Spontaneous splenic rupture (SSR) is a rare but potentially life-threatening condition, most commonly associated with underlying infectious, haematological, vascular, or neoplastic processes. Clinical presentation is often non-specific, which may lead to delayed diagnosis. Imaging, particularly contrast-enhanced computed tomography (CECT), plays a pivotal role in confirming splenic injury, identifying predisposing lesions, and guiding management. We present the case of a woman aged in her seventies with chronic atrial fibrillation on antiplatelet therapy who developed spontaneous splenic ruptu

BACKGROUND: Thrombus formation within a left atrial septal pouch is uncommon but may critically impact septostomy-based interventions.
CASE SUMMARY: A 55-year-old woman with severe rheumatic mitral stenosis and chronic atrial fibrillation on anticoagulation was evaluated for percutaneous transmitral commissurotomy. Transesophageal echocardiography revealed a large bilayered interatrial septal pouch containing a semimobile thrombus, whereas the left atrial appendage was thrombus-free. Cardiac computed tomography angiography confirmed a focal septal filling defect. Given the embolic risk associ

BACKGROUND: The Watchman device (Boston Scientific) is designed for left atrial appendage closure and has shown sufficient safety for clinical use.
CASE SUMMARY: We report a 70-year-old woman with chronic atrial fibrillation (CHA2DS2-VASc score: 4, HAS-BLED score: 3) and bronchiectasis with occasional hemoptysis who underwent successful percutaneous left atrial appendage closure with a 35-mm Watchman FLX device. The initial postoperative course was uneventful; however, she presented with persistent chest pain 2.5 years later. Computed tomography and coronary angiography revealed near-complete

Recurrent embolic events are not unusual in patients with non-valvular atrial fibrillation, and management of anticoagulation in these patients can be particularly challenging. We report a case of a patient who experienced multiple embolic events in the lower extremities and cerebral circulation despite ongoing anticoagulant therapy for atrial fibrillation. The patient was an 88-year-old woman with chronic kidney disease Stage G3b who was taking oral steroids for rheumatoid arthritis. She was also taking a direct Xa inhibitor (edoxaban) for chronic atrial fibrillation, with a CHADS2 score of 2

PURPOSE: Internal medicine departments manage a broad spectrum of chronic diseases, which are frequently associated with higher mortality rates. In this context, we evaluated the relationship between the Prognostic Nutritional Index (PNI) and mortality in elderly inpatients with chronic atrial fibrillation (AF) admitted to an internal medicine department.
PATIENTS AND METHODS: This study included 158 hospitalized patients who were followed for five years post-discharge. We compared the following variables between the survival and non-survival groups: age, sex, presence of diabetes mellitus, h

We present the case of an 84-year-old man with acute coronary syndrome, ischaemic mitral regurgitation, and chronic atrial fibrillation. The patient was successfully treated using a two-stage hybrid strategy consisting of initial transcatheter mitral valve edge-to-edge repair followed by coronary artery bypass grafting with Penditure-based left atrial appendage closure.

INTRODUCTION: Reduced composite physical reserve (CPR) may coexist with systolic heart failure (HF).
OBJECTIVES: We aimed to evaluate clinical differences among patients stratified by CPR in the 6‑minute walk test (6MWT) and maximal handgrip (MHG) strength, and to identify clinical correlates of reduced physical function.
PATIENTS AND METHODS: This post hoc analysis included 332 men from a prospective cohort of outpatients with stable HF (left ventricular ejection fraction <50%) who completed both tests. The patients were stratified into 3 categories based on median splits of 6MWT (480 m) an

Secondary mitral regurgitation (MR) is associated with substantial cardiovascular morbidity and mortality. Traditionally, secondary MR has been understood primarily as a ventricular disorder caused by structural and functional abnormalities of the left ventricle. However, atrial functional mitral regurgitation (AFMR) is increasingly recognized as a distinct pathophysiological entity characterized by left atrial and mitral annular dilatation despite relatively preserved left ventricular geometry and systolic function. AFMR is commonly encountered in older patients, particularly those with chron

Perioperative atrial fibrillation (POAF) in non-cardiac surgery (NCS) is a multifactorial condition with significant prognostic implications. This review summarizes current evidence on the incidence, risk factors, pathophysiology, complications, prevention, and management of POAF in NCS. POAF affects around 3% of all patients undergoing NCS, with rates of 10-15% in thoracic procedures. Key risk factors include advanced age, male sex, pre-existing cardiopulmonary diseases, type, duration, and urgency of surgery as well as perioperative inflammation. Pathophysiological mechanisms are not fully u

BACKGROUND: Although rare, immune thrombocytopenia can coexist with antiphospholipid syndrome, creating a management dilemma between maintaining hemostasis and preventing thromboembolism. Herein, we report our strategic perioperative management of a patient with this complex hematological profile who underwent minimally invasive cardiac surgery.
CASE PRESENTATION: A 55-year-old man with chronic atrial fibrillation and severe mitral regurgitation was diagnosed with immune thrombocytopenia and antiphospholipid syndrome. A minimally invasive cardiac surgery approach was chosen to minimize surgic

In acute heart failure (HF), the response of secondary tricuspid regurgitation (STR) to therapeutic management may indicate reverse right ventricular remodeling and response to treatment. This study sought to explore the dynamic trends of STR during acute HF hospitalization and their association with outcomes at follow-up. Among 463 acute hospitalized HF patients, 285 had significant STR on admission and were considered for this analysis. All patients underwent echocardiographic assessment on admission and prior to discharge and were divided into 2 groups: (1) significant STR on admission that

OBJECTIVE: To examine the surgical outcomes of surgical aortic valve replacement in the transcatheter aortic valve replacement era and propose a novel patient-specific prognostic model.
METHODS: We randomly divided 772 patients with aortic stenosis who underwent surgical aortic valve replacement in 2016-2021 into two cohorts (derivation, 515; validation, 257). In the derivation cohort, no data were missing for any patients for the candidate predictors including age, sex, body mass index, left ventricular ejection fraction, levels of albumin, hemoglobin, and serum creatinine, presence of chron

Rectus sheath hematoma is an uncommon but important cause of acute abdominal pain that may closely mimic intra-abdominal surgical emergencies, resulting in diagnostic uncertainty and potential delays in management. We report the case of a 64-year-old woman receiving warfarin therapy for chronic atrial fibrillation who presented with sudden-onset severe lower abdominal pain associated with abdominal distension and signs suggestive of an acute abdomen. Clinical evaluation revealed localized abdominal wall tenderness, a palpable infraumbilical mass, anemia, and supratherapeutic anticoagulation. C

Left atrial appendage aneurysm (LAAA) is a rare congenital cardiac anomaly that is associated with potentially life-threatening complications, including thromboembolism and arrhythmias. We report a 36-year-old male patient with giant LAAA (66 × 46 mm) and chronic atrial fibrillation (CAF) presenting with palpitations. Surgical resection of LAAA and Cox-Maze IV procedure were performed under cardiopulmonary bypass. The patient was discharged in sinus rhythm, remained asymptomatic, and had normal echocardiographic findings at follow-up.

BACKGROUND: Right atrial appendage (RAA) thrombosis is exceedingly rare, is usually found incidentally on imaging, and carries a high thromboembolic risk.
CASE SUMMARY: An 83-year-old man with chronic atrial fibrillation complicated by previous bleeding episodes on anticoagulation and managed most recently with a WATCHMAN device presented after a mechanical fall. Imaging work-up revealed a new incidental finding of RAA thrombosis.
DISCUSSION: This case highlights an uncommon thrombotic finding and that atrial fibrillation is a biatrial disease. Occlusion of the left atrial appendage does not

INTRODUCTION AND IMPORTANCE: Acute mesenteric ischemia (AMI) is defined as a sudden interruption of blood flow to a segment of the intestine, leading to ischemia, inflammation, and potential bowel necrosis if untreated. It is a life-threatening vascular emergency, and its occurrence following an acute ischemic stroke is exceedingly uncommon, with very few reports in the literature.
CASE PRESENTATION: We report a 76-year-old woman with chronic atrial fibrillation and degenerative valvular heart disease who presented with acute ischemic stroke and, on the third day of admission, developed sudde

An 89-year-old woman with chronic atrial fibrillation fell to the right side and was found to have right-sided weakness and impaired consciousness. Initial evaluation, including Focused Assessment with Sonography for Trauma (FAST), head computed tomography (CT), cervical CT, and thoracoabdominal CT, showed no intracranial hemorrhage, cervical epidural hematoma, aortic dissection, or thoracoabdominal bleeding. Brain magnetic resonance imaging (MRI) showed acute ischemic lesions in the left middle cerebral artery territory with a diffusion-weighted imaging-fluid-attenuated inversion recovery (DW

BACKGROUND: Left atrial appendage (LAA) closure reduces stroke in patients with atrial fibrillation.
CASE SUMMARY: A 63-year-old man with chronic atrial fibrillation had undergone mitral valve replacement and endocardial suture closure of the LAA 5 years before presentation. One year postoperatively, a giant LAA was noted, and coronary angiography revealed a newly developed coronary-LAA fistula. Progressive enlargement of the LAA was confirmed 5 years postoperatively, with expansion of the fistulous tract on coronary angiography, ultimately leading to surgical LAA resection.
DISCUSSION: Inco
