[Electrophysiological study of a Portuguese case of amyloidosis with conduction disorders].

Cardiac involvement in the course of familial Portuguese amyloidosis, as apart from the other primary amyloidosis, is characterised by the early and wide-spread intracardiac conduction disorders, and the contrasting late presentation of the clinical signs. A case is presented of Portuguese amyloidosis which was typical from the viewpoint of the neurological disorders, the familial characteristics, and the positive biopsy; the main conduction defects found in primitive amyloidoses are also recalled. The patient described had for many years suffered from first degree heart block, and then presen

[A regional experience with permanent cardiac pacing: in Friuli-Venezia Giulia the highest density of pacemaker patients in Italy, to-day (author's transl)].

In Friuli-Venezia Giulia, a region of 1,243,463 inhabitants, a total of 1,629 first implants have been carried out since the first patient was treated with permanent cardiac pacing (P.C.P.) early in 1692. This patient is still alive. In the last four years, the annual rate of implants was 185 per million inhabitants. The average age of patients is 72.9. The most frequent indication for pacing is the a-v block (64.5%), followed by the fascicular blocks (10.8%) and the sick sinus syndrome. No asinchronous fixed rate generators are implanted at present; lithium batteries are the power source of c

A 17-year follow-up study of a family with idiopathic hypertrophic cardiomyopathy and WPW syndrome.

Long term clinical courses of a patient and her family with idiopathic cardiomyopathy and WPW syndrome were described. The mother and her brother (the first generation) had died of heart disease, and 1 sibling had also died suddenly of heart disease when the study began. Seven out of the 8 siblings (the second generation) were followed for 17 years from 1958 to 1976. The 4 siblings had both typical or atypical WPW syndrome and cardiomegaly in 1958, 2 of them died suddenly and unexpectedly, 1 of them died of congestive heart failure, and 1 of them did not have any complaint during the period. Sudden death in Auckland.

A representative sample of all sudden cardiac deaths occurring in Auckland in one year is reported. Data describing demographic variables, personal characteristics and habits, previous symptoms, prodromal symptoms, the acute phase and post mortem results, is presented. Incidence values are very close to those of other similar communities. The syndrome of "sudden death" probably defines a heterogenous group of cases. Deaths were significantly more common in the cooler months, and in the lowest social class. Some evidence is presented consistent with the hypothesis that a high alcohol intake is

Idiopathic familial myocardiopathy in three generations: a clinical and pathologic study.

A peculiar non-hypertrophic myocardiopathy is described which occurred in three and possibly five generations of a single family. Clinical features included systolic murmurs, electrocardiographic abnormalities, and sudden cardiac death with a paucity of symptoms of cardiac dysfunction. Pathological studies in three generations showed a striking similarity of cardiac findings including globular and dilated ventricles, endocardial fibroelastosis, and mitral valve thickening. Myocardium in two showed basophilic degeneration and fibrosis. A retrospective genealogic analysis and a prospective clini

Evaluation of children with ventricular arrhythmias.

Ventricular arrhythmias are rare in childhood but may be associated with syncope and sudden death. This report describes 8 children with ventricular arrhythmias, 6 of whom suffered syncopal episodes. Ventricular tachycardia was documented in 5. One boy died suddenly. Complete cardiac investigation was carried out with exercise testing, Holter monitoring, echocardiography, cardiac catheterization, angiography, and electrophysiological studies. The spectrum of abnormalities related to the arrhythmias included prolapsing mitral valve, prolonged QT syndrome, sick sinus syndrome, congenital heart d

Clinicopathologic correlations. De subitaneis mortibus. XXV. Sarcoid heart disease.

Postmortem findings within the cardiac conduction system are described from the case of a black woman with sarcoid heart disease who died suddenly. Her clinical course had been characterized by recurring ventricular arrhythmias and bouts of syncope. Both the sinus node artery and the atrioventricular (A-V) node artery were sites of focal fibromuscular dysplasia, which thickened slightly the wall of the former but markedly narrowed the lumen of the latter. Small foci of sarcoid infiltration were present in the sinus node and the A-V node. Fatty replacement within the His bundle was attributable

Pathology of the heart in sudden cardiac death.

Clinical and pathologic changes in 87 patients who could not be resuscitated from an episode of sudden cardiovascular collapse are described and compared with observations from patients in the same community who were successfully resuscitated from ventricular fibrillation. Findings in patients who died suddenly generally did not differ when the patients were groups by electrocardiographic rhythm on arrival of the mobile coronary aid unit. The pathologic changes of acute thrombosis and recent myocardial infarction did not occur with sufficient frequency in the entire group to be considered caus

Infantile periarteritis nodosa or mucocutaneous lymph node syndrome. A report on four cases and diagnostic considerations.

Coronary artery aneurysm in childhood is a rare disease and has in most cases been ascribed to infantile periarteritis nodosa (IPN). In recent years a mucocutaneous lymph node syndrom (MLNS) has been found almost exclusively in Japan first described by Kawasaki 1967; this disease frequently involves the coronary arteries and myocardium. Four cases with coronary aneurysms are presented from Sweden and seem to be first described from Scandinavia. Three of these patients died a sudden death with cardiac arrest. Since MLNS and IPN have identical clinical and pathological features, we suggest that

Familial cardiac dysrhythmias: classification and review.

Disorders of cardiac rhythm and conduction are described in multiple members of one family spanning three generations. The literature is reviewed regarding other aspects of familial cardiac arrhythmias, permitting a classification of the latter. The entity is not rare, and familial screening is imperative in instances of sudden death, fainting episodes, or rhythm disorders in young individuals.

Coronary ostial stenosis complicating coronary arteriography.

Coronary ostial stenosis is a cause of angina pectoris and sudden death. It is due mainly to atherosclerosis, syphilis, and iatrogenic disease. The last is of growing importance because of the frequency of coronary arteriography and surgical procedures on the aortic valve. Since both may cause stenosis of the coronary ostia, these procedures raise the topic from an obscure morphologic entity to an important consideration in the treatment of cardiac disease. We describe a case of ostial stenosis that was complicated by coronary arteriography.

Localization by autoradiography of tritiated isoproterenol in "infarct-like" lesions of rat myocardium.

A number of physiopathogenic mechanisms have been outlined to explain the "infarct-like" lesions produced by isoproterenol (ISP) in the hearts of various animals: Excess of oxygen consumption and inotropic effect, coronary vasoconstriction, deleterious action on glucose and lipid metabolism, direct cardiotoxic effect, platelet aggregation in the small cardiac vessels and formation of microclots, excessive mobilization of fatty acids, fluid and electrolytic imbalances, loss of high-energy intracellular coupling, and inadequate activation of the "calcium pump." For this reason, localization of t

Ventricular arrhythmias in syndrome of balloon deformity of mitral valve. Definition of possible high risk group.

Twenty patients clinically identified as having balloon deformity of the mitral valve were studied to assess the incidence of ventricular arrhythmias. Echocardiography and phonocardiography were used to confirm the nature of the mitral valve lesion. Continuous 24-hour electrocardiograms were obtained from all patients and analysed by a computer and 2 observers. One patient has ventricular fibrillation and 3 patients had ventricular tachycardia. There was a high incidence of other less severe forms of ventricular arrhythmias. Eight patients had inferolateral ST and T wave abnormality on the res

Deaths in connection with chlormethiazole (heminevrin) therapy.

Six cases are described where death occurred in connection with chlormethiazole (Heminevrin) infusion. The close time relationship and indirect evidence strongly suggest that chlormethiazole had an important role in the fatal outcome of the patients, but definite proof of this way was not possible to obtain. Present or past alcohol abuse and respiratory, liver, cardiac or CNS diseases of different degrees seemed to be features in common for the patients possibly rendering them susceptible to the suggested detrimental effect of chlormethiazole therapy. Respiratory depression is supposed to be t

New approaches to the management of the Wolff-Parkinson-White syndrome.

Pre-excitation has been defined as "a condition in which all or some portion of the ventricular muscle is activated earlier in relation to atrial events, than would be expected had the impulse reached the ventricle by way of the normal atrioventricular conduction system". The susceptibility of patients with pre-excitation to paroxysmal tachycardias had been described many years previously. The Wolff-Parkinson-White syndrome was initially thought to have a relatively benign prognosis. Subsequently, it was recognised that some patients were prone to life-threatening arrhythmias and sudden death.

Hereditary dysrhythmic congestive cardiomyopathy.

A patient with hereditary congestive cardiomyopathy, who presented with recurrent episodes of life-threatening ventricular arrhythmias most often precipitated by exercise, is described. The condition is marked by either a progressive course, in which case congestive cardiac failure may set in towards the end, or by unexpected sudden death. The family tree could be traced for 10 generations. The information about the tenth generation firmly established that 4 members, 2 of whom had died, were affected. Other evidence suggests that the condition was the cause of death in 3 members of the eighth

Diagnostic features of right ventricular myxoma.

The clinical diagnostic features of right ventricular myxoma are described in a recent patient and related to the 15 cases previously reported. The presence of a pulmonic systolic ejection murmur with a delayed (120 to 140 msec.) and accentuated pulmonic second sound, or calcification in the region of the right ventricular outflow tract should suggest this lesion. Cardiac catheterization with angiocardiography in diagnostic. Ga scanning may assist in the diagnosis and followup after surgical removal of the myxoma. Early surgical removal will avoid the possibility of sudden death.

Mitral valve prolapse.

Mitral valve prolapse is a condition that is being recognized with increased frequency. It is not known whether its incidence is increasing, or whether we are better able to diagnose it today. In the idiopathic or familial variety, the mitral valve pathology is almost always that of myxomatous degeneration. Some authors have suggested the presence of a cardiomyopathy because of significant left ventricular dysfunction in many cases. Idiopathic prolapse occurs predominantly in females, often at a young age, and may be associated with chest pain, dyspnea, fatigue, presyncope, syncope, and/or sud

[Certain problems of intensive therapy of patients with acute myocardial infarct].

The communication is based on the observation of 573 patients with acute and sub-acute myocardial infarction. The arrangement of intensive care units and wards permitted to render timely and qualified aid to the patients with myocardial infarction, to reduce the percentage of complications, and in cases of the development of the signs of clinical death to undertake immediate resuscitation measures. Of the 123 patients in whom clinical death was recorded, 34 were successfully resuscitated (27.6%). The problems of the management, prognosis and prevention of clinical death are discussed. The grea
