Intermediate Atrial Tachycardia During Persistent Atrial Fibrillation Ablation: A Sub-analysis of the Extent-AF Trial.

BACKGROUND: The evidence supporting additional linear ablation for persistent atrial fibrillation (PerAF) remains limited. This study investigates the mechanisms of intermediate atrial tachycardia (AT), a transient state between PerAF and sinus rhythm (SR), during PerAF termination by catheter ablation, and provides evidence for these mechanisms. METHODS: We analyzed 136 patients who converted to organized AT after PerAF termination in the Extent-AF study. Bi-atrial activation mapping combined with entrainment mapping was used to identify the mechanisms and critical isthmus of these ATs. RES

Left Atrial Substrate Modification for Long-Standing Persistent Atrial Fibrillation and Left Atrial Macro- or Micro-Reentrant Tachycardia Using a Single-Shot Pulsed Field Ablation System-A Case Series.

Background: Pulsed field ablation [PFA] is a novel ablation technique for pulmonary vein isolation [PVI] in patients with paroxysmal and persistent atrial fibrillation. However, data for the efficacy and safety of PFA for left atrial substrate modification using a single shot PFA system, in patients with long-standing persistent atrial fibrillation [AF] and left atrial macro- as well as micro-reentrant atrial tachycardia [LAMRT], are scarce. Here, we provide a small, single-center case series regarding the efficacy and safety of left atrial substrate modification using a single-shot PFA system

Pulsed Field Ablation of Persistent Atrial Fibrillation With Continuous Electrocardiographic Monitoring Follow-Up: ADVANTAGE AF Phase 2.

BACKGROUND: There are sparse high-quality safety and effectiveness data for pulsed field ablation (PFA) of persistent atrial fibrillation (PerAF), in which lesions beyond pulmonary vein isolation (PVI) are often placed. In addition, no large trials have used insertable cardiac monitors for continuous rhythm monitoring after ablation in patients with PerAF or after PFA in any atrial fibrillation population. In Phase 2 of the ADVANTAGE AF study (A Prospective Single Arm Open Label Study of the FARAPULSE Pulsed Field Ablation System in Subjects With Persistent Atrial Fibrillation), patients with

Safety and Efficacy of Pulsed Field Ablation for Cavotricuspid Isthmus-Dependent Flutter: A Systematic Literature Review.

Cavotricuspid isthmus ablation (CTI) is a first-line therapy in patients with typical atrial flutter. With the advent of pulsed field (PF) as a new energy source, we sought to evaluate the use of PF for CTI ablation. A systematic literature search was conducted on the use of PF for CTI-flutter up to December 2024. A meta-analysis was performed for studies reporting pooled data, while individual case reports were reviewed and summarized. The mean number of PF applications, acute success rate, and prevalence of coronary vasospasm were evaluated. Eleven studies with pooled data from 155 patients

Comparison of Two High-Power Ablation Strategies for Typical Atrial Flutter: Acute and Long-Term Outcome.

BACKGROUND: Ablation of the cavo-tricuspid isthmus (CTI) is the standard treatment for typical atrial flutter. High-power strategies have been described to improve lesion efficacy and durability. OBJECTIVE: To compare the acute success, safety, and long-term outcomes of two strategies of high-power CTI ablation using 8-mm gold-tip nonirrigated and 4-mm irrigated-tip catheters. METHODS: This single-center prospective cohort study included 253 patients who underwent CTI ablation. Patients were treated with either an 8-mm gold-tip nonirrigated catheter (60 W, ≥ 30 s) or a 4-mm irrigated cathete

Fluoroless cavotricuspid isthmus ablation of typical atrial flutter achieves success with shorter radiofrequency and procedural times.

INTRODUCTION AND OBJECTIVES: Cavotricuspid isthmus (CTI) ablation for typical atrial flutter (AFL) has improved in recent years, especially with three-dimensional (3D) electroanatomic mapping systems. These tools allow a better understanding of the arrhythmia with higher ablation success rates while reducing radiation exposure. However, most ablation procedures still require varying amounts of fluoroscopy. Our aim was to examine whether fluoroless CTI ablation is effective and safe in reducing AFL recurrence, compared with CTI ablation using fluoroscopy. METHODS: We conducted a retrospective

Pulsed field ablation of the cavotricuspid isthmus using a balloon-in-basket system.

BACKGROUND: Pulsed field ablation (PFA) has emerged as a promising modality for pulmonary vein isolation (PVI) in atrial fibrillation (AF), with favourable safety and efficacy profiles. However, its application to other arrhythmias, such as typical atrial flutter (AFl), remains exploratory. The Volt™ PFA system (Abbott Laboratories), a novel balloon-in-basket catheter capable of mapping, pacing and delivering ablating energy, has been validated for PVI, but has not yet been used for the treatment of AFl cavotricuspid isthmus (CTI) dependent. We report the first documented case of successful co

Prophylactic pulmonary vein isolation in patients with typical atrial flutter and heart failure.

BACKGROUND: Heart failure (HF) readmission rates remain a concern in patients with typical atrial flutter (AFL) and HF, even after cavotricuspid isthmus (CTI) ablation. OBJECTIVE: This study aimed to explore whether additional prophylactic circumferential pulmonary vein isolation (CPVI) could improve hard outcomes among patients with AFL and HF. METHODS: In this prospective, nonrandomized, controlled study, patients with AFL and HF were consecutively enrolled from 9 centers. Patients received either CTI ablation + CPVI (CTI + CPVI group, n = 42) or CTI ablation alone (CTI-alone group, n = 39

Addition of Atrial Myopathy to HATCH Score for Predicting New-Onset Atrial Fibrillation After Ablation of Atrial Flutter.

BACKGROUND: The available risk prediction models are inadequate to identify a true low-risk patient for developing new-onset atrial fibrillation (AF) after typical atrial flutter (AFL) ablation. We aimed to determine whether adding markers of atrial myopathy to HATCH score (hypertension, age ≥75 years, transient ischemic attack/stroke, chronic obstructive pulmonary disease, and heart failure) can improve prediction of new-onset AF after ablation of typical AFL. METHODS: The study included 208 consecutive patients who underwent successful ablation of typical AFL at Minneapolis VA Medical Cente

Mechanism of Typical Atrial Flutter in a Patient After Cardiac Transplant: Functional Slowing From Geometric Distortion.

BACKGROUND: Atrial flutter and atrial fibrillation occur after orthotopic heart transplant (OHT) in 25% of patients. Hemodynamic and medication restrictions in this unique population limit treatment. CASE SUMMARY: A patient experienced prolonged episodes of paroxysmal atrial flutter and hypotension 8 days after OHT. Electrophysiology study demonstrated typical cavotricuspid isthmus (CTI)-dependent flutter, with slowing at the CTI midportion. Intracardiac echocardiography revealed that this site was distant from any suture lines or anastomoses. The slowing appeared to be secondary to geometric

Clinical Predictors of Recurrent Cavotricuspid Isthmus Flutter After Catheter Ablation in Patients With Dextro-Transposition of the Great Arteries.

INTRODUCTION: Patients with surgically repaired D-transposition of the great arteries (D-TGA) via atrial switch are at high risk of cavotricuspid isthmus (CTI) dependent atrial flutter, which can be challenging to address. High rates of CTI-dependent flutter recurrence after initial ablation have been reported, but clinical predictors of recurrence remain poorly understood. METHODS: We identified patients with a history of d-TGA repaired with atrial switch undergoing catheter ablation for CTI- dependent flutter from 2011 to 2023. Demographics, cardiac imaging, and procedural characteristics w

Incidence and Predictors of Atrial Fibrillation After Cavotricuspid Isthmus Ablation for Typical Atrial Flutter.

Cavotricuspid isthmus (CTI) ablation is a highly effective treatment of typical atrial flutter (AFL). The emergence of new-onset atrial fibrillation (AF) following CTI ablation is clinically relevant but limited data exist regarding its predictors. The goal of this study is to investigate predictors of new-onset AF following CTI ablation. Patients who underwent CTI ablation between 2016 and 2022 were included. Baseline variables including left atrial volume index (LAVI), cardiac comorbidities, CHA₂DS₂-VASc score, and medications were collected. The primary outcome was the occurrence of new-ons

Pulsed-field ablation for refractory typical atrial flutter resistant to multiple radiofrequency ablations: A technical report.

This technical report presents a case with longstanding, recurrent typical atrial flutter who experienced multiple relapses despite four prior radiofrequency (RF) ablation procedures targeting the cavotricuspid isthmus (CTI). Initial RF attempts failed to achieve durable bidirectional CTI block, leading to persistent arrhythmia and significant symptom burden. Upon current admission, comprehensive electrophysiological mapping confirmed a scar-related, clockwise right atrial flutter with residual conduction across the CTI, notably on both the ventricular and inferior vena cava (IVC) sides. Given

Catheter ablation of macro-reentrant atrial tachycardia in adults with Ebstein anomaly: A narrative review.

Improving survival rates of patients with Ebstein anomaly have increased the relevance of catheter ablation of macro-reentrant atrial tachycardia (MRAT). Our objectives were, first, to present a case report and, second, to provide a comprehensive review. A search of articles providing detailed information on adults with Ebstein anomaly who underwent catheter ablation of MRAT was performed. PubMed was queried using "Ebstein" and "ablation" as Medical Subject Headings terms, followed by a review of references and citations. Of the 56 patients included in the final review, which included the case
