[Plasma metabolomics combined with machine learning for classifying ischemic stroke subtypes and predicting the etiology of cryptogenic stroke].

OBJECTIVES: To analyze plasma metabolic profiles of different ischemic stroke subtypes and construct machine learning (ML)-based models to identify these subtypes for accurate diagnosis of cryptogenic stroke. METHODS: A total of 277 patients with acute ischemic stroke (AIS) from 3 hospitals were prospectively enrolled and stratified according to the TOAST classification criteria. Venous blood samples were collected for ¹H-NMR metabolomics detection, resulting in the identification of 46 metabolites. Metabolomics and Mendelian randomization analyses were employed to identify the potential biom

Transient sinus arrest caused by coronary spasm of the sinus node artery after pulsed field ablation for superior vena cava isolation: a case report.

BACKGROUND: Pulsed field ablation (PFA) has emerged as a novel non-thermal ablation modality with preferential myocardial selectivity. However, its effects on adjacent structures, including the sinus node and its arterial supply, remain incompletely understood. CASE SUMMARY: A 72-year-old woman underwent repeat ablation for recurrent paroxysmal atrial fibrillation. Following pulmonary vein isolation, superior vena cava (SVC) isolation was performed using a PFA system. Sinus rhythm was initially preserved. However, sinus arrest developed several minutes after ablation. High right atrial backup

Periatrial Epicardial Adipose Tissue as a Predictor of Substrate Distribution and Atrial Arrhythmia Recurrence in Patients with Early Atrial Remodeling.

BACKGROUND: Epicardial adipose tissue (EAT) is hypothesized to induce atrial remodeling. However, no prior studies have evaluated the spatial relationship between EAT and early atrial substrate. Given atrial fibrosis may progress from epi- to endocardium, unipolar voltage may be a more sensitive marker of early remodeling. OBJECTIVE: To evaluate the relationship between left atrial (LA) EAT, local unipolar and bipolar voltage, and post-ablation arrhythmia recurrence in patients with early atrial remodeling. METHODS: Consecutive patients underwent high-resolution LA electroanatomic mapping be

Five-year outcomes following catheter ablation for paroxysmal atrial fibrillation: a propensity matched analysis of 51 182 patients from a real world cohort.

AIMS: Atrial fibrillation (AF) poses significant risks of stroke and mortality. Catheter ablation (CA) has emerged as a superior rhythm control strategy compared with medical therapy, but its long-term benefits in AF, in ischaemic stroke prevention, remain underexplored. METHODS AND RESULTS: This observational study analysed data from the TriNetX Research Network, encompassing over 115 million patients. Adults diagnosed with paroxysmal atrial fibrillation (PAF) between 2012 and 2019 were stratified into CA and non-CA groups. Propensity score matching (PSM) accounted for baseline differences i

Ultra-Early Continuous ECG Monitoring for Silent Atrial Fibrillation Diagnosis in Cryptogenic Stroke: The CRIPTOFAST Randomized Controlled Trial.

BACKGROUND: Cryptogenic strokes (CS) represent one third of admissions for stroke. Silent paroxysmal atrial fibrillation (PAF) is the underlying cause of a significant proportion of cases. The use of internal loop recorders (ILR) after CS has shown controversial results, remaining unclear in guidelines. Subtle ultrasound left atrium (LA) anomalies may help select patients more prone to suffer from silent PAF who can benefit from an ILR. METHODS: Randomized, controlled, parallel-arm, open-label trial of patients with CS. We evaluated the efficacy of early ILR for detection of silent PAF episod

Gml-PAF: A Generalizable Machine Learning Algorithm for Paroxysmal Atrial Fibrillation Detection based on Short-Term Inter-Beat Intervals.

Reliable detection of paroxysmal atrial fibrillation (PAF) poses a significant challenge. We propose a generalizable machine learning (ML) algorithm for PAF detection (Gml-PAF) that uses 21-beat inter-beat intervals (IBI). Gml-PAF employs a model-agnostic framework integrating model selection, feature selection, and hyperparameter tuning. It is trained and evaluated across 16 PhysioNet electrocardiogram (ECG) databases, demonstrating robust cross-database generalization. In independent tests, it achieves F1 scores of 0.747-0.987 and AUC values of 0.933-0.999. The algorithm matches deep learnin

Thrombolysis in acute stroke complicated by limb ischaemia: a dual thrombectomy approach.

We report a case of a man in his early 70s with paroxysmal atrial fibrillation who developed acute upper limb ischaemia following intravenous thrombolysis (IVT) for acute ischaemic stroke. The patient presented with left-sided weakness and was treated with alteplase, after which he experienced left arm ischaemia. Imaging revealed a left axillary artery occlusion and a left atrial appendage thrombus, suggesting embolisation. Simultaneous mechanical thrombectomy was performed for both the intracranial and axillary artery occlusions, successfully restoring perfusion. This case highlights the risk

Reversible 2:1 Atrioventricular Block After Chronic Coronary Revascularization.

BACKGROUND: Atrioventricular (AV) conduction disturbances in elderly patients are often attributed to degeneration. However, myocardial ischemia may also cause AV block, which can be reversible with coronary revascularization. CASE SUMMARY: A 77-year-old woman with diabetes and hypertension presented with recurrent syncope. A 7-day Holter monitor showed intermittent 2:1 AV block and asymptomatic paroxysmal atrial fibrillation. Cardiac computed tomography and coronary angiography revealed severe proximal and mid left anterior descending artery stenoses, located before the origin of the first s

Ablation of atrial arrhythmias after lobectomy: high-density mapping and CT integration to guide individualized treatment.

BACKGROUND: Atrial fibrillation is a known complication following thoracic surgery, with residual pulmonary vein (PV) stumps often implicated as sources of arrhythmogenic triggers. Existing literature suggests that these stumps can maintain ectopic activity, leading to arrhythmia recurrence. In this case, contrary to common findings, a PV stump in a post-lobectomy patient was found to be electrically inert, challenging the assumption that all such stumps are pro-arrhythmic. The absence of electrical activity in the stump suggests that its anatomical characteristics, such as being extremely sho

Predictors of arrhythmia recurrence after cryoballoon ablation in paroxysmal atrial fibrillation: a comparative analysis from the CONTRAST-CRYO trial.

BACKGROUND: Factors associated with arrhythmia recurrence after atrial fibrillation (AF) ablation with different cryoballoon systems remain unclear. We sought to assess the predictors of arrhythmia recurrence after cryoballoon ablation using two different cryoballoon systems. METHODS: This substudy of the CONTRAST-CRYO trial compared the novel cryoballoon (NCB) with the conventional cryoballoon (CCB) for pulmonary vein (PV) isolation (PVI) in patients with paroxysmal AF. A total of 198 patients (66 ± 12 years; 152 men; 22 with structural heart disease [SHD]) underwent ablation (CCB, 101; NCB,

Transthoracic Transmitral Atrial Flow is Independently Associated with Ischemic Stroke Risk in Paroxysmal Atrial Fibrillation.

Atrial fibrillation (AF) significantly increases the risk of ischemic stroke. This study evaluates whether transmitral atrial flow velocity (MVA), measured non-invasively via transthoracic echocardiography (TTE), predicts stroke risk in AF patients. To assess the independent association between TTE-derived MVA and stroke incidence in paroxysmal AF patients and its value in refining risk stratification, especially in low-risk groups. This cohort study included 10,150 paroxysmal AF patients from 2010 to 2021. The primary outcome was hospitalization for ischemic stroke. Multivariable Cox regression

Thromboinflammation and hemolysis after pulsed field ablation with a pentaspline catheter: Clinical predictors, renal safety, and rhythm outcomes.

BACKGROUND: Hemolysis was recently identified as a side effect of pulsed field ablation (PFA), which raised renal safety concerns. Further systemic effects of PFA-induced hemolysis remain unknown. OBJECTIVE: This study aimed to investigate the thromboinflammatory potential of PFA and the systemic biological effects of PFA-induced hemolysis. METHODS: We prospectively included 60 patients with paroxysmal atrial fibrillation receiving pulmonary vein isolation using a pentaspline PFA system (n = 30) or radiofrequency ablation (RFA) (n = 30). Lesion characteristics after pulsed-field ablation in patients with paroxysmal atrial fibrillation and a left common ostium (PFALCO RE-MAP STUDY).

BACKGROUND: Approximately one-fifth of patients undergoing pulmonary vein isolation have a left common ostium (LCO). These patients tend to have worse arrhythmia-free survival when treated with a single-shot device, which may be related to lesion location and ablation thermodynamics. PURPOSE: This study evaluates lesion characteristics around the LCO in paroxysmal atrial fibrillation (AF) patients treated with a pentaspline pulsed-field ablation (PFA) modality. METHODS: Twenty-two patients underwent PFA. Directly after the PFA applications, an electro-anatomical map (EAM) was created using t

Myocardial and Neural Damage due to Pulmonary Vein Isolation: Comparison of Pulsed Electric Field and Thermal Ablation.

BACKGROUND: Pulmonary vein isolation (PVI) using thermal energy-radiofrequency (RF) or cryoablation (CRYO)-is associated with direct myocardial injury and collateral neural damage. In contrast, pulsed electric field (PEF) energy has emerged as a neural-sparing alternative. There is a lack of quantitative data on proportional neural damage resulting from individual ablation energies. OBJECTIVE: To assess the impact of PVI by RF, CRYO and PEF on the release of plasma biomarkers of myocardial and neural damage. METHODS: This study included 85 patients with paroxysmal atrial fibrillation undergo

Distinct Automaticity and Excitability Between Radiofrequency and Cryoballoon Ablation in Paroxysmal Atrial Fibrillation.

BACKGROUND: Cryoballoon ablation (CBA) has been shown as an effective alternative strategy to radiofrequency ablation (RFA) for the treatment of paroxysmal atrial fibrillation (PAF), with comparable efficacy and safety. However, the electrophysiological properties in pulmonary veins (PVs) after CBA and RFA are not well understood. This study compares these characteristics in PAF patients. METHODS: Consecutive patients with PAF undergoing initial catheter ablation were prospectively randomized to CBA (N = 50) or RFA (N = 50) for PV isolation (PVI). Each PV's automaticity and excitability were
