Delayed cardiac conduction disease in a young adult following childhood Chagas infection.

PRESENTATION: A Bolivian man in his 30s presented to the ED with non-exertional chest pain and intermittent second-degree atrioventricular (2:1) block. DIAGNOSIS: Given a history of untreated childhood Trypanosoma cruzi infection and family history of Chagas heart disease, late cardiac manifestation was suspected. Structural and ischaemic causes were excluded. TREATMENT: Antiparasitic therapy was initiated. Pacemaker implantation was considered but deferred following shared decision-making due to absence of symptoms. DISCUSSION: Chagas heart disease (CHD) may present decades after infection

Unraveling the mystery: congenital absence of the portal vein and its cardiac implications.

A 25-year-old male with tachycardia and right heart enlargement was diagnosed with atrial-ventricular nodal reentrant tachycardia (AVNRT) and successfully treated with radiofrequency ablation. Cardiac MRI confirmed right heart enlargement but ruled out cardiomyopathy. Further imaging revealed a rare congenital absence of the portal vein (CAPV) with an associated portosystemic shunt. This case highlights the importance of considering CAPV in unexplained cardiac symptoms and structural heart changes.

Two-Dimensional Eye Movement Characteristics of Infantile Nystagmus in Eccentric Gaze.

PURPOSE: Infantile nystagmus (IN) eye oscillations are usually predominantly horizontal. However, the contribution of the vertical component and its relationship with the horizontal component as a function of gaze angle remain unknown. This study aimed to quantify fixation performance at horizontal gaze angles, in two dimensions. METHODS: Eye-tracking data from 17 participants with IN, fixating nine horizontal gaze positions spanning ±20°, were evaluated using a bivariate probability density function method, wherein the derived 68% isocontour of eye position samples were analyzed to quantify

Targeted therapies in non-small cell lung cancer and their cardiovascular impact: mechanisms, clinical profiles, and strategies of management.

The therapeutic landscape of non-small cell lung cancer (NSCLC) has been profoundly transformed by the widespread adoption of molecular profiling and the development of targeted therapies, like tyrosine kinase inhibitors (TKIs), antibody-drug conjugates (ADCs), and bispecific antibodies (BsAbs). These agents have significantly improved survival and quality of life in molecularly selected subgroups, potentially converting NSCLC into a chronic disease requiring prolonged treatment exposure. However, extended survival has led to increasing recognition of cancer treatment-related cardiovascular (C

Influence of age and sex on the diagnostic yield of inherited cardiac conditions in sudden arrhythmic death syndrome decedents.

AIMS: Sudden arrhythmic death syndrome (SADS) refers to a sudden death, which remains unexplained despite comprehensive post-mortem examination and a toxicological screen. We aimed to investigate the impact of age and sex on the overall diagnostic yield and underlying aetiology in decedents with SADS using a combined approach of familial evaluation (FE) and molecular autopsy (MA). METHODS AND RESULTS: Consecutive referrals to a single centre for FE only, MA only or both, following a SADS death were included. First-degree family members underwent comprehensive FE and decedents with post-mortem

Long PR Interval Leads to Atrial Remodeling, Fibrosis and Inducible Atrial Fibrillation in a Swine Model.

BACKGROUND: Pacing algorithms have been developed to minimize the risk of pacing-induced ventricular dysfunction at the expense of a prolonged PR interval. However, a long PR interval has been associated with incident atrial fibrillation (AF), and the impact of a prolonged PR interval on atrial remodeling remains unclear. OBJECTIVES: This study investigated the impact of a prolonged PR interval on atrial remodeling in swine. METHODS: In total, 24 Yucatan miniswine were divided into 3 groups: control (AAI 90 beats/min, n = 8), standard (160/180 ms, n = 8), and long (350 ms, n = 8) atrioventri

Diagnostic accuracy of electrocardiogram algorithms for differentiating left from right outflow tract ventricular arrhythmia: a systematic review and network meta-analysis.

BACKGROUND: Accurate pre-ablation differentiation between left (LVOT) and right (RVOT) ventricular outflow tract arrhythmias (OTVAs) using ECG algorithms is essential for decision on vascular access and treatment strategy. However, the most reliable ECG algorithm remains unclear. We conducted a systematic review and network meta-analysis (NMA) to compare the diagnostic accuracy of available algorithms. METHODS: We searched MEDLINE, EMBASE and Cochrane databases through 7 May 2025 for studies evaluating ECG algorithms against ablation-confirmed OTVA origin. A Bayesian diagnostic test accuracy

Major arrhythmias in non-dilated left ventricular cardiomyopathy: a novel prediction score.

BACKGROUND AND AIMS: The prediction of the first major arrhythmic event (MAE) is still an unmet need in the recently defined scenario of non-dilated left ventricular cardiomyopathy (NDLVC). METHODS: A cohort of 337 patients with NDLVC and no history of MAE was retrospectively identified at two large centres. Patient-tailored diagnostic workup included cardiac magnetic resonance (CMR), endomyocardial biopsy, and genetic testing. The primary endpoint was the occurrence of the first MAE, including sustained ventricular tachycardia (VT), ventricular fibrillation, or appropriate implantable cardio

AI-Based QRS Onset Detection in the Early Ventricular Activation Site ECGs.

Identifying the onset of the QRS complex is an important step for localizing the site of origin (SOO) of premature ventricular complexes (PVCs) and the exit site of Ventricular Tachycardia (VT). However, identifying the QRS onset is challenging due to signal noise, baseline wander, motion artifact, and muscle artifact. Furthermore, in VT, QRS onset detection is especially difficult due to the overlap with repolarization from the prior beat. In this study, 7706 captured bipolar pacing beats (Stim-QRS < 40 ms) pooled from 384 anatomically widely dispersed pacing sites of 15 patients were used fo

Zero-Fluoroscopy Versus Conventional Catheter Ablation for Premature Ventricular Contractions.

BACKGROUND: Catheter ablation of premature ventricular contractions (PVCs) is an effective treatment modality, but traditionally involves fluoroscopy, which poses radiation risks. Recently, zero-fluoroscopy (ZF) techniques have emerged as a safer alternative, especially in young patients and those requiring multiple procedures. METHODS: This retrospective, single-center study evaluated 150 patients who underwent PVC ablation between January 2023 and March 2025. Patients were divided into two groups: those treated with ZF (n = 75) and conventional fluoroscopy (CF) (n = 75). Procedural characte

The Effect of Therapeutic Hypothermia on the QT Interval in Postcardiac Arrest Cases.

Postcardiac arrest (PCA) syndrome is associated with high cardiovascular morbidity and mortality. Prolongation of the QT interval can lead to life-threatening ventricular arrhythmias and sudden cardiac death. Therapeutic hypothermia (TH) is widely used to improve neurological outcomes in PCA patients, but its effects on the QT interval remain a subject of investigation. This study aimed to evaluate the impact of TH on corrected QT (QTc) intervals in PCA patients. A total of 48 patients (mean age 63.2 ± 11.3 years) who survived cardiac arrest and underwent TH were included. Standard 12-lead ele

Magnesium Supplementation and Tachyarrhythmias: A Nonrandomized Clinical Trial.

IMPORTANCE: Magnesium supplementation is regularly given to acutely ill patients with serum levels below the standard reference range, primarily for the prevention of tachyarrhythmias, such as atrial fibrillation. The evidence for this practice and the specific treatment thresholds used is limited. Conventional observational studies of this question are at risk of confounding by indication and other biases. OBJECTIVE: To determine whether giving magnesium supplementation to patients with hypomagnesemia, as defined by a given institution's usual treatment cutoff, reduces adverse clinical outco
