Rupture of ventricular aneurysm is an unusual pathology in children. This case describes a preterm infant who had been in the hospital since birth due to necrotizing enterocolitis, which was complicated by narrowing of the ascending colon that required surgery. Throughout the entire period, the infant exhibited signs of infection, first caused by Staphylococcus haemolyticus and subsequently by Staphylococcus epidermidis. Several echocardiographic examinations did not reveal any significant changes; however, the final echocardiography revealed a structure connected to the left ventricular cavit

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) in children is a rare but devastating event, with an estimated incidence of 9 per 100 000 person-years and a survival rate of less than 10%. OHCA may be the presenting event in children with undiagnosed cardiovascular disease. A substantial proportion of adults experiencing an unexpected OHCA report previous symptoms, but whether the same is true in paediatric survivors has not been systematically evaluated previously. This study aims to describe cardiac symptoms in children prior to presenting with OHCA.
METHODS: Clinical characteristics and

BACKGROUND AND AIMS: The POTCAST trial (n=1,200) showed that targeting high-normal plasma potassium levels reduced ventricular arrhythmias in patients with Implantable Cardioverter-Defibrillators (ICDs). We compared changes in plasma potassium and described outcomes across the three regimens used to increase plasma potassium.
METHODS: We studied 600 control participants and 572 intervention participants who completed potassium up-titration in POTCAST. Participants in the intervention arm were categorized according to treatment after plasma-potassium up-titration into: a) mineralocorticoid-rec

UNLABELLED: The N-terminal fragments of gasdermin (GSDM-NT) form pores on the plasma membrane that initiate pyroptosis. However, the presence of GSDM-NT pores does not necessarily result in cell death, allowing some cells to survive GSDM-mediated pyroptosis. Understanding the regulators of response to the formation of GSDM-NT pores is crucial for revealing the strategies to harness the potential of pyroptosis for treating cancer. In this study, we found that myosin 1G (MYO1G) enabled cells to resist death driven by membrane rupture. Mechanistically, MYO1G tethered GSDME-NT pores to caveolin 1

BACKGROUND: True distal left main (LM) bifurcation percutaneous coronary intervention (PCI) is technically challenging. A hybrid strategy using a drug-eluting stent (DES) in the main branch and a drug-coated balloon (DCB) in the side branch may reduce permanent metal at the bifurcation, but LM-specific data remain limited.
OBJECTIVES: To describe 1-year clinical outcomes after completed hybrid DES-DCB and two-stent strategies for true distal LM bifurcation lesions and to provide an exploratory unadjusted comparison between groups.
METHODS: This single-center retrospective cohort registry inc

Sudden cardiac death (SCD) remains a major global health challenge, characterized by high incidence, low survival rates, and often presenting as the first manifestation of underlying disease. This review provides a comprehensive overview of the genetic and epigenetic landscape of non-familial SCD, emphasizing the multifactorial nature of this complex clinical endpoint. We summarize common single nucleotide polymorphisms and insertion/deletion variants associated with SCD, identified through candidate-gene-based studies and genome-wide association studies, highlighting risk variants clustered i

Over 300,000 people die of sudden cardiac death (SCD) in the US annually. Implantable cardioverter-defibrillators (ICDs) have been shown to be more effective than antiarrhythmic drugs for the prevention of SCD in specific susceptible populations. Many patients in whom ICDs have been implanted receive concomitant therapy with antiarrhythmic drugs, for the purpose of reducing the frequency of appropriate and inappropriate defibrillation shocks. Drugs may influence defibrillation capacity and therefore influence the function of ICDs. The objective of this article is to review and update the liter

Lethal cardiac arrhythmias contribute to mortality in a number of pathological conditions. Several parameters obtained from a non-invasive, easily obtained electrocardiogram (ECG) are established, well-validated prognostic indicators of cardiac risk in patients suffering from a number of cardiomyopathies. Increased heart rate, decreased heart rate variability (HRV), and increased duration and variability of cardiac ventricular electrical activity (QT interval) are all indicative of enhanced cardiac risk. In animal models, it is valuable to compare these ECG-derived variables and susceptibility

INTRODUCTION: Sudden cardiac death (SCD) is a widespread and devastating event. As much as a quarter or half of all cardiovascular (CV) deaths are due to SCD. A lot of work is done to decrease the number of SCD. Recently several publications pointed out (very) high CV risk in Cushing's syndrome (CS) and CV risk is generally associated with SCD. Therefore, the aim of the study is to analyze the available publications regarding CS and SCD.
MATERIALS AND METHODS: We initiated the search in SCOPUS using the terms "Cushing" and "sudden death". The additional documents were retrieved from Medline,

OBJECTIVES: Inherited arrhythmia syndromes (IAS) are a group of rare disorders that result from genetic mutations in several genes including congenital long QT syndrome, Brugada syndrome (BrS), short QT syndrome, and Catecholaminergic Polymorphic Ventricular Tachycardia (CPVT). Affected individuals may have various symptoms including sudden cardiac death (SCD). Few reports have highlighted long QT syndrome from the Arbian Gulf region. The current study aims to describe demographics of children and adults with inherited arrhythmia syndromes; report the presenting clinical features, genetic muta

BACKGROUND: Congenital coronary anomalies represent a heterogeneous spectrum of anatomical variants that may predispose to myocardial ischemia, arrhythmias, and sudden cardiac death, particularly when the vessel follows an interarterial course. Although their prevalence is low, timely recognition is essential to prevent unnecessary interventions and guide appropriate treatment. The objective was to describe a case of anomalous origin of the left anterior descending (LAD) artery from the right coronary artery, highlighting the role of multimodality imaging in anatomical characterization and the

During a cardiological evaluation, premature ventricular contractions (PVCs) may be found, which require careful evaluation using diagnostic methods to discern in which patients there is a high risk of structural heart disease and sudden cardiac death. Epidemiological data show that PVCs occur in about 75% of healthy (or apparently healthy) individuals, without evidence of underlying structural heart disease. PVCs are generally asymptomatic, but can also manifest as palpitations, dyspnea, syncope and fatigue. The first clinical management includes a clinical evaluation, anamnesis and a 12-lead

Calmodulin is an intracellular Ca2+ sensor that regulates numerous cellular processes through binding effector proteins and changing their activity. Humans have three calmodulin paralogs, CALM1, CALM2 and CALM3, encoding identical proteins, and missense variants in all three are included in the ACMG recommendations for reporting of secondary findings related to early onset sudden cardiac death caused by arrhythmias long QT syndrome and CPVT. Recently, a subset of individuals with de novo variants in calmodulin genes were described who also presented with neurologic phenotypes. Here we report t

INTRODUCTION: The implantable cardioverter defibrillator (ICD) is a cardiac device recommended for use to prevent the occurrence of sudden cardiac death (SCD) in post-myocardial infarction (MI) patients with reduced left ventricular ejection fraction (LVEF). The evidence informing this guidance comes from landmark trials that are now more than 20 years old. The risk-benefit profile of ICD for the contemporary target population may have changed substantially since then, which raises the question of whether there is evidence for sparing patients a procedure associated with potentially severe com

BACKGROUND: Coronary artery anomalies represent morphological deviations from standard coronary anatomy, occurring in less than 1% of the general population. Although coronary artery anomalies are often asymptomatic, they can lead to serious complications, including sudden cardiac death and cardiac ischemia. Percutaneous coronary intervention in patients with coronary artery anomalies presents challenges, with an increased risk of ostial dissection, particularly in the setting of ST-segment elevation myocardial infarction.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) is characterized by fibrofatty myocardial replacement predisposing patients to arrhythmias and sudden cardiac death.
CASE SUMMARY: We describe a 51-year-old man with syncope, characteristic changes on electrocardiogram, and a family history of sudden cardiac death. Imaging showed right ACM features in addition to multiple left ventricular (LV) crypts. Cardiac magnetic resonance imaging (CMR) confirmed the right ACM features and further revealed LV fibrosis corresponding to the crypts. The patient was diagnosed with biventricular ACM and received

Arrhythmogenic right ventricular cardiomyopathy (ARVC) is a heritable heart disease that predisposes to sudden cardiac death. Diagnosis is challenging given the lack of specificity of diagnostic criteria and disease mimics. Cardiac magnetic resonance contributes to the diagnosis of ARVC through its advanced assessment of the right ventricle. We describe a 76-year-old woman who met 3 major and 1 minor diagnostic criteria of ARVC, including a major criterion on cardiac magnetic resonance. These criteria only emerged after severe right ventricular infarct 8 years prior. This case highlights the i

BACKGROUND: Isolated nonischemic left ventricular scar (NILVS), identified by cardiac magnetic resonance, is an increasing finding in athletes and may be the substrate for life-threatening arrhythmias. Clinical significance in asymptomatic athletes is yet to be investigated.
OBJECTIVES: This study sought to describe the clinico-genetic profile and follow-up of asymptomatic athletes diagnosed with NILVS through preparticipation screening.
METHODS: We evaluated 40 athletes (90% males, 44 [range:33-52] years) including 9 elites, with isolated NILVS involving at least 2 segments, no previous maj

BACKGROUND: Cardiac hemangiomas located in the right heart can lead to serious complications, including right ventricular outflow tract (RVOT) obstruction, arrhythmias, and sudden cardiac death. Although surgical resection remains the primary treatment, complete excision of intramural tumors poses risk of impairing cardiac function. This case report describes the successful resection of a large right ventricular hemangioma using intraoperative voltage mapping, which enable maximal tumor removal while preserving myocardial integrity and preventing postoperative heart failure. CASE PRESENTATION:

AIMS: To describe and quantify sex differences in the screening electrocardiograms (ECGs) of athletes.
METHODS AND RESULTS: Five databases (MEDLINE, EMBASE, Scopus, SPORTDiscus, and Web of Science) were searched from inception until October 2024. Included studies were original research articles examining athletes aged 16-40 years, who had a 12-lead screening ECG, and where analysis was stratified by sex. Risk of bias was assessed using a validated tool. A meta-analysis was performed, using a random-effects model, assessing proportions of athlete normal, borderline, and abnormal ECG features (
