Entrainment mapping, or continuous resetting of a re-entrant tachycardia, provides a powerful tool for distinguishing between focal and re-entrant ventricular tachycardias (VTs) and for identifying critical circuit components, such as the entrance, critical isthmus (CI), and exit sites. However, this technique is not always feasible and, in some cases, may itself terminate VT during the entrainment attempt. Whereas local capture of the CI without global ventricular capture (nonglobal capture [NGC]) can terminate VT by creating a bidirectional block (orthodromic or nonorthodromic capture) withi

The management of cardiac arrhythmias has been transformed by electroanatomical mapping, yet it is often underappreciated how dependent mapping is to electrode configuration, sensing modality, catheter geometry, and signal-processing characteristics. This review systematically evaluates how these features influence mapping, in the context of catheters in contemporary linear, circular, grid, star-shaped and basket configurations. We compare direct sensing strategies in unipolar and bipolar configurations, monophasic action potentials, and discuss post-processing approaches including omnipolar,

We describe a case of swallowing-induced ventricular tachycardia in a young, otherwise healthy male with a structurally normal heart which was refractory to multiple anti-arrhythmic medication and was successfully treated with ablation of a cardiac ganglion in the crux of the heart. We use this case to describe an unusual disease presentation and highlight current evidence related to evaluation and management of ventricular arrhythmias and add to the limited body of evidence related to swallowing induced tachycardia.

BACKGROUND: Left atrial appendage closure (LAAC) with the Watchman™ device is typically performed with the use of transesophageal echocardiography (TEE) and fluoroscopy. LAAC guided by intracardiac echo (ICE) without the use of fluoroscopy has not been previously described.
OBJECTIVE: In this series, we describe the safety and feasibility of Watchman™ implantation using only ICE imaging and without the use of fluoroscopy.
METHODS: 214 patients undergoing LAAC with the Watchman FLX™ or Watchman FLX™ Pro device and using a planned fluoroscopy-free approach were studied. ICE was used for echoca

BACKGROUND: Myocardial scar heterogeneity is a key determinant of ventricular arrhythmias after myocardial infarction (MI). Late gadolinium enhancement dispersion mapping (LGE-DM) quantifies structural complexity, but its prognostic role in ischemic cardiomyopathy remains unevaluated.
METHODS: We retrospectively included 364 consecutive patients with prior MI (>90 days). For each LGE image, a parametric map was generated and each enhanced pixel scored (0-8) based on surrounding signal distribution; the global dispersion score (GDS) was calculated as the mean pixel score, quantifying pixel-to-

BACKGROUND: Congenital long QT syndrome (LQTS) is typically diagnosed in youth. Recognition in advanced age is rare and may be delayed when arrhythmic syncope mimics epilepsy.
CASE SUMMARY: A 93-year-old man with recurrent convulsive syncope presented with marked QT prolongation, notched T waves, and pause-dependent torsades de pointes/pulseless ventricular tachycardia initiated by premature ventricular contractions. Four years earlier, torsades de pointes had been attributed to mild hypokalemia and famotidine, although QTc interval remained prolonged after correction. Genetic testing identif

BACKGROUND: Fulminant myocarditis (FM) carries high rates of mortality, heart transplantation, or durable ventricular assist device (dVAD) implantation. Deciding when to transition to dVAD is challenging, particularly when complicated by multiorgan failure or infection.
CASE SUMMARY: A 45-year-old man with FM was transitioned from a failing extracorporeal membrane oxygenation-Impella (combined use of venoarterial extracorporeal membrane oxygenation and Impella CP) to a surgical temporary biventricular assist device. Despite dialysis-dependent renal failure and Candida fungemia-which subsequen

BACKGROUND: Major cardiac arrhythmias after NSTEMI are associated with increased mortality and adverse clinical outcomes. However, evidence comparing the ability of established acute coronary syndrome risk scores to predict in-hospital arrhythmic events remains limited.
METHODS: We conducted a retrospective cohort study of consecutive NSTEMI patients who underwent coronary angiography at Taipei Medical University Hospital between January 2017 and August 2022. Major cardiac arrhythmias were defined as ventricular tachycardia, ventricular fibrillation, new-onset atrial fibrillation, and high-de

BACKGROUND: peak frequency (PF) analysis has recently emerged as a novel tool to assess electrogram properties through their frequency spectrum. PF is an indirect marker of local myocardial activity, and its reduction after radiofrequency ablation reflects effective tissue modification.
OBJECTIVE: our study aims to evaluate PF analysis as an objective method to validate lesion efficacy during ventricular tachycardia (VT) ablation.
METHODS: we retrospectively analyzed 43 (out of 247) consecutive patients who underwent VT ablation between January 2023 and December 2024, using the EnSite X mapp

BACKGROUND: Premature ventricular complexes (PVCs) originating from epicardial anterior interventricular groove and closely adjacent to the left anterior descending artery (LAD) are difficult to eliminate and carry high risks of coronary damage or occlusion. We describe a successful guidewire-assisted ethanol ablation in this high -risk patient.
CASE SUMMARY: A 30-year-old man who presented with palpitations and high-burden PVCs was admitted to our hospital. Pace mapping performed at the epicardial anterior interventricular groove yielded QRS morphology nearly identical to the native PVCs. Co

PURPOSE: Drug-induced QT interval prolongation increases the risk of ventricular arrhythmias and cardiac arrest. Assessing these outcomes in electronic health databases and evaluating their association with drug exposure requires prior validation of administrative diagnostic codes. This study aimed to evaluate the positive predictive value (PPV) of International Classification of Diseases, 10th revision (ICD-10) codes for ventricular arrhythmias and cardiac arrest in the French hospital discharge summary database (Programme de Médicalisation des Systèmes d'Information (PMSI)).

BACKGROUND AND PURPOSE: Cardiorespiratory ultrasound motion monitoring may enhance the precision of dose delivery for stereotactic arrhythmia radioablation (STAR) of ventricular tachycardia. This treatment planning study aimed to evaluate the feasibility of photon-based STAR planning in the presence of an ultrasound probe.
MATERIALS AND METHODS: For sixteen targets in fourteen patients, three treatment plans were retrospectively created and compared: a reference plan without an ultrasound probe and two plans in which the ultrasound probe was spared from the primary treatment beam (apical or p

BACKGROUND: Polymorphic ventricular tachycardia (VT) mediated by short-coupled premature ventricular complexes in patients with recent myocardial infarction is termed 'Angry Purkinje Syndrome'. These arrhythmias are typically refractory to conventional antiarrhythmic therapy but may respond to quinidine.
METHODS & RESULTS: We present a case series of patients with short-coupled polymorphic VT occurring 2-8 days after acute myocardial infarction and coronary revascularisation. All three patients had recurrent polymorphic VT episodes that were consistently initiated by ventricular ectopy with a

Cardiac allograft vasculopathy (CAV) is a major cause of mortality and morbidity within the first 3 years of heart transplantation (HT). CAV results in progressive coronary artery stenosis and may lead to arrhythmias and sudden cardiac death. In some cases, sudden cardiac death represents the initial presentation of CAV, as cardiac denervation blunts anginal symptoms that otherwise could have preceded sudden cardiac death (SCD). Here, we describe the case of a 56-year-old man with non-ischemic cardiomyopathy. He underwent left ventricular assist device implantation followed by orthotopic HT. F

BACKGROUND AND AIMS: Radiotherapy (RT) with a single application of ionizing radiation (STAR) has been suggested as a non-invasive alternative to radiofrequency in ablating ventricular tachycardia (VT). Emerging data reveal that STAR may suppress VT without substrate destruction, by enhancing impulse conduction instead, through increased expression of NaV1.5 channels and connexin 43. The study aims to investigate electrophysiology and intracellular Ca2+ dynamics in cardiomyocytes (CMs) from mice subjected to in-vivo RT. The emerging data led us to evaluate biochemical changes potentially linki

Inherited arrhythmias and cardiomyopathies are leading causes of sudden cardiac death in young individuals. Although global registries and genetic databases have advanced diagnosis and prevention globally, data from the Middle East (ME) remain limited and fragmented. This review synthesizes clinical and genetic evidence on long QT syndrome, catecholaminergic polymorphic ventricular tachycardia, Brugada syndrome, arrhythmogenic right ventricular cardiomyopathy, hypertrophic cardiomyopathy, and dilated cardiomyopathy across the ME. A defining feature of ME cohorts is the high prevalence of consa
