PURPOSE: Evaluation of the prolonged PR interval and its predictive value for trifascicular block in individuals with bifascicular His bundle branch block.
PATIENTS AND METHODS: 55 patients, 36 male and 19 female aged 57 + 15.8 years with bifascicular bundle branch block type have been studied. 11 cases had complete left bundle branch block and 44 cases had complete right bundle branch block with left anterior and superior division block. 127 patients aged 0.9 to 18 years (mean 4.0 +/- 2.6 years) underwent total correction for Fallot's tetralogy from March 1975 to November 1989. 118 survivors (early mortality 7.0%) were reinvestigated between 0.2 and 15.3 years (mean 5.1 +/- 4.1 years) after operation by means of clinical investigation, ECG, X-ray, echocardiography (98%) and by recatheterization (48%). 97% are in NYHA-class I and only 3% are in NYHA-class II. An Inuit family from Cape Dorset, Northwest Territories is described; six had documented complete heart block, one high grade atrioventricular block, one bi- or trifascicular block and all suffered syncope. One other member received a pacemaker for unknown reasons. Two of three middle-aged relatives had minor conduction abnormalities. All younger family members who were examined had no conduction problems. This suggests a heritable form of adult onset complete heart block, the largest series reported in the English literature.

Postoperative clinical findings from 25 patients with surgical A-V block and 12 with SSS following surgery for ASD who received permanent pacemakers were analyzed in order to consider the long-term management of postoperative bradyarrhythmias. Surgical A-V block: Episodes of Adams-Stokes were observed in 11/25 patients before pacemaker implantation, and in 6 of these (55%) the onset of episodes occurred more than a year after open heart surgery. Fifty-nine patients operated for Fallot's tetralogy were reviewed over 3 years after surgery. The average age at surgery was 7.4 years (range 6 months to 37 years). The review included ECG, chest X-ray, echocardiography, exercise stress testing and Holter monitoring, completed by cardiac catheterisation in 10 cases and electrophysiological investigation in 4 cases. A 21 year old patient was operated for bilateral ptosis and external ophthalmoplegia at 13 years of age.
