A series of thirty sudden deaths (twenty-nine men, one woman) associated with squash playing is described. The subjects' mean age was 46.7 +/- SD 9.58 years (range 22-66). Necropsy results were available for twenty-seven subjects. Significant coronary heart disease (CHD) was found in twenty-three, valvular heart disease in three, and hypertrophic obstructive cardiomyopathy in one, and cardiac arrhythmia, in the absence of other pathology, was thought to have preceded death in two. Documentation of the mechanism of sudden death is described in a patient with a prolonged QT interval. Ventricular tachycardia was initiated by a ventricular premature beat (VPB) with a prematurity index similar to previous isolated VPBs. Six cases of acute myocardial infarction with blood in the pericardial sac are described. In one case rapid death followed myocardial rupture leaving no time for the possibility of intervention. Electrophysiologic testing in patients after acute myocardial infarction may predict subsequent sudden death. We used a permanent pacemaker for repetitive electrophysiologic testing in a patient after acute infarction. Programmed, twice-threshold ventricular extrastimulation at two weeks after anteroseptal infarction failed to induce ventricular tachycardia. A previously healthy 34-year-old Dutch immigrant to Australia died unexpectedly in his sleep. At autopsy the only significant finding was a floppy aortic valve (FAV). Histologic, histochemical and electron microscopic studies corroborated the macroscopic diagnosis. Previously described associations of the FAV include the floppy mitral valve, Marfan's syndrome, aortic root dilatation and aortic cystic medial necrosis. The pathologic substrate for sudden death in the middle-aged or elderly adult is usually ischemic heart disease. In contrast, few data are available regarding the pathology of sudden death in teenagers. This report describes three teenagers without clinically suspected heart disease dying suddenly. The authors describe their clinical strategy and experience with electrophysiologic testing in a group of patients who have had at least one episode of sudden cardiac death or nonbradycardic syncope in the absence of coronary artery disease. Their study shows that for this population programmed stimulation commonly induces ventricular tachycardia, which can then be suppressed by drug therapy.

Originally described in 1964, lipomatous hypertrophy of the atrial septum currently remains a diagnosis established primarily at autopsy. Clinical interest in this disorder has centered on the reported association with supraventricular arrhythmias and sudden death. Fifty-two patients with refractory symptomatic supraventricular and ventricular arrhythmias were treated with amiodarone (Cordarone-X; Reckitt & Colman) and followed up for a mean period of 36,4 months (range 3-54 months). The material and implantation technique of the implantable defibrillator developed by M. Mirowski and used by the authors in 3 patients are described. Through lateral thoracotomy, 4 electrodes are installed, including one patch on the apex of the heart. In case of sustained ventricular arrhythmia the instrument delivers, after less than 30 seconds, a 25 joules shock increased to 30-35 joules if this fails. The results were very encouraging since conversion of severe ventricular arrhythmia was obtained in all 3 patients after the first shock. There was no false detection or inappropriate start.

A patient with pulmonary hypertension (PH) complicating mixed connective tissue disease (MCTD) is described. The patient died suddenly after catheterization documenting the pulmonary hypertension. Pathologic examination demonstrated marked medial hypertrophy and intimal proliferation of the pulmonary arterial vessels. Review of the literature revealed few and differing reports concerning the pathology of PH as seen in MCTD. Sudden death in patients with pulmonary hypertension undergoing invasive procedures is not an uncommon occurrence.
