Use and effectiveness of the Valsalva maneuver for supraventricular tachycardia in prehospital emergency care: a retrospective cohort study.

BACKGROUND: Vagal maneuvers are recommended as first-line therapy for hemodynamically stable supraventricular tachycardia (SVT). However, real-world data on their use in prehospital emergency medical services (EMS) remain limited. AIM: To evaluate the utilization and effectiveness of the Valsalva maneuver in the prehospital management of tachyarrhythmia, with particular focus on patients with confirmed SVT. METHODS: A retrospective cohort study was conducted using EMS documentation from a regional EMS system in Poland between 2023 and 2024. Among 93,847 EMS interventions, 235 cases coded as un

Towards Feasible Home ECG Monitoring: AI-Driven Detection of Clinically Critical Arrhythmias Using Single-Lead Signals.

Differentiating life-threatening arrhythmias, such as ventricular tachycardia and supraventricular tachycardia, from non-threatening ones is crucial for clinical applications. This study aimed to develop a deep learning model to classify five key Electrocardiogram (ECG) patterns: normal sinus rhythm, sinus tachycardia, sinus bradycardia, supraventricular tachycardia, and ventricular tachycardia. We collected 1500 single-lead 10 s ECG signals from public datasets, including PhysioNet/Computing in Cardiology (CiC) Challenge 2020 and the Malignant Ventricular Ectopy Database, for training and 229

Severe Myocarditis after Nivolumab and Ipilimumab in a Patient with Microsatellite Instability-High Gastric Adenocarcinoma: A Case Report.

Myocarditis attributable to immune checkpoint inhibitors is regarded as among the most serious complications of immunotherapeutic treatment. We report a case of gastric cancer who developed myocarditis, and subsequent late complications following treatment with nivolumab and ipilimumab. The patient was managed with corticosteroids, mycophenolate mofetil (MMF), and intravenous immunoglobulin G (IVIG), but later developed supraventricular tachycardia and deep vein thrombosis. This report emphasizes the need to enhance recognition of immune-related toxicities within non-oncology specialties to su

All That Is Wide Is Not Ventricular Tachycardia: A Case Highlighting Supraventricular Tachycardia as an Alternative in Stable Wide Complex Tachycardia With Reversible Cardiomyopathy.

Wide complex tachycardia (WCT) is often treated as ventricular tachycardia (VT). However, it can also be supraventricular tachycardia (SVT) with aberrant conduction, which is rarely life-threatening. Accurately differentiating VT from SVT with aberrancy guides appropriate therapy. We present a case that highlights the value of structured electrocardiogram (ECG) analysis in hemodynamically stable WCT and demonstrates reversible alcohol-related SVT-induced cardiomyopathy in a patient with familial susceptibility. A 51-year-old previously healthy woman presented to urgent care with four hours of

A tale of two QRS: Misleading irregular tachycardia in an ICD carrier.

We report a 52-year-old man with coronary artery disease, reduced left ventricular ejection fraction, and a cardiac resynchronization therapy-defibrillator (CRT-D), admitted to the emergency department for palpitations. Surface ECG showed a wide-complex tachycardia with a 2:1 ventricular-to-atrial (V:A) relationship. Device interrogation revealed episodes detected in the ventricular tachycardia monitor zone, according to the atrioventricular discrimination algorithms. Detailed analysis of surface ECG and device intracardiac electrograms was performed, followed by an electrophysiological study

Emapalumab's role in a severe and treatment-resistant paediatric macrophage activation syndrome.

Introduction
Macrophage activation syndrome (MAS) is a life-threatening hyperinflammatory condition. Emapalumab, an IFNγ-directed antibody, is approved for use in the USA but not in Europe. Case
A 15-year-old girl presented with fever, odynophagia and a transient rash. After 9 days of hospitalization under empirical antibiotics, she developed pancytopenia, hypofibrinogenaemia, elevated ALT, AST, LDH, triglycerides, soluble CD25, serum calprotectin and ferritin (peak 357,976 ng/ml), and hepatosplenomegaly. Infectious and immune workups were negative, and bone biopsy confirmed haemophagocytosis.

Beyond Rhythm Control: Health-Related Quality of Life and Attention-Behavioral Outcomes After Catheter Ablation in Children with Supraventricular Tachycardia.

Background: Supraventricular tachycardia (SVT) is the most common symptomatic arrhythmia in childhood and may negatively affect physical, emotional, and psychosocial functioning. Although catheter ablation is an established treatment for SVT, most outcome studies have focused primarily on rhythm control rather than broader psychosocial recovery. Objective: This study aimed to evaluate changes in health-related quality of life (HRQoL) following catheter ablation in children with symptomatic SVT and to characterize attention-behavioral outcomes at follow-up. Methods: In this retrospective-prospe

A prospective randomised evaluation of voltage propagation vs anatomical atrioventricular nodal re-entry tachycardia mapping in paediatric patients.

BACKGROUND: There is currently no consensus on the optimal mapping technique for atrioventricular nodal re-entry tachycardia ablation. OBJECTIVE: This, the first of its kind, prospective randomised trial compared procedural characteristics and ablation outcomes between an anatomic approach for atrioventricular nodal re-entry tachycardia ablation and an approach guided by low voltage signals, local activation time, and the propagation wave collision. METHODS: A randomised, prospective, multi-centre clinical trial was performed at 5 paediatric cardiac centres. 3D mapping was used on all patien

Saturation Gap During Anesthesia in Glucose-6-Phosphate Dehydrogenase Deficiency and Methemoglobinemia: A Case Report and Review of Perioperative Management.

The coexistence of glucose-6-phosphate dehydrogenase (G6PD) deficiency and methemoglobinemia may produce misleading pulse oximetry readings, creating a significant diagnostic challenge during the perioperative assessment of oxygenation. Methemoglobinemia results from the oxidation of hemoglobin iron to the ferric state, impairing oxygen delivery and altering light absorption, often leading to falsely low oxygen saturation values. A 20-year-old man with confirmed G6PD deficiency and suspected chronic methemoglobinemia underwent an elective electrophysiologic study for recurrent supraventricular

ECG puzzle: Persistent narrow-QRS tachycardia in a 62-year-old woman.

We report a 62-year-old woman with a long history of palpitations who presented with fatigue and acute dyspnea. ECG demonstrated a regular narrow-QRS tachycardia with long RP' intervals and 1:1 atrioventricular conduction. Episodes were recurrent despite amiodarone and beta-blockers, and transthoracic echocardiography revealed severe biventricular dysfunction which improved after rhythm control. We discuss the ECG features, mechanisms, and diagnostic clues of long RP' supraventricular tachycardia in adults.

Soccer-related blunt chest trauma inducing aberrant supraventricular tachycardia and unmasking Wolff-Parkinson-White syndrome in an adolescent.

We report a rare case of a hemodynamically unstable adolescent boy who developed aberrant supraventricular tachycardia following minor blunt chest trauma during soccer, successfully terminated with vagal maneuvers alone. Post-termination electrocardiography revealed previously unrecognized Wolff-Parkinson-White syndrome. This case demonstrates that seemingly minor blunt chest trauma during contact sports can precipitate clinically significant arrhythmias in children with occult conduction abnormalities. It further underscores the safety, effectiveness and the practical utility of vagal maneuve

Cardiac and Perinatal Outcomes Among Pregnancies With Maternal Cardiac Arrhythmias.

Atrial fibrillation/flutter (AF/AFL) and supraventricular tachycardia (SVT) are common arrhythmias in pregnancy. While known to contribute to adverse outcomes, large studies have not evaluated cardiac and perinatal outcomes in patients with arrhythmias. The objective of this study is to compare the cardiac/perinatal outcomes of pregnant patients with AF/AFL and SVT in pregnancy versus those without arrhythmias. We performed a retrospective cohort study utilizing Epic Cosmos, a dataset from 1,500 US hospitals. We identified pregnancies between October 25, 2021, and October 24, 2024, of patients

Hepatic vein injury after atrial tachycardia ablation: a rare cause of hemoperitoneum: case report.

BACKGROUND: Catheter ablation for atrial arrhythmias is a common procedure with the possibility of causing complications, typically involving vascular access sites or cardiac tamponade. However, hepatic haemorrhage as a complication of cardiac catheter ablation is extremely rare. We present a case of life-threatening intra-abdominal bleeding originating from a hepatic vein following a catheter ablation for atrial tachycardia. CASE SUMMARY: A 67-year-old man with a history of mitral valve repair and multiple ablation procedures for atrial fibrillation, atrial flutter, and macro-reentrant atria

Comment on "Supraventricular Tachycardia Ablation in the Elderly-Characteristics and Outcomes".

Catheter ablation for supraventricular tachycardia demonstrates high acute procedural success and acceptable immediate safety in selected elderly patients. However, the absence of long-term follow-up, substrate diversity, and broader patient inclusion limits conclusions regarding durable clinical effectiveness.

Case Report: Complex atrial arrhythmia ablation with transbaffle puncture in a Fontan patient with intracardiac lateral tunnel anatomy.

Atrial arrhythmias are frequent and potentially life-threatening late sequelae in adults with Fontan physiology. We present the case of a 36-year-old woman with an intracardiac lateral tunnel Fontan and prior Amplatzer Septal Occluder closure of a residual fenestration, who developed recurrent, drug-refractory supraventricular tachycardia. High-density electroanatomical mapping revealed a macro-reentrant flutter circuit within the conduit, successfully interrupted with linear ablation. Preprocedural cardiac CT guided a transbaffle puncture, performed inferior and right to the occluder, enablin

Retrospective evaluation of the relationship between catheter ablation and systemic immune-inflammation index in cardiac arrhythmias.

BACKGROUND: Catheter ablation induces controlled myocardial injury and transient endothelial dysfunction, both of which may trigger a systemic inflammatory response. The systemic immune-inflammation index (SII) has emerged as a sensitive biomarker reflecting the interplay between inflammation, thrombosis, and immune regulation; however, its peri-procedural behavior after ablation remains unclear. OBJECTIVE: To evaluate changes in SII following catheter ablation across different arrhythmia subtypes and to determine independent predictors of ΔSII. METHODS: In this retrospective cohort study, 642

Zero- or minimal-fluoroscopy vs conventional fluoroscopy in catheter ablation of supraventricular tachycardias: An updated systematic review and meta-analysis.

BACKGROUND: Catheter ablation is the treatment of choice for supraventricular tachycardias. Conventional fluoroscopy (CF) enables real-time guidance but exposes patients and staff to ionizing radiation. Zero- (ZF) or minimal-fluoroscopy (MF) approaches are primarily enabled by electroanatomic mapping, with adjunctive tools such as intracardiac echocardiography used selectively when clinically indicated, to reduce radiation. OBJECTIVE: This study aimed to evaluate the efficacy and safety of ZF/MF ablation compared with CF for supraventricular tachycardias. METHODS: We performed a systematic r
