Towards credible digital twins for basic and preclinical research.

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Mapping generative AI in higher education teaching and learning: a scoping review.

BACKGROUND: Dry mouth is a common perioperative patient complaint. There are a number of treatments used for dry mouth in other settings which are effective. None have been tested previously in the perioperative setting. Interventions to Manage Dry mouth (IM DRY) compared the effect of water and a saliva substitute on mouth dryness. The primary objective was to demonstrate the feasibility of conducting a large randomised controlled trial and secondary scientific aims were to assess treatment potential efficacy. METHODS: Single blind, pilot randomised controlled trial (RCT) of 101 pre-operativ

Pericardial and interstitial alcianophilic gelatinous degeneration in anorexia nervosa.

Anorexia nervosa is associated with severe cardiovascular complications and an increased risk of sudden death. While cardiac atrophy is well recognized, unusual connective-tissue alterations of the heart are rarely described. A 28-year-old woman with a longstanding history of anorexia nervosa was found deceased at home. She was profoundly underweight, with extreme cachexia and a body mass index of 10.6. External examination revealed severe emaciation with near-complete loss of subcutaneous fat and marked muscular atrophy. Autopsy demonstrated near-total depletion of subcutaneous and visceral a

Fatal ascending aortic dissection in a 12-year-old boy with previously undiagnosed Loeys-Dietz syndrome: A molecular autopsy case.

Loeys-Dietz syndrome (LDS) is a heritable connective tissue disorder caused by pathogenic variants in genes involved in transforming growth factor-β signaling. It can present in childhood with fatal aortic dissection or rupture. We describe a medicolegal autopsy case of a 12-year-old boy who collapsed shortly after complaining of jaw pain and died despite resuscitation. Non-contrast postmortem computed tomography showed high-attenuation pericardial fluid, consistent with hemopericardium. At autopsy, 621 g of dark-red blood clot was present in the pericardial cavity, and the ascending aorta sho

Surgical treatment of pulmonary hydatidosis: six years of experience and results at the National Hospital Guillermo Almenara Irigoyen, Lima, Peru.

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Novel use of far UV-C light as a surface disinfectant in the poultry industry: trials on broiler farms show reduced bacterial burden in poultry anterooms and on wellington boots.

Osteoarthritis (OA) is condition which poses a main concern to the aging population and its severity is expected to increase with the increasing life expectancy. In the future, several possible targets for OA treatment need to be defined. Dickkopf‑related protein 3 (DKK3) is an atypical member of the Wnt‑antagonistic dickkopf‑related protein (DKK) family. The availability of research into the role of DKK3 in the abnormal remodeling of subchondral bone in human knee joints is currently limited. Thus, the aim of the present study was the evaluation of DKK3 expression in the abnormal bone remodel

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Unveiling the Unforeseen: Left Ventricular Fibroma Recurrence or Rare Complication? A Multimodality Imaging Revelation.

HIGHLIGHTS: Postoperative intramyocardial abscess may present with echocardiographic features indistinguishable from tumor recurrence. Cardiac magnetic resonance (CMR) with late gadolinium enhancement is critical for accurate differentiation and diagnosis. The surgical technique of using two dissimilar xenopericardial patches can create a potential dead space, serving as a nidus for infection and subsequent abscess formation despite an initially uncomplicated postoperative course.CMR findings of a T2-hyperintense core without early enhancement, surrounded by an enhancing rim, are characteristic

PPA2-associated cardiac arrest survivor.

Biallelic pathogenic variants in the inorganic pyrophosphatase 2 (PPA2) gene are a rare but established cause of sudden infant death, which may be precipitated by a pyrexial or viral illness. It has also been associated with sudden death secondary to alcohol ingestion in young adults. We describe the case of a thirteen-month-old female who presented following out-of- hospital cardiac arrest and was subsequently diagnosed with compound, heterozygous pathogenic variants of PPA2. She survived this episode and made excellent neurological recovery despite her prolonged out-of-hospital resuscitation

[Protection from sudden cardiac death using a new extravascular implantable cardioverter-defibrillator device: a literature review and a case series from a high-volume single center].

The extravascular implantable cardioverter-defibrillator (EV-ICD, Aurora, Medtronic, Minneapolis, MN, USA) has recently been validated as a new device for preventing arrhythmic death with the aim of reducing vascular and mechanical complications of transvenous ICDs and overcoming the limitations of subcutaneous ICDs. A dedicated learning program is required and, in addition to the electrophysiologist, the implantation procedure must be performed with the assistance of an anesthesiologist for general anesthesia induction and, especially in the early stages of training, a cardiothoracic surgeon

Sports-Related Sudden Cardiac Arrest in Canada: Incidence and Survival.

BACKGROUND: Sports-related (Sr-) sudden cardiac arrest (SCA) is widely recognized in young competitive athletes, yet occurs more frequently in middle-aged, recreational athletes. Our objective was to describe the epidemiology and characteristics of Sr-SCA in 5 Canadian Provinces (population: 10.9 million). METHODS: We conducted a retrospective cohort study using emergency medical services records from consecutive out of hospital SCAs, for patients aged 18-85 years, who were treated, and whose SCA was from a presumed cardiac cause, during or ≤ 1 hour after sports activity. RESULTS: A total of

Eighteen-year analysis of anaesthetic management in Brugada syndrome: The BRUGANAES study.

BACKGROUND: Brugada syndrome (BrS) is a genetic disorder that increases the risk of ventricular tachyarrhythmias and sudden cardiac death (SCD). Certain drugs (propofol, local anaesthetics), fever, bradycardia, increased vagal tone and electrolyte imbalances can trigger or worsen BrS arrhythmias. OBJECTIVE: To evaluate the incidence of malignant ventricular arrhythmias during the perioperative period in patients with BrS, hypothesising that common anaesthetic drugs may be safe to use during daily clinical practice. DESIGN: The BRUGANAES study was an observational, retrospective project inclu

Kambo Administration and Its Association With Sudden Death: Clinical and Forensic Perspectives From a Systematic Review.

Kambo (Kambô) is a secretion of an amphibian traditionally used by indigenous tribes in the Amazon region. This secretion is derived from the giant monkey frog (Phyllomedusa bicolor) and is utilized in tribal cleansing rituals known as Kambo ceremonies. Kambo is purported to offer several benefits, both physical and psychological. Proponents of this practice claim that it can enhance physical endurance, sharpen mental clarity, and boost overall health. However, the side effects, such as tachycardia, blood pressure fluctuations, and gastrointestinal distress, highlight the need for caution. In

Natural History, Phenotype Spectrum, and Clinical Outcomes of Desmin (DES)-Associated Cardiomyopathy.

BACKGROUND: Pathogenic/likely pathogenic (LP) desmin (DES) variants cause heterogeneous cardiomyopathy and skeletal myopathy phenotypes. Limited data suggest a high incidence of major adverse cardiac events (MACEs), including cardiac conduction disease, sustained ventricular arrhythmias (VA), and heart failure (HF) events (HF hospitalization, left ventricular assist device/cardiac transplant, HF-related death) in patients with pathogenic/LP DES variants. However, pleiotropic presentation and small cohort sizes have limited clinical phenotype and outcome characterization. We aimed to describe t

Sudden Death of a Four-Day-Old Newborn Due to Mitochondrial Trifunctional Protein/Long-Chain 3-Hydroxyacyl-CoA Dehydrogenase Deficiencies and a Systematic Literature Review of Early Deaths of Neonates with Fatty Acid Oxidation Disorders.

Mitochondrial trifunctional protein (MTP) and long-chain 3-hydroxyacyl-CoA dehydrogenase (LCHAD) deficiencies have been a part of the Slovenian newborn screening (NBS) program since 2018. We describe a case of early lethal presentation of MTPD/LCHADD in a term newborn. The girl was born after an uneventful pregnancy and delivery, and she was discharged home at the age of 3 days, appearing well. At the age of 4 days, she was found without signs of life. Resuscitation was not successful. The NBS test performed using tandem mass spectrometry (MS/MS) showed a positive screen for MTPD/LCHADD. Genet

Advances in Cardiovascular Pharmacotherapy. I. Cardiac Myosin Inhibitors.

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiomyopathy. The disease is characterized by asymmetric left ventricular (LV) remodeling with myocyte disarray and interstitial fibrosis, a hypercontractile state, dynamic subaortic obstruction of the LV outflow tract, impaired LV diastolic function, atrial and ventricular arrhythmias, and sudden cardiac death. HCM occurs as a result of pathological alterations in the cardiac myocyte's chemomechanical cycle, in which an enhanced rate of myosin-actin crossbridge formation and destabilization of the energy-conserving "super-relaxe

Sudden Cardiac Death: The Role of Molecular Autopsy with Next-Generation Sequencing.

Molecular autopsy is a term employed to describe the investigation of the cause of death through the analysis of genetic information using biological samples collected post-mortem. Its utility becomes evident in situations where conventional medico-legal autopsy methods are not able to identify the cause of death, i.e., in sudden cardiac death (SCD) cases in young individuals, where deaths are commonly due to genetic cardiac conditions, such as cardiomyopathies and channelopathies. The recent advancement in high-throughput sequencing techniques, such as next-generation sequencing (NGS), has al
