Fulminant flecainide intoxication in an adolescent: cardiogenic shock requiring VA-ECMO support.

We describe the case of a previously healthy adolescent female who presented with chest discomfort and near-syncope, accompanied by trifascicular block and hypotension. Her clinical condition rapidly progressed to cardiogenic shock with neurological impairment. Despite multiple attempts, transvenous pacing failed to achieve consistent ventricular capture. Flecainide intoxication was subsequently confirmed through toxicological analysis. The patient was managed with invasive ventilation, veno-arterial extracorporeal membrane oxygenation (VA-ECMO), sodium bicarbonate and intravenous lipid emulsi

Rosenbaum Syndrome Successfully Treated With Left Bundle Branch Area Pacing.

BACKGROUND: Trifascicular block can progress to complete atrioventricular (AV) block and sudden cardiac death. Its clinical and electrocardiographic presentation is usually dramatic but may occasionally be subtle and intermittent, making recognition difficult. CASE SUMMARY: A 45-year-old man presented with exertional dyspnea and no structural heart disease. Surface electrocardiography revealed right bundle branch block with alternating left anterior and left posterior fascicular block and intermittent AV conduction delay, consistent with Rosenbaum syndrome and advanced His-Purkinje disease at

Previously undescribed transient trifascicular block: left septal fascicular block associated with left anterior fascicular block and complete right bundle branch block.

Exercise-induced complete right bundle branch block (CRBBB) remains a rare finding during a treadmill test. Both exercise-induced CRBBB and left anterior fascicular block (LAFB) have been associated with coronary artery disease, more specifically with stenosis of the left anterior descending coronary artery. Left septal fascicular block (LSFB) has been associated with acute and chronic coronary syndromes. We describe a patient case with severe left main coronary artery disease. In an exercise test, a rare combination of these three ventricular conduction disorders developed. In a second exerci

Redo-Transcatheter Aortic Valve Replacement With a 26 mm Sapien Valve in a 26 mm Evolut Valve to Correct Significant Paravalvular Leak via Transcarotid Access.

An 84-year-old man with multiple comorbidities including severe aortic stenosis, heart failure with a reduced ejection fraction, severe peripheral artery disease with prior bilateral iliac artery stents, and trifascicular block presented for evaluation for aortic valve replacement. He was deemed high risk for surgical valve replacement. Preprocedural computed tomography angiography (CTA) for transcatheter aortic valve replacement (TAVR) planning demonstrated an annular area of 295 mm2 and perimeter of 62 mm. There was significant slice misregistration on CT; thus, we performed multiple re-meas

Pacemaker lead perforations: a five-year study from a high-volume center in India.

BACKGROUND: Device therapy for various cardiac rhythm disturbances has seen a tremendous increase in recent times, and so have the various complications associated with this therapy. Pacemaker lead perforation is one of the most feared complications associated with these device implantations. This prospective observational study was conducted to evaluate the clinical features, diagnosis, and outcome of pacemaker lead perforation in our setting. RESULTS: A total of 5493 patients were included in the study. It included 3438 temporary pacemaker (TPM) lead placements and 2055 patients who had und

A ryanodine receptor 2 gene variant associated with left ventricular non-compaction, cardiac conduction disease, ventricular arrhythmias, and sudden cardiac death.

A case of a 3-year-old asymptomatic male patient, who previously lost two brothers and was found to have a homozygote ryanodine receptor 2 gene mutation, is presented. The patient's initial electrocardiogram showed right bundle branch block, first-degree AV block, and left anterior hemiblock (trifascicular block), and echocardiographic examination revealed increased trabeculation in the left ventricle. Since the patient's mother and father were consanguineous, a homozygous missense mutation was detected in the ryanodine receptor type 2 gene within the scope of clinical exome sequencing in the

Takotsubo Cardiomyopathy Complicated by High-Grade Atrioventricular Block in a Patient With Preexisting Conduction Disease: A Case Report.

Takotsubo cardiomyopathy (TTC) often mimics acute coronary syndrome, presenting with chest pain, electrocardiogram (ECG) changes, and troponin rise. While usually reversible, it can rarely be complicated by conduction abnormalities such as high-grade atrioventricular (AV) block. We report a 79-year-old woman with poorly controlled hypertension and preexisting trifascicular block who presented with chest pain precipitated by extreme emotional stress due to the inability to obtain her antihypertensive tablets prescription via her General Practitioner, coinciding with the anniversary of her husba

Masquerading Bundle Branch Block: A Critical ECG Pattern Hidden in Plain Sight.

Masquerading bundle branch block (MBBB) is a rare and often overlooked electrocardiographic (ECG) pattern characterized by right bundle branch block (RBBB) morphology in the right precordial leads combined with features resembling left bundle branch block in the limb leads and/or the left precordial leads. This pattern reflects bilateral conduction system disease and carries a significant risk of progression to high-grade atrioventricular block (AVB). We report the case of an 84-year-old woman admitted for acute decompensated heart failure with preserved ejection fraction and atrial flutter. H

Study of Conduction Blocks in ST Elevation Myocardial Infarction – A Cross-Sectional Analysis.

Conduction blocks complicating ST(ST-segment)-elevation myocardial infarction are associated with increased morbidity and mortality. Research indicates that anterior and inferior wall myocardial infarction were the most encountered causes of blocks but with conflicting results. However, patterns of conduction blocks have not been widely established in our population. The aim was to study the various patterns of conduction blocks following ST-elevation myocardial infarction and their prognostic implications. Prospectively, 70 patients, aged > 18 years, diagnosed with ST segment elevation myocar

Diagnostic Dilemma of Syncope: Esophageal Hiatal Hernia and High-risk Bundle-branch Block.

We herein report a complicated case of recurrent syncope accompanying bundle branch block and hiatal hernia of the esophagus. An 83-year-old woman presented with syncope. Echocardiography visualized the left atrium compressed by an esophageal hiatal hernia, which had potential to decrease the cardiac output. Although she underwent esophageal repair surgery, two months after the surgery, she presented to the emergency department again with complaints of syncope. At the return visit, her face was pale and her pulse rate was 30 beats per minute. Electrocardiography showed complete atrioventricula

Transient conduction disturbances acutely after pulsed-field cavotricuspid isthmus ablation: a case report.

BACKGROUND: Cavotricuspid isthmus pulsed-field ablation has been recently described to be safely performed despite initial reports on coronary arterial spasm while conduction disturbances as a complication of cavotricuspid isthmus ablation are rare and have been reported exclusively for radiofrequency catheter ablation. CASE SUMMARY: A 64-year-old female patient with mechanical prosthetic valves underwent atrial fibrillation ablation using the pentaspline pulsed-field ablation catheter. At the end of the uneventful pulmonary vein isolation, an atrial tachycardia depended to the cavotricuspid

Resolution of Trifascicular Heart Block with Effective Closure of Congenital Atrial Septal Defect Followed by Later Coronavirus Disease 2019-associated Cardiac Strain.

Heart block (HB) is one of the most serious arrhythmias. Higher degrees of HB-for example, trifascicular HB-result in a more intense patient condition. Atrial septal defects (ASDs) represent the most common congenital heart disease in adults. All ASDs generally result in a left-to-right shunt, commonly causing right-side enlargement and dilation and, to a lesser extent, left atrial enlargement. A 26-year-old woman presented to the physician outpatient clinic with a complicated ASD with trifascicular HB and severe mitral and tricuspid regurgitations. The trifascicular HB with valvular regurgita

Syncope in the setting of trifascicular block and retrograde concealed conduction: A case report.

In clinical practice, the accurate diagnosis of the causes of syncope is often challenging and demanding. Moreover, certain rare electrocardiographic phenomena may complicate the diagnostic workup, leading to imprecise diagnoses. The present study briefly describes the case of an 82-year-old male patient with ischemic cardiomyopathy who suffered syncopal episodes in the setting of trifascicular block. The 12-lead electrocardiogram revealed premature ventricular contractions and non-conducted P waves due to the phenomenon of retrograde concealed conduction. Following the exclusion of myocardial

A Case With Inferior Wall Myocardial Infarction and Conduction Abnormalities: Addressing the Diagnostic Challenges.

Conduction disturbances are an important complication of ST-elevation myocardial infarction (STEMI). Conduction disturbances such as fascicular blocks and bundle branch blocks are associated with alteration of QRS morphology and secondary ST-T wave changes that can influence the diagnosis of acute myocardial ischemia. We report an interesting case where a patient presented with inferior wall myocardial infarction (MI), right bundle branch block (RBBB), and left anterior hemiblock (LAHB). We discuss the challenges in diagnosing MI in such patients, including the impact of QRS changes in RBBB an
