"Supportive relationships over support services"-A qualitative study of patient and caregiver perspectives on the implementation of community-based palliative care for people living with advanced noncommunicable diseases.

BACKGROUND: Community-based palliative care (CBPC) shows promise for supporting patients with advanced noncommunicable diseases, yet implementation remains inconsistent. Most research focuses on organizational and provider perspectives, with limited understanding of patient and caregiver views on service acceptability and implementation. OBJECTIVES: To understand barriers and facilitators to implementing CBPC services from the perspectives of patients with advanced congestive heart failure, chronic obstructive pulmonary disease and end-stage renal failure, and their primary caregivers. DESIG

Fetomaternal hemorrhage: pathogenesis, diagnosis, and clinical management.

Clinically significant fetomaternal hemorrhage (FMH) can have devastating consequences on the newborn infant. Acute FMH close to childbirth can present with hypovolemia, shock, metabolic acidosis, and encephalopathy. Chronic FMH can be associated with congestive heart failure, pulmonary edema, hydrops and hepatomegaly. Early recognition and timely management of FMH are crucial in improving outcomes. This review article summarizes the epidemiology, pathogenesis, diagnosis, management and outcomes of clinically significant FMH. Current knowledge gaps in diagnosis and management of FMH are additi

Effect of Frailty on Postoperative Complications From Colpocleisis.

IMPORTANCE: Colpocleisis is an obliterative procedure for pelvic organ prolapse commonly offered to medically complex patients who may not be candidates for reconstructive surgery. Although frailty is a well-established predictor of surgical morbidity, its effect on colpocleisis outcomes is not well defined. The 5-factor Modified Frailty Index (mFI-5) is a validated tool for surgical risk prediction. OBJECTIVE: This study evaluated the association between mFI-5 and 30-day postoperative complications after colpocleisis. STUDY DESIGN: We conducted a retrospective cohort study using the America

Applications of Venous Excess Ultrasound Score (VExUS) in Volume Status Assessment in Patients With Acute Decompensated Heart Failure and Cardiorenal Syndrome.

Accurate assessment of volume status and venous congestion is essential in the management of congestive heart failure (CHF), particularly in patients with cardiorenal syndrome (CRS). Traditional approaches, including physical examination, laboratory biomarkers, and noninvasive hemodynamic monitoring, have notable limitations in reliably detecting venous congestion. The Venous Excess Ultrasound (VExUS) score, introduced in 2020, offers a noninvasive and widely available method to evaluate systemic congestion by integrating Doppler assessment of the inferior vena cava, hepatic, portal, and intra

The Detrimental Outcomes Associated With Kidney Delayed Graft Function Persist Beyond the Short Term.

BACKGROUND: Delayed graft function (DGF) is associated with acute rejection (AR) and decreased graft survival, mainly in the early posttransplant period. However, its impact beyond this time is not well known. METHODS: All deceased donor kidney-only transplant recipients at our center between June 2005 and December 2019 were stratified based on DGF status (+ versus -). Adjusted hazard ratios (aHRs) of all-cause graft failure (GF), death-censored GF (DCGF), death with a functioning graft (DWFG), and acute rejection (AR) were analyzed within the short-term (0-6 mo) and medium-term (7-36 mo) pos
