Risk of Cancer Comparing Warfarin and Direct Oral Anticoagulants: Population-Based Cohort Studies in England and Hong Kong.

BACKGROUND: Previous evidence suggests a potential protective effect of warfarin against cancer, compared to non-users. However, it may be prone to immortal time bias and residual confounding. OBJECTIVE: We aimed to examine the association between cancer and warfarin, compared with active comparator (direct oral anticoagulants). METHODS: We conducted studies using population-based databases from England and Hong Kong to investigate the association between warfarin and hazard of cancer using a new-user active-comparator cohort design. People with atrial fibrillation aged ≥ 18 years who had fi

Left atrial appendage closure versus direct oral anticoagulants after pulmonary vein isolation for atrial fibrillation: protocol for a multicentre, prospective, randomised, non-inferiority trial (PROMOTE study).

INTRODUCTION: Atrial fibrillation (AF), with a prevalence of 1-2%, is the most common cardiac arrhythmia. AF is associated with a fivefold increased risk of cardioembolic events; approximately 20% of all strokes are caused by AF. Pulmonary vein isolation (PVI) has become the first-line treatment for AF. However, PVI cannot eliminate the residual stroke risk. Current guidelines recommend that anticoagulation be continued in this specific group of patients, regardless of the presence or absence of AF. In this large AF population post-PVI, who are considered to be in an earlier stage of AF, it is

Ionized magnesium levels and atrial fibrillation in patients undergoing cardiac surgery - The iMagic Prospective Cohort Study.

BACKGROUND: New-onset atrial fibrillation (AF) after cardiac surgery is associated with unfavorable outcomes. Electrolyte imbalances have been described as potential risk factors. The effects of ionized magnesium (iMg), a component of total magnesium (tMg) concentration in the blood, and the influence of iMg-dynamics on the incidence of postoperative atrial fibrillation (POAF) are unclear. METHODS: Ionized Magnesium Levels and Atrial Fibrillation in Patients undergoing Cardiac surgery (iMagic) was a single-center prospective cohort study. A total of 200 patients were enrolled over six months.

Pitfalls in the reporting of clinical events in unblinded device trials: analysing performance bias in the BIO|GUARD-MI trial.

BACKGROUND: The BIO|GUARD-MI study was an unblinded trial that investigated the clinical effect of continuous arrhythmia monitoring with an implantable cardiac monitor in patients after myocardial infarction. Unexpectedly, a pre-specified interim analysis identified a higher incidence of non-cardiovascular adverse events in the treatment group. This was assumed due to a performance bias and ultimately led to premature study termination. By study protocol, participants were expected to visit the study site only when invited after detected arrhythmias. Here, we provide a secondary analysis of pa

A privileged ER compartment for post-translational heteromeric assembly of an ion channel.

Mechanisms underlying heterotypic subunit assembly of ion channels and other oligomeric complexes are poorly understood. In the human heart, heteromeric assembly of two isoforms encoded by the human ether-à-go-go related gene (hERG) is essential for the normal function of cardiac IKr in ventricular repolarization, with loss of hERG1b contributing to arrhythmias associated with long QT-syndrome (LQTS). While hERG1a homomers traffic efficiently to the plasma membrane, hERG1b homomers are retained in the endoplasmic reticulum (ER). When expressed together, the two subunits avidly associate during

Colchicine in patients with aortic stenosis undergoing transcatheter aortic valve replacement: a double-blind randomized trial.

An inflammatory process may increase the risk of arrhythmias after transcatheter aortic valve replacement (TAVR). In this single-centre, double-blind, placebo-controlled, randomized trial we investigated the efficacy of colchicine to reduce a composite of new-onset atrial fibrillation or atrioventricular conduction disturbances requiring the implantation of a permanent pacemaker at 30 days after TAVR. Between September 21, 2021 and April 25, 2024, 120 patients with aortic stenosis undergoing TAVR (mean age 80.6 ± 5 years, 64% male) were randomly allocated to treatment with colchicine (n = 60)

Incidence and predictors of persistent left atrial appendage patency and its subtypes after percutaneous closure: a pre-specified analysis of the SWISS-APERO trial.

AIMS: Left atrial appendage (LAA) patency after percutaneous closure is associated with higher thromboembolic risk, especially when detected at later time points after procedure. We aim to investigate the incidence and predictors of persistent LAA patency and its different subtypes at cardiac computed tomography angiography (CCTA) during the first year after LAA closure (LAAC). METHODS AND RESULTS: In the SWISS APERO trial, patients undergoing LAAC were randomly assigned (1:1) to Amulet or Watchman/FLX across eight European centers and received CCTA follow-up at 45 days and 13 months. The pre

External validation of atrial fibrillation risk scores in heart failure under continuous device surveillance.

BACKGROUND: Population-derived atrial fibrillation (AF) risk scores are widely used, but their transportability to populations with heart failure (HF) under continuous rhythm surveillance is uncertain. OBJECTIVE: The goals of this study were to externally validate established AF risk scores in patients with HF monitored continuously using implantable devices and to assess discrimination and clinical utility for guiding surveillance strategies. METHODS: This study included a prospective multicenter cohort of outpatients with HF without prior AF. All patients carried implantable loop recorders

Proactive esophageal cooling during radiofrequency cardiac ablation: data update including applications in very high-power short duration ablation.

INTRODUCTION: Proactive esophageal cooling reduces injury during radiofrequency (RF) ablation of the left atrium (LA) for the treatment of atrial fibrillation (AF). New catheters are capable of higher wattage settings up to 90 W (very high-power short duration, vHPSD) for 4 s. Varying power and duration, however, does not eliminate the risk of thermal injury. Furthermore, alternative energy sources such as pulsed field ablation (PFA) also exhibit thermal effects, with clinical data showing esophageal temperatures up to 40.3°C. The ensoETM esophageal cooling device (Attune Medical, now a part o

Adherence to the Atrial fibrillation Better Care pathway and the risk of adverse health outcomes in older care home residents with atrial fibrillation: a retrospective data linkage study 2003-18.

BACKGROUND: The Atrial fibrillation Better Care (ABC) pathway is the gold-standard approach to atrial fibrillation (AF) management, but the effect of implementation on health outcomes in care home residents is unknown. OBJECTIVE: To examine associations between ABC pathway adherence and stroke, transient ischaemic attack, cardiovascular hospitalisation, major bleeding, mortality and a composite of all these outcomes in care home residents. METHODS: A retrospective cohort study of older care home residents (≥65 years) in Wales with AF was conducted between 1 January 2003 and 31 December 2018

[Drug therapy for arterial hypertension and atrial fibrillation in frail patients : Are there any new insights and recommendations?].

The frail and elderly are considered to be at particular risk of suffering an adverse drug reaction. Empirical studies confirm the increased rate of adverse drug reactions. Whether frailty per se impairs drug metabolism or the underlying organ ageing processes and multimorbidity cannot be answered with certainty based on current data. Cardiovascular diseases exhibit a considerable interdependence with frailty. For example, there is a disproportionate syndromal interdependence between heart failure and frailty, and the typical ageing processes of the sinus node can be interpreted as heartbeat f

Incidence and predictors of 2-year mortality following percutaneous left atrial appendage occlusion in the EWOLUTION trial.

AIMS: Sufficient survival time following left atrial appendage occlusion (LAAO) is essential for ensuring the efficacy and cost-effectiveness of this strategy for stroke prevention. Understanding prognostic factors for early mortality after LAAO could optimize patient selection. In the current study, we perform an in-depth analysis of 2-year mortality after LAAO, focusing particularly on potential predictors. METHODS AND RESULTS: The EWOLUTION registry is a real-world cohort comprising 1020 patients that underwent LAAO. Endpoint definitions were pre-specified, and death was categorized as car

Failure to treat ventricular tachycardia or ventricular fibrillation by implantable cardioverter-defibrillators.

BACKGROUND: Normally functioning implantable cardioverter-defibrillators (ICDs) with intact lead systems occasionally fail to deliver therapy for ventricular tachycardia/fibrillation (VT/VF) or deliver it only after clinically significant delays (failure-to-treat). OBJECTIVE: We aimed to investigate ICD failure-to-treat VT/VF in a large patient cohort. METHODS: We searched the US Food and Drug Administration's online Manufacturer and User Facility Device Experience (MAUDE) database from 2019 to 2023 for manufacturer-verified reports in which normally functioning ICDs failed to treat VT/VF.
