Linear ablation of right atrial free wall flutter: demonstration of bidirectional conduction block as an endpoint associated with long-term success.

INTRODUCTION: Ablation for atypical atrial flutter (AFL) is often performed during tachycardia, with termination or noninducibility of AFL as the endpoint. Termination alone is, however, an inadequate endpoint for typical AFL ablation, where incomplete isthmus block leads to high recurrence rates. We assessed conduction block across a low lateral right atrial (RA) ablation line (LRA) from free wall scar to the inferior vena cava (IVC) or tricuspid annulus in 11 consecutive patients with atypical RA free wall flutter. METHOD AND RESULTS: LRA block was assessed following termination of AFL, by

Three-dimensional mapping of atypical right atrial flutter late after chest stabbing.

We present the case of a female patient who previously underwent cardiac surgery for traumatic anterior right atrial perforation after a stabbing attack. Four years later the patient presented with right atrial common type flutter and isthmus ablation was performed subsequently. However, three years after isthmus ablation the patient was readmitted with atypical right atrial flutter. Electrophysiological study revealed persistent bidirectional isthmus block. Three-dimensional mapping (NavX, St. Jude Medical, St. Paul, MN, USA) demonstrated an incisional tachycardia with the critical isthmus at

Prediction of the atrial flutter circuit location from the surface electrocardiogram.

Identification of atypical atrial flutter (AFL) (non-cavo-tricuspid isthmus-dependent) prior to the electrophysiology laboratory is potentially useful because it allows appropriate procedural planning and enables discussion of the likely success rates and risks of the procedure with the patient. Typical counterclockwise AFL has a stereotypic appearance, the electrocardiogram (ECG) is predictive of the diagnosis in the majority of cases, and ablation procedures are associated with a high degree of safety and success. Atypical right atrial and left AFLs have a highly variable flutter wave morpho

Atrial tachycardia: mechanisms and management.

Atrial tachycardias comprise a heterogeneous group of arrhythmias that include focal atrial tachycardia, typical atrial flutter and atypical atrial flutter. Focal atrial tachyardias arise from automatic, triggered or microreentrant mechanisms, while typical and atypical flutters are macroreentrant in nature. Typical flutter describes a reentrant circuit that is dependent on the cavotricuspid isthmus in the right atrium while atypical flutter includes various lesional and de novo macroreentrant circuits in the right and left atria. Electrocardiographic criteria have been proposed to distinguish

Impact of remote magnetic catheter navigation on ablation fluoroscopy and procedure time.

BACKGROUND: Remote magnetic catheter navigation (RCN) is gaining acceptance in clinical cardiac electrophysiology, but details regarding how RCN affects procedure execution are not well characterized. METHODS: From January 1, 2005, to November 30, 2007, 721 cases were retrospectively analyzed and compared. Of these, 127 used RCN and 594 used manual catheter navigation (MCN). Data including procedure time, fluoroscopy time, ablation catheter, procedural success, and complications were extracted from our procedure database and compared between RCN and MCN. RESULTS: RCN use significantly decrea

Feasibility and safety of remote-controlled magnetic navigation for ablation of atrial fibrillation.

Radiofrequency ablation for atrial fibrillation (AF) involves complex catheter manipulation resulting in prolonged procedure time and fluoroscopy exposure. Remote magnetic navigation (RMN) represents a novel approach toward improving the ability to perform complex ablation. Forty patients underwent ablation for AF, 20 using RMN (NIOBE II, Stereotaxis, Inc) with a 4-mm-tip magnetic catheter (Celsius, Biosense Webster) and 20 using a conventional 8-mm-tip bidirectional ablation catheter (Blazer, Boston Scientific). All patients underwent a combined wide area circumferential ablation and segmenta

[Proarrhythmic effect of propafenone in patients with atrial fibrillation and atrial flutter].

We present two cases of proarrhythmia after propafenone treatment. In the first case slowing of the flutter cycle length from 205 ms to 290 ms resulted in a change from well tolerated two to one atrioventricular conduction to one to one conduction with hypotension and presyncope. In the second case organization of atrial fibrillation to atypical atrial flutter and use-dependent left bundle branch block resulted in fast broad QRS tachycardia that mimicked ventricular tachycardia.

[Right atrial flutter or tachycardia?].

Since the early days of Jenner, vaccination against infectious agents appeared a very efficacious way for preventing millions of deaths and patients with serious sequelae. However, only limited success has been obtained in eradicating infectious diseases. Smallpox is the only human infectious disease that has been eradicated so far by vaccination, and eradication of polio is in its endgame. Data from the WHO in 2003 show that 2.5 million children under five still die from vaccine preventable diseases, but many more die from the major microbial killers in the world, i.e. HIV/AIDS, malaria and t

Atrial fibrillation is common after ablation of isolated atrial flutter during long-term follow-up.

BACKGROUND: Previous studies have shown that the incidence of atrial fibrillation after atrial flutter ablation is approximately 20% among patients presenting with typical atrial flutter and no history of fibrillation. However, studies involving this population have been small, with follow-up typically less than 2 years. OBJECTIVE: The purpose of this study was to provide a more accurate perspective on the long-term risk of atrial fibrillation in patients presenting with isolated typical flutter. METHODS: Clinical records of consecutive patients who had flutter ablations at Presbyterian Medi

Stepwise linear approach to catheter ablation of atrial fibrillation.

BACKGROUND: This study attempted to convert atrial fibrillation (AF) to sinus rhythm using a stepwise linear catheter ablation approach. METHODS: One hundred and ninety-six patients (43 with persistent AF) were enrolled in the study. A multiple electrode array was used for anatomical navigation and activation mapping. Continuously incremental stimulation was used to induce AF if spontaneous AF was not present. Stepwise linear ablation was applied until AF was converted to sinus rhythm or atypical atrial flutter (AAFL) or atrial tachycardia (AT). The stepwise approach initially utilized a figu

Radiofrequency ablation of atypical atrial flutter after cardiac surgery or atrial fibrillation ablation: a randomized comparison of open-irrigation-tip and 8-mm-tip catheters.

BACKGROUND: The efficacy of radiofrequency ablation of atypical atrial flutter (AAFL) remains relatively low. This is probably related to the complex mechanism of this arrhythmia or may be due to an inability to deliver sufficient energy during ablation. OBJECTIVE: The aim of this study is to assess whether an open-irrigation-tip catheter or an 8-mm-tip catheter is more effective for ablation of AAFL in patients with prior history of cardiac surgery and/or catheter ablation of atrial fibrillation. METHODS: Seventy patients with AAFL after cardiac surgery/atrial fibrillation ablation were ran

Effect of right ventricular apex pacing on the Tei index and brain natriuretic peptide in patients with a dual-chamber pacemaker.

BACKGROUND: Asynchronous electrical activation induced by right ventricular apex (RVA) pacing can cause various abnormalities in left ventricular (LV) function, particularly in the context of severe LV dysfunction or structural heart disease. However, the effect of RVA pacing in patients with normal LV and right ventricular (RV) function has not been fully elucidated. The aim of this study was to characterize the effects of RVA pacing on LV and RV function by assessing isovolumic contraction time and isovolumic relaxation time divided by ejection time (Tei index) and by assessing changes in pl

Electrophysiological mechanisms of atrial flutter.

Atrial flutter (AFL) is a common arrhythmia in clinical practice. Several experimental models such as tricuspid regurgitation model, tricuspid ring model, sterile pericarditis model and atrial crush injury model have provided important information about reentrant circuit and can test the effect of antiarrhythmic drugs. Human atrial flutter has typical and atypical forms. Typical atrial flutter rotates around tricuspid annulus and uses the crista terminalis and sometimes sinus venosa as the boundary. The IVC-tricuspid isthmus is a slow conduction zone and the target of radiofrequency ablation.

Superior vena cava flutter: electrophysiology and ablation.

INTRODUCTION: Reentry within a major thoracic vein has been suggested as a cause of atrial arrhythmias. However, little is known about these potential reentrant circuits. METHODS AND RESULTS: Atypical atrial flutter was induced and mapped in 67 out of 225 atrial flutter ablation procedures. Reentry around the superior vena cava (SVC) was suspected in three patients. The suspected SVC flutter was induced and terminated by pacing in all patients. Fusion was demonstrated during flutter entrainment by subeustachian isthmus pacing in all of them. The postpacing interval following entrainment by pa

Catheter ablation of atypical atrial flutter and atrial tachycardia within the coronary sinus after left atrial ablation for atrial fibrillation.

OBJECTIVES: The goal of this study was to describe the prevalence and ablation of coronary sinus (CS) arrhythmias after left atrial ablation for atrial fibrillation (AF). BACKGROUND: The CS has been implicated in a variety of supraventricular arrhythmias. METHODS: Thirty-eight patients underwent mapping and ablation of atypical flutter that developed during (n = 5) or after (n = 33) ablation for AF. Also included were two patients with focal CS arrhythmias that occurred during an AF ablation procedure. A tachycardia was considered to be originating from the CS if the post-pacing interval in
