Genomic Structural Equation Modeling Combined With Post-GWAS Analysis Identifies Two Risk Gene Loci and Functionally Sensitive Genes Associated With Cardiac Conduction Block.

BACKGROUND: Cardiac conduction disorders (CCDs) represent a broad spectrum of severe cardiovascular conditions associated with syncope and sudden cardiac death. Therefore, identification of reliable biomarkers is necessary to significantly improve the diagnostic accuracy and therapeutic outcomes of CCDs. This study analyzed GWAS summary datasets using a genomic structural equation model (Genomic-SEM), fine mapping, linkage disequilibrium score regression (LDSC), and two-sample Mendelian randomization (TSMR) analyses to identify genetic loci and genes associated with CCDs. METHODS: GWAS summar

Atrioventricular synchrony maintained by a dual-chamber leadless pacemaker in real-world conditions.

AIMS: A dual-chamber leadless pacemaker (LP) system relies on a beat-to-beat, wireless, implant-to-implant (i2i™) communication between implanted devices in the right atrium and right ventricle (A-to-V and V-to-A). Atrioventricular (AV) synchrony was evaluated during an ambulatory 24 h period of daily living to determine if its effectiveness can be maintained under real-world conditions. METHODS AND RESULTS: A prospective, single-arm, multicentre clinical study (ClinicalTrials.gov identifier NCT05252702) evaluated the overall safety and performance of the dual-chamber LPs in patients with sta

uPVC-Net: A Universal Premature Ventricular Contraction Detection Deep Learning Algorithm.

INTRODUCTION: Premature Ventricular Contractions (PVCs) are common cardiac arrhythmias originating from the ventricles. Accurate detection remains challenging due to variability in electrocardiogram (ECG) waveforms caused by differences in lead placement, recording conditions, and population demographics. METHODS: We developed uPVC-Net, a universal deep learning model to detect PVCs from any single-lead ECG recordings. The model is developed on four independent ECG datasets comprising a total of 8.3 million beats collected from Holter monitors and a modern wearable ECG patch. uPVC-Net employs

Very High-Power Short-Duration Ablation for Premature Ventricular Complexes From Sites With Suboptimal Catheter Stability.

INTRODUCTION: While very high-power short-duration (vHPSD) ablation has been shown to be safe and effective for ablation of atrial fibrillation, the utility of vHPSD ablation for targeting premature ventricular complexes (PVCs) remains unclear. We aimed to describe our experience of PVC ablation using vHPSD ablation targeting areas with suboptimal catheter contact. METHODS AND RESULTS: We included 8 patients (mean age 66.5 ± 11.3 years, 77% female gender, mean LV ejection fraction 52.8 ± 8.2%, baseline PVC burden 23.3 ± 10.1% [range 9-41%]) with PVCs originating from intracavitary structures

Alternatives to right ventricular pacing in patients with atrioventricular block: can we prevent pacing-induced cardiomyopathy?

INTRODUCTION: Right ventricular apical pacing (RVAP) has been a standard treatment for patients experiencing symptomatic advanced atrioventricular (AV) block. However, due to electrical and subsequent mechanical dyssynchrony, RVAP might lead to pacing-induced cardiomyopathy (PICM). Various predisposing risk factors for PICM have been identified. AREAS COVERED: This review, based on a literature search of PubMed and Scopus databases (up to April 2025), focuses on alternative pacing strategies, including conduction system pacing (CSP) and biventricular pacing (BiVP), to reduce PICM. Left bundle branch area pacing in patients with and without interventricular septal hypertrophy: a systemic review and meta-analysis.

BACKGROUND: Conduction disease is common in patients with interventricular septal hypertrophy (IVSH). Left bundle branch area pacing (LBBAP) is a physiological pacing approach which preserves interventricular synchrony. There is limited data regarding the success rate and outcomes of LBBAP in patients with IVSH. In this study, we aimed to synthesize all available evidence on the feasibility and outcomes of LBBAP in patients with IVSH compared to those without IVSH. METHODS: We systematically reviewed bibliographic databases to identify studies published through August 2025. We included observ

Automated Detection of Normal, Atrial, and Ventricular Premature Beats from Single-Lead ECG Using Convolutional Neural Networks.

Accurate detection of premature atrial contractions (PACs) and premature ventricular contractions (PVCs) in single-lead electrocardiograms (ECGs) is crucial for early identification of patients at risk for atrial fibrillation, cardiomyopathy, and other adverse outcomes. In this work, we present a fully convolutional one-dimensional U-Net that reframes beat classification as a segmentation task and directly detects normal beats, PACs, and PVCs from raw ECG signals. The architecture employs a ConvNeXt V2 encoder with simple decoder blocks and does not rely on explicit R-peak detection, handcraft

Arrhythmia substrates accessible from the aortic root and immediate sub-aortic areas: Mapping and ablation.

The aortic root overlies the left ventricular ostium and the interatrial septum. Muscle sleeves supporting the aortic sinuses and extensions to the inter-leaflet triangle are sources for outflow tract arrhythmias. The sinuses of Valsalva, in addition, provide access to the inter-atrial septum and left ventricular infra-aortic regions, where substrates for arrhythmias reside in both idiopathic arrhythmias and structural heart disease. This review summarizes the anatomical relationship of the aortic root to the various arrhythmic substrates and discusses approaches to ablation.

P-wave peak time: An emerging electrocardiographic marker of atrial stress and ischemic burden-review.

The 12‑lead electrocardiogram (ECG) remains a cornerstone of cardiovascular assessment, providing a noninvasive window into cardiac electrical and structural function beyond other markers of ischemia and arrhythmias. P-wave peak time (PWPT), defined as the interval from P-wave onset to its maximal amplitude, has emerged as a novel electrocardiographic marker of atrial conduction and hemodynamic stress. Increasing evidence suggests that PWPT prolongation accompanies elevated ventricular filling pressures, atrial stretch, and chronic atrial remodeling across a spectrum of ischemic conditions, in

Unmasking Brugada ECG Pattern in Myotonic Dystrophy Type 2 With an ANK2 Variant.

BACKGROUND: Brugada syndrome has been reported in myotonic dystrophy type 1, whereas its association with myotonic dystrophy type 2 (DM2) remains largely unexplored. CASE SUMMARY: We report a patient with genetically confirmed DM2 who presented with an electrocardiogram showing a Brugada type-2 ECG pattern. Sodium-channel blocker testing unmasked a diagnostic type 1 Brugada pattern. Genetic analysis excluded SCN5A variants and identified a rare heterozygous ANK2 missense variant, a gene involved in cardiac electrical stability. CONCLUSION: This case expands the spectrum of Brugada phenotypes

Inappropriate ICD shock occurred 34 years after the last syncope in a symptomatic Brugada patient: does arrhythmia risk shift from VF to AF with age in Brugada syndrome?

Atrial fibrillation (AF) is a significant cause of inappropriate implantable cardioverter defibrillator (ICD) shocks. A male patient in his 70s was diagnosed with Brugada syndrome (BrS) in his 40s following ventricular fibrillation-induced syncope. He had implanted a single-lead ICD in his 50s. 34 years after the last syncope, the ICD delivered four inappropriate shocks due to paroxysmal AF, successfully treated with pulmonary vein isolation. Ageing likely increased AF burden, despite its infrequency during follow-up. This case highlights the limitations of single-lead ICDs in AF detection and

Heart Rate Variability Nomogram Predicts Atrial Fibrillation in Patients with Moderate to High Burden of Premature Ventricular Complexes.

Background and Objectives: There is a well-established correlation between premature ventricular contractions (PVCs) and atrial fibrillation (AF), with a higher burden of PVCs increasing the likelihood of new-onset AF. This study aims to investigate the impact of heart rate variability (HRV) on the onset of AF in patients with moderate to high burdens of PVCs, as observed through 24 h ambulatory electrocardiogram (ECG) analysis. Materials and Methods: Our study was a retrospective analysis involving 187 patients at a single tertiary center. We analyzed PVC counts from 24 h ECG recordings, cate
