Long QT syndrome in children and adolescents: risk factors and outcomes in a large German cohort.

BACKGROUND AND AIMS: This study describes outcomes in a large contemporary national cohort of children and adolescents with long QT syndrome (LQTS). METHODS: Data analysis of 548 paediatric LQTS patients at 12 German tertiary care centres. Primary outcome was the occurrence of a major arrhythmic event (MAE) defined as sudden cardiac death (SCD), aborted cardiac arrest (ACA) and appropriate implantable cardioverter-defibrillator (ICD) therapy before age 18 years. RESULTS: Patients (48.9% male) presented at a median age of 6.3 (interquartile range 0.5-11.7) years with incidental findings (26.5

Left Anterior Descending Artery to Right Coronary Artery Bifurcation Stenting with Culotte Technique in Acute Inferior Myocardial Infarction.

Coronary artery anomalies are described as abnormal origin, course, or termination of any of the three main epicardial coronary arteries. They are generally not seen in cardiac complaints and are detected incidentally. However, some may be related to an increased risk of myocardial infarction, syncope, ventricular arrhythmias, and exercise-induced sudden cardiac death. Single coronary artery anomaly is very rare in coronary artery anomalies and provides a large myocardial perfusion area. Here we described an unusual very rare single coronary artery anomaly that the right coronary artery (RCA)

Ventricular electrical storm refractory to 250 defibrillations during myomectomy: the critical role of VA-ECMO and multidisciplinary management.

Ventricular electrical storm (VES) is a critical cardiac emergency with high mortality, often refractory to treatment. They are typically associated with certain cardiovascular diseases, cardiac surgeries, hereditary arrhythmia syndromes or ICD shocks. Malignant intraoperative VES during non-cardiac surgeries represents a rare subset within cardiovascular medicine, yet their under-representation in clinical literature is disproportionate to the critical nature of the condition. This case report describes a previously healthy patient who developed sudden intraoperative ventricular fibrillation

Chronic Cardiac Resynchronization Therapy-Defibrillator Infection Leading to Sepsis and Bilateral Septic Pulmonary Emboli: A Case Report.

Cardiac implantable electronic devices (CIEDs), including cardiac resynchronization therapy-defibrillators (CRT-Ds) and other pacemaker systems, are widely used in patients with varying degrees of heart failure to improve cardiac function and reduce the risk of sudden cardiac death. Despite their life-saving benefits, infections related to CRT-Ds present significant clinical challenges. These infections can lead to serious complications, including endocarditis, pocket infections, and device-associated bloodstream infections, often resulting in prolonged hospitalization and potentially life-thr

Kawasaki disease with persistent severe psoriatic-like skin manifestations and sudden cardiac death.

This case report describes a 1.3-year-old infant with an atypical presentation of Kawasaki Disease (KD) following exposure to COVID-19 within the family and positive SARS-CoV-2 IgG antibodies. The clinical course was marked by significant left ventricular (LV) dysfunction, progressive coronary artery involvement, and complicated by severe dermatologic manifestations including Steven-Johnson-like lesions and psoriatic-like eruptions. Despite initial improvement in cardiac function, the infant developed multiple coronary aneurysms and succumbed to sudden cardiac death. This case highlights the d

Cardiac involvement in female elite athletes with carrier status of Duchenne muscular dystrophy.

Duchenne muscular dystrophy is a muscle-wasting, progressive, X-linked inherited disease in young male individuals, who-aside from peripheral muscular impairment-may also suffer from severe cardiac complications. In women who are muscular dystrophy carriers (MDCs), muscular symptoms and cardiac complications are less severe or even absent. While male individuals with muscular dystrophy are not usually able to perform strenuous exercise, women who are MDCs can exercise at mild, or even high, intensity. However, the impact of participating in elite sports, particularly endurance sports with high

The Diagnostic Value of Copy Number Variants in Genetic Cardiomyopathies and Channelopathies.

Sudden cardiac death represents an unexpected death for which a strong underlying genetic background has been described. The primary causes are identified in cardiomyopathies and channelopathies, which are heart diseases of the muscle and electrical system, respectively, without coronary artery disease, hypertension, valvular disease, and congenital heart malformations. Genetic variants, especially single nucleotide variants and short insertions/deletions impacting essential myocardial functions, have shown that cardiomyopathies display high heritability. However, genetic heterogeneity, incomp

QT variability and myocardial repolarization in sleep apnea: implications for cardiac risk.

PURPOSE: Due to the increased risk of sudden cardiac death, we decided to assess ECG parameters related to the stability of the myocardial repolarization period, i.e. the corrected QT interval (QTc) and derivatives describing its variability (QTV and QTVi). METHODS: Healthy volunteers (n = 187) with visceral obesity, aged 35-65 years, were included. Each participant underwent Holter-ECG and polygraphy simultaneously. According to the severity of breathing disorders during sleep the cohort was divided into 3 groups: Respiratory Event Index (REI) 5-14/hour, REI 15-30/hour, REI ≥ 30/hour. The

Long QT Interval Syndrome and Female Sex-Review and Case Report.

Background and Clinical Significance: Congenital LQTS is a life-threatening condition, resulting from a mutation of the gene encoding the cardiac ion channels, which results in prolongation of the ventricular action potential. Genetic screening of family members in symptomatic and asymptomatic patients is crucial for the prevention of sudden cardiac death. There are a number of detected mutations of congenital LQTS, of which the three forms LQT1, LQT2, and LQT3 are the best described. In addition to the described ECG morphology, the key triggers and treatment approach are described. This empha

Optimizing the Primary Prevention of Sudden Cardiac Death in Patients With Heart Failure.

Implantable cardioverter-defibrillators (ICDs) protect patients from sudden cardiac death (SCD). Landmark trials demonstrating their efficacy for primary prevention in patients with heart failure (HF) used reduced left ventricular ejection fraction (LVEF) as a major inclusion criterion and current recommendations for ICD implantation rely on this variable in patient selection. However, contemporary medical management has reduced the risk of SCD in patients with reduced LVEF so that an increasingly large proportion of this population never requires the protection offered by the device. Although

Electrical alternans of the Q-T interval and fatal arrhythmias caused by neonatal cardiac tumor: a case report.

Primary cardiac tumors in the fetal and neonatal populations are rare. In neonates, these tumors may present with a range of clinical manifestations, from asymptomatic cases to severe arrhythmias, valvular obstruction, and cardiac dysfunction. This report describes a case of fatal malignant arrhythmia caused by a cardiac tumor in an 8-day-old neonate. The electrocardiogram at birth revealed multiple abnormalities, including ST-T segment alterations, intraventricular conduction block, and bundle branch block. By the sixth postnatal day, the patient developed rapid, polymorphic malignant ventric

Frequency, Management, and Outcomes of Outpatient Hyperkalemia: A Population-Based Cohort Study.

BACKGROUND: Hyperkalemia is a potentially life-threatening condition, with guidelines recommending urgent treatment when the serum potassium level is greater than 6.0 mmol/L. However, these recommendations are inconsistent, leading to diverse approaches to patient care. OBJECTIVES: The primary objectives were to use population-based datasets to determine how often outpatient hyperkalemia (K > 6.2 mmol/L) occurs and how frequently patients present to the emergency department (ED) within 24 hours of the hyperkalemia report. Secondary objectives were to compare the characteristics of patients wh

Unusual catecholaminergic polymorphic ventricular tachycardia and bradycardia caused by a novel triadin variant in 2 siblings from a Malian family.

RATIONALE: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a hereditary arrhythmia syndrome that can cause sudden cardiac death, particularly in young individuals. CPVT is often linked to triadin (TRDN) variants that disrupt calcium regulation in the cardiac muscle. Although TRDN-associated CPVT is well documented, its association with resting sinus bradycardia is rarely reported. We describe 2 siblings from a consanguineous Malian family presenting with exertional syncope and bradycardia, in whom a novel homozygous pathogenic TRDN variant was identified. PATIENT CONCERNS: The

Primary Angioplasty for STEMI Patients With Anomalous Left Coronary Artery Arising From Right Coronary Sinus.

BACKGROUND: Anomalous origin of the left main coronary artery from the right sinus of Valsalva (ALCA-RSV) is exceedingly rare yet complicates emergency angioplasty for ST-segment elevation myocardial infarction. CASES SUMMARY: We describe 2 ST-segment elevation myocardial infarction patients in whom ALCA-RSV was identified during primary percutaneous coronary intervention. Despite procedural challenges, timely revascularization was achieved using customized access, catheter strategies, and adjunctive devices. Postprocedural computed tomography coronary angiography excluded malignant courses.

A Comprehensive Review of a Mechanism-Based Ventricular Electrical Storm Management.

The electrical ventricular storm (VES) is defined as multiple sustained ventricular arrhythmias arising in a short time, often refractory to standard antiarrhythmic treatment. The three pillars of the physiopathogenesis of the VES are autonomic dysfunction, triggers, and an altered ventricular substrate. Incessant or highly recurrent ventricular arrhythmia impacts the hemodynamic status by worsening heart failure and increasing mortality. A stepwise, team-based, and tailored therapeutic approach is required to stop ventricular arrhythmia and regain the hemodynamic and electric stability of the
