Conflicting invasive electrophysiological study results in a suspected fasciculoventricular pathway: a case report.

BACKGROUND: Fasciculoventricular pathways (FVPs) are variants of pre-excitation syndrome with uncertain prevalence in the human heart. This case report presents findings from an invasive electrophysiological study (EPS) that mostly fulfil the established diagnostic criteria for FVP, but also exhibit atypical features, highlighting diagnostic ambiguity in clinical practice. CASE SUMMARY: A 23-year-old patient presented with a history of brief, weekly episodes of palpitations. The resting surface ECG demonstrated a pre-excitation pattern suggestive of either a parahisian or fasciculoventricular

Using Open-Window Mapping Strategy for Wolff-Parkinson-White Syndrome in a Patient With Dextrocardia With Situs Inversus.

The coexistence of dextrocardia and Wolff-Parkinson-White (WPW) syndrome is exceedingly rare and presents unique challenges for electrophysiological mapping and ablation. We report the first successful radiofrequency catheter ablation guided by three-dimensional open-window mapping (OWM) in such a patient. Pre-procedural right-sided electrocardiography analyzed with the EASY-WPW algorithm predicted a right inferoparaseptal accessory pathway, consistent with OWM localization. Despite reversed cardiac anatomy, precise mapping enabled effective ablation without complications. This case illustrate

Impact of catheter ablation in asymptomatic patients with pre-excitation syndrome - real world evidence.

BACKGROUND: Pre-excitation syndromes such as Wolff-Parkinson-White increase risks of tachyarrhythmias and sudden cardiac death. While symptomatic patients undergo ablation, optimal management of asymptomatic individuals remains debated. This study aims to evaluate long-term outcomes of catheter ablation versus conservative management in asymptomatic patients utilizing real-world data. METHODS: In this observational cohort study using the U.S. Collaborative Network in TriNetX, patients ≥ 12 to 50 years with asymptomatic pre-excitation between September 1, 2010 and October 1, 2024 were identifi

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A Fasciculoventricular Accessory Pathway Featuring Functional Decremental Conduction and QRS Variability.

Fasciculoventricular accessory pathways (FVAPs), once considered rare variants of pre-excitation syndrome, are now recognised as ubiquitous in both humans and murine. Nonetheless, most FVAPs are likely electrically silent. However, they can become evident, as reported for some glycogen storage diseases (such as Danon disease and PRKAG2) and during high-voltage septal pacing. Typically, FVAPs only exhibit antegrade and non-decremental conducting properties. A block at the FVAP results in a normal His-to-ventricle interval and a narrow QRS complex without signs of pre-excitation. Decremental con

A Novel Approach to Explore Internal Cardiac Electrophysiological Pattern under Emotional Stress.

Numerous psychological and clinical studies have confirmed a correlation between mental and cardiac health. We aim to explore this relationship further by examining how emotions influence cardiac health. By collecting body surface potential and utilizing the electrocardiographic imaging (ECGI) model, we can noninvasively and continuously reconstruct internal cardiac electrical activity. To enhance the existing ECGI model on various datasets, we propose an information fusion strategy called Emotional Potential Conversion CycleGAN. It enables data alignment across diverse datasets while preservi

Severe Aortic Stenosis and Pre-Excitation Syndrome in Pregnancy-A Multidisciplinary Approach.

Background/Objectives: Heart disease affects 0.1% to 4% of pregnant women, with congenital heart defects being the leading cause in developed countries. While maternal mortality is generally low, pre-existing cardiac conditions substantially increase adverse outcome risks. This report describes the multidisciplinary management of a pregnant patient with a bicuspid aortic valve, severe aortic stenosis, and ascending aortic ectasia. Case Presentation: A 34-year-old pregnant woman, asymptomatic but at high risk (World Health Organization Class III) for hemodynamic decompensation, was closely moni

Drug review: mTOR-inhibitor therapy in fetal cardiac rhabdomyoma-a tightrope walk.

OBJECTIVE: Mechanistic/mammalian target of rapamycin (mTOR) inhibitors have been used successfully to reduce the size of cardiac rhabdomyomas. However, the number of published cases is small and thus there is no consensus about therapeutic approaches, especially regarding dosing regimens and safety profiles of mTOR inhibitors. Based on a systematic literature review and one new case report, we discuss in detail the indication and adverse effects of fetal and neonatal mTOR-inhibitor therapy. METHODS: A comprehensive search was conducted on PubMed/MEDLINE and Web of Science for studies using co

Pregnancy outcomes in patients complicated with pre-excitation syndrome.

INTRODUCTION: Pregnant women with pre-excitation syndrome are more likely to develop supraventricular tachycardia (SVT) during pregnancy and delivery, leading to an increased risk of adverse events. METHOD: This was a retrospective study of 309 pregnancies in 280 women (29 women had two pregnancies in this series) with pre-excitation syndrome who delivered at West China Second University Hospital from June 2011 to October 2021. All the 309 pregnant women with pre-excitation syndrome were divided into SVT and non-SVT groups to analyze the cardiac and obstetric complications. RESULTS: Among th

Characteristics of Right-Sided Accessory Pathways Associated With Right Cardiac Veins.

BACKGROUND: The anatomy of myocardial fibers around the right cardiac veins (RCVs) and their roles in accessory pathways (APs) are rarely reported. METHODS: Six RCV-APs were identified from 566 patients with right-sided APs. Mapping of retrograde atrial activation was performed using CARTO 3 system under orthodromic tachycardia or right ventricular pacing. Venography of RCVs was acquired at the earliest retrograde atrial activation. RESULTS: Patients enrolled had a median age of 30 (11-51) years, 5 of them were male. Venography of RCVs could be classified into 3 distinct patterns based on th

Abnormal Conduction-Induced Cardiomyopathy: JACC Review Topic of the Week.

Nonischemic cardiomyopathies are a frequent occurrence. The understanding of the mechanism(s) and triggers of these cardiomyopathies have led to improvement and even recovery of left ventricular function. Although chronic right ventricular pacing-induced cardiomyopathy has been recognized for many years, left bundle branch block and pre-excitation have been recently identified as potential reversible causes of cardiomyopathy. These cardiomyopathies share a similar abnormal ventricular propagation that can be recognized by a wide QRS duration with left bundle branch block pattern; thus, we coin

Successful catheter ablation of the accessory pathway in an unusual location in a 13-year-old girl with a coronary sinus diverticulum and Wolff-Parkinson-White syndrome.

Wolff-Parkinson-White syndrome is a congenital cardiac pre-excitation syndrome that is effectively treated by ablating the accessory pathway. However, accessory pathways located in the posteroseptal region can sometimes be challenging. In this paper, we present the successful ablation of the epicardial posteroseptal accessory pathway through the middle cardiac vein in a 13-year-old girl with a coronary sinus diverticulum and Wolff-Parkinson-White syndrome, after unsuccessful ablation attempts at different locations. If the ablation procedure fails, the possibility of the posteroseptal pathway

Sudden Onset of Broad Complex Tachycardia in a Fit Young Man: A Case Report.

Wolff-Parkinson-White (WPW) syndrome is a clinical pre-excitation syndrome often strongly associated with tachyarrhythmias that are predominantly atrioventricular re-entrant tachycardia (AVRT). It is generally considered to be a relatively benign arrhythmogenic condition associated with a slightly higher risk of sudden cardiac death (SCD) in comparison to the general population. Epidemiological data suggests that 0.1%-0.3% of the general population have electrocardiographic (ECG) findings suggesting that during sinus rhythm, in addition to atrioventricular (AV) conduction over the AV node-His

Assessment of the physical performance in children with asymptomatic pre-excitation.

AIMS: Pre-excitation syndrome can lead to recurrent supraventricular tachycardias (SVTs) and carries a risk of sudden cardiac death (SCD). However, an underestimated consequence of antegrade conduction through an accessory pathway is fusion of intrinsic and accessory conduction that causes asynchronous activation and myocardial contraction that could be a cause for cardiac dysfunction and dilation. It is not known to what extent pre-excitation affects myocardial and physical performance in those patients. The aim of the study was to assess to what degree ventricular pre-excitation affects phys

Fasciculoventricular pathways-A rare and innocent variant: A Retrospective study focusing on clinical and electrophysiologic characteristics.

BACKGROUND: Fasciculoventricular pathways (FVPs) are variants of pre-excitation syndrome which were investigated insufficiently because of its rarity. OBJECTIVE: This report aimed to represent one of the largest series of FVP, focusing on its clinical and electrophysiological properties. METHODS: We analyzed retrospectively 26 consecutive patients who underwent electrophysiological study (EPS) for FVP between January 1998 and June 2020. RESULTS: Among 1437 patients with accessory pathways, 26 had FVP (1.80%). All the 26 patients (100%) were males, with a mean age of 22.15 ± 3.50 years (rang
