Post-transseptal puncture re-entrant atrial tachycardia mimicking typical flutter.

This case report discusses the management of a 75-year-old man who developed an unusual type of atypical atrial flutter following a previous pulmonary vein isolation for paroxysmal atrial fibrillation. Despite a second attempt to re-isolate the pulmonary veins and performing cavotricuspid isthmus ablation (which was suspected to be part of the arrythmia circuit), the flutter continued and was converted to sinus rhythm through electrical cardioversion. A few weeks later, the patient's atrial tachycardia relapsed. Initial ablation attempts at the coronary sinus ostium were unsuccessful due to in

Atypical Atrial Flutter: Electrophysiological Characterization and Effective Catheter Ablation.

Atrial flutter (AFL), defined as macro-re-entrant atrial tachycardia, is associated with debilitating symptoms, stroke, heart failure, and increased mortality. AFL is classified into typical, or cavotricuspid isthmus (CTI)-dependent, and atypical, or non-CTI-dependent. Atypical AFL is a heterogenous group of re-entrant atrial tachycardias that most commonly occur in patients with prior heart surgery or catheter ablation. The ECG pattern is poorly predictive of circuit anatomy but may still provide mechanistic insight. AFL is difficult to manage medically and catheter ablation is the preferred

In-hospital outcomes of catheter ablation in atrial arrhythmias: a nationwide analysis of 2,901 patients with adult congenital heart disease compared to 787,995 without.

BACKGROUND: Advances in pediatric cardiology and congenital heart surgery have increased the adult population with congenital heart disease (CHD), now facing long-term complications like atrial arrhythmias. Given the limited data and safety concerns in this unique and vulnerable patient group, this study analyzes in-hospital outcomes of atrial catheter ablation in CHD patients versus non-CHD patients from a German nationwide real-world registry. METHODS: Using health records, all atrial catheter ablation procedures in Germany from 2008 to 2021 were analyzed. After adjustment for confounders,

Using a Novel Pulsed Field Ablation Technique to Identify the Critical Isthmus Within a Tachycardia Circuit.

Pulsed field ablation uses irreversible electroporation to interrupt cellular membranes and induce myocyte apoptosis. Reversible electroporation has been used in other specialties, but its utility in cardiac ablation is unknown. Here, a 69-year-old woman undergoing repeat ablation for atypical atrial flutter presented with extensive scar after cardiac surgery (including MAZE) and previous ablation, leading to a macro re-entry circuit. To minimize superfluous lesions and further arrhythmia, we used a single pulse confirming the isthmus location, with cessation of the arrhythmia. As a conclusion

Checking Ablation Lines-Is Bidirectional Block Sufficient?

This case report presents a 71-year-old man undergoing repeat ablation for atypical atrial flutter after prior pulmonary vein isolation and subsequent re-ablation involving an anterior mitral line and a posterior box. High-density mapping revealed reconnection at the left superior pulmonary vein, which was successfully re-isolated. Although bidirectional block of the anterior mitral line was confirmed via local activation time (LAT) mapping during differential pacing, burst stimulation induced atrial flutter. Further LAT mapping during flutter identified very slow conduction through a gap in t

Serendipitous Supernormality.

We describe a patient who underwent AV node modification to create complete heart block in the setting of incessant, ablation-and-drug-refractory, symptomatic atypical atrial flutter. His dual chamber defibrillator (previously implanted for resuscitated cardiac arrest) was programmed to the VVIR mode at a faster pacing rate of 85 bpm. Serendipitously, this rate was an almost exact factorial of his flutter rate of 250-260 bpm. This resulted in every 6th flutter wave falling in the supernormal period, resulting in fixed-coupled supraventricular bigeminy and trigeminy in the setting of complete h

Clinical and Electrophysiological Predictors of Isthmus Dependency in Atrial Flutter.

Background: Atrial flutter (AFL) is a macro-reentrant tachycardia classified as cavotricuspid isthmus (CTI)-dependent or non-CTI-dependent based on its reliance on the CTI for conduction. CTI dependence can present as type I ECG (sawtooth flutter waves in inferior leads and positive P-waves in V1) or type II ECG (absence of these characteristics). This study aimed to identify clinical and electrophysiological parameters to improve CTI dependence prediction in AFL. Methods: Patients at the Ulm University Heart Center between 2010 and 2019 with AFL undergoing electrophysiological studies and abl

One-year outcomes of focal pulsed-field ablation for redo procedures in atrial fibrillation and atypical flutter.

BACKGROUND: Redo ablation procedures are frequently required in patients with recurrent atrial fibrillation (AF) and atypical atrial flutter following initial pulmonary vein isolation (PVI). While focal pulsed-field ablation (PFA) has emerged as a promising nonthermal alternative, data on its long-term efficacy and safety in redo procedures remain limited. This study evaluates the 1-year outcomes of focal PFA in redo ablations for AF and atypical flutter, focusing on arrhythmia recurrence, procedural success, and safety. METHODS: A retrospective analysis was conducted on 54 patients undergoin

Long-Term Results of Atrioventricular Node Ablation After His Bundle Pacing in Uncontrolled Atrial Tachyarrhythmias.

INTRODUCTION: In the evolving treatment of atrial fibrillation (AF), atrioventricular (AV) node ablation is being reconsidered as an early option for patients with inadequate AF control and limited cure potential. Although interest in physiological pacing is growing, concerns about the long-term safety of permanent His bundle pacing (p-HBP) persist. Our current study aims to evaluate the long-term outcomes of patients who underwent AV node ablation and p-HBP, focusing on left ventricular ejection fraction (LVEF), NYHA class, readmissions, and pacing parameters. METHODS: This descriptive obser

Endocardial Ablation of Atrial Flutter with Involvement of the Vein of Marshall: A Case Report.

Background: Atypical atrial flutter (AFL) is a complex clinical challenge, particularly in patients with prior atrial fibrillation (AF) treated with pulmonary vein isolation (PVI). Arrhythmias involving the vein of Marshall (VOM) often require extensive lesion sets, including ethanol infusion, to effectively target the epicardial substrate. To minimize tissue damage, an alternative strategy has been proposed, emphasizing advanced electroanatomical mapping, entrainment maneuvers, and highly targeted ablation techniques. Case Presentation: We describe a 72-year-old woman with recurrent atrial ar

Persistent Atrial Flutter Treated by Coronary Sinus Ablation in Persistent Left Superior Vena Cava.

Ablation of atypical atrial flutter may be challenging, particularly when a complex substrate is present. The persistence of left superior vena cava and severe coronary sinus dilatation carries an arrhythmogenic potential. In this case of atrial flutter, extensive substrate and activation mapping revealed the critical substrate within the coronary sinus.

Can Artificial Intelligence Identify the Ideal Ablation Area for the Patients With Atypical Atrial Flutter? A Proof of Concept Study Using Developed Software.

BACKGROUND: Catheter ablation for atypical atrial flutter (AFL) is challenging owing to its complex circuit. Consequently, the success rate varies depending on the circuit complexity and the operator's level of experience. OBJECTIVE: This study aimed to create software using artificial intelligence to identify the critical isthmus of atypical AFL on 3D mapping. METHODS: Raw data from 19 episodes of atypical AFL were extracted from the 3D mapping system. Local data, including location, activation timing, and voltage amplitude, were used to design software to identify areas where wavefronts co

Outcomes in patients with cardiac amyloidosis undergoing catheter ablation for atrial arrhythmias.

BACKGROUND: Cardiac amyloidosis (CA) is characterized by atrial myopathy, which predisposes patients to atrial fibrillation (AF) and other atrial arrhythmias (AA). Although catheter ablation of AA is effective in the general population, its efficacy and safety in patients with CA remain unclear. OBJECTIVE: The study aimed to evaluate outcomes in patients with CA undergoing catheter ablation for typical atrial flutter (TAFL) and left atrial (LA) arrhythmias and to assess the presence and influence of LA low-voltage areas (LVA) in the latter. METHODS: We retrospectively analyzed patients with

Breaking the Circuit: A Case of Macro-re-entrant Biatrial Tachycardia.

We present a case of a 71-year-old woman with symptomatic paroxysmal atrial fibrillation and atypical atrial flutter (AFL), ultimately diagnosed with a rare type 3 macro-re-entrant biatrial tachycardia (BiAT). Despite initial pulmonary vein isolation and anterior line ablation for atypical AFL, she experienced recurrent AFL requiring a complex redo ablation. Successful termination of the tachycardia was achieved by extending ablation to the septal regions of both atria. This case highlights the complexity of managing BiAT.

Subxiphoid epicardial approach for catheter ablation of refractory perimitral atrial flutter: a case series.

BACKGROUND: Perimitral atrial flutter is a complex atrial tachycardia involving endocardial and epicardial structures. The standard ablation strategy includes endocardial radiofrequency and vein of Marshall ethanol infusion, but restoring sinus rhythm can be challenging even in experienced centres. CASE SUMMARY: We report the cases of three male patients (60-, 75-, and 58-year-old) with persistent and symptomatic atypical atrial flutter. They shared a previous history of numerous atrial fibrillation catheter ablations. Endocardial left atrial activation mapping found a perimitral macro-reentr
