Paediatric patient with FLNC and CTNNA3 variants presenting with frequent premature ventricular contractions and systolic dysfunction: a case report.

We present the case of an 8-year-old male with frequent premature ventricular contractions and mild left ventricular dysfunction, who was found to carry heterozygous variants in both FLNC and CTNNA3. Medical therapy resulted in improved systolic function and reduced arrhythmia burden. This rare combination suggests a potential genotype-phenotype correlation in paediatric arrhythmogenic cardiomyopathy.

A case of endocardial mapping for the epicardial pure origin of premature ventricular complex using the time difference between bipolar and unipolar potential.

In endocardial mapping of premature ventricular complexes of intramural or epicardial origin, we must be aware that pure origin and exit do not always coincide. In this study, we describe a case of endocardial mapping for epicardial-side premature ventricular complexes of pure origin using the time difference between bipolar and unipolar potentials.

Catheter Ablation of Idiopathic Premature Ventricular Contractions Using Robotic Magnetic Navigation: Extensive Experience and Review.

AIMS: This retrospective analysis of our extensive experience focuses on idiopathic premature ventricular contractions (PVC) ablation guided by robotic magnetic navigation (RMN). Critical evaluation is given to the causes and impact of acute procedural failure. METHODS: This retrospective cohort study enrolled 824 patients (mean PVC burden 21.0 ± 6.6%) undergoing first-time RMN-guided PVC ablation. Acute failure was defined as recurrent clinical PVC during the observation period after the last radiofrequency (RF) application. The Navigation Index was calculated as the ratio of RF delivery tim

Cycle length variation assists in differentiation of atrial, ventricular, and sinus arrhythmias in horses at rest.

OBJECTIVE: Criteria for arrhythmia classification in horses are insufficient, particularly when differentiating atrial premature complexes (APCs) from sinus arrhythmia (SA). This study investigated RR and PP interval changes between SA, APCs, and ventricular premature complexes (VPCs). METHODS: 30 resting ECGs were reviewed: 10 each containing SA, APC, and VPC. Sequences of 10 RR and PP intervals were recorded for each arrhythmia (20 events/ECG). Percent RR and PP deviations were calculated (1) between sequential intervals (%ΔSEQPP and %ΔSEQRR, respectively) and (2) compared to averaged basel

Arrhythmias detected by implanted cardiac rhythm recorders in transthyretin cardiac amyloidosis.

INTRODUCTION AND OBJECTIVES: Arrhythmias are a common complication of transthyretin cardiac amyloidosis (ATTR-CM). We sought to characterize the prevalence, risk factors, and prognosis of arrhythmias in ATTR-CM using implantable loop recorders (ILRs). METHODS: ILRs were implanted in 49 patients with ATTR-CM (84% male; median age, 73.2 [66.2-78.8] years; 71% wild-type, 51% with known atrial fibrillation [AF]). Factors associated with the development of AF or pacemaker implantation were analyzed, and clinical outcomes were compared with those of a contemporary cohort of patients with 125 ATTR-C

Influence of mode of delivery on neonatal QTc.

OBJECTIVE: To evaluate the influence of mode of delivery on repolarisation and QT interval in the neonatal electrocardiogram (ECG). STUDY DESIGN: KUNO-Kids is a prospective, population-based birth cohort study. Neonates received an ECG within the first week of life; subgroups were divided according to mode of delivery. Effects on neonatal QT interval were tested by linear and logistic regression analyses, adjusting for age at ECG recording. RESULTS: In total, 712 neonates were included. The primary caesarean section group showed a significant higher mean QTc (regression coefficient 6.03, p =

Serum NT-proBNP Concentrations in Pediatric Patients with a History of Idiopathic Cardiac Arrhythmias: A Pilot Study.

Diagnosis and management of pediatric cardiac arrhythmias is challenging due to heterogeneous clinical presentation and limited availability of validated biomarkers. N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a well-established marker of myocardial stress; its role in pediatric arrhythmias has not been fully elucidated. This pilot study included 26 children and adolescents with a history of idiopathic ventricular arrhythmias (VAs, n = 17) or supraventricular tachycardia (SVT, n = 9) and 20 hospital-based controls without arrhythmias. Serum NT-proBNP was measured by ELISA. Group d

Long-term predictors of outcome in Brugada syndrome patients after implantable cardioverter-defibrillator implant.

AIMS: Various clinical risk factors impact the natural history of Brugada syndrome (BrS) and establish the basis of current clinical practice guidelines (CPG) indications for implantable cardioverter-defibrillator (ICD). Due to the time-dependent expressivity of BrS, their performance in predicting the actual occurrence of arrhythmic events after ICD implantation remains unknown. OBJECTIVES: To assess the occurrence of arrhythmic events based on the CPG indications and whether the genetic substrate can add to the prediction of arrhythmic events at follow-up. METHODS AND RESULTS: We enrolled

Histomorphometric characterization of cardiac nodes in cattle, ponies, and cats.

The sinoatrial node (SAN) and atrioventricular node (AVN) are essential components of the cardiac conduction system and play a critical role in initiating and propagating the electrical impulse. To characterise these cardiac nodes across different species, we examined the hearts of ten cattle, ten cats, and five male ponies without evidence of cardiac disease using conventional histological and immunohistochemical techniques. Morphometric parameters of the nodes and their constituent cells were quantified at different magnifications. The SAN displayed an oval morphology in all species. Its dia

Genetic and functional characterization of a novel SERPIND1 variant in a family with isolated gastric varices and venous thrombosis.

BACKGROUND: Isolated gastric varices type I (IGV1) is a rare but severe manifestation of splenic vein thrombosis. While thrombophilia is a recognized cause, the underlying genetic determinants are often elusive. This study investigates the case of a proband presenting with IGV1, deep vein thrombosis (DVT), and Long QT syndrome Type 2 (LQT2), aiming to elucidate the genetic basis and functional impact of the identified SERPIND1 variants. METHODS: Whole-exome sequencing identified candidate variants in the proband, which were validated by Sanger sequencing. Functional assessment included quanti

Arrhythmias in patients with transthyretin cardiac amyloidosis and the impact of contemporary disease-modifying therapies.

Transthyretin cardiac amyloidosis (ATTR-CM) is an increasingly recognized infiltrative cardiomyopathy characterized by extracellular deposition of misfolded transthyretin fibrils, leading to progressive myocardial stiffening, electrical remodeling, and conduction-system disease. These pathophysiological changes create a complex arrhythmic substrate, resulting in a high burden of atrial, bradyarrhythmia, and ventricular arrhythmias that significantly contribute to morbidity and mortality. Despite advances in disease recognition, the optimal management of arrhythmias in ATTR-CM remains challengi

Active Exsanguination Behind a Urinary Tract Infection (UTI) Diagnostic Label: A Quadruple-Hit Cardiorenal Collapse Following Ambulatory Urological Care.

We report the case of a 95-year-old man with a previous history of myocardial infarction, pulmonary oedema, and hypertension who developed recurrent collapse while undergoing ambulatory urological follow-up for a six-week history of macroscopic haematuria. Following a recent outpatient review, he had received oral antibiotics for a presumed urinary tract infection (UTI). Throughout this six-week ambulatory period, no full blood count or other blood tests were performed, leaving the extent of blood loss and its haematological consequences clinically occult. The patient subsequently experienced

Perinatal management and delivery-room stabilization in congenital complete heart block: an 11-year experience at a tertiary center.

Congenital complete heart block (CCHB) is a rare condition associated with significant perinatal hemodynamic instability. Although advances in prenatal diagnosis and pacing have improved survival, evidence guiding delivery-room stabilization remains limited. To describe our institutional experience in the perinatal management of CCHB, focusing on delivery-room stabilization and neonatal outcomes. We conducted a retrospective study at a tertiary center including newborns with CCHB over an 11-year period (2009-2020). Fetal characteristics, delivery-room interventions, and neonatal outcomes were

Seizures Secondary to Aconitum Plant Poisoning: a case report.

BACKGROUND: Aconitum plant poisoning typically manifests as isolated early cardiotoxicity or late neurotoxicity. However, the dynamic temporal evolution from arrhythmia-driven cardiogenic syncope to true epileptic seizures mediated by direct central nervous system injury remains rarely documented and poorly characterized. CASE PRESENTATION: We report a 53-year-old Han Chinese male who presented with ventricular tachycardia rapidly degenerating into ventricular fibrillation and cardiac arrest following ingestion of homemade Aconitum kusnezoffii (caowu)-infused liquor. Concentration-QTc Analysis Supporting Thorough QT Study Waiver for Forvisirvat: Electrocardiogram Assay Sensitivity Demonstrated via Meal Effect.

Concentration-QTc analysis from Phase 1 studies is increasingly used to evaluate QTc prolongation risk as an alternative to thorough QT (TQT) studies. According to International Conference on Harmonization E14 documents, such analyses should include exposures at twice the highest clinically relevant exposure instead of a positive control to establish electrocardiogram (ECG) assay sensitivity; however, achieving this may be challenging when the highest dose in Phase 1 studies is restricted for various reasons. This study evaluated the QTc prolongation risk of forvisirvat via concentration-QTc a

Cardiac repolarization wave dynamics and its role in arrhythmogenesis.

Premature ventricular contractions (PVCs) are commonly associated with action potential duration (APD) prolongation through early afterdepolarizations (EADs) or repolarization failure (RF), yet the mechanisms linking spatial heterogeneity of repolarization to PVC initiation remain unclear. Previous studies have emphasized the role of repolarization gradients, but a unifying dynamical framework connecting repolarization wave (RW) propagation and PVC initiation is lacking. Here, we investigate PVC initiation in one-dimensional heterogeneous tissue using models of increasing biophysical complexit

Excessive supraventricular ectopic activity and new-onset ischemic stroke: a retrospective cohort study with prospective outcome ascertainment.

BACKGROUND: Excessive supraventricular ectopic activity (ESVEA), a marker of both paroxysmal atrial fibrillation (pAF) and underlying atrial cardiomyopathy, may serve as an earlier electrographic predictor of cryptogenic ischemic stroke than pAF itself. This study aimed to investigate the association between ESVEA and the risks of new-onset ischemic stroke and all-cause mortality in an Asian population, and to explore a potential data-driven threshold that requires external validation. METHODS: In this retrospective cohort study with prospective outcome ascertainment, data were collected from

Life-Threatening Ventricular Tachycardia and Transient Brugada-Like Electrocardiographic Abnormalities Caused by Severe Hyperkalemia Due to Urinary Retention in a 55-Year-Old Man.

BACKGROUND Hyperkalemia is a potentially fatal electrolyte disturbance that typically manifests as progressive conduction abnormalities, bradyarrhythmias, and sine-wave electrocardiographic changes. In rare cases, however, severe hyperkalemia also presents with malignant ventricular tachyarrhythmias and transient electrocardiographic abnormalities involving the right precordial leads, potentially leading to diagnostic confusion with primary arrhythmic disorders. CASE REPORT A 55-year-old man with no known cardiovascular disease presented with palpitations, dyspnea, and diaphoresis. Electrocard

Epicardial fat enhances prediction of exercise-induced hypertension and ventricular arrhythmias in asymptomatic normotensive individuals.

BACKGROUND: Exercise-induced hypertension (EIH) is recognized as an independent cardiovascular risk factor. Epicardial fat thickness (EFT) has been implicated in various cardiovascular pathologies. However, the relationship between EFT, EIH, and ventricular arrhythmias remains poorly characterized. This study aimed to investigate the predictive value of EFT for both EIH and premature ventricular beats (PVB). METHODS: A total of 2658 participants were initially screened for eligibility, and normotensive participants of age > 18 were considered for enrollment. All participants underwent a compr
