BACKGROUND: Exercise-induced hypertension (EIH) is recognized as an independent cardiovascular risk factor. Epicardial fat thickness (EFT) has been implicated in various cardiovascular pathologies. However, the relationship between EFT, EIH, and ventricular arrhythmias remains poorly characterized. This study aimed to investigate the predictive value of EFT for both EIH and premature ventricular beats (PVB).
METHODS: A total of 2658 participants were initially screened for eligibility, and normotensive participants of age > 18 were considered for enrollment. All participants underwent a compr

To illustrate the effectiveness of radiofrequency catheter ablation as a therapy for paediatric patients with frequent premature ventricular contractions that are refractory to medical treatment and associated with reduced cardiac function. In patients with multifocal ectopic premature contractions of localised regional origins, radiofrequency catheter ablation may be more beneficial than pharmacologic therapy.

OBJECTIVE: To determine whether neurological injury in metabolic syndrome (MetS) extends beyond hyperglycaemia-driven neuropathy.
METHODS: We employed a translational design combining controlled rat models of isolated and combined MetS components with a community-based cross-sectional human study. Preclinically, ten rat models (n = 6 per group) representing various MetS phenotypes were assessed for metabolic parameters and peripheral neurological function. Clinically, 1,176 adults were stratified by glycaemic and MetS status according to International Diabetes Federation (IDF) criteria. Parti

OBJECTIVES: Identifying immune features that distinguish asymptomatic autoantibody-positive at-risk individuals from patients with established autoimmune disease is key to understanding the evolution from autoreactivity to established autoimmune disease. This study investigated immune characteristics of anti-SSA-positive at-risk individuals, including pregnant women, compared with patients with Sjögren's disease (SjD) and healthy donors (HDs).
METHODS: Peripheral blood samples were obtained from asymptomatic anti-SSA-positive women (at-risk individuals; n = 7, including two ongoing pregnancie

INTRODUCTION: Leadless pacemakers (LP) have significantly reduced device-related complications compared with conventional systems, yet limitations in atrial pacing and atrioventricular (AV) synchrony restrict their widespread use.
METHODS AND RESULTS: We report an 87-year-old man with a history of stage 5 CKD on haemodialysis with complete AV block and a previously implanted Micra-AV who developed symptomatic sinus node dysfunction. To restore AV synchrony and avoid transvenous implantation, an Abbott AVEIR AR atrial LP was implanted, achieving functional dual-chamber leadless pacing. The pro

BACKGROUND: PRKAG2 syndrome is a rare autosomal dominant glycogen storage cardiomyopathy characterized by left ventricular hypertrophy, ventricular pre-excitation, and progressive conduction system disease.
CASE SUMMARY: We report the case of a 25-year-old woman with genetically confirmed PRKAG2 syndrome who presented with an out-of-hospital cardiac arrest. Echocardiography revealed preserved left ventricular systolic function with mild concentric hypertrophy and marked apical thickening. Cardiac magnetic resonance imaging showed anterolateral trabecular hypertrophy without evidence of myocar

Cardiac conduction system abnormalities, such as sinus nodal dysfunction and AV block, can lead to significant bradyarrhythmias and pauses, potentially causing symptoms like dizziness and loss of consciousness. Monitoring these rhythm disturbances is crucial, as they may evolve and indicate underlying structural heart disease or lead to more serious arrhythmias. We present the case of a 76-year-old man with an exceptionally prolonged 62-second ventricular pause, followed by a single ventricular beat and an additional 24-second pause before normal rhythm resumes. He presented recurrent, intermi

The management of bradyarrhythmias requiring permanent pacing in pediatric patients and those with congenital heart disease (CHD) remains uniquely challenging due to continuous somatic growth, complex cardiac anatomy, limited vascular access, and long-term risks associated with conventional cardiac implantable electronic devices. Traditional transvenous endocardial and surgical epicardial pacing systems are associated with high rates of lead failure, venous occlusion, infection, and ventricular dyssynchrony. Leadless pacemakers and conduction system pacing have emerged as promising alternative

Left bundle branch block (LBBB) describes a disruption or delay in electrical conduction through the left bundle branch of the His-Purkinje system (HPS). While new-onset LBBB commonly emerges in individuals with a history of hypertension, cardiomegaly or coronary artery disease, it can also occur in the absence of other cardiovascular abnormalities. Whether LBBB is a cause or a consequence of heart failure remains unclear. In this work, we used six different electromechanical in silico heart models with an integrated HPS to simulate both a healthy control and a pure LBBB case for each geometry

BACKGROUND: Initially discovered and published in 2017, the heterozygous mutation (p.Arg52Leu) in the GNB2 gene is associated with sinus node dysfunction (SND) and atrioventricular block (AVB). Electrophysiological characterization using heterologous expression systems has demonstrated that this R52L GNB2 variant results in a larger G-protein-activated K+ (GIRK/IK, ACh) current. So far, this has not been functionally validated in patient-derived disease modelling using human induced pluripotent stem cell-derived cardiomyocytes (CM).
METHODS: Here, we differentiated CMs derived from human indu

Amiodarone-induced thyrotoxicosis (AIT) is a potentially life-threatening complication of amiodarone therapy, particularly hazardous in patients with underlying structural heart disease. We report the case of a man in his mid-60s with severe cardiomyopathy and recurrent ventricular tachycardia who developed refractory thyrotoxicosis following amiodarone exposure. Overlapping features of type 1 and 2 AIT suggested mixed-type disease, posing a significant diagnostic challenge. Despite aggressive conventional therapy with high-dose thionamides and glucocorticoids, supplemented by adjunctive agent

OBJECTIVE: Frequent premature ventricular complexes (PVCs) can lead to adverse health conditions such as cardiomyopathy. The linear correlation between PVC frequency and heart rate (as positive, negative, or neutral) on a 24-hour Holter recording has been proposed as a way to classify patients and guide treatment with beta-blockers. Our objective was to evaluate the robustness of this classification to measurement methodology, different 24-hour periods, and nonlinear dependencies of PVCs on heart rate.
MATERIALS AND METHODS: We analyzed 82 multi-day Holter recordings (1-7 days) collected from

INTRODUCTION: The estimated prevalence of ventricular extra systole (VES) in the general population is about 1-4% on ECG, but 24-hour Holter monitoring has shown a prevalence of 40-75%. While it may be asymptomatic in many patients, frequent VES persisting for a long time can negatively affect left ventricular (LV) systolic function in patients without structural heart disease. The etiology of VES is not completely known. In this study, we investigated the role of neuropeptide Y (NPY) in the occurrence of VES.
MATERIALS AND METHODS: In this study, we included 150 patients with VES and 86 case

BACKGROUND: Ajmaline challenge (AC) is used for diagnosing suspected Brugada syndrome (BS) in patients with unexplained syncope, survived cardiac arrest, or for family screening.
PURPOSE: To evaluate baseline ECG markers predicting a positive AC in the absence of a spontaneous diagnostic Brugada ECG.
METHODS: Baseline ECGs of 221 consecutive patients undergoing AC (up to 1 mg/kg bodyweight) were analyzed. ECGs from positive and negative tests were compared, with Q-, R-, S-, J-, and T-wave amplitudes and intervals measured in all 12 leads.
RESULTS: 221 patients underwent AC; the cohort was 7

Acute occlusive myocardial infarction (OMI) complicated by ventricular pre-excitation can present diagnostic challenges. This case describes a 78-year-old male with an 11-h history of chest tightness. A reduction was observed in the extent of localized ventricular pre-excitation, accompanied by secondary ST-T segment abnormalities in leads V1-V3. Emergency coronary angiography revealed occlusion of the left anterior descending artery. This case highlights that a decrease in the extent of local ventricular pre-excitation in R-wave-dominant leads, combined with inference of the accessory pathway

Entrapment of the ablation catheter in the mitral valve apparatus during transaortic ablation of premature ventricular complexes is a rare complication that usually requires surgical removal. Random rotating and pulling of the catheter exacerbate the extent of the entrapment, causing chest pain and a drop in blood pressure. The catheter can be loosened by changing the direction of the vector at the tip of the catheter, pulling in the reverse direction and rotating. We describe the case of the thought process and catheter maneuvers used to diagnose, loosen, and remove a trapped catheter.

Delayed afterdepolarizations (DADs), which occur during the diastolic phase of a cardiomyocyte action potential (AP), are frequently observed under specific pathophysiological conditions. The synchronization of DAD-capable myocytes can effectively overcome the inherent source-sink mismatch with adjacent normal myocytes, so it is an important mechanism for the genesis of premature ventricular contractions (PVCs). Our study elucidates the role of mechano-electrical feedback in modulating this critical source-sink requirement and its interplay with diffusional anisotropy in cardiac tissue. We com

BACKGROUND: It is well-established that obesity increases the risk of cardiovascular illnesses. Even in the absence of apparent illness, obesity may affect cardiac electrical activity. The present study sought to explore the association between body mass index (BMI) and the occurrence of arrhythmias, average heart rate, and fragmented QRS (fQRS) patterns in a population of healthy individuals aged 40 years or younger. METHODS: A study population of 193 asymptomatic individuals without known cardiovascular disease under 40 years of age was collected and categorized into three groups according t

BACKGROUND: Premature ventricular contractions (PVCs) are the most prevalent ventricular arrhythmia in adults. High PVC burden can lead to left ventricular systolic dysfunction, eccentric hypertrophy, and an increased risk of heart failure and sudden cardiac death. Inadequate angiogenesis is a key determinant in the transition from adaptive to maladaptive cardiac hypertrophy, and fibrosis is a risk factor for arrhythmia and sudden cardiac death. We quantitatively assessed structural remodeling and transcriptional alterations in PVC-induced cardiomyopathy (PVC-CM).
METHODS: Animals were implan

Cardiac conduction is a central determinant of normal rhythm and arrhythmia susceptibility. Although arrhythmias have traditionally been attributed to abnormal automaticity, triggered activity, and re-entry, emerging evidence indicates that conduction abnormalities integrate structural, electrical, and immune-derived signals into a common arrhythmogenic substrate. This review summarizes multiscale mechanisms of impulse propagation, with an emphasis on gap junction-mediated coupling. Connexin 43 (Cx43), the principal ventricular connexin, maintains intercellular current flow through phosphoryla
