Eighteen-year analysis of anaesthetic management in Brugada syndrome: The BRUGANAES study.

BACKGROUND: Brugada syndrome (BrS) is a genetic disorder that increases the risk of ventricular tachyarrhythmias and sudden cardiac death (SCD). Certain drugs (propofol, local anaesthetics), fever, bradycardia, increased vagal tone and electrolyte imbalances can trigger or worsen BrS arrhythmias. OBJECTIVE: To evaluate the incidence of malignant ventricular arrhythmias during the perioperative period in patients with BrS, hypothesising that common anaesthetic drugs may be safe to use during daily clinical practice. DESIGN: The BRUGANAES study was an observational, retrospective project inclu

Non-juvenile familial form of life-threatening arrhythmias caused by the Ryanodine Receptor type 2 c.13823 G>A, p.(Arg4608Gln) pathogenic variant: Atypical catecholaminergic polymorphic ventricular Tachycardia or misdiagnosis?

BACKGROUND: Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT) is a rare inherited channelopathy, responsible for potentially lethal malignant arrhythmic episodes. Atypical non-juvenile form of CPVT may not mislead an alternative diagnosis of calcium release deficiency syndrome (CRDS). CASE: The index case was a 58 years-old woman who experienced aborted sudden cardiac arrest. The initial complete diagnostic workup (including norepinephrine challenge) was completely negative. She was implanted with an entirely subcutaneous defibrillator. During her follow-up, she received an appropr

Kambo Administration and Its Association With Sudden Death: Clinical and Forensic Perspectives From a Systematic Review.

Kambo (Kambô) is a secretion of an amphibian traditionally used by indigenous tribes in the Amazon region. This secretion is derived from the giant monkey frog (Phyllomedusa bicolor) and is utilized in tribal cleansing rituals known as Kambo ceremonies. Kambo is purported to offer several benefits, both physical and psychological. Proponents of this practice claim that it can enhance physical endurance, sharpen mental clarity, and boost overall health. However, the side effects, such as tachycardia, blood pressure fluctuations, and gastrointestinal distress, highlight the need for caution. In

Natural History, Phenotype Spectrum, and Clinical Outcomes of Desmin (DES)-Associated Cardiomyopathy.

BACKGROUND: Pathogenic/likely pathogenic (LP) desmin (DES) variants cause heterogeneous cardiomyopathy and skeletal myopathy phenotypes. Limited data suggest a high incidence of major adverse cardiac events (MACEs), including cardiac conduction disease, sustained ventricular arrhythmias (VA), and heart failure (HF) events (HF hospitalization, left ventricular assist device/cardiac transplant, HF-related death) in patients with pathogenic/LP DES variants. However, pleiotropic presentation and small cohort sizes have limited clinical phenotype and outcome characterization. We aimed to describe t

Sudden Death of a Four-Day-Old Newborn Due to Mitochondrial Trifunctional Protein/Long-Chain 3-Hydroxyacyl-CoA Dehydrogenase Deficiencies and a Systematic Literature Review of Early Deaths of Neonates with Fatty Acid Oxidation Disorders.

Mitochondrial trifunctional protein (MTP) and long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) deficiencies have been a part of the Slovenian newborn screening (NBS) program since 2018. We describe a case of early lethal presentation of MTPD/LCHADD in a term newborn. The girl was born after an uneventful pregnancy and delivery, and she was discharged home at the age of 3 days, appearing well. At the age of 4 days, she was found without signs of life. Resuscitation was not successful. The NBS test performed using tandem mass spectrometry (MS/MS) showed a positive screen for MTPD/LCHADD. Genet

Advances in Cardiovascular Pharmacotherapy. I. Cardiac Myosin Inhibitors.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiomyopathy. The disease is characterized by asymmetric left ventricular (LV) remodeling with myocyte disarray and interstitial fibrosis, a hypercontractile state, dynamic subaortic obstruction of the LV outflow tract, impaired LV diastolic function, atrial and ventricular arrhythmias, and sudden cardiac death. HCM occurs as a result of pathological alterations in the cardiac myocyte's chemomechanical cycle, in which an enhanced rate of myosin-actin crossbridge formation and destabilization of the energy-conserving "super-relaxe

Sudden Cardiac Death: The Role of Molecular Autopsy with Next-Generation Sequencing.

Molecular autopsy is a term employed to describe the investigation of the cause of death through the analysis of genetic information using biological samples collected post-mortem. Its utility becomes evident in situations where conventional medico-legal autopsy methods are not able to identify the cause of death, i.e., in sudden cardiac death (SCD) cases in young individuals, where deaths are commonly due to genetic cardiac conditions, such as cardiomyopathies and channelopathies. The recent advancement in high-throughput sequencing techniques, such as next-generation sequencing (NGS), has al

25-year follow-up on marked ventricular repolarization abnormalities in athletes: Long-term outcomes and cardiovascular prognosis.

BACKGROUND: The presence of ventricular repolarization abnormalities (VRA) in young asymptomatic athletes is rare and may represent initial expression of underlying ongoing cardiomyopathy, increasing risk of sudden cardiac death. This study aims to evaluate the long-term prognosis of VRAs in this specific population. METHODS: A cohort of 28 young asymptomatic Caucasian male athletes with VRA on 12‑leads electrocardiogram (ECG), initially underwent transthoracic echocardiography (TTE) and treadmill test between1990-2000 which excluded cardiac diseases. The same tests where repeated between 202

Sympathetic stimulation can compensate for hypocalcaemia-induced bradycardia in human and rabbit sinoatrial node cells.

Regular activation of the heart originates from cyclic spontaneous depolarisations of sinoatrial node cells (SANCs). Variations in electrolyte levels, commonly observed in haemodialysis (HD) patients, and the autonomic nervous system (ANS) profoundly affect the SANC function. Thus we investigated the effects of hypocalcaemia and sympathetic stimulation on the SANC beating rate (BR). The β-adrenergic receptor (β-AR) signalling cascade, as described by Behar et al., was incorporated into the SANC models of Severi et al. (rabbit) and Fabbri et al. (human). Simulations were conducted across variou

Rare Metastatic Mesothelioma Occupying Intra-Atrial Cavity, Released by an Emergency Surgery: A Case Report and Literature Review.

INTRODUCTION: Cardiac surgery for cardiovascular-associated mesothelioma has a poor prognosis. However, life-saving surgery is unavoidable to maintain circulation. This report describes a case in which metastatic intracardiac mesothelioma triggered sudden respiratory failure, which was reduced by surgical resection. CASE PRESENTATION: An 81-year-old man with a history of asbestos exposure presented to our hospital with sudden onset of dyspnea. Prior to this event, the pleura was involved in an epithelial malignancy, which was immunohistochemically negatively stained with anti-D2-40, WT-1, or

Autoimmune cardiac channelopathies and heart rhythm disorders: A contemporary review.

Cardiac arrhythmias still represent a major health problem worldwide, at least in part because the fundamental pathogenic mechanisms are not fully understood, thus affecting the efficacy of therapeutic measures. In fact, whereas cardiac arrhythmias are in most cases due to structural heart diseases, the underlying cause remains elusive in a significant number of patients despite intensive investigations even including postmortem examination and molecular autopsy. A large body of data progressively accumulated during the last decade provides strong evidence that autoimmune mechanisms may be inv

Autopsy results and factors associated with sudden cardiac death in young individuals with congenital heart disease - a nationwide study.

OBJECTIVES: Sudden cardiac death (SCD) is a leading cause of mortality among individuals with congenital heart disease (CHD), and risk stratification remains challenging. This study aimed to describe the underlying structural cardiac abnormalities in a national cohort of SCD victims with CHD, their socioeconomic status, and interactions with the healthcare system before death. METHODS: The Swedish study of Sudden Cardiac Death in the Young, 2000-2010, included SCD victims under 36 years, along with population-based controls and their parents. Of 903 SCD victims, 39 with autopsy-defined CHD we

Sudden cardiac death due to isolated right ventricle thromboembolism in a young male.

Sudden cardiac death due to an isolated right ventricular thromboembolism is an uncommon cause of death. In the reported case, a healthy, asymptomatic young male with no past medical history outside of obesity and his age was found unresponsive by a family member who suspected him to be sleeping. Examination of the heart at autopsy revealed a large serpiginous, organized and obstructive blood clot compromising most of the right ventricle with no other cardiac abnormalities and without any evidence of a pulmonary embolism. The patient was found to not have any history of substance abuse, geneti

Epicardial Adipose Tissue in Arrhythmogenic Cardiomyopathy.

Arrhythmogenic cardiomyopathy (ACM) is an inherited heart disease characterized by fibrofatty replacement of the ventricular myocardium, with an estimated prevalence of 1:5000 people in the general population. Sudden cardiac death is the first manifestation of this disease in 16-23% of patients with ACM. Fibrofatty infiltration can be identified with noninvasive cardiac magnetic resonance. Studies of epicardial fat deposits have suggested pathogenic roles of epicardial fats in mediating cardiac diseases and arrhythmias. Although myocardial fat infiltration has been well described in ACM, chang

Ventricular fibrillation induced by atrial threshold search: a case report.

BACKGROUND: Implantable cardioverter defibrillators (ICDs) have been proven to reduce the risk of sudden cardiac death from ventricular tachyarrhythmias. However, ICDs can sometimes induce malignant arrhythmias. We describe a case of ventricular fibrillation triggered by an automatic atrial threshold search. CASE SUMMARY: A 72-year-old man presented after a syncopal episode. His medical history included moderate aortic regurgitation and a symptomatic second-degree atrioventricular (AV) block, for which he received a dual-chamber pacemaker in 2013, later upgraded to a CRT-D due to pacing-induc

The diagnostic role of pharmacological provocation testing in cardiac electrophysiology: a clinical consensus statement of the European Heart Rhythm Association and the European Association of Percutaneous Cardiovascular Interventions (EAPCI) of the ESC, the ESC Working Group on Cardiovascular Pharmacotherapy, the Association of European Paediatric and Congenital Cardiology (AEPC), the Paediatric & Congenital Electrophysiology Society (PACES), the Heart Rhythm Society (HRS), the Asia Pacific Heart Rhythm Society (APHRS), and the Latin American Heart Rhythm Society (LAHRS).

The pharmacological provocation test is a pivotal tool in cardiac electrophysiology for the diagnosis of potential causes of sudden cardiac death, sudden cardiac arrest (SCA), arrhythmias, symptoms, or ECG abnormalities. The 2022 European Society of Cardiology Guidelines for the Treatment of Ventricular Arrhythmias and Prevention of Sudden Cardiac Death offered guidance on provocation testing but did not describe the indications and requirements in depth. This clinical consensus statement, led by the European Heart Rhythm Association and approved by major international stakeholders, aims to ad

AI-based detection and classification of anomalous aortic origin of coronary arteries using coronary CT angiography images.

Anomalous aortic origin of the coronary artery (AAOCA) is a rare cardiac condition that can lead to ischemia or sudden cardiac death, yet it is often overlooked or falsely classified in routine coronary CT angiography (CCTA). Here, we developed, validated, externally tested, and clinically evaluated a fully automated artificial intelligence (AI)-based tool for detecting and classifying AAOCA in 3D-CCTA images. The discriminatory performance of the different models achieved an AUC ≥ 0.99, with sensitivity and specificity ranging 0.95-0.99 across all internal and external testing datasets. Here,

Acquired Genotype-Positive Long QT Syndrome After Pediatric Heart Transplantation.

BACKGROUND: Congenital long QT syndrome (LQTS) is rare but significant, as it carries a risk for ventricular arrhythmias and sudden cardiac death. Its diagnosis can be made clinically by serial ECGs, ambulatory ECG monitoring, and exercise stress testing; however, genetic testing is confirmatory in the majority of cases. METHODS: Here, we describe a rare case of phenotype-positive LQTS in a 6-year-old heart transplant recipient, confirmed 5 years after transplantation to be genotype-positive and thus "acquired" from the transplanted heart. RESULTS: Recognition of a persistently prolonged QTc
