Residential radon in Peru: challenges for health surveillance and lung cancer prevention.

The Journal of Genetics, started by William Bateson in 1910, played a distinguished role in the early years of genetics. However, it stopped publishing in 1978. The Indian Academy of Sciences revived it in 1985, and has published it regularly since then. To commemorate this landmark, I highlight one of the 17 articles published that year. 'The isolation and genetic analysis of a Caenorhabditis elegans… X-chromosome balancer' by András Fodor and Péter Deak, of the Institute of Genetics, Biological Research Centre, Hungarian Academy of Sciences, Szeged, Hungary. Subcentimeter Ileal Neuroendocrine Tumor Resection for Obscure Gastrointestinal Bleeding Followed by Pacemaker-Treated Sick Sinus Syndrome: A Case Report.

BACKGROUND Obscure gastrointestinal bleeding (OGIB) denotes recurrent hemorrhage after nondiagnostic bidirectional endoscopy. In overt presentations with suspected small-bowel origin, capsule endoscopy, deep enteroscopy, and multiphase computed tomographic enterography (CTE) can improve lesion detection and operative planning. However, subcentimeter small-intestinal neuroendocrine tumors (siNETs) may remain undetected unless cross-sectional imaging enables targeted exploration. CASE REPORT A 72-year-old man presented with 2 days of melena and vague abdominal discomfort in the absence of hemate

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Assessment of the safety and efficacy of catheter ablation for atrial fibrillation in very elderly patients: insight from the national prospective registry study.

BACKGROUND AND AIMS: This study evaluated the safety and efficacy of catheter ablation in treating atrial fibrillation (AF) among the elderly population. METHODS AND RESULTS: A total of 170 017 AF ablation procedures prospectively enrolled from 482 facilities between 2017 and 2020 were analysed. They were stratified into six age groups, ranging from <65 to ≥85 years, in 5-year increments. A cut-off of 80 years was set for dividing participants into two groups. The primary endpoints included procedure-related complications and 1-year arrhythmia recurrence after a 3-month blanking period. Patie

The impact of common and rare genetic variants on bradyarrhythmia development.

To broaden our understanding of bradyarrhythmias and conduction disease, we performed common variant genome-wide association analyses in up to 1.3 million individuals and rare variant burden testing in 460,000 individuals for sinus node dysfunction (SND), distal conduction disease (DCD) and pacemaker (PM) implantation. We identified 13, 31 and 21 common variant loci for SND, DCD and PM, respectively. Four well-known loci (SCN5A/SCN10A, CCDC141, TBX20 and CAMK2D) were shared for SND and DCD, while others were more specific for SND or DCD. SND and DCD showed a moderate genetic correlation (rg =

Atrial Fibrillation-Related Bradycardia and/or Bradycardia-Related Atrial Fibrillation: When and How to Intervene.

INTRODUCTION/OBJECTIVE: Atrial fibrillation (AF) could present with slow ventricular-response; bradycardia could facilitate the emergence of AF. The conviction that one "does not succumb" from bradycardia as an escape rhythm will emerge unless one sustains a fatal injury following syncope is in stark difference with ventricular tachyarrhythmia (VA), which may promptly cause cardiac arrest. However, this is not always the case, as a life-threatening situation may emerge during the bradycardic episode, i.e., the development of bradycardia-induced VAs, which could be fatal if there is no prompt i

High prevalence of atrial conduction abnormalities in Lewy body disease - a marker of cardiac complications?

Increasing evidence suggests that Lewy body disease (LBD) is associated with clinically important cardiac complications, including sick sinus syndrome, atrial fibrillation and sudden cardiac death. The high prevalence of sick sinus syndrome and atrial fibrillation in LBD suggests the presence of disease-related atrial conduction disorders. To explore whether LBD is associated with atrial conduction disorders, electrocardiographic (ECG) P wave parameters were analyzed in a cohort of LBD patients (n = 74), using age-matched Alzheimer's disease (AD) patients (n = 25) as controls. P wave terminal

Long-term outcome after ablation of persistent atrial fibrillation in patients with postprocedurally unmasked sinus node disease.

BACKGROUND: About three percent of patients with persistent atrial fibrillation (AF) additionally suffer from a concealed sinus node disease (SND). We sought to determine the success of ablation one year after ablation of persistent AF in patients with postprocedurally unmasked SND. METHODS AND RESULTS: In total, 2539 patients with an ablation of persistent AF at our centre were screened for a postprocedurally unmasked SND, which made acute cardiac pacing necessary within one week after ablation. In a propensity score-matched case-control study (1:2 matching), the long-term ablation success o

Transvenous Dual-Chamber Pacemaker Implantation in a Patient with Persistent Left Superior Vena Cava Undergoing Maintenance Hemodialysis.

BACKGROUND: Persistent left superior vena cava (PLSVC) is a rare congenital venous anomaly. Permanent pacemaker implantation (PPI) in patients with PLSVC presents challenges in placing both the right ventricular and atrial leads. The article describes a technique for dual-chamber PPI using standard leads in a PLSVC patient with chronic kidney disease on maintenance hemodialysis. CASE PRESENTATION: A 69-year-old male patient with sick sinus syndrome (SSS), hypertension, moderate mitral regurgitation, dilated left ventricle, diabetes, chronic kidney disease (on hemodialysis), and NYHA III heart

Physiological Ventricular Pacing from the Right Side of the Septum by Engaging the Subendocardial Purkinje Network.

Right ventricular (RV) pacing, particularly from the RV apex, causes bilateral ventricular dyssynchrony, reducing systolic and diastolic function, by delayed activation of the lateral left ventricle, resulting in a wide QRS with a left bundle branch block (LBBB) morphology. Alternative pacing strategies, such as His-bundle pacing and LBB area pacing, tend to be more physiological, avoiding this problem. The feasibility of attaining a narrow paced QRS from the RV septum has not been methodically examined. This study aimed to test the hypothesis that, through pacing at select RV septal sites by

Benefits of Chronic Atrial Pacing in Fontan Physiology with Sinus Node Dysfunction and Preserved Ejection Fraction: Novel Therapeutic Approach for Failing Fontan.

Increased heart rate (HR) can reduce central venous pressure (CVP) in patients with preserved ventricular systolic function. We aimed to investigate whether this theory works in failing Fontan physiology. Pacemaker implantation (PMI) was performed in six Fontan patients (age: 6-40 years) with sinus node dysfunction (SND) without junctional rhythm (JR). Baseline median (interquartile [IQR]) values of CVP, ventricular end-diastolic pressure (EDP), and ejection fraction (EF) were 12 (11-15) mmHg, 13 (10-16) mmHg, and 57% (53%-64%), respectively. Before PMI, the relationships among HR, cardiac ind

The Role of Electrophysiologic Study in Device Selection for Leadless Atrial Pacing.

The AVEIR DR (Abbott Medical) was the first dual-chamber leadless pacing system approved by the US Food and Drug Administration. Many patients with sinus node dysfunction have physiologically normal atrioventricular conduction, so an atrial-only pacing system may be appropriate for them. Three patients with symptomatic sinus node dysfunction underwent device implantation with electrophysiologic study, including incremental atrial pacing until loss of 1:1 atrioventricular conduction (atrioventricular Wenckebach cycle length) and His-ventricular interval measurement. If the atrioventricular Wenc

Causality of Childhood and Adult Body Mass Index on Sick Sinus Syndrome: A Mendelian Randomization Study.

Background The relationship between body mass index (BMI) and the risk of sick sinus syndrome (SSS) remains unclear. Clarifying the impact of BMI on SSS at different life stages is essential for advancing precision medicine and implementing effective prevention strategies to reduce the burden of SSS. Methods The causalities of childhood and adult BMI with SSS were investigated by univariate and multivariate Mendelian randomization. Reverse causalities were also explored to improve the accuracy of causality findings. Different sources of exposure data were used for replication analysis, and the

Transient conduction disturbance during pulsed-field ablation for atrial fibrillation in a patient with pre-existing conduction disease.

Pulsed-field ablation (PFA) is a novel, non-thermal technique for atrial fibrillation (AF) ablation, known for its tissue specificity and reduced risk of collateral damage. However, its impact on cardiac conduction pathways in patients with pre-existing conduction abnormalities is not well understood. We present the first documented case of transient conduction block observed during PFA for AF. A late 60s male with paroxysmal AF and a dual chamber pacemaker for sinus node disease underwent successful PFA using a 31 mm pentaspline FARAPULSE catheter. During pulmonary vein isolations, transient
