[Anesthetic management of patients with pre-excitation syndrome].

The authors reviewed the experience of anesthesia in 30 patients under extracorporeal circulation for surgical interruption of Kent's bundle. The key points of anesthetic management are to prevent and control supraventricular tachycardia, which had 35 episodes in 20 of the 30 patients, and disappeared after appropriate treatment. In this group, 18 patients had mild pre-excitation syndrome. After discontinuation of cardiopulmonary bypass, fifteen of the 18 patients were free from pharmacologic support, while 8 of the remaining 12 associated with other cardiac anomalies often necessitated positi

[Ventricular pre-excitation syndrome: anesthesiologic aspects].

Two cases of pre-excitation syndrome are reported and the anaesthesiological management adopted is discussed. After a brief overview of the pathophysiology of the syndrome relative indications and contraindications of the main anaesthetic drugs and volatile agents are analyzed. The high incidence of perioperative atrial fibrillation, atrial flutter or supraventricular tachycardia and the importance of maintaining sinus rhythm is recalled. The task confronting the anaesthetist is to ensure an adequate level of anaesthesia during induction and maintenance while carefully selecting specific drugs

[Electrophysiologic characteristics of multiple atrioventricular accessory pathways in patients with pre-excitation syndrome].

The electrophysiologic studies in 4 patients with multiple accessory pathways (AP) were reported. The results showed that in these patients, the electrophysiologic characteristics were diversified: The VA intervals often changed abruptly during ventricular pacing at a set rate; and during ventricular pacing with incremental rate or a single premature stimulus scanning, the VA intervals revealed several different constant values. At the onset of AV reentrant SVT with wide QRS complex, the HBE showed VA-VA sequence and no H wave could be found; when orthodromic reentrant SVT, the VA interval and

[The inheritance of the ventricular pre-excitation syndrome].

Medico-genetic studies including preparation of the family pedigree, interviews, examinations, electro- and echocardiography were carried out in the families of 75 patients with pre-excitation syndrome (PES). Of these, 35 patients presented with Wolff-Parkinson-White (WPW) syndrome and 40 with Clerc-Lévy-Cristesco (CLC) syndrome. The studies made in possible to define the autosomal-dominant type of the syndrome or PES inheritance and to diagnose for the first time WPW syndrome in 3, PES in 1, CLC syndrome in 32 and CLC phenomenon in 89 persons out of 233 relatives of the first and second degre

[Disorders of cardiac rhythm in combined ventricular pre-excitation syndrome and primary mitral valve prolapse].

Primary prolapse of the mitral valve was diagnosed in 27 (11.7%) out of 230 patients with preexcitation syndrome: 10 of them presented with Wolff-Parkinson-White syndrome, 15 with Clerc-Lévy-Critesco syndrome, and two patients with combination of both syndromes. Preexcitation syndrome combined with mitral valve prolapse manifested itself by different disorders of the cardiac rhythm, mainly by the supraventricular forms: by PST in 17, nonparoxysmal tachycardia in 1, atrial fibrillation paroxysms in 7, sinus tachycardia in 2, supraventricular extrasystole in 3, pacemaker migration in the atria i

Deceptive surgical results in three siblings with familial Wolff-Parkinson-White syndrome.

Three siblings with familial Wolff-Parkinson-White syndrome and two instances of sudden death are described. In all of them, multiple accessory pathways with a very short anterograde refractory period and rapid ventricular responses during atrial fibrillation had been documented, thus surgical ablation of the bypass tracts had been performed. Although abolition of the accessory pathway conduction had been demonstrated post-operatively, an electrophysiologic evaluation performed after 2-8 years showed resumption of conduction over the anomalous connections, with life-threatening arrhythmias dur

Evaluation of fetal heart rate artifacts, hemodynamics and digoxin treatment in fetal tachyarrhythmia by Doppler measurement of fetal blood flow--case report of a pre-excitation syndrome.

A fetal cardiac arrhythmia may cause misleading fetal heart rate tracings with unreliable signs of fetal distress. Assessment of such artifacts and of the hemodynamic relevance of a fetal arrhythmia by alternative methods is necessary for management and therapy. A 28-year-old healthy woman was referred at 30 weeks of gestation because of fetal tachy-brady-arrhythmia, but cw-Doppler assessment of umbilical artery blood flow revealed periods of pseudobradycardia during bigeminal and trigeminal fetal pulse. FHR turned to regular tachycardia, and transplacental digitalization was started. Between

Wolff-Parkinson-White syndrome: illustrative case and brief review.

The Wolff-Parkinson-White syndrome (WPWS) is a pre-excitation syndrome manifested on the 12-lead ECG by a short PR interval (less than .12 sec), a prolonged QRS interval (greater than .10 sec), and an initial slurring of the QRS complex called a delta wave. The anatomical etiology is the presence of accessory atrioventricular conduction fibers called the Kent bundle. Patients with the WPWS may present to the emergency department with a wide spectrum of symptoms, ranging from mild palpitations, to unstable tachydysrhythmias and sudden cardiac death. WPWS should be suspected in any patient prese

[The natural history of 270 cases of Wolff-Parkinson-White syndrome in a survey of the general population].

Among 226,464 ambulatory subjects who underwent medical check-ups over a 15-year period, 270 were found to have Wolff-Parkinson-White syndrome (1.2 case in 1,000). The syndrome was more frequent in men (181 cases, 1.4 p. 1,000) than in women (89 cases, 0.9 p. 1,000). 222 subjects were aged from 20 to 49 years (1.4 p. 1,000) and only 48 were between 50 and 80 years of age (0.7 p. 1,000). 197 subjects were re-evaluated: 119 (60.4 p. 100) complained of palpitations and 78 (39.6 p. 100) were asymptomatic. Palpitations began at all ages, even after 50 years, and usually proceeded in short attacks l

[Pharmacodynamics and pharmacokinetics of flecainide in cardiac pre-excitation of the bundle of Kent].

In order to identify the flecainide plasma concentrations capable to inhibit the abnormal Kent's pathways, 9 patients affected by a Wolff-Parkinson-White syndrome were studied. One of them (a female 21 years old) previously received 200 mg oral flecainide "una tantum", 8 received 100 mg bid for at least 2 weeks. They had a short PR interval and an evident delta wave on the surface ECG, diagnostic for pre-excitation syndrome. The flecainide plasma half-life calculated in 6 patients chronically treated correlated directly and strictly with age. In all patients the flecainide treatment blocked th

[A case of intermittent ventricular pre-excitation syndrome with a Mahaim fiber].

A 61 year-old woman with intermittent ventricular pre-excitation syndrome using a Mahaim fiber was reported. The electrocardiogram showed QRS-complex with small delta wave (100 msec in QRS width) or without small delta wave (80 msec in QRS width). On electrophysiological study, during sinus rhythm, A-H interval was 100 msec. H-V intervals were 25 msec in QRS-complex with small delta wave and 45 msec in QRS-complex without small delta wave. From these findings, it was considered that the small delta wave was produced by a Mahaim fiber.

[Fulguration and pre-excitation syndrome. Results in 121 patients].

Ablation of accessory pathway (AP) of any location was performed either with a right or a left approach (patent foramen ovale, transpeptal or a retrograde transvalvular aortic catheterism in 121 patients. The best ablation site was localized: 1) by the recording of a potential likely with the Kent bundle activation; 2) the earliest site of retrograde atrial activation during orthodromic reciprocating tachycardia (80 +/- 35 ms); 3) first ventricular potentials recorded ahead or synchronous with the delta wave in standard leads; 4) disappearance of preexcitation due to the pressure of the cathet

Cryoablation of the accessory pathway in Wolff-Parkinson-White syndrome: initial results and long term follow up.

Cryoablation of the accessory pathway was used in the management of 20 patients with pre-excitation syndromes. All patients had presented with paroxysmal atrioventricular reentrant tachycardia; in addition, six had experienced atrial fibrillation. In 16 patients pre-excitation was overt and in four the accessory pathway was concealed. Intraoperative epicardial and endocardial mapping showed 10 left free wall pathways, seven septal pathways, and four right free wall pathways. One patient had two right free wall accessory pathways. There was one postoperative death (from a ruptured cerebral haem
