Upper common pathway analysis using late atrial premature depolarization in atrioventricular nodal reentry tachycardia.

BACKGROUND: Anatomic and electrophysiologic findings suggest that the actual circuit of atrioventricular nodal reentrant tachycardia (AVNRT) involves the perinodal atrium. However, occasional instances in which the atrium is dissociated from the AVNRT have led to the concept of an upper common pathway (UCP). OBJECTIVE: We aimed to assess the prevalence of UCP in AVNRT using a late atrial premature depolarization (LAPD) maneuver. METHODS: Patients who were diagnosed with typical AVNRT by electrophysiologic studies were enrolled. For evaluation of the presence of UCP, an LAPD was given at the

Authors reply regarding "A-V-V-A response to single atrial premature depolarization in a narrow QRS tachycardia: What is the mechanism?".

We respond to a letter by Dr. A. Goyal. If the tachycardia were junctional ectopic tachycardia (JET), the occurrence of the ventriculoatrial block following an atrial premature depolarization could not be explained. Therefore, we conclude that atrioventricular nodal reentrant tachycardia was more likely than JET.

A-V-V-A response to single atrial premature depolarization in a narrow QRS tachycardia: What is the mechanism?

We present an atypical response to single atrial premature depolarization (APD) in a long RP' tachycardia. APD advanced the His-bundle potential immediately after it and resulted in a VA block; however, tachycardia persisted and consequently exhibited an A-V-V-A response. We propose the mechanism for an A-V-V-A response to APD in a long RP' tachycardia.

Change in VA interval by a single atrial premature depolarization - What is the mechanism?

A long VA tachycardia during a typical atrioventricular nodal reentrant tachycardia (AVNRT) can be a concomitant atypical AVNRT, atrial tachycardia or rarely atrio-ventricular reentrant tachycardia (AVRT). There are reported associations of AVNRT with other tachycardia substrates. Maneuvers are useful for differentiating the mechanism of the second tachycardia. Atrial tachycardia (AT) is one common association. When the AT originates from the lower triangle of Koch/near coronary sinus ostium, it can mimic slow-slow/fast-slow AVNRT. We encountered an interesting case where a longer VA tachycard

Atrial Premature Depolarization-Induced Changes in QRS and T Wave Morphology on Resting Electrocardiograms in Horses.

BACKGROUND: The electrocardiographic differentiation between atrial (APDs) and ventricular (VPDs) premature depolarizations is important. P wave prematurity and normal QRS and T wave morphology generally are used as discriminating criteria for APDs. HYPOTHESIS/OBJECTIVES: The aim of this study was to determine whether P, Q, R, S, and T wave amplitude, PQ interval, QRS and P wave duration and P and T wave morphology differ between APDs and sinus beats. To determine the relationship between the RR coupling interval and the change in S wave amplitude between sinus beats and APDs. METHODS: Case-

Fast T Wave Detection Calibrated by Clinical Knowledge with Annotation of P and T Waves.

BACKGROUND: There are limited studies on the automatic detection of T waves in arrhythmic electrocardiogram (ECG) signals. This is perhaps because there is no available arrhythmia dataset with annotated T waves. There is a growing need to develop numerically-efficient algorithms that can accommodate the new trend of battery-driven ECG devices. Moreover, there is also a need to analyze long-term recorded signals in a reliable and time-efficient manner, therefore improving the diagnostic ability of mobile devices and point-of-care technologies. METHODS: Here, the T wave annotation of the well-k

Atrial pacing for suppression of early reinitiation of atrial fibrillation after successful internal cardioversion.

AIMS: To evaluate the efficacy of atrial pacing in the suppression of early reinitiation of atrial fibrillation after successful internal cardioversion. METHODS AND RESULTS: The efficacy of atrial pacing in suppressing early reinitiation of atrial fibrillation was studied in 12 of 45 (29%) patients with early reinitiation of atrial fibrillation after successful cardioversion. These patients were randomized to undergo either repeated defibrillation alone or repeated defibrillation followed by high right atrial pacing at 500 ms in a crossover fashion. In patients with persistent early reinitiat

[Comparison of atrial premature depolarization topography during vagal stimulation and humoral acetylcholine administration].

Epicardial atrial mapping in open-chest dogs during different cholinergic influences has shown that, in acetylcholine administration and vagal stimulation, spatial distribution of atrial premature depolarisation (APDs) seems to be similar to prevalence of ectopic sources from both atria and atrial septum. Spatial distribution of the APDs in acetylcholine administration in the sinus node artery was limited to the region of this artery so that the APDs mainly arise from intercaval area of the right atrium and from atrial septum, but never from the left atrium. The latent pacemakers spread over b

Computer modeling of DDD pacemakers for use in prophylaxis of junctional reentry tachycardia.

A computer simulation model of cardiac rhythm and heart-pacemaker interactions has been used to examine the response of short AV delay DDD pacing to an atrial premature depolarization in patients with a potential junctional reentry circuit. Special attention was given to the possibility of using the pacemaker to prevent atrioventricular reentry tachycardia mediated by a bidirectional accessory pathway. The computational experiments examined: (a) the differences between pacing modes with constant and quasi-Wenckebach prolongation of AV delay; (b) the effects of imposing the restrictions of a pa

The dynamic nature of ventriculoatrial conduction.

An endless loop tachycardia starts when the atrial sensory amplifier of a dual chamber pacemaker identifies an early atrial signal originating from a ventricular or atrial premature depolarization or from myopotential noise. The tachycardia will continue as long as ventriculoatrial conduction is sustained. By selecting the appropriate atrial sensitivity setting, postventricular atrial refractory period, or upper rate limit, it is possible to eliminate sustained endless loop tachycardia. Electrophysiological data obtained at the time of dual chamber pacemaker implantation can assist the physici

Indirectly estimated sinoatrial conduction time by the atrial premature stimulus technique: patterns of error and the degree of associated inaccuracy as assessed by direct sinus node electrography.

The atrial premature stimulus method for estimating sinoatrial conduction time (SACT) is commonly used. When the stimulated atrial premature depolarization (APD) does not appear to affect sinus node automaticity or conduction, the indirectly estimated SACT (SACT1) is quite accurate. That is, SACT1 correlates quite highly with SACT measured directly (SACTD) on sinus node electrograms (SNE). In this study we used direct SNE recordings in 17 patients to assess SACT1 when factors thought to produce inaccuracy in SACT1 were present. Three patients had sinoatrial entrance block, which might make som

Evidence that AV nodal re-entrant tachycardia does not require participation of the entire AV node.

Electrophysiologic studies in a case of AV nodal re-entrant tachycardia showed that a programmed atrial premature depolarization induced during the tachycardia did not change the tachycardia cycle but caused a delay in the following atrial echo. Analysis of such a phenomenon suggests that the atrial premature depolarization was conducted to the upper part of the AV node but not to the site of the re-entry. Therefore, AV nodal re-entry can persist without the participation of the upper part of the AV node. This case illustrates that the upper common pathway connecting the dual AV nodal pathways

Tachyarrhythmias associated with programmable automatic atrial antitachycardia pacemakers.

A multiprogrammable automatic antitachycardia pacemaker (Cybertach-60), connected to a tined "J" wire electrode, was positioned in the right atrial appendage in three patients with recurrent supraventricular tachycardia (SVT). Mechanisms of SVT included atrioventricular (AV) nodal reentry (two patients) and reciprocating tachycardia using an accessory pathway (one patient). Indications for implantation included frequent symptomatic episodes, multiple drug failures, and reproducible termination of the arrhythmia by the automatic pacemaker during electrophysiologic studies. Following discharge,

A second zone of compensation during atrial premature stimulation: evidence for decremental conduction in the sinoatrial junction.

125 consecutive patients with premature atrial stimulation were studied. Three demonstrated sinus node return cycles that were fully compensatory following premature atrial stimuli delivered early in diastole. This second zone of compensation was unaccompanied by significant alterations in the post-return cycle lengths or in P-wave morphology of the return cycle. To account for the occurrence of a complete compensatory pause following very early premature atrial depolarizations, we consider the possibility that retrograde conduction of the early atrial premature depolarization (APD) in the sin

Spontaneous atrial premature depolarizations during paroxysmal reentrant tachycardia.

The spontaneous onset and termination of many episodes of paroxysmal tachycardia, each initiated by an atrial premature depolarization, were studied in one patient. Surface electrocardiograms alone were inadequate to define the mechanisms underlying the frequent irregularity of atrial and ventricular cycle lengths during the tachycardia and the nature of spontaneous termination of the tachycardia. Unipolar atrial electrograms demonstrated that the irregularity during the tachycardia was due to premature atrial depolarizations that reset the reentrant cycle sustaining the tachycardia, and each
