Persistent Supraventricular Tachycardia Following Central Venous Catheter Insertion: When Withdrawal of the Mechanical Stimulus Is Not Enough.

Cardiac arrhythmias are a recognised complication of central venous catheterisation and are usually attributed to mechanical irritation of the endocardium by the guidewire or catheter tip. These arrhythmias typically resolve following withdrawal of the mechanical stimulus. We report a 42-year-old man with heart failure with reduced ejection fraction and diabetic ketoacidosis who developed an abrupt regular narrow-complex tachycardia at approximately 217 beats/min immediately following advancement of a right internal jugular central venous catheter. The arrhythmia was associated with transient

Outcomes of a Simplified Two-Catheter Approach for Catheter Ablation of SVT in Pediatric Patients.

BACKGROUND: Catheter ablations are an established treatment for supraventricular tachycardia (SVT) in pediatric patients with high success and low complication rates typically. This study evaluated the procedural outcomes, safety, and recurrence rates of pediatric patients undergoing SVT ablation using a two-catheter approach. METHODS: A retrospective review was performed of 150 pediatric patients who underwent electrophysiology studies and catheter ablation for SVT. All patients were younger than 21 years, with arrhythmia mechanisms including atrioventricular nodal reentrant tachycardia (AVN

A comprehensive review of usefulness of sodium butyrate for the management of inflammatory bowel disease: from molecular mechanisms to clinical application.

Sodium butyrate (SB), a salt of the short-chain fatty acid butyrate, has emerged as a promising adjunctive therapy in inflammatory bowel disease (IBD). IBD is commonly associated with intestinal dysbiosis characterized by depletion of butyrogenic bacteria. This leads to a functionally relevant butyrate deficiency that contributes to epithelial barrier dysfunction, immune dysregulation, and persistent mucosal inflammation. Experimental studies demonstrate that butyrate supports colonocyte energy metabolism, strengthens tight-junction integrity, enhances mucin and antimicrobial peptide productio

A Multisystem Presentation of West Nile Neuroinvasive Disease: Lessons in Diagnostic Complexity.

West Nile virus (WNV) is an uncommon but serious case of encephalitis and meningitis, often presenting with nonspecific symptoms that can mimic a wide range of infectious, metabolic, and endocrine disorders. We present the case of a 61-year-old male with multiple comorbidities who arrived with 5 days of fever, chills, profuse diarrhea, black stools, and worsening confusion. Initial evaluation in the emergency department revealed hyponatremia, transaminitis, and concern for tick-borne illness, prompting empiric doxycycline. However, the patient denied any tick bites. Given his known thyroid nod

Dantrolene and cyproheptadine as salvage therapy for severe intrathecal baclofen withdrawal: A case report.

PURPOSE: Withdrawal from intrathecal baclofen can be a life-threatening situation. Despite this well-described complication, guidance for prevention and treatment when intrathecal baclofen cannot be reestablished is not well described. We report a case involving use of dantrolene and cyproheptadine for treatment of baclofen withdrawal not responsive to other therapies. SUMMARY: A 51-year-old man presented for intrathecal baclofen pump explantation due to a nonhealing surgical wound post catheter revision. At the time of pump removal, the patient was clinically stable and oral baclofen was sch

Clinical significance and efficacy of radiofrequency catheter ablation in the treatment of tachycardia-induced cardiomyopathy in 12 children.

BACKGROUND: Tachycardia-induced cardiomyopathy (TIC) in children is significantly affected by factors such as type of arrhythmia and duration of the arrhythmia. Early detection and effective radiofrequency catheter ablation (RFCA) are highly beneficial in improving the prognosis of children with TIC. However, related research in this area is still limited, in particular with regard to detailed clinical characteristics and treatment efficacy in a specific pediatric population. METHODS: A retrospective analysis was carried out on the medical records of TIC children admitted to Wuhan Children’s H

Radiation exposure and fluoroscopy time in AVNRT ablation: role of 3D electroanatomical mapping.

BACKGROUND: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most frequent regular supraventricular tachycardia in adults. Catheter ablation of the slow pathway is considered first-line therapy, offering high success with low complication risk. Conventional fluoroscopy-guided ablation, however, exposes both patients and operators to ionizing radiation. Three-dimensional (3D) electroanatomical mapping systems have emerged as a promising alternative to reduce radiation while maintaining efficacy. METHODS: We retrospectively reviewed 170 patients who underwent AVNRT ablation between Ju
