A Cohort Study on Prognostic Factors for Long-Term Success after Catheter Ablation of Supraventricular Tachycardia in Children.

INTRODUCTION: Radiofrequency catheter ablation (RFCA) is an established therapy for pediatric supraventricular tachycardia (SVT). However, data on long-term outcomes and predictors of success from sizable contemporary cohorts are limited. This study aimed to evaluate the long-term clinical success rate of RFCA in a pediatric cohort and to identify independent predictors of arrhythmia-free survival. METHODS: We conducted a retrospective analysis of 219 consecutive pediatric patients (age ≤18 years) who underwent their first RFCA for SVT (including atrioventricular reentrant tachycardia, atriov

Case Report: Pseudo-wide QRS complex tachycardia in an infant with complex congenital heart disease.

The initial electrocardiogram (ECG) of a 5-month-old infant with complex congenital heart disease (pulmonary valvular stenosis, ventricular septal defects, and patent ductus arteriosus) revealed a regular wide QRS tachycardia at 169 bpm, initially suggestive of ventricular tachycardia (VT) or supraventricular tachycardia (SVT) with aberrant conduction. However, a subsequent ECG obtained during spontaneous heart rate deceleration to 143 bpm showed narrow QRS complexes with discernible sinus P waves and a markedly prolonged PR interval (300 ms). These findings establish the diagnosis of sinus rh

Admission rates for pediatric supraventricular tachycardia are affected by primary insurance payor: An analysis of the NEDS database.

BACKGROUND: Recent data have highlighted potential disparities in the care of children with SVT. The impact of socioeconomic factors on emergency department (ED) outcomes for pediatric SVT has not been well characterized. METHODS: We performed a retrospective cohort study utilizing the 2006 to 2018 US Nationwide Emergency Department Sample (NEDS). Encounters for patients age 0-18 years with a primary diagnosis of SVT were evaluated, excluding those with congenital heart disease. The primary exposure was insurance payor. The primary outcome was hospital admission/transfer. Secondary outcomes i

Supraventricular Tachycardias Using Multiple Accessory Pathways.

Three atrioventricular reciprocating tachycardias (AVRTs) using 3 accessory pathways (APs) occurred with the His bundle eliminated by the previous ablation. Two AVRTs using 2 right APs rotated reversely, and the other AVRT was one with a retrograde conduction through the left AP and anterograde conduction through the 2 right APs.

Association Between Protocolized Magnesium Supplementation and Atrial Fibrillation or Flutter in Critically Ill Patients: A Multicenter Retrospective Cohort Study.

OBJECTIVES: Most ICUs use protocolized magnesium supplementation, yet the clinical effect of this practice is unknown. DESIGN: Pseudo-randomized retrospective study comparing patients who were and were not assigned to receive magnesium supplementation, using a protocol where supplementation occurs when serum levels are less than or equal to 0.95 mmol/L (2.31 mg/dL). Primary outcome was atrial fibrillation or flutter within 24 hours. Secondary outcomes were tachyarrhythmia (supraventricular tachycardia or ventricular arrhythmia) and death within 24 hours. SETTING: ICUs with a shared magnesium

FHGNet: A Feature-Centric Hierarchical Network with Graph Attention Layer for Supraventricular Tachycardia Classification.

Automated electrocardiogram (ECG) classification plays a critical role in arrhythmia diagnosis. However, current deep learning-based methodologies frequently fail to account for physiological rhythms and clinical diagnostic reasoning, thereby compromising their reliability and interpretability. This study proposes a clinically inspired multi-lead oscillatory Transformer framework, named FHGNet, to enhance the precision and interoperability of classifying ventricular tachycardia (VT) and supraventricular tachycardia (SVT). The proposed architecture integrates R-peak detection for heartbeat segm

Supraventricular Tachycardia Ablation in the Elderly-Characteristics and Outcomes.

BACKGROUND: Catheter ablation is an effective treatment for symptomatic supraventricular tachycardia (SVT). Most studies target the general adult population; data on the elderly are less robust. We studied the clinical and procedural characteristics and outcomes in elderly patients undergoing SVT ablation. METHODS: All patients undergoing atrioventricular nodal re-entry tachycardia (AVNRT), atrioventricular re-entry tachycardia (AVRT), and/or atrial tachycardia (AT) ablation between May 2011 and May 2022 at a tertiary center were included. Cases with concurrent ablation of atrial flutter, atr

Antiarrhythmics Management During Electrophysiology Procedures: A Stepwise Approach.

This review examines the protocols and rationale for stopping antiarrhythmic drugs (AADs) before interventional electrophysiology (EP) procedures to establish a drug-free baseline, ensuring diagnostic accuracy and procedural success. The review provided a detailed analysis of procedure-specific AADs washout requirements for supraventricular tachycardia, atrial fibrillation, and ventricular tachycardia ablation, and a comprehensive, drug-by-drug guide to calculate washout periods, based on pharmacokinetic and pharmacodynamic principles. This guide details the half-life, metabolism, and eliminat

[Atrial tachycardia after previous ablation-right atrial, left atrial, biatrial].

BACKGROUND: Catheter-assisted ablation of supraventricular tachycardia, especially of atrial fibrillation, often results in atrial tachycardia (AT) in the further course. ATs are favored by incomplete linear lesions, scarring of the atria or persistent pulmonary vein connections, and vary in incidence depending on the type of the preceding procedure. OBJECTIVES: This article discusses different forms of postablation AT in terms of diagnosis, treatment options, and perspectives for prevention and effective treatment. MATERIALS AND METHODS: Overview-based review of the literature and case-base

Multivessel Coronary Artery Disease Presenting as Supraventricular Tachycardia in an Elderly Woman: A Case Report.

Troponin elevations occurring during supraventricular tachycardia (SVT) are often interpreted as "demand ischemia" rather than indicative of obstructive coronary artery disease (CAD). Nonetheless, differentiating transient tachycardia-related injury from an acute coronary syndrome (ACS) can be difficult. We report a case of a 71-year-old woman whose SVT episode ultimately served as the first clue to underlying severe CAD. Her progressively rising high-sensitivity troponin (HS-Tn) required further evaluation. The patient subsequently underwent coronary angiography that revealed significant mult

Electrocardiogram-based false positive diagnosis of left ventricular hypertrophy during tachycardia: What more needs to be done.

This short communication aims at raising an insight about an observation made 26 years ago, describing a transient false positive electrocardiogram (ECG)-based diagnosis of left ventricular hypertrophy (LVH) in patients with various tachycardias (e.g, sinus, supraventricular, atrial fibrillation) with or without evidence of LVH as assessed by cardiac imaging. The mechanism is purported to be due to a tachycardia-mediated shortening of the diastolic left ventricular (LV) dimensions due to tachycardias, with the diastolic LV volume centroid displaced closer to the anterior chest wall (e.g. "Wils

Cardiovascular complications of traditional vs surrogate pregnancy.

BACKGROUND: Surrogacy utilises assisted reproductive technology including artificial intrauterine insemination, in-vitro fertilization (IVF), and frozen embryo transfer to help achieve parenthood. The differences in adverse cardiovascular outcomes of surrogate pregnancy compared to traditional pregnancy are not well studied. METHODS: Pregnant patients were procured from the National Inpatient Sample database. Patients were stratified into surrogate and non-surrogate pregnancy groups. The primary outcome was major adverse cardiovascular event (MACE), conduction abnormalities, pre-eclampsia, ge

Investigating explainable arrhythmia classification using class-specific ensemble model from segmented ECG signals.

Accurate arrhythmia classification from short clinical electrocardiograms (ECGs) remains challenging, particularly in terms of performance and interpretability. Prior studies are often limited to single-lead, use uniform model fusion, or report point estimates without uncertainty. To address this, we propose a novel class-specific weighted ensemble for 12-lead, 10 s ECG segments that fuses multiple models with per-class weights and provides lead-resolved SHapley Additive exPlanations (SHAP) explanations over 360 features. Four rhythms (sinus bradycardia (SB), sinus tachycardia (ST), atrial flu

The Human Factor: Physicians' Unbiased Interpretation of Atrial Fibrillation in the Emergency Department.

BACKGROUND: Misinterpretation of atrial fibrillation (AF) poses significant clinical risks. While the impact of computerized ECG interpretations (CIE) on clinical decision-making is well-documented, there is limited real-world evidence on the diagnostic accuracy of emergency department (ED) clinicians in the absence of such tools. This study examines the frequency and clinical impact of AF misdiagnosis in an ED operating without CIE. METHODS: ECGs of consecutive patients discharged with a diagnosis of AF over a five-year period were reevaluated by two cardiologists, with an electrophysiologis

Prehospital diagnosis and management of supraventricular tachycardia - a scoping review.

BACKGROUND: Supraventricular tachycardias (SVTs) are tachyarrhythmias that may result in haemodynamic instability and require prompt recognition and management in the prehospital setting. Current guidelines recommend a stepwise approach- vagal manoeuvres, adenosine, and electrical cardioversion (ECV) for unstable or refractory cases. However, prehospital care is characterized by diagnostic uncertainty, limited resources, and variable provider expertise and evidence specifically addressing prehospital care of SVT remains fragmented. This scoping review aimed to map the existing literature and i

Treatment-Resistant Fascicular Ventricular Tachycardia: A Case Report.

Fascicular ventricular tachycardia (VT) is a form of VT that arises through a re-entrant pathway utilising the Purkinje fibres. It can be categorised based on the anatomical origin of the re-entrant pathway. Fascicular VT typically affects young males. We present a case of a young male patient with treatment-resistant fascicular VT. A young male in his twenties spent much of his adolescence (8 years) suffering from sudden-onset and offset 'panic attack'-like symptoms. A routine preoperative ECG showed posterior fascicular VT. After a poor response to medical therapy, the patient underwent two
