An ECG biomarker for sudden cardiac death discovered with deep learning.

Sudden cardiac death is, in theory, preventable with defibrillators. But every year, many patients die without defibrillators because doctors fail to predict their risk1. The only predictive biomarker in wide use, cardiac left ventricular ejection fraction (LVEF), misses most sudden cardiac deaths2, and flags many low-risk patients for futile defibrillators that never fire3,4. Here we apply deep learning to a dataset linking all electrocardiograms (ECGs) in a Swedish region to death certificates. The resulting model isolates a high-risk group (2.2% of the sample) with a 7.0% annual rate of sud

Cardiovascular Effects of Droperidol: A Clinically-Focused Review for the Consultation-Liaison and Emergency Psychiatrist.

BACKGROUND: Droperidol carries a black box warning from the US Food and Drug Administration regarding the risk for corrected QT (QTc) prolongation and torsades de pointes but is experiencing a resurgence of use for agitation in emergency medicine settings. OBJECTIVE: This structured review aims to investigate effects of droperidol on cardiac conduction and risk of arrhythmias when used at standard doses. METHODS: We searched PubMed from inception through February 2025. We included studies describing cardiac effects or changes in vital signs in the setting of droperidol usage, excluding those

Case report: exercise-associated sudden death in a Thoroughbred racehorse with cardiac lesions.

Exercise-associated sudden death (EASD), defined as acute collapse and death in an apparently healthy racehorse during or within ~1 h after exercise, is currently among the top concerns in the racing industry. Cases of EASD with no cause of death identified at necropsy are frequently presumed to be of cardiac origin. In such cases, in addition to a regular necropsy, a cardiac examination protocol is performed that includes a detailed gross examination and microscopic evaluation of 11 regions of the heart. Very often this protocol is unrewarding because there are no microscopic lesions to expla

Biopsy-confirmed Mycobacterium kansasii peritonitis with CT peritoneography in a young PD patient: a case report.

BACKGROUND: Mycobacterium kansasii is an exceptionally rare cause of peritoneal dialysis (PD)-related peritonitis and may present with subtle, indolent features that delay recognition. This case illustrates how "silent" atypical peritonitis can be unmasked by pairing contrast-enhanced computed tomography (CT) with CT peritoneography: the former identified nodular biopsy targets, while the latter revealed loculated dialysate flow and failure of free intraperitoneal circulation. CASE PRESENTATION: A 20-year-old male had received automated PD for 3 years and was being evaluated for combined hear

Sports and Exercise Cardiology: A New Frontier in Cardiovascular Disease Management.

Exercise and physical activity are recognized as essential tools to optimize fitness and longevity, improve overall health, and reduce the risk of cardiovascular disease and other major comorbidities. However, there are inherent risks associated with participation in sports and exercise, regardless of the level of competition or skill required. As such, there is a need for cardiologists to understand the unique aspects of competitive and recreational athletics, as well as sport and exercise science, as it relates to overall heart health and cardiovascular risk. In this article, we provide an o

Charged to death: Energy drinks, smart drugs, and sudden cardiac death.

Sudden cardiac death (SCD) in individuals ≤35 years remains a major challenge for cardiology and forensic medicine, often affecting subjects without prior symptoms. At the same time, the consumption of energy drinks (EDs) and the medical or non-medical use of cognitive enhancers or "smart drugs" (SDs), including methylphenidate, amphetamine derivatives, and modafinil, has increased markedly among adolescents, students, and athletes. This review integrates mechanistic, clinical, epidemiological, and forensic evidence to clarify the role of EDs and SDs as arrhythmogenic triggers, identify at-ris

Multimodality Risk Stratification in Athletes With Long QT Syndrome.

BACKGROUND: Long QT syndrome has traditionally led to exercise restriction due to concerns for exertion-triggered arrhythmia and sudden cardiac death. Emerging data, however, support individualized participation guided by genotype and physiologic risk markers. CASE SUMMARY: We describe 3 young athletes with distinct genotypes (KCNE1 D85N [long QT syndrome type 5-Lite], KCNQ1 R555H [long QT syndrome type 1], and KCNH2 p.T613M [long QT syndrome type 2]) evaluated through electrocardiography, exercise testing, and echocardiographic assessment of the electromechanical window. Noticing and responding to end-of-life suffering in the PICU: A co-designed conceptual framework for nursing practice.

OBJECTIVES: Pediatric intensive care unit (PICU) nurses spend the most time with patients and families during a critical illness. However, nurses, families, and other clinicians conceptualize suffering differently, leaving some types of suffering when a child is nearing the end-of-life (EOL) unaddressed. PICU nurses are thus well-positioned but underequipped to address suffering during EOL. We aimed to 1) characterize distinctive features that indicate the presence and/or absence of suffering; and 2) identify nursing care responses that help attend to EOL suffering. METHODS: Using qualitative

Durable stable disease and immune activation in metastatic uveal melanoma treated with tebentafusp: case report.

Metastatic uveal melanoma (mUM) is a rare, aggressive malignancy with limited therapeutic options in the metastatic setting. Tebentafusp (IMCgp100) is an immune-mobilizing monoclonal T cell receptor against cancer (ImmTAC), targeting gp100 in HLA-A*0201+ patients. Tebentafusp has demonstrated an overall survival (OS) benefit in clinical trials, usually without marked radiographic tumor regression. We describe a mUM patient who was treated with tebentafusp for more than two years and achieved durable stable disease on imaging but passed away due to sudden cardiac death. Autopsy examination reve

Effects of curcumin and nicorandil on nilotinib-induced QT interval prolongation in rats: A telemetry-based study.

BACKGROUND: Long QT syndrome (LQTS) is characterized by QT prolongation, ventricular arrhythmias and sudden death. Nilotinib (Nilo), a tyrosine kinase inhibitor used in chronic myeloid leukemia, prolongs QTc mainly through hERG (IKr) channel inhibition. Curcumin (Curc), often co-administered, modulates cardiac ion channels, whereas nicorandil (Nico) acts as a KATP channel opener. OBJECTIVES: This study evaluated the effects of Curc and Nico on Nilo-induced QTc prolongation. METHODS: Male Sprague-Dawley rats implanted with radiotelemetry transmitters received 10 mg/kg nilotinib (selected as t

High-degree atrioventricular block in a child with an ANK2 variant and long QT phenotype requiring leadless pacemaker implantation.

Ankyrin-B syndrome, which has been linked to variants in ANK2 and classified as long QT syndrome type 4, is characterised by heterogeneous arrhythmic manifestations, including sinus node dysfunction and conduction abnormalities. Clinically significant atrioventricular block requiring permanent pacing in paediatric patients has rarely been described. We report a 10-year-old boy who presented with recurrent syncope and seizure-like episodes. Electrocardiography demonstrated QT prolongation, and continuous monitoring revealed persistent high-degree atrioventricular block. Genetic testing identifi

Clinical characteristics of multifocal premature ventricular complexes originating from the basal septal region of the left ventricle: Anatomy, mapping, and ablation.

BACKGROUND: Clinical recognition of multifocal premature ventricular complexes (PVCs) poses diagnostic and therapeutic challenges. Despite their morphological diversity, these arrhythmias may originate from a confined anatomical region, allowing a systematic mapping and ablation strategy. OBJECTIVE: To describe clinical characteristics and propose a stepwise approach to mapping and ablation of multifocal PVCs originating from the basal septal left ventricular (LV) ostium. METHODS: Patients with multifocal septal PVCs who underwent coronary venous mapping were included. Ablation targeted the

Fatal cardiac inflammatory pseudotumor: a case report and literature review.

Inflammatory pseudotumor is a rare fibroinflammatory lesion described in nearly all organ systems, whereas cardiac involvement is exceptional. This case presents a sudden death due to cardiac inflammatory pseudotumor diagnosed at autopsy in an adult male with a recent diagnosis of Human Immunodeficiency Virus infection. Autopsy revealed cardiomegaly with asymmetric septal hypertrophy and an irregular tan white fibrous lesion infiltrating the interventricular septum. Histologic examination demonstrated replacement of myocardial tissue by a spindle cell mesenchymal lesion within a myxoid stroma,

Chest Pain Due to Anomalous Origin of the Left Circumflex Artery From the Opposite Sinus of Valsalva in a 70-Year-Old Woman.

BACKGROUND An anomalous origin of a coronary artery from the opposite sinus of Valsalva (ACAOS) is a rare congenital coronary abnormality that is usually asymptomatic throughout life. However, an anomalous coronary artery arising from an atypical aortic location can lead to myocardial ischemia, exertional symptoms, or even sudden cardiac death. Therefore, early recognition and risk stratification are essential. Clinical management of ACAOS should be highly individualized and guided by detailed anatomical features, clinical symptoms, and evidence of ischemia. This report describes an older woma

Homozygous Nonsense DSP Variants Selectively Affecting Desmoplakin I Cause Carvajal Syndrome in a Consanguineous Arabian Family with Four Affected Female Siblings.

Introduction: Arrhythmogenic cardiomyopathy (ACM) is a myocardial disorder characterized by fibrofatty replacement of the myocardium and is associated with heart failure and sudden cardiac death. It can involve either ventricle, including left-dominant forms defined partly by genetic findings. Methods: We investigated a consanguineous family with four affected daughters presenting with ACM, epidermolytic palmoplantar keratoderma, and woolly hair. The two eldest siblings died suddenly during childhood. The remaining two affected siblings underwent genetic analysis using autozygosity mapping and

Methadone administration and QT interval prolongation: protocol for a systematic review and meta-analysis.

INTRODUCTION: Methadone is a long-acting opioid used to treat opioid use disorder, cancer pain and provide perioperative analgesia. Methadone administration has been associated with QTc interval prolongation and consequently an increased risk of ventricular arrhythmias. This mechanism appears to be mediated by HERG channel inhibition, drug-drug interactions and genetic susceptibility. This protocol describes the methodology for a systematic review and meta-analysis aiming to evaluate the association between methadone administration and QTc prolongation and estimate the odds of major cardiac ev

The Hodgkin-Huxley-Katz Prize Lecture: Heart digital twins: A new paradigm in arrhythmia medicine.

Digital twins offer a way to translate rich patient data into individualized models that can be interrogated rather than merely described. In cardiac electrophysiology this approach has advanced to the point where the geometry, fibrosis, adiposity, inflammation and genotype of an individual heart can be incorporated into biophysically grounded simulations. This review accompanies The Physiological Society's annual Hodgkin-Huxley-Katz Award Lecture, presented to Dr. Natalia Trayanova in recognition of her contributions to computational cardiology and its clinical translation. It surveys the bod

A Pilot Randomised Controlled Trial of a Targeted Dietary Approach to Potassium Management in Hemodialysis: A Clinical Research Protocol for the Evidence for Optimal Potassium Intake in Hemodialysis (EvoKe-HD) Study.

BACKGROUND: Hyperkalemia affects more than one in five patients receiving maintenance hemodialysis and remains a major contributor to arrhythmias and sudden cardiac death. Traditional potassium restriction has centered on limiting fruits and vegetables, even as emerging evidence challenges the assumption that all high-potassium foods pose equal risk. Meanwhile, awareness of potassium additives has grown, and recommendations to avoid additive-containing processed foods have been layered onto existing restrictions. Together, the uncertainties around long-standing to newer recommendations create

Cardiomyocyte hypertrophy co-localizes with accumulation of hyaluronan in cMyBPC+/- mice hearts.

Hypertrophic cardiomyopathy (HCM) is a genetic heart disease characterized by left ventricular hypertrophy. Mutations in genes for sarcomere proteins, e.g., in cardiac myosin-binding protein C (cMyBPC), may cause symptoms like heart failure and sudden cardiac death. In HCM, the heart utilizes high amounts of glucose. Increased glucose metabolism creates intermediates for hyaluronan (HA) synthesis, resulting in HA accumulation within the extracellular space, which could contribute to cardiomyocyte hypertrophy. We aimed to describe the connection between HA and cardiomyocyte hypertrophy using cM
