Early repolarization pattern in patients with malignant coronary artery anomalies.

BACKGROUND: Potentially malignant coronary artery anomalies (CAAs) are single coronary artery, myocardial bridges with deep course, ALCAPA, large or multiple fistulas, and anomalous coronary artery from the opposite sinus, interarterial course (IAC) anomalies, which can lead to life-threatening complications. It is well-known that the early repolarization pattern (ERP) is associated with sudden cardiac death (SCD).This investigation aimed to assess the relationship between ERP and poor cardiac outcomes in patients with malignant CAAs. METHODS: Imaging findings, electrocardiographic features,

Family Members' Experiences Communicating With Coroners in Suspected Hereditary Sudden Cardiac Death Cases: A Mixed Methods Study in Canada.

BACKGROUND: Sudden cardiac death (SCD) can be caused by heritable cardiac conditions, as such, up to 50% of family members may be at risk. Coroners are responsible for communicating this risk, in addition to investigating the cause of death. The purpose of this study was to explore how family members of SCD victims experience the type, timing, and suitability of communication with coroners about the cause of death and their own risk for SCD. METHODS: We conducted an explanatory sequential mixed methods study. Eligible family members of SCD victims aged 2 to 70 years, who died in 2021 from a p

Avian Infective Endocarditis Associated with Vagococcus fluvialis: A Case Report and Literature Review.

Avian infective endocarditis is an uncommon but severe disease that is typically diagnosed postmortem because of nonspecific clinical signs and rapid progression. In the present study, five broiler chickens (n = 5) from a commercial flock were examined with septicemia and lesions suggestive of endocarditis. This study reports the first molecularly confirmed and characterized case of valvular endocarditis associated with multidrug-resistant Vagococcus fluvialis in poultry and provides a comprehensive review of bacterial endocarditis in avian species. The case involved a broiler chicken that pre

Trapped in the Trabeculae: A Rare Case of Noncompaction Cardiomyopathy and Large Left Ventricular Thrombus.

Left ventricular noncompaction (LVNC) is a rare primary cardiomyopathy characterized by a failure of myocardial morphogenesis, resulting in a prominent, characteristic "sponge-like" trabeculated endocardial layer. Clinical presentation is classical for heart failure, ventricular arrhythmias, and systemic thromboembolism, which defines its severity. Here, we describe a 34-year-old male with no prior medical history who presented with progressive dyspnea and volume overload. On initial evaluation, severe biventricular failure, stage 3b chronic kidney disease (type 2 cardiorenal syndrome), and si

How Advanced is Nanomedicine to Treat Atherosclerosis? A Comprehensive Review of the Literature.

Atherosclerosis (AS) is a major underlying cause of cardiovascular disease. Rupture of unstable atherosclerotic plaques can induce severe acute cardiovascular events and sudden cardiac death. Therefore, developing targeted interventional therapies for atherosclerotic plaques is clinically important to improve cardiovascular mortality. With the advancement of nanomedicine, nanomaterials have demonstrated great potential in atherosclerosis treatment due to their unique compositional/structural features, synthesis strategies, and surface modifications. Based on the pathological characteristics of

Ventricular conduction is a marker for arrhythmic risk in SCN5A-E1784K overlap sodium channel disease.

AIMS: SCN5A-E1784K (c.5350G>A) is the most common variant associated with the long QT (LQTS) and Brugada syndromes (BrS). It can manifest variably as LQTS, BrS, and/or conduction disorders. This presents a challenge for risk stratification. We aimed to describe clinical and ECG characteristics and identify risk markers that associate with arrhythmic events. METHODS AND RESULTS: We undertook a retrospective observational multicentre study of a large cohort of 231 subjects with SCN5A-E1784K from Europe, USA, and Japan. Comprehensive demographic and clinical data, including initial presentation

Validation of circulating miR-323a-3p and miR-625-3p to classify hypertrophic cardiomyopathy in Friedreich's ataxia.

Friedreich's ataxia (FRDA) is an inherited neurodegenerative disorder frequently complicated by hypertrophic cardiomyopathy (HCM), a major cause of morbidity and mortality in these patients. Conventional protein biomarkers, such as high-sensitivity troponin or collagen turnover markers, provide only modest diagnostic accuracy, highlighting the need for more sensitive tools. Circulating microRNAs (miRNAs) have emerged as promising non-invasive biomarkers, but independent validation in FRDA remains limited. We analyzed a cohort of FRDA patients (n = 34) and age-, sex-, and race-matched healthy c

Isolated Brugada Pattern in Limb Leads: An Indicator of Sudden Cardiac Death.

Brugada syndrome is classically characterized by coved-type ST-segment elevation in leads V1 or V2. Repolarization abnormalities in peripheral leads have been previously reported in patients with Brugada syndrome; however, spontaneous isolated type 1 Brugada patterns confined to non-right precordial leads remain rarely described, particularly in individuals without a prior diagnosis of Brugada syndrome. TAKE-HOME MESSAGE: Spontaneous isolated type 1 Brugada patterns confined to non-right precordial leads may suggest underlying Brugada syndrome and serve as potential markers of malignant ventri

Effectiveness of Online Training on Cardiopulmonary Resuscitation and Automated External Defibrillator Use in Simulations With Rural Populations: Quasi-Experimental Community Intervention Study.

BACKGROUND: Sudden death due to cardiorespiratory arrest has a high mortality rate and often occurs outside hospital settings. Prompt initiation of cardiopulmonary resuscitation (CPR) by bystanders, along with the use of an automated external defibrillator (AED), has been shown to double survival rates. Given the challenges of ensuring timely emergency response in rural areas, implementing basic CPR training programs can help improve survival outcomes. OBJECTIVE: This study aimed to evaluate the effectiveness of online CPR-AED training delivered to participants from a rural area of Camp de Ta

A Rare Presentation of Infective Endocarditis Complicating Severe Aortic Valve Stenosis.

BACKGROUND: Although less frequently encountered, aortic valve stenosis is associated with complications separate from its hemodynamic burdens, such as infective endocarditis. CASE SUMMARY: We report the case of a 77-year-old female patient with regular cardiac follow-up in the setting of an asymptomatic severe aortic stenosis, who presented to the emergency department with signs and symptoms of sepsis and acute decompensated heart failure. Echocardiography revealed two vegetations attached to the tricuspid valve, an abscess of the anterior aortic ring, and a high-velocity ventricular septal

Subcutaneous ICD therapy in patients with monomorphic ventricular tachycardia: Arrhythmia recurrence, shock rate, and management strategies.

BACKGROUND: The subcutaneous implantable cardioverter-defibrillator (S-ICD) is recommended for patients at risk of sudden cardiac death who do not require pacing or antitachycardia pacing (ATP). However, patients with a history of monomorphic ventricular tachycardia (MVT), which is often amenable to ATP, are underrepresented in clinical studies dealing with S-ICD. OBJECTIVE: This study aimed to evaluate outcomes of S-ICD therapy in patients with previous MVT and describe subsequent management strategies in clinical practice. METHODS: From 2164 consecutive de novo S-ICD implantations, we iden

Clinical Diagnosis of Calcium Release Deficiency Syndrome in a Family With Sudden Cardiac Death.

IMPORTANCE: Calcium release deficiency syndrome (CRDS) is a recently described inherited arrhythmia caused by loss-of-function variants in the cardiac ryanodine receptor (RYR2). Diagnosis currently requires in vitro functional testing and availability limits diagnosis; a recently proposed burst pacing provocation test offers a potential clinical diagnostic tool, but its performance remains to be validated. OBJECTIVE: To demonstrate how the novel CRDS provocation test can clinically establish the diagnosis in a family with unexplained sudden cardiac arrest and death. DESIGN, SETTING, AND PART

Retrosternal extravascular implantable cardioverter-defibrillators: a systematic review of implant characteristics, defibrillation testing, and acute safety.

INTRODUCTION: Implantable cardioverter-defibrillators (ICD) terminate potentially deadly ventricular arrhythmias in at-risk patients. Retrosternal extravascular ICDs (EV-ICD) place a lead in the mediastinal space to provide both shock and antitachycardia pacing therapy without the need for venous access. A systematic review was performed to assess acute outcomes in patients implanted with a retrosternal EV-ICD. METHODS: A query of EMBASE and MEDLINE databases was performed to identify unique retrosternal EV-ICD cohort studies published between JAN-1-2020 and JAN-31-2026. Two known retrosterna

Heart Disease in Arrest-Related Death.

It is accepted that congenital and acquired heart disease increase the risk of sudden cardiac death. To our knowledge, there are no published studies on the types of heart disease in cases of arrest-related deaths. The goal of this study, therefore, was to consider a large body of autopsy reports of non-firearm arrest-related deaths to analyze the presence of heart disease with regard to numerous decedent variables and toxicology findings. We analyzed 1650 autopsy reports covering the years 2005-2024 and classified cardiac findings into larger subgroups. Most of the cases (95%) were men, and t

Contextual patterns in publicly reported non-traumatic sudden deaths among university students in physical education and campus sports settings in China: a retrospective thematic analysis.

BACKGROUND: Physical education classes, physical fitness testing, and other campus-based sports activities are important settings in which university students engage in organized and semi-organized physical activity. Although non-traumatic sudden death in these settings is rare, its consequences are severe for students, families, universities, and campus public health governance. Existing research has mainly focused on pathological mechanisms, clinical interpretation, emergency medicine, and legal responsibility, whereas less attention has been paid to how such events are publicly represented

Case Report: Two cases of PD-1 inhibitor-associated myocarditis with bidirectional ventricular tachycardia.

BACKGROUND: The clinical application of targeted anti-programmed cell death protein 1 (PD-1) therapy has significantly improved the prognosis of patients with various advanced malignancies. However, life-threatening immune-related adverse events remain problematic, including rare but highly lethal myocarditis. Bidirectional ventricular tachycardia (BVT) is a rare but dangerous form of ventricular arrhythmia. If immune checkpoint inhibitor (ICI)-associated myocarditis is complicated with BVT, patients face an extremely high risk of mortality. CASE DESCRIPTION: The first case was a 45-year-old
