Intraoperative voltage mapping-assisted resection of a giant right ventricular giant tumor: a case report.

BACKGROUND: Cardiac hemangiomas located in the right heart can lead to serious complications, including right ventricular outflow tract (RVOT) obstruction, arrhythmias, and sudden cardiac death. Although surgical resection remains the primary treatment, complete excision of intramural tumors poses risk of impairing cardiac function. This case report describes the successful resection of a large right ventricular hemangioma using intraoperative voltage mapping, which enable maximal tumor removal while preserving myocardial integrity and preventing postoperative heart failure. CASE PRESENTATION:

Sex differences in electrocardiograms of trained athletes: a systematic review and meta-analysis.

AIMS: To describe and quantify sex differences in the screening electrocardiograms (ECGs) of athletes. METHODS AND RESULTS: Five databases (MEDLINE, EMBASE, Scopus, SPORTDiscus, and Web of Science) were searched from inception until October 2024. Included studies were original research articles examining athletes aged 16-40 years, who had a 12-lead screening ECG, and where analysis was stratified by sex. Risk of bias was assessed using a validated tool. A meta-analysis was performed, using a random-effects model, assessing proportions of athlete normal, borderline, and abnormal ECG features (

Early Onset Heart Failure due to RBM20 Variant: A Case Report Emphasizing Genetic Diagnosis and Arrhythmic Risk Stratification.

The RBM20 gene, located on chromosome 10q25.2, encodes a serine/arginine-rich protein essential for post-transcriptional splicing of several cardiac genes, including titin. Pathogenic variants in RBM20 are increasingly recognized as causes of familial dilated cardiomyopathy (DCM) with a high risk of heart failure and sudden cardiac death. We describe a 23-year-old man who presented with heart failure with mildly reduced ejection fraction secondary to DCM caused by a heterozygous missense variant in the RBM20 gene (c.1907G>A; p.Arg636His). Comprehensive clinical evaluation excluded non-genetic

One origin, diverse courses: coronary CTA evaluation of single coronary artery variants from the right sinus of Valsalva.

Single coronary artery (SCA) anomalies, in which the entire coronary circulation arises from a solitary ostium, are exceedingly rare congenital variants with variable clinical implications depending on their morphologic course. Recognition of malignant trajectories, particularly interarterial courses, is critical given their association with myocardial ischemia and sudden cardiac death (SCD). This study aimed to describe the anatomical diversity, clinical presentations, and prognostic implications of SCA originating from the right sinus of Valsalva (RSV), with emphasis on the diagnostic value

Myocardial Bridging and Cardiac Arrest.

BACKGROUND: Myocardial bridging (MB) is a congenital coronary anomaly in which a segment of an epicardial artery tunnels through the myocardium. Although frequently considered benign, MB has been increasingly linked to ischemia, malignant arrhythmias, and sudden cardiac death. CASE SUMMARY: We describe a case of a 39-year-old woman with mid-left anterior descending artery MB who presented with sudden chest pain followed by cardiac arrest. Coronary angiography revealed MB without obstructive disease. Despite prolonged resuscitation and hypoxic brain injury, she achieved functional recovery aft

Cobalt-Induced Cardiomyopathy: Mitochondrial Dysfunction, Oxidative Stress, and Reversible Cardiac Toxicity: A Systematic Review.

Metal-on-metal (MoM) joint replacements were designed to improve durability in younger, active patients. However, cobalt-induced cardiomyopathy (CIC) has emerged as a rare but serious complication, often misattributed to idiopathic or ischemic causes. We systematically reviewed published case reports, case series, and laboratory studies describing CIC in patients with MoM implants. Data extraction included clinical presentation, diagnostic criteria, treatment, and outcomes. Methodological quality and risk of bias were assessed qualitatively. Eighteen cases were included. Implant wear and corro

Case Report: Genomic and clinical insights into MYBPC3-related hypertrophic cardiomyopathy in Ecuadorian patients: implications for sudden cardiac death risk.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disease and a leading cause of sudden cardiac death (SCD) in young adults and athletes. It exhibits marked clinical variability, which may be influenced by genetic background and environmental factors. Although MYBPC3 is the most frequently implicated gene, data from Latin American and admixed populations remain scarce. In this study, we describe three unrelated Ecuadorian patients with clinically diagnosed HCM who harbored MYBPC3 variants. Two patients carried likely pathogenic mutations (p.Glu258Lys and p.His875Profs*8),

Clinical usefulness of genetic diagnosis in early-onset cardiomyopathies.

INTRODUCTION AND OBJECTIVES: Early-onset nonsyndromic cardiomyopathies (eoNSCM) are rare in pediatrics and are often associated with cardiac events (CE). The prognostic impact of multiple genetic variants in eoNSCM remains incompletely characterized. This study aimed to describe clinical outcomes in a pediatric cohort with eoNSCM and to analyze their association with genetic burden. METHODS: Single-center retrospective study analyzing clinical and genetic data from patients with eoNSCM. The primary outcome was the occurrence of CE: ventricular tachycardia or fibrillation, appropriate implanta

Fatal gelsemium alkaloid poisoning versus cardiovascular cause in a sudden death: a forensic case report.

Gelsemium alkaloids, such as gelsemine and koumine, are highly toxic compounds that can induce rapid respiratory and cardiovascular failure. Their clinical manifestations-dizziness, ptosis, and sudden collapse-closely resemble those of acute myocardial infarction, presenting diagnostic challenges in forensic practice, especially in distinguishing between poisoning and cardiovascular events. We describe a sudden death in a middle-aged male following a restaurant meal. The decedent developed acute dizziness and eyelid drooping shortly after ingestion and died within hours despite resuscitation.

Forensic Diagnosis of Sudden Cardiac Death Due to Thrombosis Secondary to Coronary Artery Ectasia: A Description of Two Cases.

Coronary artery ectasia (CAE), whether it is diffuse or localized, is an uncommon finding. It is sparsely documented, especially by post-mortem imaging and histology. In this article, we will describe two cases of sudden cardiac death secondary to coronary thrombosis in the context of CAE. The cause of death was myocardial infarction in both cases. Diagnosis was based on post-mortem computed tomography angiography, forensic autopsy, and histological examination, offering a rare and complete illustration of the disease.

Incidence and Predictors of Appropriate Implantable Cardioverter-Defibrillator Therapy in Light Chain Cardiac Amyloidosis.

BACKGROUND: Sudden cardiac death represents a common mode of death in patients with light chain cardiac amyloidosis (AL CA). However, data evaluating the role of implantable cardioverter defibrillator (ICD) in these patients are scarce. OBJECTIVES: This study aimed to describe the incidence and identify factors associated with appropriate ICD therapies in AL CA patients. METHODS: This was a retrospective, single referral center study including all AL CA patients implanted with ICDs between November 2014 and May 2024. The primary endpoint was the first appropriate ICD therapy (shock or antita

Pediatric viral myocarditis: mechanisms, experimental models, and research gaps.

Viral myocarditis is a major cause of pediatric cardiac inflammation and contributes to dilated cardiomyopathy and sudden cardiac death. Despite its impact, most mechanistic insights derive from adult models, limiting understanding of pediatric disease. This review highlights age-specific differences in immune responses, viral tropism, and clinical outcomes. We summarize the major viral pathogens associated with pediatric myocarditis and their distinct roles in acute and chronic cardiac injury. We discuss experimental infection models with attention to viral species, host strain, and route of

The examination of the atrio-ventricular node region should be a routine practice in all cases of sudden unexpected death in infancy (SUDI) and childhood (SUDC).

KEY POINTS: To describe 3 pediatric cases in which the sampling of the atrioventricular (AV) node was crucial to identify the cause and/or mechanism of death. We also describe 6 other pediatric autopsy cases with a non-cardiac cause of death in which cartilage was present in the fulcrum, adjacent to the AV node. We routinely sample the AV node region in cases of SUDI and SUDC. This experience provides the authors with a comprehensive understanding of regional normal histological variation. Samples are stained with H&E, Trichrome, Alcian Blue and S100 immunohistochemistry. In 3 cases features p

Sudden unexpected death due to hypokalemic rhabdomyolysis associated with chronic alcohol abuse: An autopsy case.

Severe hypokalemia-induced rhabdomyolysis is rare but potentially fatal, particularly among chronic alcohol abusers who are predisposed to multiple electrolyte disturbances. This case report describes an early 40s male with chronic heavy alcohol consumption who presented with progressive neck stiffness and shoulder discomfort over several days, ultimately resulting in inability to stand. Laboratory findings revealed profound hypokalemia (1.4 mEq/L), hypophosphatemia, markedly elevated creatine kinase, and macrocytic hyperchromic anemia, suggesting alcohol-related organ dysfunction. Despite sup

SLC4A3-related short QT syndrome assessed in human induced pluripotent stem cell-derived cardiomyocytes: mechanisms of ventricular arrhythmia and sudden cardiac death.

BACKGROUND AND AIMS: Short QT syndrome (SQTS) is an inherited channelopathy that can cause sudden cardiac death. Recent research has implicated mutations in the SLC4A3 gene as a cause of SQTS, but the mechanisms of shortened action potential duration (APD) and arrhythmia vulnerability have not been described. This study aims to evaluate the underlying pathophysiology causing a shortened APD and ventricular arrhythmia vulnerability in SLC4A3-associated SQTS through mechanistic studies of novel SLC4A3 mutations responsible for familial SQTS. METHODS: This study evaluated the function and pathop

Introducing Cardiac Magnetic Resonance in Athlete Screening: An Initial Moroccan Experience With Professional Football Players.

Background and objectives Pre-participation cardiovascular screening in athletes relies primarily on electrocardiography and transthoracic echocardiography. While cardiac magnetic resonance (CMR) is increasingly recognized as a valuable complementary imaging modality, its integration into routine athlete evaluation remains variable across healthcare settings. In our context, the recent expansion of access to CMR has enabled its use as a second-line tool in professional athletes initially assessed by first-line screening examinations. This study aimed to describe CMR findings in asymptomatic pr

Acute cardiovascular changes following heat exposure during simulated shipboard firefighting.

Like structural firefighting, shipboard firefighting requires extreme exertion. However, shipboard firefighting may be a unique cardiovascular stress as most sailors lack extensive firefighting experience and may complete significant work before reaching the fire scene. Several indices of acute cardiovascular impairment have been associated with the high prevalence of sudden cardiac death among structural firefighters. However, acute cardiovascular responses to shipboard firefighting have yet to be described. The aim of this study was to investigate cardiac, macrovascular and microvascular res

The Novel ACTC1 p.Gly50Ser Variant Is Associated With Arrhythmia and Secondary Features of HCM Without Hypertrophy.

The key diagnostic criterion for hypertrophic cardiomyopathy is the presence of otherwise unexplained hypertrophy. Current definitions of HCM rely on specific thresholds to establish a diagnosis, while guideline directed risk stratification algorithms take its magnitude into consideration. Presently, secondary features such as myocardial crypts and valve abnormalities in the absence of hypertrophy are solely considered as markers of preclinical disease in variant carriers, with no perceptible excess risk for arrhythmic events. Strictly adhering to hypertrophy as the sole criterion for a diagno

PPI in LifeMap-QUEST: an example of co-producing videos in different languages to support inclusion in a clinical study.

Recruiting participants into clinical trials is often challenging, particularly when trying to involve people from diverse or disadvantaged communities. This article describes how we co‑produced video resources to support inclusion in the LifeMap‑QUEST clinical study. LifeMap is a new approach to diagnosing people who may be at risk of Sudden Cardiac Death (SCD). Recognising the longstanding under‑representation of ethnic minority groups in research, and the barriers posed by language, literacy, and trust, the LifeMap‑QUEST Patient and Public Involvement (PPI) Advisory Group worked with commun
