Device occlusion of perimembranous ventricular septal defect is gaining popularity with the emergence of newer, softer occluders and improved technical know-how. We report a 26-year-old lady with a moderate size perimembranous ventricular septal defect who had a new onset of bundle branch block shortly after device closure. The patient subsequently developed a complete atrio-ventricular heart block.

INTRODUCTION: Conduction system pacing (CSP) has emerged as an ideal physiologic pacing strategy for patients with permanent pacing indications. We sought to evaluate the safety and feasibility of CSP in a consecutive series of unselected patients with congenital heart disease (CHD).
METHODS: Consecutive patients with CHD in which CSP was attempted were included. Safety and feasibility, implant tools and electrical parameters at implant and at follow-up were evaluated.
RESULTS: A total of 20 patients were included (10 with a previous device). A total of 10 patients had complex forms of CHD,

BACKGROUND Brugada syndrome is a rare inherited channelopathy that can lead to sudden cardiac death. The discovery of new variants of variable penetrance along with the current guidance for cascade family screening can be expected to lead to an increase in identified asymptomatic carriers of potentially causative mutations of channelopathies. A single coronary artery is a rare congenital anomaly of the coronary anatomy. We present a rare case of a 59-year-old woman with a family history of Brugada syndrome with the c.664C>T variant of the SCN5A gene, congenital absence of the right coronary ar

Background Congenital ventricular septal defects (VSDs) are considered to have benign long-term outcome when treated correctly in childhood. However, abnormal parameters are described in younger adults, including impaired heart rate variability (HRV). It is not known whether such abnormalities will deteriorate with age. Therefore, HRV and cardiac events, such as premature ventricular contraction, were evaluated in patients aged >40 years with congenital VSDs and compared with healthy peers. Methods and Results A total of 30 surgically closed VSDs (51±8 years, repair at median age 6.3 years wit

Atrial septal defect (ASD) is the most common congenital heart disease observed in adult. Several ECG findings are considered sensitive for the diagnosis of ASD. We describe a 50 years old man who displayed Crochetage sign, incomplete right bundle branch block (IRBBB) and right ventricular strain pattern on ECG. Crochetage sign is highly specific for ASD and it correlates with shunt severity. The diagnostic specificity for ASD increases if the R waves have both Crochetage patterns and IRBBB. It is important not to confuse Crochetage signs with IRBBB abnormalities on ECG. Our patient was ultima

A 61-years-old male underwent left bundle branch pacing for nonischemic dilated cardiomyopathy with recurrent heart failure. Left bundle branch pacing (LBBP) resulted in reduction in QRS duration along with improvement in left ventricular ejection fraction (LVEF) to 64% during follow-up. Two years after implantation he had recurrence of symptoms along with decline in LVEF to 51%. Late lead dislodgement was diagnosed and re-do LBBP was planned. The lead was extracted en-masse without complication and a new 3830 lead was positioned deep inside the proximal septum to capture the left bundle. Post

AIMS: The management of heart failure (HF) in young children is challenging. The present study aimed to clarify the effect of left univentricular epicardial pacing on dilated cardiomyopathy with left bundle branch block (LBBB) in children.
METHODS AND RESULTS: A total of five cases (30.86 ± 16.39 months, three female) of children weighing 5.8-15 kg with dilated cardiomyopathy and LBBB were included in this study. LBBB in one child occurred after device closure of peri-membranous ventricular septal defects, and the remaining four were idiopathically discovered early after birth. Before implant

INTRODUCTION AND OBJECTIVES: Pregnancy can affect the cardiac conduction system, causing changes to the electrocardiographic indices that may mimic pathologic ones. The aim of this study was to discover these changes in normal pregnant women.
METHODS: We included 103 healthy pregnant women, aged between 18-35 years. Standard 12-lead electrocardiogram was performed in the first and third trimester and electrocardiographic indices were calculated. The patients were classified in two age categories.
RESULTS: The mean PR interval in trimester one (T1) was 133.36±20.9 ms and in third trimester (T

We report a 25-year-old woman who was diagnosed with atrial septal defect (ASD). An ECG showed only first-degree atrioventricular block and incomplete right bundle branch block. One day after the percutaneous ASD closure, she had a slight fever and an ECG showed a type 1 Brugada pattern. ECG characteristics of ASD are similar to those of a Brugada ECG. This case is rare combination of Brugada syndrome with ASD.

BACKGROUND: We aimed to evaluate early outcomes of septal myectomy in patients with hypertrophic cardiomyopathy.
METHODS: We retrospectively analyzed data collected over a 9-year period from 583 patients who underwent septal myectomy for hypertrophic cardiomyopathy at our institution.
RESULTS: The mean age was 55.7 ± 13.1 years, and 338 (58%) patients were in New York Heart Association class III or IV. There were 11 (1.9%) early deaths, including 3 (0.5%) intraoperative deaths. Early mortality was lowest after isolated septal myectomy (0.8%) and highest after concomitant mitral valve replace

BACKGROUND: The risk factors for complete atrioventricular block (CAVB) after device closure of perimembranous ventricular septal defect (pmVSD) remain unclear.
OBJECTIVE: The purpose of this study was to analyze the incidence and risk factors for CAVB after device closure for pmVSD.
METHODS: We reviewed 1884 patients with pmVSD who had undergone successful device occlusion between June 2005 and January 2020. Permanent CAVB was defined as CAVB requiring implantation of a permanent pacemaker (PPM) or extraction of the occluder.
RESULTS: In total, 14 patients (0.7%) developed permanent CAVB.

Tetralogy of Fallot (ToF) is one of the most prevalent cyanotic congenital heart disease (CHD). Subsequent to the introduction of surgical repair, long-term outcomes for patients with ToF have improved significantly. Pulmonary atresia with ventricular septal defect (PA-VSD) may be considered as an extreme form of classic ToF. Surgical repair not only corrects the haemodynamic but also creates scars that will be source for ventricular arrhythmias. Patients with tetralogy of Fallot are at risk for ventricular arrhythmias and sudden cardiac death. We present the successful treatment of a 7-years-

BACKGROUND: Multisystem inflammatory syndrome in children (MIS-C) is a rare life-threatening clinical condition that can develop in patients younger than 21 years of age with a history of infection/exposure to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). The cardiovascular system is a main target of the inflammatory process that frequently causes myocardial dysfunction, myopericarditis, coronary artery dilation, hypotension, and shock. Multisystem inflammatory syndrome in children-associated myocarditis is usually characterized by fever, tachycardia, non-specific electrocardio

Ductus diverticulum is either a remnant of the ductus arteriosus or the right dorsal aortic root. The most common diagnosis is the differentiation of an aortic isthmus pseudoaneurysm from a type III ductal diverticulum. Both aortic entities occur at roughly the same anatomical location often leading to diagnostic confusion. Aortic dissection should be considered if it is Takayasu's disease. We are reporting the case of a 31-year-old woman with chest pain and muscle weakness, functional impotence of the left upper limb with intermittent palpitations. Thoraco-abdominal CT angiography was in favo

BACKGROUND: QRS duration and morphology including left bundle branch block (LBBB) are the most widely used electrocardiogram (ECG) markers for assessing ventricular dyssynchrony and predicting heart failure (HF). However, the vectorcardiographic QRS area may more accurately identify delayed left ventricular activation and HF development.
OBJECTIVE: We investigated the association between QRS area and incident HF risk in patients with LBBB.
METHODS: By crosslinking data from Danish nationwide registries, we identified patients with a first-time digital LBBB ECG between 2001 and 2015. The vect
