Randomized comparison of two targets in typical atrial flutter ablation.

Typical atrial flutter ablation has become anatomically guided to 2 separate sites within the isthmus at the inferior right atrium: (1) between the inferior vena cava and the tricuspid annulus (anterior side of the isthmus [A]), (2) between the eustachian crest, the coronary sinus ostium and tricuspid annulus (posterior side of the isthmus [P]). We prospectively compared ablation results at these sites in 72 consecutive patients. Patients were randomized in group P or A according to the initial target site. If ablation failed at 1 site after 15 radiofrequency (RF) pulses, the other side of the

Cardioversion of persistent atrial flutter in non-anticoagulated patients at low risk for thromboembolism.

BACKGROUND: The true risk of thromboembolic events after cardioversion of atrial flutter was not addressed carefully. Nevertheless, thromboembolic events were thought to be rare and less likely to occur after cardioversion of atrial fibrillation. The aim of this study was to prospectively evaluate if the interruption of persistent typical atrial flutter could be safely performed without anticoagulation in a group of patients at low risk for thromboembolic events. METHODS: We studied 64 subjects selected among 138 consecutive patients with persistent typical atrial flutter (minimal duration 72

Relationship between surface electrocardiogram characteristics and endocardial activation sequence in patients with typical atrial flutter.

OBJECTIVES: The relationships that exist between endocardial and surface electrocardiogram (ECG) activity in patients with AFl have not been satisfactorily delineated. The aim of the study was to determine the relationships that exist between the atrial endocardial activity and the surface ECG in patients with atrial flutter (AFl). METHODS: In 40 patients with AFl, a 64-electrode basket catheter (BC) was deployed in the right atrium (RA). A decapolar catheter was inserted into the coronary sinus (CS) to record from the left atrium. The temporal relationship between endocardial and surface ECG

Management of atrial flutter.

Typical atrial flutter is a macroreentrant arrhythmia of the right atrium. The isthmus area between the tricuspid annulus, the inferior vena cava, and the ostium of the coronary sinus is a critical zone of the reentry circle. Atrial flutter has been treated with class I and III antiarrhytmic drugs to maintain sinus rhythm, with moderate success. Catheter ablation has been highly successful in treating atrial flutter. A contiguous line of bidirectional electrical block is created in the isthmus area between the tricuspid annulus and the inferior vena cava by application of radiofrequency energy

Differential pacing for distinguishing block from persistent conduction through an ablation line.

BACKGROUND: Because complete linear conduction block is necessary to minimize the recurrence of reentrant tachycardias such as typical atrial flutter, we investigated a simple technique to recognize a persistent gap or complete linear block. METHODS AND RESULTS: We prospectively evaluated cavotricuspid isthmus conduction in 50 patients (age 63+/-8 years, 43 men) after radiofrequency ablation. The distal and proximal bipoles of a quadripolar catheter placed close to the ablation line were successively stimulated during recording from the ablation line. We hypothesized that because the initial

The role of the crista terminalis in atrial flutter and fibrillation: a computer modeling study.

Although atrial fibrillation is a common arrhythmia, the underlying mechanisms are incompletely understood. Recent studies have determined the role of the crista terminalis in the mechanisms of a simpler arrhythmia, atrial flutter. We hypothesize that as transverse coupling across the crista terminalis increases, the activation pattern that results is less like typical atrial flutter and more like atrial fibrillation. 6480 Van Capelle elements were coupled in an icosahedron, simulating the right atrium. Atrial simulations were created which incorporated no heterogeneity, heterogeneous coupling

Comparison of effectiveness of an 8-mm versus a 4-mm tip electrode catheter for radiofrequency ablation of typical atrial flutter.

An 8-mm catheter does not appear superior to 4-mm tip electrode for atrial flutter ablation. The potential advantage of allowing higher energy delivery on a larger surface is compensated by the lack of consistent contact with the endocardial surface.

Electroanatomic mapping to identify breakthrough sites in recurrent typical human flutter.

The accuracy of conventional techniques in localizing previous radiofrequency (RF) ablation sites and thus breakthrough sites of recurrent atrial flutter is somewhat limited. We investigated the role of electroanatomic mapping for identifying breakthrough sites or "gaps" at the tricuspid annulus and inferior vena cava (IVC)/eustachian ridge isthmus to help RF ablation in patients with recurrent typical flutter. Twelve patients (8 men, 4 women, age 63 +/- 10 years) with recurrent typical atrial flutter were included in the study. An electroanatomic mapping system (CARTO) was used to create a vo

Supraventricular Tachyarrhythmia.

The main treatment modalities for supraventricular tachycardia are medications and catheter ablation. Ablation is appropriate therapy for paroxysmal supraventricular tachycardia in patients who have a preference for ablation over medications, symptoms that are refractory to medications, severe symptoms, Wolff-Parkinson-White syndrome, or incessant tachycardia. Ablation also is reasonable as first-line therapy in patients with recurrent typical atrial flutter.

Treatment of mixed atrial fibrillation and typical atrial flutter by hybrid catheter ablation.

Successful isthmus ablation of typical atrial flutter mixed with atrial fibrillation (AF) may favorably modify the subsequent course of paroxysmal AF. However, the source of ectopic beats triggering AF may be located in the pulmonary veins (PV). This study compared the results of combined isthmus and focal ablation with ablation limited to the isthmus in patients with mixed AF and typical atrial flutter. Thirty patients with typical atrial flutter and AF were treated. Ablation limited to the isthmus was performed in 14 patients (group A), and 16 patients underwent focal ablation of triggering

Clinical significance of residual slow cavotricuspid isthmus conduction after ablation of typical atrial flutter.

The purpose of this study was to evaluate the clinical significance of residual slow cavotricuspid isthmus (IT) conduction on the ablation line after typical atrial flutter (AF) ablation, undetected by analysis of right atrial (RA) activation. Seventy patients with AF underwent IT ablation. In the first 35 patients (group I), IT block was verified only by the RA activation sequence. In the subsequent 35 patients (group II), IT block was verified by the presence of parallel double potentials with an isoelectric interval through the entire ablation line (in addition to RA activation sequence cri

How to map and ablate atrial scar macroreentrant tachycardia of the right atrium.

A special form of macroreentrant atrial tachycardia (MRAT), due to reentrant activation around surgical scars, can occur in patients after cardiac surgery. Scar MRAT occurs usually after correction of congenital defects, such as atrial or ventricular septal defects, and especially after Mustard, Senning or Fontan procedures, but it can occur also after myxoma, valvular or coronary bypass surgery. The simplest form of scar MRAT is reentry around a lateral right atrial surgical scar. A basic mapping array with multiple simultaneous recordings from the anterior and septal right atrium is very use

Detection of atrial arrhythmia for cardiac rhythm management by implantable devices.

Implantable atrial defibrillators (IAD) should provide pacing therapy whenever appropriate (ie, typical atrial flutter) to minimize shock-related patient discomfort. Additionally, IADs should provide diagnostics regarding atrial arrhythmia type and frequency of occurrence to enable improved physician management of atrial arrhythmia. To achieve this, IADs should accurately classify atrial arrhythmia such as atrial fibrillation (AF) and atrial flutter (AFL) This article evaluates the performance of an algorithm, atrial rhythm classification (ARC), designed to classify AF and AFL. The ARC algorit

Electromagnetic versus fluoroscopic mapping of the inferior isthmus for ablation of typical atrial flutter: A prospective randomized study.

BACKGROUND: Radiofrequency catheter ablation within the tricuspid annulus-inferior caval vein isthmus can cure typical atrial flutter. The target for ablation, nonetheless, is relatively wide, and standard ablation procedures may require significant exposure to radiation. METHODS AND RESULTS: A total of 50 patients (mean age, 58+/-11 years) with typical atrial flutter were prospectively randomized to receive isthmus ablation using conventional fluoroscopy for catheter navigation (group I, n=24) or electromagnetic mapping (group II, n=26). Complete bidirectional isthmus block was verified with

Atypical atrial flutter originating in the right atrial free wall.

BACKGROUND: Data from experimental models of atrial flutter indicate that macro-reentrant circuits may be confined by anatomic and functional barriers remote from the tricuspid annulus-eustachian ridge atrial isthmus. Data characterizing the various forms of atypical atrial flutter in humans are limited. METHODS AND RESULTS: In 6 of 160 consecutive patients referred for ablation of counterclockwise and/or clockwise typical atrial flutter, an additional atypical atrial flutter was mapped to the right atrial free wall. Five patients had no prior cardiac surgery. Incisional atrial tachycardia wa

Electrophysiologic characteristics and ablation of an atypical atrial flutter in the right atrium.

Subeustachian isthmus-dependent typical atrial flutter has been well studied. We demonstrate a case with atypical atrial flutter involving only the base of the right atrium around the inferior vena cava. Entrainment pacing and mapping studies documented a distinct circuit traversing the subeustachian isthmus, propagating through the posterobasal right atrium, and skirting the inferior vena cava. Successful radiofrequency ablation of the arrhythmia was accomplished by creating a linear lesion at the subeustachian isthmus. Mapping of the inferior vena cava region and the demonstration of conceal

High-density mapping of activation through an incomplete isthmus ablation line.

BACKGROUND: Activation mechanisms through gaps in ablation lines and resulting electrograms are poorly understood. METHODS AND RESULTS: Eight patients (all men; age, 59+/-9 years) were studied during a recurrence of typical atrial flutter (cycle length, 233+/-19 ms) after a previous catheter ablation in the cavotricuspid isthmus. High-density 3-dimensional mapping of the isthmus was performed with the Cordis-Biosense EP Navigation system, and local conduction velocity (CV) was estimated. Maps created with 96+/-19 points revealed 0.8+/-0.3-cm gaps of recovered conduction in the ablation line.

Rate-dependent conduction block of the crista terminalis in patients with typical atrial flutter: influence on evaluation of cavotricuspid isthmus conduction block.

BACKGROUND: The crista terminalis (CT) has been identified as the posterior boundary of typical atrial flutter (AFL) in the lateral wall (LW) of the right atrium (RA). To study conduction properties across the CT, rapid pacing was performed at both sides of the CT after bidirectional conduction block was achieved in the cavotricuspid isthmus by radiofrequency catheter ablation. METHODS AND RESULTS: In 22 patients (aged 61+/-7 years) with AFL (cycle length, 234+/-23 ms), CT was identified during AFL by double electrograms recorded between the LW and posterior wall (PW). Human histopathologic findings following radiofrequency ablation of the tricuspid-inferior vena cava isthmus.

Radiofrequency (RF) ablation of the tricuspid valve-inferior vena cava isthmus is now the first line of treatment in the management of typical atrial flutter. Successful ablation is associated with conduction block in this region, although the histopathologic changes following this procedure have never been reported. We describe the pathologic changes following RF ablation of this region in an explanted heart of a patient undergoing heart transplantation 4 months after successful atrial flutter ablation. The findings confirm the ability of RF ablation to create in the isthmus a chronic full th

Partial cavotricuspid isthmus block before ablation in patients with typical atrial flutter.

OBJECTIVES: The purpose of this study was to prospectively evaluate preexisting partial isthmus block in the context of an electrophysiologically directed linear ablation strategy for typical atrial flutter (AF). BACKGROUND: Double potentials (DPs) separated by an isoelectric interval have been recognized as markers of local block. However, the presence and significance of DPs in the cavotricuspid isthmus during AF before ablation have not been evaluated. METHODS: Thirty consecutive patients with AF (counterclockwise: 24, clockwise: 6) were studied during AF. Sequential withdrawal mapping wa
