Characteristics of patients with obesity presenting with cardiac arrest and the potential role of cardioverter-defibrillator therapy: an analysis using the National Inpatient Sample database.

BACKGROUND: Obesity is a well established risk factor for cardiovascular disease and has been associated with an increased incidence of cardiac arrest. This study utilizes the National Inpatient Sample (NIS) database to analyze the characteristics of obese patients presenting with cardiac arrest and explores the potential role of implantable cardioverter-defibrillator (ICD) therapy in this population. METHODS: We analyzed data from the NIS database between 2016 and 2020, including obese (body mass index ≥30) adult patients with and without cardiac arrest. Baseline characteristics and comorbid

Practical Approaches to Coronary Venous Ethanol Ablation for Refractory Ventricular Arrhythmias: Technique Selection and Clinical Outcomes.

BACKGROUND: Coronary venous ethanol ablation (CVEA) is a promising alternative for refractory ventricular arrhythmias (VAs); however, it has limited availability, lacks guideline support, and comprises heterogeneous procedural techniques. OBJECTIVE: To evaluate the acute efficacy, safety, and long-term outcomes of CVEA in refractory VAs and outline practical techniques of CVEA. METHODS: Consecutive patients undergoing VA ablation at our center between December 2017 and December 2025 were screened; CVEA was performed after unsuccessful endocardial RFA. We performed CVEA using either tip injec

Legionella Septic Cardiomyopathy Requiring Venoarterial Extracorporeal Membrane Oxygenation Followed by Presumed Complement-Mediated Thrombotic Microangiopathy: A Case Report.

Background:Legionella pneumophila rarely causes profound myocardial dysfunction and malignant ventricular arrhythmias. Persistent thrombocytopenia and acute kidney injury during recovery may additionally raise concern for thrombotic microangiopathy (TMA), although distinguishing complement-mediated disease from secondary causes in critically ill patients is challenging. Case Presentation: A 64-year-old man presented after five days of fever with recurrent ventricular tachycardia/ventricular fibrillation, cardiac arrest, and refractory shock. Echocardiography demonstrated diffuse biventricular

An Adrenal Adenoma as the Culprit Behind 30 Years of Recurrent Syncope.

BACKGROUND: Primary aldosteronism (PA) is a treatable endocrine cause of secondary hypertension and, rarely, ventricular tachycardia (VT), syncope, or sudden cardiac death. CASE SUMMARY: A 59-year-old woman with hypertension presented with recurrent syncope for 30 years, worsening over 2 weeks. Electrocardiogram showed multifocal premature ventricular complexes (PVCs), with one dominant morphology triggering predominantly monomorphic VT, plus corrected QT interval prolongation. Hypokalemia, renal potassium wasting, left ventricular enlargement, and mild systolic dysfunction were also present.

Seizures Secondary to Aconitum Plant Poisoning: a case report.

BACKGROUND: Aconitum plant poisoning typically manifests as isolated early cardiotoxicity or late neurotoxicity. However, the dynamic temporal evolution from arrhythmia-driven cardiogenic syncope to true epileptic seizures mediated by direct central nervous system injury remains rarely documented and poorly characterized. CASE PRESENTATION: We report a 53-year-old Han Chinese male who presented with ventricular tachycardia rapidly degenerating into ventricular fibrillation and cardiac arrest following ingestion of homemade Aconitum kusnezoffii (caowu)-infused liquor. Initial convulsions were

Novel SCN5A Variant in a Patient With Dilated Cardiomyopathy Complicated by Conduction Disorders and Recurrent Ventricular Arrhythmias.

BACKGROUND: Although sodium voltage-gated channel alpha subunit 5 (SCN5A) was traditionally linked to electrophysiological disorders, growing evidence underscores its significant involvement in structural abnormalities. CASE SUMMARY: A young dilated cardiomyopathy patient presented with a distinctive electro-structural dual phenotype, characterized by progressive left ventricular systolic dysfunction, conduction disorders, and concomitant severe ventricular arrhythmias. Comorbidities included coronary artery disease, chronic kidney disease, obesity, and obstructive sleep apnea. Genetic testin

Global Advances in Ventricular Tachycardia Ablation.

Ventricular tachycardia (VT) is a potentially life-threatening arrhythmia associated with substantial morbidity and mortality, particularly in patients with structural heart disease. Over the past several decades, the management of VT has evolved significantly with the development of catheter-based and noninvasive ablation strategies aimed at eliminating arrhythmogenic myocardial substrates. This review provides a comprehensive overview of global advances in VT ablation, highlighting historical foundations, current therapeutic modalities, and emerging technologies shaping the future of ventric

Acute Lymphocytic Myocarditis Complicated By Bidirectional Ventricular Tachycardia Treated With Early Steroid Use.

Acute lymphocytic myocarditis can precipitate malignant ventricular arrhythmias, including the rare presentation of bidirectional ventricular tachycardia (BDVT), a rhythm associated with significant hemodynamic instability. Although BDVT is classically linked to catecholaminergic polymorphic ventricular tachycardia and digitalis toxicity, it is increasingly recognized in myocarditis, likely reflecting diffuse inflammatory involvement of the myocardium and Purkinje system. Emerging evidence suggests that early corticosteroid therapy may rapidly reduce arrhythmia burden and improve ventricular f

Pilsicainide Intoxication Presenting With Alternating Accelerated Idioventricular and Junctional Escape Rhythms During Acute Kidney Injury: A Case Report.

Pilsicainide is a class Ic sodium channel blocker that is eliminated predominantly by the kidneys and may accumulate when renal function deteriorates. An 85-year-old man receiving pilsicainide 150 mg/day developed acute kidney injury (AKI) during hospitalization for hemorrhagic colitis. He subsequently developed impaired consciousness, dyspnea, pulmonary congestion, and alternating episodes of junctional escape rhythm at 30-37 beats/min and a regular, wide QRS rhythm at 79 beats/min. The QRS duration was approximately 150 ms, and the serum pilsicainide concentration was 3.19 µg/mL. The wide QR

Impact of 18FDG-Avidity and Immunosuppression on Idiopathic and Genetic Cardiomyopathies.

BACKGROUND: Myocardial 18fluorodeoxyglucose (18FDG)-avidity is frequently seen in patients with genetic cardiomyopathy, as well as a growing "idiopathic 18FDG-avidity" group of genotype-negative patients who do not clearly have cardiac sarcoidosis. The prognostic implications of 18FDG-avidity in patients with and without genetic cardiomyopathy, and the effects of immunosuppression in the latter, remain uncertain. METHODS: This multicenter, retrospective study included all patients who were referred for both 18FDG-PET and cardiomyopathy genetic testing. Patients with acute myocarditis, biopsy-

Refractory Ventricular Tachycardia-Induced Severe Cardiomyopathy in a Child: Successful Radiofrequency Ablation With ECMO Support.

BACKGROUND: Ventricular tachycardia (VT)-induced cardiac dysfunction is a recognized but uncommon cause of cardiomyopathy in children. CASE SUMMARY: A 7-year-old girl presented with severely depressed left ventricular function and incessant VT that was refractory to multiple antiarrhythmics and attempted cardioversions. Because of hemodynamic instability, the patient required extracorporeal membrane oxygenation (ECMO) support and ultimately radiofrequency ablation of a focal VT that was localized to the right midventricular septum near the moderator band. The patient had a subsequent return o

Identifying the High-Risk Arrhythmic Mitral Valve Prolapse Phenotype by Imaging.

Within arrhythmogenic mitral valve prolapse (AMVP), a distinct subset of mitral valve prolapse (MVP), there is a small group that carries a higher risk of malignant ventricular arrhythmias and sudden cardiac death called malignant MVP. The malignant MVP phenotype is over-represented in young women with unexplained sudden cardiac death on autopsy. AMVP is defined by myxomatous, often bileaflet MVP with mitral annular disjunction, abnormal valvular-myocardial mechanics, complex ventricular ectopy, and nonsustained ventricular tachycardia. Excessive mechanical force from systolic annular expansio

From call to home: The role of dispatcher-assisted CPR in patient survival.

BACKGROUND: Dispatcher-assisted cardiopulmonary resuscitation (DA-CPR) is a key strategy to reduce no-flow time in out-of-hospital cardiac arrest (OHCA), yet its impact on long-term survival remains various across emergency medical systems. PURPOSE: We aimed to evaluate the association between DA-CPR and resuscitation outcomes in a nationwide cohort of OHCA patients presenting with an initial shockable rhythm. METHODS: We conducted a retrospective observational study including all adult OHCA cases with ventricular fibrillation or pulseless ventricular tachycardia (VF/VT) in which resuscitati

Electrophysiologic Phenotype Outperforms Left Ventricular Ejection Fraction for Mortality Prediction in Immune Checkpoint Inhibitor Myocarditis.

BACKGROUND: Immune checkpoint inhibitor (ICI)-associated myocarditis carries mortality rates approaching 50%. Risk stratification relies heavily on left ventricular ejection fraction (LVEF), yet many fatal cases occur despite preserved systolic function. OBJECTIVES: The study aims to compare the prognostic value of electrophysiologic involvement vs LVEF in ICI-associated myocarditis. METHODS: We systematically searched PubMed for ICI-associated myocarditis cases with extractable patient-level data. Electrophysiologic phenotype (EP) was defined by clinically significant conduction disease or

Life-Threatening Ventricular Tachycardia and Transient Brugada-Like Electrocardiographic Abnormalities Caused by Severe Hyperkalemia Due to Urinary Retention in a 55-Year-Old Man.

BACKGROUND Hyperkalemia is a potentially fatal electrolyte disturbance that typically manifests as progressive conduction abnormalities, bradyarrhythmias, and sine-wave electrocardiographic changes. In rare cases, however, severe hyperkalemia also presents with malignant ventricular tachyarrhythmias and transient electrocardiographic abnormalities involving the right precordial leads, potentially leading to diagnostic confusion with primary arrhythmic disorders. CASE REPORT A 55-year-old man with no known cardiovascular disease presented with palpitations, dyspnea, and diaphoresis. Electrocard

Ionizing radiation-induced remodelling of the heart: Cellular pathways and crosstalk relevant to stereotactic arrhythmia radioablation (STAR).

Stereotactic arrhythmia radioablation (STAR) has emerged as a promising non-invasive therapy for refractory ventricular tachycardia, yet its biological mechanisms remain poorly defined. While conventional catheter ablation achieves arrhythmia control through structural remodelling, radiotherapy was initially assumed to act similarly. However, STAR reduces ventricular arrhythmia burden within weeks, preceding the development of radiation-induced structural remodelling. This temporal mismatch indicates that early anti-arrhythmic effects are probably driven by functional adaptations. Human tissue
