Transcription Factor TBX5 Enhances Structural Complexity and Calcium Transient Synchronization in Spontaneously Active Neonatal Rat Cardiomyocyte Cultures.

Transduced cardiomyocyte-based constructs provide a promising platform for in vitro modeling cardiac disease and a source of cell products for treating cardiac diseases and, particularly, conduction system disorders. Neonatal rat cardiomyocyte cultures were transduced with a vector carrying a gene for the TBX5 transcription factor, which is involved in controlling the bioelectrical properties of cardiomyocytes. TBX5 overexpression was found to increase the number of functionally active cardiomyocyte clusters in a culture, to enhance their morphological complexity, to stimulate the establishmen

Physical activity and Brugada ECG pattern during 12-lead Holter monitoring.

BACKGROUND: Brugada Syndrome (BrS) is a hereditary channelopathy with a dynamic ECG pattern, often latent at rest. While several triggers can unmask the type 1 pattern, the influence of physical activity (PA) remains uncertain. This study investigated whether PA is associated with spontaneous type 1 ECG, its unmasking in induced cases, and the occurrence of arrhythmic events in Brugada athletes. METHODS: We enrolled 202 BrS patients (18-55 years), collecting demographic data, PA levels, resting ECG, and 12-lead Holter. Subjects exercising ≥4 hours/week were defined as "recreational athletes"

Left bundle branch area pacing vs right ventricular pacing for atrioventricular block: the MELOS RELOADED study.

BACKGROUND AND AIMS: Left bundle branch area pacing (LBBAP) promotes physiological synchronous activation of the left ventricle and may be particularly beneficial in patients with atrioventricular block (AVB), but its mortality benefit remains unclear. This study aims to compare long-term survival in AVB patients receiving either LBBAP or right ventricular pacing (RVP) and to analyse predictors of mortality during LBBAP. METHODS: MELOS RELOADED, a multicentre European collaboration, was a registry-based study of pacemaker patients with AVB, left ventricular ejection fraction (LVEF) >40% and v

Short-term efficacy and safety of radiofrequency ablation for idiopathic premature ventricular contractions.

Premature ventricular contractions (PVCs) are common arrhythmias that may impair quality of life and lead to PVC-induced cardiomyopathy. Radiofrequency ablation (RFA) has emerged as an effective treatment option for symptomatic and drug-refractory idiopathic PVCs, yet real-world data remain limited. This retrospective single-center study included 53 patients who underwent RFA for idiopathic PVCs between December 2022 and June 2024. Demographic, clinical, and procedural data were analyzed. Acute procedural success, 6-month recurrence, and complications were evaluated according to PVC localizati

Right internal jugular implantation of the AVEIR leadless pacemaker: a case series demonstrating feasibility and safety.

OBJECTIVES: Leadless pacemakers provide an alternative to transvenous pacing in patients at risk for lead- and pocket-related complications. While the femoral vein is the standard route of implantation, certain anatomical or clinical factors may make this approach technically challenging. The objective of this study was to evaluate the feasibility and safety of AVEIR (Abbott) implantation through the right internal jugular (RIJ) vein in patients in whom the femoral approach was not feasible. METHODS: The authors report a single-center case series of patients who underwent AVEIR implantation u

The Underscreening of Cardiac Amyloidosis in Patients With Pacemakers-A Single Centre Retrospective Audit.

BACKGROUND: Cardiac amyloidosis is commonly associated with cardiac conduction disease. We sought to determine the prevalence of advanced conduction disease requiring a pacemaker in patients with known cardiac amyloidosis to evaluate current screening practices among patients receiving pacemakers. METHODS: A two-stage analysis was conducted. First, a retrospective chart review was conducted for all patients with known transthyretin cardiac amyloidosis who attended the Cardiac Amyloidosis clinic at the Victorian and Tasmanian Amyloidosis Service (Australia) from 2014 to 2024 to identify those

Novel insights on ventricular arrhythmias in cardiac amyloidosis: an updated narrative review.

Cardiac amyloidosis (CA) poses a significant risk for ventricular arrhythmias and sudden cardiac death. Arrhythmic risk differs by subtype: the light-chain variant is often more arrhythmogenic due to direct light-chain toxicity, while the transthyretin form is dominated by conduction system disease. Although advanced imaging, particularly cardiac magnetic resonance, accurately predicts all-cause mortality, it fails to reliably distinguish patients who will have a shockable arrhythmic event from those who will die of progressive heart failure or electromechanical dissociation. This limitation u

Clinical Value of High-Channel 12-Lead Holter in Assisting Early Identification of Arrhythmias in High-Risk Populations.

AIMS/BACKGROUND: Cardiac arrhythmias are common in high-risk populations, often presenting paroxysmally and asymptomatically, leading to missed diagnoses and increased cardiovascular risks. This study aimed to retrospectively analyze the clinical value of high-channel 12-lead Holter electrocardiogram (ECG) in the early identification of arrhythmias in high-risk populations and to provide a reference for cardiovascular risk prevention and control in these groups. METHODS: Clinical data from 300 patients who met the criteria for high-risk populations and received diagnosis and treatment in Zibo

Therapeutic Strategies Targeting the Molecular Pathogenesis of Myotonic Dystrophy Type 1: Current Status and Future Directions.

Myotonic dystrophy type 1 is the most prevalent adult-onset muscular dystrophy and is characterized by progressive muscle weakness, myotonia, cardiac conduction defects, endocrine dysfunction, and central nervous system involvement. Myotonic dystrophy type 1 is caused by an unstable CTG repeat expansion in the 3' untranslated region of the DMPK gene, which produces toxic CUG-expanded transcripts that sequester RNA-binding proteins such as Muscleblind-like, induce widespread alternative splicing defects, and drive an RNA gain-of-function mechanism rather than simple DMPK haploinsufficiency. [Consensus recommendations on the diagnosis and treatment of hyperkalemia of the Austrian Society of Nephrology 2026].

Hyperkalemia represents a potentially life-threatening electrolyte disturbance that often necessitates prompt diagnosis and treatment. Although uncommon in the general population, the risk is substantially increased in the presence of comorbidities, such as chronic kidney disease, diabetes mellitus or heart failure. Pharmacological therapies that interfere with the renin-angiotensin-aldosterone system further increase this risk. The treatment of hyperkalemia is based on acute interventions to stabilize cardiac conduction abnormalities, promote intracellular potassium redistribution, and enhanc

Integrated Multi-Cohort Transcriptomic Analysis Reveals Molecular Networks and Signaling Pathway Landscape of Aconitine-Induced Cardiotoxicity and Myocardial Injury.

Aconitine-induced cardiotoxicity is a serious adverse effect of Aconitum-containing medicines, with myocardial injury and malignant ventricular arrhythmias representing important clinical manifestations. However, the molecular responses underlying aconitine-induced myocardial injury remain incompletely understood. This study aimed to construct an integrated molecular network and signaling pathway landscape of aconitine-induced cardiotoxicity through multi-cohort transcriptomic analysis and to identify key hub genes associated with myocardial injury and ventricular tachyarrhythmias. Rat myocard

NS1643 enhances the activity of T1019PfsX38 and Q1070X hERG channels associated with long QT syndrome type 2.

Type 2 long QT syndrome (LQT2) increases the risk of cardiac arrest and sudden death due to reduced rapid delayed rectifier potassium currents (IKr) in the heart. Caused by loss-of-function genetic variants in the hERG K+ channel, LQT2 might benefit from variant-targeted therapy approaches. T1019PfsX38 and Q1070X are LQT2-causing variants reported in Oman and Saudi Arabia. Although nonsense-mediated mRNA decay (NMD) has been proposed to contribute to reduced expression of these truncating variants, it remains unclear whether residual mutant hERG channel function can be pharmacologically enhanc

Conservation and divergence of transcriptional heterogeneity in the cardiac conduction system.

The cardiac conduction system (CCS) consists of specialized cardiomyocytes that initiate and propagate electrical activity throughout the heart. While the transcriptional programs underlying CCS development and function have been studied within individual species, how these programs compare across species and developmental stages remains unclear. Here, we present a comprehensive cross-species and cross-stage analysis of the CCS transcriptome using single-cell/single-nucleus RNA sequencing and spatial transcriptomic datasets from human, mouse, rat, zebrafish, and medaka hearts. We identify shar

Left Bundle Branch Area Pacing: Current Clinical Practice and Future Perspectives.

Conventional right ventricular pacing (RVP) is associated with electromechanical dyssynchrony and may contribute to pacing-induced cardiomyopathy (PICM). Left bundle branch area pacing (LBBAP) has emerged as a physiological pacing strategy that engages the native conduction system. This review summarizes the current clinical evidence and future perspectives for LBBAP in both bradycardia and heart failure indications. LBBAP has been associated with improved electrical synchrony, preservation of left ventricular function, and lower rates of heart failure hospitalization and mortality compared wi

Severe pulmonary valve stenosis associated with advanced atrioventricular conduction disease: A rare case report.

Congenital pulmonary valve stenosis is a common cause of right ventricular (RV) outflow tract obstruction. Advanced atrioventricular (AV) block in congenital heart disease is usually associated with abnormalities of AV septation or conduction system malposition. An association between isolated pulmonary valve stenosis and advanced AV block has not previously been reported. A 16-year-old male presented with exertional dyspnea and recurrent presyncope. Electrocardiography (ECG) revealed 2:1 AV block. Transthoracic echocardiography demonstrated severe valvular pulmonary stenosis with a peak gradi

Apheresis-Triggered Torsades De Pointes in Congenital Long QT Syndrome Type 2: Citrate-Related Arrhythmia Risk and Safe Modality Switching During Myasthenic Crisis.

Citrate anticoagulation during therapeutic plasma exchange (TPE) chelates divalent cations and can precipitate torsades de pointes (TdP) in patients with pre-existing repolarization vulnerability. We report a 44-year-old woman with genetically determined long QT syndrome Type 2 (LQT2) presenting in myasthenic crisis due to anti-MuSK-positive myasthenia gravis. TPE was initiated urgently before confirmatory antibody results were available, as alternative rapid immunomodulatory options were not immediately accessible. Despite prophylactic electrolyte supplementation, the corrected QT interval (Q

The first case of Reel Syndrome in a dog after pacemaker implantation: A case report.

Reel Syndrome is a rare complication of permanent pacemaker implantation, characterised by the rotation of the device around its transverse axis, which causes the lead to coil around the generator and displacement, leading to pacing dysfunction. This report presents the first documented case of Reel Syndrome in a dog. The, 6-year-old female Labrador Retriever required a pacemaker implantation due to a diagnosed third-degree atrioventricular block. Forty-eight hours after pacemaker implantation, the dog developed clinical signs, including diaphragmatic contractions and a decrease in heart rate.
