whereas in cases of simple QRS axis of +60 degrees to +100 degrees in the frontal plane, the diagnosis of LPFB is made erroneously. Both facts rely on controversial and partially misleading opinions in the literature. Therefore, a retrospective and prospective study was performed in order to determine the prevalence of LPFB and to correlate its presence to the underlying disease. Retrospective study: Of a cohort of 830 patients referred

Body surface isopotential mapping of the entire QRST complex in the Wolff-Parkinson-White syndrome. Correlation with the location of the accessory pathway.

Body surface potential maps were recorded during sinus rhythm and during atrial pacing at the time of electrophysiologic studies in 42 patients with Wolff-Parkinson-White syndrome. The locations of the accessory pathways were determined by epicardial mapping during surgery in 34 patients and by multicatheter endocavitary electrophysiologic studies in eight additional patients. During delta wave inscription, the shape and extension of areas of the negative and positive potentials on the thorax correlated better with the preexcitation site (69% of patients) than with the localization of the mini

Electrophysiologic characteristics of sudden QRS axis deviation during orthodromic tachycardia. Role of functional fascicular block in localization of accessory pathway.

We analyzed the effect of functional fascicular block (FFB) on ventriculoatrial conduction time (VACT) during orthodromic tachycardia (OT) in 32 patients with single accessory pathway (AP) of the Kent bundle type. The location of AP was left free wall (LFW-AP) in 21 patients, left posteroseptal in 6, right free wall in 2, and right anteroseptal in 3. FFB either alone or in combination with functional left or right bundle branch block (LBBB or RBBB) occurred predominantly at the onset of OT and was initiated with ventricular extrastimulus technique more often than with atrial extrastimulation.

Left bundle branch block: a continuously evolving concept.

Eppinger and Rothberger in 1909 and 1910 first acknowledged the importance of the conduction system, yet a confusion of the pattern of left bundle branch block with right bundle branch block resulted which persisted for 25 years. In left bundle branch block, right ventricular endocardial activation begins before, and is often completed before, initiation of left ventricular endocardial activation. Most likely, right to left septal activation then follows, resulting in left ventricular endocardial activation. Although it is hazardous to make definitive diagnoses of infarction in the presence of

Myotonic dystrophy: ambulatory electrocardiogram, electrophysiologic study, and echocardiographic evaluation.

Myotonic dystrophy is frequently associated with functional and anatomic derangements in the myocardium. Ten myotonic dystrophy patients (seven men and three women, ages ranging from 35 to 58 years) were evaluated with a 12-lead ECG, 24-hour Holter monitor recording, invasive electrophysiologic studies, and echocardiographic examination. Nine patients displayed abnormalities in the conduction system. ECG and Holter monitor abnormalities were first-degree atrioventricular block (n = 8), second-degree atrioventricular block (n = 1) (Wenckebach type), complete left bundle branch block (n = 2), le

The effect of fascicular block on ventriculoatrial conduction during AV reentrant tachycardia.

Two patients with Wolff-Parkinson-White syndrome and surgically mapped anterior left free wall atrioventricular bypass tracts had orthodromic atrioventricular reentry tachycardia conducted with complete left bundle branch block (CLBBB), complete right bundle block (CRBBB), left anterior fascicular block (LAFB), and a narrow QRS. Ventriculoatrial conduction increased by 35 and 85 ms with CLBBB compatible with the left free wall location of the bypass tracts. In one patient, resolution of CLBBB occurred in two stages. Initially, left posterior fascicular block (LPFB) resolved, decreasing VA cond

Recent advances in the diagnosis of fascicular blocks.

The electrocardiographic criteria for the diagnosis of left fascicular blocks have been a subject of debate and speculation. Nevertheless, careful analysis of certain electropathologic studies, extrapolations from animal experiments, and studies resulting from the inadvertent section of the various fascicles and of tracings obtained at the moment that the conduction disturbances appeared or disappeared have led to the acceptance of characteristic patterns. Although the specificity and sensitivity of electrocardiographic criteria of left anterior and left posterior fascicular block still requir

Paroxysmal fascicular tachycardia and ventricular tachycardia due to mechanical stimulation by a mitral valve prosthesis.

Electrophysiologic studies were performed in a woman who had two varieties of paroxysmal wide QRS tachycardia after mitral valve replacement with a Starr-Edwards prosthesis. One tachycardia originated in the left anterior fascicle; QRS complexes were 100 ms wide and resembled right bundle branch block with left posterior fascicular block, and a His bundle potential preceded each QRS by an interval of 20 ms (compared with 50 ms during sinus rhythm). The other tachycardia originated in the left ventricle. Clinical and echocardiographic observations suggested that the tachycardias were caused by

A family with myotonic dystrophy associated with diffuse cardiac conduction disturbances as demonstrated by His bundle electrocardiography.

In a family with myotonic dystrophy, three siblings showing ECG abnormalities were subjected to His bundle ECG studies. The duration of the disease ranged from 9 to 24 years. All three siblings had prolonged HV intervals (His-Purkinje conduction times) of 70 to 80 msec. The sister with the shortest duration of the disease had second-degree atrioventricular (AV) nodal block as well as left posterior fascicular block, the younger brother had left anterior fascicular block, and the elder brother with the longest history had first-degree AV block without bundle branch or fascicular block. With reg

Wolff-Parkinson-White VCG patterns that mimic other cardiac pathologies: a correlative study with the preexcitation pathway localization.

Vectorcardiograms (VCGs) of 44 patients with a Wolff-Parkinson-White (WPW) syndrome have been analyzed with the aim to correlate the QRS loop patterns with specific preexcitation sites. The VCG QRS loops were analyzed to determine whether conduction abnormalities and myocardial infarction (MI)-like patterns observed in the WPW syndrome could be related to specific preexcitation sites identified by surgery as well as by body surface potential mapping (BSPM). Left bundle branch block pattern was observed with anteroseptal (AS) preexcitation, anterior MI pattern was seen with lateral right ventri

Criteria for intraventricular conduction disturbances and pre-excitation. World Health Organizational/International Society and Federation for Cardiology Task Force Ad Hoc.

In an effort to standardize terminology and criteria for clinical electrocardiography, and as a follow-up of its work on definitions of terms related to cardiac rhythm, an Ad Hoc Working Group established by the World Health Organization and the International Society and Federation of Cardiology reviewed criteria for the diagnosis of conduction disturbances and pre-excitation. Recommendations resulting from these discussions are summarized for the diagnosis of complete and incomplete right and left bundle branch block, left anterior and left posterior fascicular block, nonspecific intraventric
