Effects of septal pacing on P wave characteristics: the value of three-dimensional echocardiography.

Interatrial septum (IAS) pacing has been proposed for the prevention of paroxysmal atrial fibrillation. IAS pacing is usually guided by fluoroscopy and P wave analysis. The authors have developed a new approach for IAS pacing using intracardiac echocardiography (ICE), and examined its effects on P wave characteristics. Cross-sectional images are acquired during pullback of the ICE transducer from the superior vena cava into the inferior vena cava by an electrocardiogram- and respiration-gated technique. The right atrium and IAS are then three-dimensionally reconstructed, and the desired pacing

Hemodynamic effects of alternative atrial pacing sites in patients with paroxysmal atrial fibrillation.

Recently, multisite atrial pacing has been suggested as an alternative therapy to prevent recurrences of paroxysmal atrial fibrillation (PAF). A study was conducted to compare the acute effects of biatrial (BiA), left atrial (LA), and right atrial appendage (RAA) pacing on cardiac hemodynamics. In 14 patients with PAF and a BiA pacemaker (with leads in the RAA and coronary sinus), cardiac output (CO), right (RV) and left ventricular (LV) filling, RA-LA contraction delay [PA(m-t)] and the difference in A wave duration [Adif(m-p)] at the level of the mitral valve (Adurm) and pulmonary veins (Adu

Radiation exposure during radiofrequency catheter ablation for atrial fibrillation.

RF catheter ablation of paroxysmal atrial fibrillation (PAF) is associated with prolonged fluoroscopy. The procedural duration and fluoroscopic exposure to patients and medical staff were recorded and compared among 43 ablation procedures for PAF, 20 for common atrial flutter, and 16 for accessory pathways. Patient radiation exposure was measured by dosimeters placed over the xyphoid, while that of physicians and nurses was measured by dosimeters placed outside and inside the lead apron. The mean fluoroscopy time was 57 +/- 30 minutes for PAF, 20 +/- 10 minutes for common flutter, and 22 +/- 2

Cost analysis of catheter ablation for paroxysmal atrial fibrillation.

RF ablation for paroxysmal atrial fibrillation (PAF) is a curative treatment, which when successful, eliminates the need to take antiarrhythmic drugs, be anticoagulated, and have recurrent physician visits or hospital admissions. The authors performed a retrospective cost comparison of RF ablation versus drug therapy for PAF. The study population consisted of 118 consecutive patients with symptomatic, drug refractory PAF who underwent 1.52 +/- 0.71 RF ablation procedures (range 1-4) for PAF. During a follow-up of 32 +/- 15 weeks, 85 (72%) patients remained free of clinical recurrence in absenc

Bepridil prevents paroxysmal atrial fibrillation by a class III antiarrhythmic drug effect.

BACKGROUND: [corrected] Bepridil, a multiple ion-channel blocker, has been reported to prevent paroxysmal atrial fibrillation (PAF). The f-f interval of PAF during treatment with bepridil versus class Ic antiarrhythmic drugs was compared. METHODS: Fifty-two patients with PAF were randomized to bepridil, 200 mg/day (n = 14) versus flecainide, 100 to 200 mg/day (n = 15) or pilsicainide, 75 to 150 mg/day (n = 23). The drug was considered effective when symptomatic episodes of PAF were decreased to < 50% during a follow-up of 2 to 6 months. The f-f interval was measured in 12-lead ECGs of initial

[Radiofrequency ablation in the treatment of focal atrial fibrillation using circumferential mapping and segmentary disconnection of pulmonary veins].

INTRODUCTION: The treatment of paroxysmal atrial fibrillation in patients without structural heart disease using radiofrequency ablation of pulmonary veins is a recently developed procedure with a good clinical success rate, although there have only been a few published series. We analyzed our results with this technique. METHOD: The procedure was performed in 33 highly symptomatic patients with focal atrial fibrillation that had probably originated in the pulmonary veins. The electrophysiological study was carried out with a decapolar catheter inserted by transeptal catheterization. Ablation

Pulmonary vein stenosis by ostial irrigated-tip ablation: incidence, time course, and prediction.

INTRODUCTION: The incidence of pulmonary vein (PV) stenosis and its time course for ostial trigger elimination in paroxysmal atrial fibrillation (PAF) is uncertain. In addition, the clinical value of serial computed tomographic (CT) scanning of the PV ostia and the predictive value of energy requirements for radiofrequency ablation have yet to be established. METHODS AND RESULTS: We performed irrigated-tip ablation in 37 patients with drug-resistant PAF. Serial spiral CT scans were taken prospectively in 34 patients the day before the procedure, at prehospital discharge, and at 3- and 6-month

Pharmacodynamic equivalence of two flecainide acetate formulations in patients with paroxysmal atrial fibrillation by QRS analysis of ambulatory electrocardiogram.

The pharmacodynamic equivalence of flecainide acetate immediate-release (IR) and controlled-release (CR) formulations was assessed from QRS duration in patients currently treated with the IR formulation. Patients were blindly assigned randomly to the IR (100 mg BID, n = 25) or to the CR group (200 mg OD, n = 23). Electrocardiographic parameters were measured at baseline and at week 8 from 24-h Holter monitoring. Mean (SD) normalized flecainide trough plasma concentration (measured 12 h after last intake) at week 8 was 381.3 ng/ml (104.8) with the IR and 381.4 ng/ml (123.8) with the CR formulat

P wave polarities of an arrhythmogenic focus in patients with paroxysmal atrial fibrillation originating from superior vena cava or right superior pulmonary vein.

INTRODUCTION: The superior vena cava (SVC) and right superior pulmonary vein (RSPV) are anatomically close structures. Using 12-lead ECG may facilitate identification of ectopic foci from SVC or RSPV. The aim of this study was to assess whether P wave polarity on surface ECG is helpful in distinguishing an arrhythmogenic focus of paroxysmal atrial fibrillation (AF) from SVC or RSPV. METHODS AND RESULTS: Thirty-four patients with paroxysmal AF from the SVC (group I: 17 patients, 10 men and 7 women; mean age 57 +/- 12 years) or RSPV (group II: 17 patients, 15 men and 2 women, mean age 62 +/- 14

Anticoagulation and spontaneous retropharyngeal hematoma.

A 57-year-old man treated with Coumadin for an episode of paroxysmal atrial fibrillation presented to the Emergency Department with progressive dysphagia and odynophagia. Physical examination revealed a large right-sided lateral and posterior pharyngeal wall mass. Laboratory studies were significant for an international normalized ratio (INR) of 5.4 with a white blood cell count of 11,600/muL and a hematocrit of 33.2%. A lateral soft tissue radiograph and computed tomography (CT) scan of the neck demonstrated a retropharyngeal hematoma compromising the airway. Rapid reversal of the coagulopath

Giant multiperforated atrial septal aneurysm in a patient with paroxysmal atrial fibrillation.

With the widespread use of transoesophageal echocardiography, atrial septal aneurysm (ASA) is frequently and easily recognized. Diagnostic criteria, however, remain arbitrary. A meticulous search for interatrial shunting (most often patent foramen ovale (PFO)) and associated cardiac diseases is important. ASA is considered a risk factor for cardioembolism, especially in association with patent foramen ovale. We describe a patient with a giant multiperforated ASA presenting with refractory paroxysmal atrial fibrillation. Resection of the aneurysm was performed in combination with a surgical Maz

Lessons from the Stroke Prevention in Atrial Fibrillation trials.

Atrial fibrillation predisposes to left atrial thrombus formation and carries a sixfold increased risk for stroke. Antithrombotic therapies are the mainstay for stroke prevention. The National Institute of Neurological Disorders and Stroke-sponsored Stroke Prevention in Atrial Fibrillation (SPAF) studies assessed the value of warfarin, aspirin, and their combination for preventing stroke in six multicenter trials involving 3950 participants. This review presents the major results and implications, which offer unique perspectives on antithrombotic therapies for stroke prevention in atrial fibri

Sudden death of a case of hypertrophic obstructive cardiomyopathy 19 months after successful percutaneous transluminal septal myocardial ablation.

A 56-year-old male with hypertrophic obstructive cardiomyopathy complicated with medically refractory paroxysmal atrial fibrillation and congestive heart failure was treated with percutaneous transluminal septal myocardial ablation. The resting left ventricular outflow tract gradient decreased from 70 mmHg to 0 mmHg after the procedure, and clinical symptoms improved dramatically. However, the patient died suddenly 19 months later and autopsy revealed nontransmural myocardial fibrosis with an irregular border in the interventricular septum.

A completely endoscopic approach to microwave ablation for atrial fibrillation.

BACKGROUND: Surgical therapy for atrial fibrillation is extremely effective but not widely applied. Minimally invasive methods may be more attractive. We report a case of complete pulmonary vein isolation performed entirely through a thoracoscopic approach. METHODS: A 74-year-old woman with paroxysmal atrial fibrillation underwent a microwave ablation procedure using 3 access ports in the right pleural space and 3 access ports through the left pleural space. RESULTS: After the 3-hour procedure the patient was extubated in the operating room and discharged home in sinus rhythm on the second p

Catheter ablation of paroxysmal atrial fibrillation initiated by non-pulmonary vein ectopy.

BACKGROUND: Most of the ectopic beats initiating paroxysmal atrial fibrillation (PAF) originate from the pulmonary vein (PV). However, only limited data are available on PAF originating from the non-PV areas. METHODS AND RESULTS: Two hundred forty patients with a total of 358 ectopic foci initiating PAF were included. Sixty-eight (28%) patients had AF initiated by ectopic beats (73 foci, 20%) from the non-PV areas, including the left atrial posterior free wall (28, 38.3%), superior vena cava (27, 37.0%), crista terminalis (10, 3.7%), ligament of Marshall (6, 8.2%), coronary sinus ostium (1, 1

Dissociated activity and pulmonary vein fibrillation following functional disconnection: impact for the arrhythmogenesis of focal atrial fibrillation.

The present study sought to investigate the electrophysiological properties of isolated pulmonary veins following successful radiofrequency (RF) catheter ablation in patients with paroxysmal atrial fibrillation (PAF). Overall, 71 pulmonary veins in 37 consecutive patients (age: 56 +/- 9 years) with recurrent PAF were targeted for RF ablation at the ostial region in order to achieve a complete functional block. Following disconnection, the incidence of dissociated pulmonary vein (PV) activity and its response to orciprenalin were studied. RF ablation abolished conduction in 67 (94%) of 71 poten

Dilatation of the pulmonary veins in atrial fibrillation: a transesophageal echocardiographic evaluation.

Ectopic beats originating from sleeves of atrial tissue within the pulmonary veins (PVs) can induce and sustain paroxysmal atrial fibrillation (AF). Left atrial stretch and dilatation favors the development of atrial ectopy and AF. Similarly, PV dilatation, if present, might trigger PV ectopy in patients with AF. This study was designed to evaluate whether PV dilatation is present in patients with nonfocal AF and whether the PV diameter correlates to the left atrial diameter (LAD). The diameters of the right superior (RSPV) and left superior PV (LSPV) were measured at the ostium and at a depth

Electrophysiological demonstration of dissociation of pulmonary vein potentials through electroanatomically guided circumferential ablation.

A 57-year-old woman with frequent isolated and repetitive premature atrial contractions initiating paroxysmal atrial fibrillation, underwent electrophysiological study and catheter ablation. A real-time three-dimensional map of the left atrium was reconstructed using a nonfluoroscopic navigation system. By means of a deflectable decapolar catheter, the left superior pulmonary vein (PV) was identified as the arrhythmogenic vein, and PV potentials were found in the left inferior and right superior veins. Ablation was performed under electroanatomic guidance. After circumferential ablation outsid

Increased p-wave duration and p-wave dispersion in patients with aortic stenosis.

BACKGROUND: P-wave dispersion (PWD), defined as the difference between the maximum and minimum P-wave duration, has been proposed as being useful for the prediction of paroxysmal atrial fibrillation (AF). AF is the most common arrhythmia and an important prognostic indicator for clinical deterioration in patients with aortic stenosis (AS). The aim of the present study was to evaluate PWD in patients with AS. METHODS: The study population consisted of two groups: Group I consisted of 98 patients with AS (76 men, 22 women; aged 63 +/- 8 years) and group II consisted of 98 healthy subjects (same
